        The Obturator Hernia: A Challenge for Early Diagnosis with Computed Tomography in Emergency Situations

        Obturator hernias usually occur in elderly persons. We sometimes miss the correct diagnosis because of vague symptoms and signs in older patients at initial presentation. For good clinical outcomes, we should make the correct diagnosis earlier, especially in aged patients. Here we report cases of obturator hernia in two aged patients without specific medical or surgical histories. Two older women suffered obturator hernias, which were characterized by clinical and radiographic features. They did not have any specific surgical and medical histories. For further evaluation of abdominal symptoms and signs, we did an abdominopelvic computed tomography (CT) scan in the emergency department. The CT scan revealed findings compatible with an obturator hernia in the area of the obturator foramen. They were treated and discharged without complications. When obturator hernia is suspected in a patient with related symptoms and signs, early CT scans should be considered to detect obturator hernia.

        응급센터에 내원한 정신분열증 환자에서 급성 충수돌기염의 임상적 고찰

        Purpose: To investigate the clinical characteristics and Alvarado scores of schizophrenia with patients acute appendicitis presenting at the emergency department. Methods: We retrospectively reviewed medical records and identified schizophrenia patients who were diagnosed with acute appendicitis between January 2000 to December 2009 and who presented at the emergency department in 6 University hospitals. Comparisons were made with non-schizophrenic patients with acute appendicitis. Data collected included the patient's age, sex, clinical features, time interval between onset of symptoms and visiting the hospital, Alvarado score, disease complications,and diagnostic imaging modality. Results: We identified 27 schizophrenia patients with acute appendicitis. Mean age was 34.5±8.3; 51.9% were women. The median time from onset of symptoms to visiting the emergency department was 3.8±1.4 days in the schizophrenic group and 2.3±2.1 in the normal control group. The mean Alvarado scores were 5.4±1.5 for the schizophrenic group and 6.5±2.1 for controls. Perforation of the appendix and formation of abscesses was higher in schizophrenic patients with appendicitis. Conclusion: These findings suggest that acute appendicitis is diagnosed late and with difficulty in schizophrenic patients and these patients may be the most vulnerable to failure to obtain timely surgical care. Early and careful access is recommended in suspected cases of appendicitis in schizophrenia patients.

        술로 오인된 차량 유리 세정액 음독으로 인한 메탄올 집단 중독

        Almost all cases of acute methanol intoxication result from ingestion. Absorption of methanol following oral administration is rapid and peak concentration occurs within 30-60 min. Foromic acid converted metabolites of methanol are responsible for its toxic effect. Methanol poisoning typically induces nausea, vomiting, abdominal pain, and mild central nervous system depression. Then there is a latent period lasting, followed by an uncompensated metabolic acidosis and visual dysfunction, depending on the dose of methanol ingested. Here we report on three patients who were victims of methanol intoxication due to ingestion of windshield washer fluid, which was misidentified as an alcoholic beverage. One patient had a mild form, which included abdominal discomfort,another patient had a moderate case, which included nausea, vomiting, and acidosis, and the other patient had severe acidosis and altered mentality and visual dysfunction. Except one case of visual dysfunction, all three patients recovered successfully following medical treatment and hemodialysis.

        비대칭 하이브리드 합성보-기둥 접합부 개발 및 내진성능평가

        최근 저자는 일반강재와 고강도강재를 단면에 함께 사용하는 비대칭 하이브리드 합성보를 제안하였다. 본 연구에서는 후속연구로서 비대칭 하이브리드 합성보–기둥 접합부를 개발하고 실물대 실험을 통해 해당 접합부의 내진성능을 평가하였다. 보경간 전체의 단면 활용도를 극대화하기 위해 중력하중을 받는 보의 모멘트 분포에 따라 두가지 타입(내측경간과 외측경간)으로 실험체를 구분하였다. 실험체 설계시 안정적인 반복거동을 기대할 수 있도록 정모멘트와 부모멘트의 소성힌지 위치를 다르게 유도하는 것에 가장 중점을 두었다. 실험결과 모든 실험체가 특수모멘트골조에 해당하는 성능을 발휘하였으며 의도한 위치(정모멘트시 기둥면, 부모멘트시브라켓 내부)에 소성힌지가 발생하였다. 하지만 내측경간 실험체에서 볼트연결부 아래의 브라켓 웨브에 조기(3% 층간변위레벨)에 국부좌굴이 발생하였고, 이후 부모멘트강도가 저하되었기에 브라켓 웨브의 보강이 필요할 것으로 보인다. Asymmetric hybrid composite beams, utilizing both ordinary- and high-strength steels in one beam section, have been recently proposed by the authors. As a continuing work, asymmetric hybrid composite beam-to-column connections were developed and their full-scale seismic testing was conducted in this study. In order to maximize the cross-section efficiency along the beam span, the connections were developed in two types (for exterior and interior spans) by considering typical bending moment distribution of the continuous beams under gravity loading. In order to obtain stable cyclic response, inducing the plastic hinging at the different locations for the positive and negative moments was among the most significant design considerations. All the four specimens tested in this study developed the plastic hinging at the intended locations (i.e., at the column face for the sagging moment and at the bracket region for the hogging moment), and satisfied the requirements of special moment frame connections. However, the bracket web below the bolted splice experienced rather earlier local buckling (at 3% story drift level)and subsequent strength degradation under the hogging (negative) moment, thus indicating the need to stiffen the bracket web.

        SPEED CONTROL OF REMOTELY OPERATED MULTI-WHEEL MOTOR-DRIVEN VEHICLES FOR UNPAVED ROADS AND OBSTACLES

        This paper describes the speed control of a remotely operated multi-wheel motor-driven electric vehicle that can be effectively utilized not only on paved roads but also on unpaved roads or obstacles. The proposed speed control algorithm consists of an upper-level control module and two lower-level control modules. The upper-level module generates acceleration commands and the weighting factor, allowing the tractive and braking forces to intersect smoothly. Two lower-level modules generate motor torque and hydraulic brake commands, respectively. This speed controller tracks the speed commands of each axle to reduce wheel slip on unpaved roads. In addition, it uses the pitch angle of the vehicle body for mobility on slopes or obstacles. The disturbance observer is also applied to the lower-level control modules for robustness against modeling errors and frequently changing road conditions. Finally, the proposed speed control algorithm is applied to a simulation model and a six-wheel motor-driven prototype vehicle used for military purposes. Its effectiveness is verified through various scenarios, including unpaved roads and obstacles.

        Eff ects of Moment Gradient on Elastic Flange Local Buckling of I-Shaped Beams

        Recently, after clearly pointing out the fl aws associated with the current AISC equation for fl exural strength of I-shaped beams with slender fl ange, the authors proposed new equation, more accurate and more consistent than the current AISC equation, based on the mixed variational approach. However, the new equation was derived under the condition of uniform moment loading. In this paper, elastic fl ange local buckling strength formula for I-shaped beams with slender fl ange under moment gradient was proposed as a follow-up study on the basis of mixed variational approach and supplemental numerical works. The fl ange plate buckling coeffi cient predicted by the formula of this study is again shown to be much more accurate and consistent.

        이러닝 시스템의 품질지각이 체계만족도와 학습몰입에 미치는 영향

        The purpose of this study is to investigate the effect of Digital Textbook Learning in the Army on User Satisfaction and Learning Immersion. By using the Information System Success Model, system quality, content sufficiency, faculty involvement, and knowledge sharing are hypothesized as the precedents of user satisfaction, which leads to students’ learning immersion. Survey results show that content sufficiency, faculty involvement, and knowledge sharing each has positive impact on user satisfaction. However, system quality has no influence on system satisfaction of users who use the digital textbook. User satisfaction with the e-Learning system leads to learning immersion. Theoretically, this study developed e-Learning theory based on the perspective of the Information System Success Model and knowledge sharing. Practical implications were suggested e-Learning systems to be developed continuously. 본 연구의 목적은 최근 육군에서 스마트러닝의 일환으로 추진 중인 디지털교과서 활용 간 사용자의 체계만족도와 학습몰입에 미치는 영향요인을 조사하고 그 효과를 밝히는 데 있다. 이를 위해 시스템품질, 콘텐츠충족성, 교수자의 노력, 지식 공유를 선행요인으로 고려하였으며, 이러한 선행요인이 디지털교과서 활용간 사용자가 느끼는 만족도와 학습 몰입에 미치는 영향관계를 설명하기 위하여 정보시스템 성공모형을 응용하여 가설로 설정하였다. 설문 분석결과 사용자들의 디지털교과서 활용을 위한 교수자의 노력, 디지털교과서콘텐츠의 충족성, 사용자들 간의 지식공유행위가 체계 만족도에 긍정적인 영향을 미치는 것을 알 수 있었다. 한편, 디지털 교과서를 활용하는 사용자의 체계만족도 향상에 시스템 품질이 미치는 영향은 유의하지 않았다. 이론적 측면에서 정보시스템 성공모형과 지식공유의 관점에서 이러닝의 효과성을 측정하였다는데 의의가있으며 실무적으로는 이러닝 시스템의 활용과 발전을 위한 시사점을 제시하였다는데 의의가 있다.

        웨브 구속과 모멘트 구배가 H형강 보 세장 플랜지 국부좌굴 강도에 미치는 영향

        Compared to 2016 KBC, very large flexural overstrength was observed in the recent full-scale testing of welded H-shaped beams with slender flange. In this paper, based on critical review of the background research of the relevant provisions, it is first shown that the KBC provisions do not properly reflect the effects of web restraint and moment gradient on elastic flange local buckling (FLB). In this study, elastic FLB strength equation with sound theoretical background is newly proposed using the mixed variational approach. The proposed equation is more accurate but still simple for practical use. 최근 수행한 조립 H형강보의 실물대 실험에서 세장플랜지를 갖는 실험체가 설계기준에 비해 상당한 초과 휨강도를 발휘하였다. 분석결과 현재 설계기준은 웨브에 의한 플랜지 구속효과를 과소평가하고 있으며, 모멘트 구배가 플랜지좌굴강도에 미치는 영향을 반영하지못하고 있음을 확인하였다. 본 연구에서는 혼합변분법을 통하여 과도하게 보수적인 현행 설계기준의 플랜지국부좌굴식의 정확성을 높이고 모멘트 구배효과를 반영할 수 있는 탄성 플랜지국부좌굴강도식을 새로이 유도하였다. 또한 혼합변분법으로 계산된 다양한 해석결과를 바탕으로 보다 정확하고 실용적인 약산식을 제시하였다.
