Background and Objectives Sudden sensorineural hearing loss (S-SNHL) has been reportedto be associated with hypertension, diabetes mellitus, hyperlipidemia, and stroke inprevious studies. The purpose of this study was to confrm whether metabolic syndrome (MS)infuences the hearing outcome of S-SNHL in the Korean population. Subjects and Method We prospectively investigated the clinical variables of 231 cases ofS-SNHL from 2010 March to 2014 February. MS was defned according to the National CholesterolEducation Program Adult Treatment Panel III with Asian modifcations. We analyzedand compared the basic and clinical characteristics, the thresholds of pure tone audiometry(PTA), and the hearing outcomes of patients with MS (MS group) and without MS (NMSgroup). Results There were no signifcant differences in the basic characteristics and initial hearingthresholds between the MS and NMS groups; on the other hand, there were signifcant differencesin all of the clinical values of MS between the two groups. The average thresholds of finalPTA for the MS and NMS groups were 52.9±30.9 dB and 40.5±27.2 dB, respectively,with meaningful diversity (p<0.01). According to Siegel’s criteria, the recovery rate of theNMS group (95/159, 59.7%) was signifcantly higher than those of the MS group (29/72, 40.3%). The proportion of recovery within 5 days after starting the treatment was signifcantly higherof the NMS group (37/95, 39.0%) than those of the MS group (4/29, 13.8%) (p<0.01). Conclusion The hearing outcome of the NMS group was signifcantly better than those ofthe MS group.
장대터널에서 수소연료전지 차량의 수소 누출에 대한 수소 거동의 수치해석 연구

In the present study, the dispersion characteristics of hydrogen leakage from a Fuel Cell Vehicle (FCV) were analyzed by numerical simulation in order to assess the risk of a hydrogen leakage incident in a long road tunnel. In order to implement the worst case of hydrogen leakage, the FCV was located at the center of a tunnel, and hydrogen was completely discharged within 63 seconds. The Leakage velocity of hydrogen was adopted sub-sonic speed because that the assumption of the blockage effect of secondary device inside a vehicle. The temporal and spatial evaluation of the hydrogen concentration as well as the flammable region in a road tunnel was reported according to change of ventilation operating conditions. The hydrogen was blended by supply air form a ventilation fan, however, the hydrogen was discharged to outside in the exhaust air. It is observed that the efficiency way to eliminate of hydrogen is supply air operating condition under the hazardous hydrogen leaking incident. The present numerical analysis can be provided useful information of ventilation under the hydrogen leaking situation.

Mucosa-associated lymphoid tissue (MALT) lymphoma has specific clinical and pathologic features. The most common site MALT lymphomas is the stomach; however, it can also occur in other organs, such as the salivary glands. MALT lymphoma is rare, but its prognosis is good. A 32-year-old man visited Konyang university hospital with parotid mass. Superficial partial parotidectomy was performed to exclude lymphoid neoplasms. IgH gene rearrangement analysis of the surgical specimen led to the diagnosis of MALT lymphoma. The patient underwent esophagogastroduodenoscopy, positron emission tomography-computed tomography, and whole-body bone scan. Regional or distant metastasis was not observed on staging workup. The patient underwent postoperative radiation therapy, there has been no recurrence of MALT lymphoma to date. Here, we report this rare case of parotid MALT lymphoma that was treated with surgery and postoperative radiation therapy.

Epithelial-myoepithelial carcinoma (EMC) is a rare type of low-grade malignant tumor that account for pproximately 0.5% to 1% of salivary gland neoplasm and arises most commonly in the parotid gland (80%). We introduce three cases of parotid EMC arose as painless cystic mass in male patients over 70 years old. All patients were diagnosed as benign tumors (pleomorphic adenoma) by image and pathologic study (fine needle aspiration) before surgery, but the final histopathologic results were EMC. All three patients underwent parotidectomy without adjuvant adiotherapy. There were no complications such as facial paralysis. No complications or recurrences were observed during follow-up for 6 to 9 months. Since the reports of EMC are still relatively few, we report our three cases with the clinical and pathological review.

Eosinophilic chronic rhinosinusitis with nasal polyps (CRSwNP) is one of the most challenging problems in clinical rhinology. FZD5 is a receptor for Wnt5A, and its complex with Wnt5A contributes to activating inflammation and tissue modification. Nasal polyps and eosinophil/non-eosinophil counts are reported to be directly correlated. This study investigated the expression and distribution of FZD5, and the role of eosinophil infiltration and FZD5 in eosinophilic CRSwNP pathogenesis. The prognostic role of eosinophil levels was evaluated in seven patients with CRSwNP. Fifteen patients with CRS were classified based on the percentage of eosinophils in nasal polyp tissue. Methylated genes were detected using methylCpG-binding domain sequencing, and qRT-PCR and immunohistochemistry were used to detect FZD5 expression in nasal polyp tissue samples. The results showed that mRNA expression of FZD5 was upregulated in nasal polyps. FZD5 expression was significantly higher in nasal polyp samples from patients with eosinophilic CRSwNP than in those from patients with non-eosinophilic CRSwNP, as indicated by immunohistochemistry. Furthermore, inflammatory cytokine levels were higher in eosinophilic CRSwNP-derived epithelial cells than in normal tissues. In conclusion, FZD5 expression in nasal mucosal epithelial cells is correlated with inflammatory cells and might play a role in the pathogenesis of eosinophilic CRSwNP.
