        각종 음성분석기에 따른 음성장애 환자의 주기간 주파수 및 진폭변동률 분석

        Background and Objectives : Voice perturbation measures, such as jitter and shimmer has been importantly used for diagnosis and treatment efficacy of laryngeal dysfunction. This study was conducted to investigate validity of newly developed multi-channel voice analyzer program by comparing with MDVP, PRAAT, TF32. In addition, we compared the voice perturbation measures with different voice analyzer program by type of signals. Materials and Methods : Nineteen mild-severe dysphonic patients participated in our study. Fundamental frequency, jitter and shimmer values were obtained from different voice analyzer program using the same sustained/ah/phonation. Results : Fundamental frequency and shimmer were highly correlated whereas jitter was weakly correlated between newly developed multi-channel voice analyzer program and the others though different pitch computation algorithm except MDVP, In addition, Type 2 and 3 signals were weakly correlated than Type 1. Conclusion : In the clinical setting, clinician may have sufficient information of voice analyzer and control conditions properly for severity of pathologic voice before voice perturbation measure to obtain reliable results.

      • 잔류농약 위해평가를 위한 국제화 및 독성DB 마련 연구

        [연구목적] 잔류농약 등 식품 중 오염물질에 대한 사회적 우려가 지속되고 있어, 효율적인 규제 관리를 위하여 신속하고 정확한 위해평가 방법의 활용이 필요하다. CODEX, EU 등 농약 잔류허용 기준 설정 위해평가 시 만·급성평가를 동시 수행하고 있어 국내에서도 급성 위해평가방법의 적용이 필요한 상황이다. 이에 본 연구에서는 국외 급성위해평가방법을 조사하여 잔류농약 허용기준 설정 시 적용 가능한 급성위해평가 시스템 개발을 목적으로 한다. [연구 방법] 국내외 잔류농약의 급성 노출에 대한 위해평가모델을 분석하여 각 위해평가모델이 가지고 있는 장단점과 알고리즘을 비교하였다. 또한, 노출 및 위해평가를 위해 요구되는 급성독성 참고치(aRfD) 등을 포함한 변수에 대해 데이터베이스를 개별적으로 구축하고 각각의 데이터베이스를 체계적으로 조직화하고 국내 현실에 적합한 시스템을 개발하고자 제외국의 위해평가 시스템을 조사 비교한다. [연구결과 및 고찰] 4개국에 대한 잔류농약 위해평가모델에 대해 분석하였다. 일본 위해평가 모델을 사용하지 않는 것으로 확인되었으며, 미국EPA 위해평가 모델과 유럽 위해평가 모델을 중점으로 비교하여, 각 모델에 대한 알고리즘에 대한 확인하였다. 실제 노출과 규제를 위한 모델로 구분되어 사용되며, 실제 노출평가를 위한 모델은 급·만성이 구별없이 사용되고 몬테카를로 방법으로 통한 확률론적 노출방법에 따라 계산된다. 또한, 국가, 기관별로 모델 내 적용되는 고정변수가 달라 결과가 차이나는 것을 확인하였다. 잔류농약 노출 및 위해평가를 위한 급성독성참고치(aRfD)를 4개국(CODEX, EU, 미국, 일본)의 보고서와 기관별 데이터베이스를 통해 DB구축하였다. 각국의 급성독성참고치(aRfD)의 값이 일부 상이한 부분이 있으며, 급성독성참고치(aRfD)가 설정되지 않은 농약도 확인 할 수 있었다. 사용목적에 따라 국가, 기관별로 모델에 따라 서로 다른 알고리즘과 변수의 차이가 있어 국내 실정에 적합한 급\성위해평가 체계가 필요할 것으로 판단된다.

        성인둔상환자에서 평균동맥압과 위해사건발생의 관련성: 단면 조사 연구

        Purpose: Non-invasive blood pressure measurement is widely used as a pre-hospital triage tool for blunt trauma patients. However, scant data exits for using the mean arterial pressure (MAP), compared to the systolic blood pressure, as a guiding index. The aim of this study was to determine the association between adverse outcomes and mean arterial pressure (MAP) and to exhibit the therapeutic range of the MAP in adult blunt trauma patients. Methods: The electronic medical records for all trauma patients in a single hospital from January 2010 to September 2012 were retrospectively reviewed. Patients below 17 years of age, patients with penetrating injuries, and patients with serious head trauma (injuries containing any skull fractures or any intracranial hemorrhages) were excluded. Adverse outcomes were defined as one of the following: death in the Emergency Department (ED), admission via operating theater, admission to the intensive care unit, transfer to another hospital for emergency surgery, or discharge as hopeless. Results: There were 14,537 patients who met entry criteria. Adverse outcomes occurred for MAPs in range from 90 to 120 mmHg. Adverse outcomes were found, after adjusting for confounding variables, to occur increasingly as the MAP declined below 90 mmHg or rose above 120 mmHg. Conclusion: Not only lower but also higher mean arterial pressure is associated with increased adverse outcomes in adult blunt trauma patients. Thus, patients with a MAP above 120 mmHg should be considered as a special group requiring higher medical attention, just as those with a MAP below 90 mmHg are.

        In general, when we look at the demand value of electricity use by time of buildings, it is concentrated in the day time as well as in certain seasons, which occurs an imbalance of demand. Ice Storage System among Heat Storage systems which is an improvement plan for demand management is the most space-efficient which is stored in Ice in the storage tank by using latent heat as well as sensible heat. This study aimed to confirm economical part such as reduction of running cost etc. through the detailed process of the work and the operation result of the installed system which is designed and constructed the ice storage system.

        일측 성대마비 환자에서 성대내전술 후 성대접촉율의 증가가 음질 개선에 미치는 영향

        Purpose : To assess perceptual, acoustic and aerodynamic measure of voice quality in patients with unilateral vocal cord paralysis before and after type I thyroplasty. Methods : The clinical records of patients operated type I thyroplasty in the Departement of otorhinoalryngolgy, Yongdong Severance hospital from November 2001 to November 2003 were reviewed. All patients uderwent a vocal function evaluation including perceptual, acoustic and aerodynamic measures of voice preoperative and on $60^{th}$ postoperative day. The perceptual and acoustic measures were obtained from recording of patients' reading a 'Sanchak' passage. The perceptual evaluation was performed by 2 speech pathologist using a 4-point rating scale. Acoustic parameters(voice range profile low(RAL), voice range profile high(RAH), average fundamental frequency(AFX), closed quotient, harmonic to noise ratio, jitter and shimmer) were investigated by Lx speech studio. Mean flow rate(MFR), subglottic pressure(Psub) and intensity were measured using the Phonatory function analyzer. The maximum phonation time was also measured. The data were statistically analyzed. A paired t-test (p<0.1) was used to compare preoperative and postoperative results. And multiple regression test was used to find which parameter was most correlated to improvement of postoperative voice quality. Results : Among aerodynamic parameters, Psub $(88.11mmH_2O{\rightarrow}58.7mmH_2O)$, MPT(7.87sec${\rightarrow}$12.53sec), MFR (359.8ml/sec${\rightarrow}$161.06ml/sec) were statistically improved. AFx(205.5Hz${\rightarrow}$163.27Hz), AQx(23.9%${\rightarrow}$48.3%), RAL, RAH. Jotter and shimmer were improved. In multiple regression test, AFx and AQx was noted as the two meost correlated parameters to improvement of postoperative breathiness. But general grade of voice quality was more correlated to Psub and shimmer. Conclusion : Vocal fold medialization procedures effectively reduce glottic gap. Increasing of contact area of both vocal folds induced improvement in aerodynamic parameters and leaded stabilizing of vocal fold vibration. That effect results in improvement in acoustic parameters (shimmer, jitter, signal-to-noise ratio, voice range profile) and voice quality.

        Robust pitch estimation is an important study in many areas of speech processing. In voice pathology, diverse statistics extracted form pitch were commonly used to test voice quality. In this study, we compared several established pitch detection algorithms (PDAs) for verification of adequacy of the PDAs. In the database of total pathological voices of 99 and normal voices of 3D, an analysis of errors related with pitch detection was evaluated between pathological and normal voices, or among the types of pathological voices such as benign vocal fold lesions; polyp, nodule, and cysts. Consequently, it is required to survey the severity of tested voice in order to obtain accurate pitch estimates.

        In voice pathology, various measurements calculated from pitch values are proposed to show voice quality. However, those measurements frequently seem to be inaccurate and unreliable because they are based on some wrong pitch values determined from pathological voice data. In order to solve the problem, we compared several pitch estimation methods to propose a better one in pathological voices. From the database of 99 pathological voice and 30 normal voice data, errors derived from pitch estimation were analyzed and compared between pathological and normal voice data or among the vowels produced by patients with benign vocal fold lesions. Results showed that gross pitch errors were observed in the cases of pathological voice data. From the types of pathological voices classified by the degree of aperiodicity in the speech signals, we found that pitch errors were closely related to the number of aperiodic segments. Also, the autocorrelation approach was found to be the most robust pitch estimation in the pathological voice data. It is desirable to conduct further research on the more severely pathological voice data in order to reduce pitch estimation errors.

        도마표면의 병원성미생물 제어를 위한 오존수 처리효과

        식품 및 식품접촉 표면의 식중독균을 제어하기 위해 오존수를 사용하는 사례가 증가하고 있다. 본 연구는 HDPE 도마와 나무 도마에 오염된 식중독균 제거를 위해 오존수 처리의 살균효과를 평가하였다. E. coli O157:H7, S. aureus, S. Typhimurium, B. cereus 모두 오염원 도마 표면을 오존수로 처리 시 농도 증가에 따른 1-5 log의 살균력이 관찰되었고, 세척보다는 침지 처리가 살균력이 높았다(p < 0.05). E. coli O157:H7, S. Typhimurium 등 그람 음성균이 S. aureus, B. cereus 등 그람양성균보다 오존수에 대한 저항성이 낮았으며, 나무도마보다 HDPE 도마에서 더 높은 살균력을 보였다. 본 연구 결과는 식품가공공장의 식품접촉 표면 식중독 미생물 제어에 효과적으로 활용될 것이다. The efficacy of ozone water in reducing food-borne pathogenic bacteria on High Density Polyethylene (HDPE) and wooden chopping boards in food industry was investigated in this study. 1-5 log reductions of E. coli O157:H7, S. aureus, S. Typhimurium, and B. cereus were observed with increasing concentrations of ozone water. The immersion treatment evidenced superior capability to inactivate food-borne pathogens than washing treatment. The Gram-negative bacteria, such as E. coli O157:H7 and S. Typhimurium, evidenced lower resistance against ozone water than was seen with the grampositive bacteria, which included S. aureus and B. cereus. The sterilizing effects of ozone water on HDPE chopping boards was superior to that on wooden boards. This result might be utilized to remove food-borne pathogens from food contact surfaces in the food industry.
