        Silastic을 이용한 내전 갑상성형술-적용 및 술기

        Unilateral vocal fold paralysis resulting in glottal incompetence can cause significant morbidity attributable to impaired speech, swallowing, and ability to protect the airway. The treatment of unilateral vocal cord paralysis has a long history, marked by technical innovations and improvements. These methods typically use endoscopic injection or implants to augment the volume of the affected vocal fold. The first known treatment, reported by Brunnings in 1911, was paraffin injection. The first thyroplasty medializing the paralysed vocal cord was performed by Payr in 1915 ; here, a cartilage door-flap was created from the thyroid ala to obtain better voice quality. In the 1970s, Isshiki systematized and developed the use of the external medialization by Payr. Later he modified his original technique, and achieved safer and better results. Many other methods were introduced for external medialization during the 1980s and 1990s. There has been couple of materials using for medialization laryngoplasty: silicone bloc, cartilage, goretex (polytetrafluoroethylene), titanium, etc. Among them, silicone bloc is the most popularly used material. Type I thyroplasty in combination with arytenoid adduction is a proven technique for medialization of the paralysed vocal fold. In this paper, personal experience for using silicone bloc type I thyroplasty : decision making and practical points, long-term results and complication of the procedure will be discussed.

      • [제3주제]열원 영양소 및 열원식품에 대한 이해

        인간의 기본적인 생명유지에 필요한 열원영양소는 주로 탄수화물과 지방질이며 부분적으로 단백질이 여기에 포함되어 활용된다. 이들은 다양한 열원식품의 섭취, 소화, 흡수 대사과정을 통하여 이용되어지고 있다. 본 주제에서는 광범위한 이 분야의 연구 업적 및 관련 현황을 개괄적으로 살펴보고 최근 관심사를 좀 더 깊숙이 논하고자 하였다. 열원영양소와 이에 관련된 열원식품섭취 양상은 많은 변화를 갖고 왔으며, 일부 열량과잉 현상이 나타나고 있으며 또 한편으로는 이들의 결핍현상이 주목되고 있다. 에너지에 대한 축적된 연구결과를 바탕으로 필요량이 산정되고 권장량이 제정되고 있으나 이에 대한 꾸준한 검토가 필요하다. 또한 에너지 과잉 섭취와 질병에 대한 지속적인 연구가 요망되고 있다. 지방질은 효과적인 열원식품이나 지방질 과산화 등 부정적인 문제점을 안고 있으며 이로 이한 공존 비타민 그리고 단백질 영양의 손실 기타 독성 문제를 유의할 필요가 있다. 또한 최근 필수지방산 그리고 eicosanoids가 강조되고 있으며 이들의 생물활성 연구와 함께 고도불포화 지방산의 생체 내 유리라디칼 생성과 억제에 대한 내용이 더 밝혀져야 할 부분이 있다. 또한 이들의 과다섭취와 trans지방산 영양에 대한 관심도 더 높여야 할 것 이다. 아직도 다양한 지방질의 섭취와 고지혈증 그리고 기타 질환과의 관계는 분명하지 않는 점이 많다. 한편, 당질은 가장 유용한 열원자원이며, 섭취면의 비율도 가장 높으나 앞으로의 섭취방향제시와 영양교육이 필요한 상황에 있다. 그리고 문제가 되고 있는 설탕의 과다섭취, 성인유당 과민성, 갈락토스혈증 등에 대하여서도 재인식할 필요가 있다. 단백질은 열량원으로서보다 다른 여러가지 역할이 매우 중요하며, 단백질과 아미노산의 소화, 흡수, 대사필요량과 결핍, 식품단백질의 영양가 변화에 대한 많은 연구결과가 있고 앞으로 더욱 이 분야의 연구가 요망된다. Lipid, carbohydrate, and protein are the most important nutients, which may be used for the energy source and other specific nutritional roles for human health. Even though undernutrition remains as a leading cause of mortality and morbidity in developing countries, the major nutritional problem in developed areas is related with excessive intake of enlarged fat, causing the disproportionate increase in metabolic diseases such as obesity, diabetes mellitus, hypertension, and hyperlipidemia. There is an increasing evidence that the information and approach used in establishing energy requirements were inadequate. Dietary fat has the highest caloric value among all nutrients. However, lipid oxidation is one of the serious problems in food deterioration, nutritional loss, and biological function. It has been suggested that free radicals from lipid oxidation process are involved in the protein interaction, loss of carotene and tocopherol, and cardiovascular diseases. A lot of basic researches on the biochemical reaction and biological role of essential fatty acids, prostaglandins and related eicosanoids have been accomplished with many unanswered questions. Carbohydrate is the main source of energy for human beings, supplying from 35 to 70 percent of total calories consumed. High intakes of sucrose may be the risk for dental caries, but is not a causative factor either for the cardiovascular disease or diabetes. Carbohydrate intolerlance is the result of heredity or acquired enzyme deficiencies which affect the related metabolism and digestion. On the other hand, dietary protein, essential amino acid and their requirments for human being are briefly reviewed and discussed, particularly in relation to their metabolism and utilization.

        편측성대마비환자에서의 성문면적파형(Glottal Area Waveform)의 정량적 측정

        Type Ⅰ thyuroplasty in conjunction with arytenoid adduction is one of the excellent techniques in the treatment of unilateral vocal fold paralysis. But perioperative objective evaluation of the patients is difficult. With the development of the videostroboscopy and image analysis program, we could quantify the Glottal Area Waveform(GAW) in patients with unilateral vocal fold paralysis and investigated the relationship between the glottal area and aerodynamic and acoustic parameters. Eight female patients who were performed type Ⅰ thyroplasty in conjunction with arytenoid adduction and 5 females with normal vocal function were involved in this study. Preoperative and postoperative videostroboscopy and vocal function study wire performed. GAW was analysed quantitatively with image analysis program (Kay Stroboscope Image analysis, KSIP) Peak Glottal Area(PGA), Baseline Offset(BO), and Closing Phase(CP) were increased in patients with unilateral vocal fold paralysis and they were reduced after the operation. Mean flow Rate (MFR) was well correlated with the PGA in normal control group and unilateral vocal fold paralysis patients. Noise to harmonic ratio(NHR) was correlated with PGA only in preoperative unilateral vocal fold paralysis patients. In conclusion quantitative measurement of the GAW is useful method in evaluation of unilateral vocal f31d paralysis patients.

        후두에 발생한 소낭낭포 1례

        Laryngeal saccular cyst may develop when obstruction of the orifice of the saccule occurs with resultant dilatation of saccule. It is classified into anterior saccular cyst and lateral saccular cyst. When saccular cyst occurs in infancy, it usually may cause respiratory distress, inspiratory stridor, cyanosis and dysphagia. In adults, it may cause hoarseness, dyspnea, dysphagia, pain and neck mass. The laryngeal saccular cyst may be treated by its removal with endoscopic or external approach, depending on the classification of saccular cysts, its size and individual patient factor. Recently, we have experienced a case of laryngeal saccular cyst so we report our clinical experience with brief review of literature.

        정상인과 후두폴립환자에서의 음성학적 측정

        Though normal vocal cords show regular vibration, pathologic vocal cords show irregularity between peaks. Jitter means fluctuation in the time interval between peaks, and Shimmer means cycle to cycle variation in the amplitude of the peaks. We investigated the vocal vibration of Korean normal persons objectively. The fundamental frequency, Jitter, Shimmer and SNR(signal to noise ratio) of normal persons were compared with that of vocal Polyp Patients with CSpeech Program for the possibility of distinguishing the pathologic vocal vibration from normal. The results were as follows ; Comparing the fundamental frequency of vocal Polyp Patients with normal persons, great change was noted only in female cases. But the Jitter and Shimmer of vocal polyp patients were greater than normal significantly in both male and female cases. SNR was lower than normal in vocal polyp patients. In the conclusion, fundamental frequency, Jitter, Shimmer and SNR might be meaningful parameters distinguisuing pathologic vibration from normal.
