      • A case of incontinentia pigmenti in a male

        Incontinentia pigmenti (IP) is a rare X-linked genodermatosis caused by a mutation in nuclear factor-κ B essential modulator (NEMO) gene located on Xq28. The condition is characterized by typical cutaneous lesions and abnormalities of the central nervous system and eyes. IP predominantly affects female infants (in excess of 37:1), and is usually lethal in males in utero. The survival of affected males is attributed to the presence of an extra X chromosome (Klinefelter’s syndrome), hypomorphic mutations, and somatic mosaicism. Herein, we report a male patient with IP as a rare case. A 5-day-old boy presented with linearly arranged, irregular shaped, erythematous papules and vesicles on the left leg along the blaschko line in a linear pattern. The boy was born after 36+6 weeks of dizygotic twin pregnancy as the second child of healthy parents with no family history of IP. His Apgar score was 4, weight 2830g, length 50cm. He had undergone immediate resuscitation for low Apgar score and had a seizure at the day 1 and 2. All neurologic, ophthalmologic, and dental examinations revealed normal results. The diagnosis was confirmed by histopathologic examination that showed eosinophils within intraepidermal, multiloculated vesicles. We diagnosed this case as incontinentia pigmenti. At 6 months of age, the vesicles and pustules resolved spontaneously with residual hyperpigmented streaks.

      • Establishment of Co-culture System to Enhance Milk Protein Expression in Mac-T Cells with Preadipocyte 3T3-L1 Cells

        Mammary gland development is critically dependent on the interaction between the stromal and the epithelial compartments within the gland. In this study, we established a co-culture of bovine mammary epithelial Mac-T cells and murine preadipocyte 3T3-L1 cells. Mac-T cells were co-cultured with 3T3-L1 cells for four days and production of milk proteins was induced for three days. After seven days of co-culturing, the number of alveolar-like colonies in the presence of 3T3-L1 cells was significantly higher than that in control group. Expression levels of αS1-casein and β-casein mRNAs were significantly increased by co-culturing with 3T3-L1 cells. In addition, casein protein production was significantly higher in the co-culture of Mac-T cells with 3T3-L1 cells than in control Mac-T cells. Substances that induced casein production in Mac-T cells also stimulated adipogenesis in 3T3-L1 cells. We suggest that a co-culture system of bovine mammary epithelial cells and preadipocyte cells is an efficient method for in vitro milk protein production.

      • Putative therapeutic mechanisms of simvastatin in the treatment of alopecia areata

        Background: Alopecia areata (AA), a common, relapsing hair-loss disorder, is considered to be a T-cell-mediated autoimmune disease with limited treatment options and success rate. Recently, simvastatin, a well-known treatment modality for hyperlipidemia and cardiovascular disorder, has been used for AA treatment. However, the putative therapeutic mechanism of simvastatin in AA has not been elucidated. Objectives: To investigate the therapeutic mechanism of the simvastatin treatment in AA. Methods: Outer root sheath (ORS) cells were pretreated with simvastatin and stimulated with poly(I:C) and IFNγ to induce inflammatory responses. And the expression and secretion of inflammatory cytokines were determined by qPCR and ELISA. Effect of simvastatin on signaling pathways such as NFκB and STAT3 were also examined. And the effect of simvastatin on hair growth was also determined using TOPflash assay and qPCR. Results: Simvastatin inhibited poly(I:C) and IFNγ-induced cytokine production in ORS cells and activation of NFκB and STAT3 signaling pathway by decreasing intracellular ROS levels. Moreover, simvastatin increased cell viability and transcriptional activation of β-catenin in ORS cells. Conclusion: These results suggest that simvastatin has an anti-inflammatory effect by inhibiting NFκB and STAT3 signaling pathway and also could stimulate hair growth by regulating β-catenin activation in ORS cells.

        Osteogenic Differentiation of Human Adipose-Derived Stem Cells(hADSCs) on a Dexamethasone Eluting Nanofiber Scaffolds

        Dexamethasone(DEX), a synthetic steroidal anti-inflammatory drug, is an efficient and dependable drug that induces osteogenic differentiation. The aim of this study is to fabricate the DEX loaded PLGA nanofibers by electrospinning method. Adipose-derived stem cells(ADSCs) were seeded into the nanofiber and the sustained release of DEX from PLGA nanofiber scaffolds promoted their osteogenic differentiation. The properties of DEX loaded PLGA nanofiber scaffold were characterized by scanning electron microscopy(SEM) and the release kinetics of DEX from PLGA nanofibers in vitro(1 h to 14 days) was evaluated by high performed liquid chromatography(HPLC). To evaluate the cellular response of the ADSCs seeded onto DEX loaded PLGA nanofiber, we performed F-actin, cytotoxicity, alkaline phosphatase activity, alizarin red S, von Kossa staining and immunocytochemistry assays for osteogenic differentiation. DEX loaded PLGA nanofiber scaffold was observed to have sustained release in vitro during experimental periods. The cytotoxicity test of the DEX loaded PLGA nanofiber scaffold indicated there was almost no-toxic effects in regards to proliferation and differentiation of adiposederived stem cells(ADSCs) as compared with a control. Alkaline phosphatase activity and alizarin red S were more significantly increased after 14days with increased DEX concentration of PLGA nanofiber scaffold than with PLGA nanofiber only. Also, von Kossa staining results confirmed a larger area of calcium deposition with increased DEX concentration of PLGA nanofiber scaffold. We observed that osteocalcin was also increased with increasing DEX concentration. These results demonstrate that DEX acts as an osteogenic inductive factor, and increased DEX concentration promoted more osteogenic differentiation of ADSCs. In conclusion, DEX loaded PLGA nanofiber would be valuable tool for bone tissue regeneration.

      • A case of granular cell tumor on the chest wall

        Granular cell tumor (GCT) is an uncommon tumor derived from Schwann cells, and usually presents in the oral cavity, especially on the tongue. When GCTs are located on the other body site, many tumors should be considered as differential diagnosis. Histopathological examination including S-100, CD68 immunohistochemistry is very useful for distinguishing GCTs from other tumors. In this report, we present a case of a gradually enlarging firm mass on the chest diagnosed as GCT withcharacteristic histopathological features, and this report would remind dermatologists of the differential diagnosis, immunohistochemical analysis and treatment of GCT. A 54-year-old woman presented with an 8-month history of a gradually enlarging mass on the chest, without associated symptoms. A physical examination revealed an asymptomatic, smooth-surfaced, well-circumscribed, oval-shaped, yellowish to brownish, firm mass on her chest, measuring 15 mm in diameter. Histologically, the dermis showed polygonal cells with abundant granular eosinophilic cytoplasm which showed positivity at S-100 and CD68 stains. As a result, the patient was diagnosed as GCT. Total excision was done, and no recurrence was observed during 3-month follow-up. Herein, we report a case of GCT on the chest, which is an unusual site of the tumor.

        8-(Tosylamino)quinoline inhibits tumour progression through targeting phosphoinositide-3-kinase/Akt pathway.

        <P>We examined whether 8-(tosylamino)quinoline (8-TQ), a structural analogue of BAY 11-7082, is able to modulate various tumourigenic responses using various in vitro and in vivo experimental conditions. 8-TQ exhibited the strongest suppressive activity on the proliferation of C6, A431, HeLa and MDA-MB-231 cells with IC550 values ranging from 10 to 30 microM. According to the analysis of level of active caspase-3, and morphologies of C6, HeLa and MDA-MB-231 cells, it was revealed that 8-TQ is able to induce apoptosis. Furthermore, this compound strongly diminished the invasion of MDA-MB-231 cells, the migration of HeLa cells, and the new generation of blood vessels under non-toxic conditions. Reduction of the phospho-form levels of intracellular signalling enzymes by 8-TQ strongly indicated that molecular signalling machineries composed of phosphoinositide 3-kinase (PI3K)/phosphoinositide-dependent kinase-1 (PDK1)/Akt and extracellular-signal-regulated kinase (ERK) could be targeted by 8-TQ treatment. Indeed, the specific inhibitors (LY294002 and U0126) of PI3K/PDK1/Akt and ERK showed similar anti-cancer properties to 8-TQ. Finally, 8-TQ intraperitoneally injected suppressed the increase of tumour volume up to 40% compared to vehicle-treated control. Taken together, our results clearly suggest that 8-TQ might have applications as a novel anti-cancer drug or may be served as a lead compound to be further optimized.</P>

The battery architecture incorporates a design feature that can be easily scaled up in size for use in large systems.         Prevalence and Clinical Characteristics of the Metabolic Syndrome in Middle-Aged Korean Adults

        Background : The aim of this study was to analyze the prevalence and clinical characteristic of the metabolic syndrome of adults, over 40 years old, living in Korea. Methods : This study was carried out for 2 years, 2003-2004, on total 5,330 individuals (2,197 men and 3,133 women) selected by the stratified random cluster sampling among adults over 40 years old. Metabolic syndrome was defined based on both the NCEP-ATP III criteria and Modified ATP III criteria applying the WHO-APR (Asian Pacific Region)`s abdominal obesity criteria (waist circumference > 90 cm in men, 80 cm in women) instead of NCEP-ATP III criteria. Results : Using NCEP-ATP III criteria, the age-adjusted overall prevalence of metabolic syndrome was 24.8% (17.6% in men, 30.0% in women). Age-adjusted overall prevalence of metabolic syndrome as defined by modified-ATP III criteria was 34.3% (26.3% in men, 40.1% in women). The prevalence of metabolic syndrome for each age group (40-49, 50-59, 60-69, ≥ 70) in men was as follows: 18.8%, 17.4%, 18.3%, 14.5%. In women: 22.3%, 32.7%, 39.9%, 39.3%. The prevalence of hypertriglyceridemia (triglycerides ≥ 1.7 mmol/l) was well correlated with the changing pattern of the prevalence of metabolic syndrome both in men and women. Conclusions : The peak age of metabolic syndrome in men was age 40 through 49, and the prevalence decreased with aging. Therefore, early intervention for risk factors of metabolic syndrome might be required in men. On the other hand, prevention for cardiovascular disease will be needed for perimenopausal women due to considerably increased prevalence in the age 50 through 59.

        High-resolution Manometry and Globus: Comparison of Globus, Gastroesophageal Reflux Disease and Normal Controls Using High-resolution Manometry

        Background/Aims Globus is a foreign body sense in the throat without dysphagia, odynophagia, esophageal motility disorders, or gastroesophageal reflux. The etiology is unclear. Previous studies suggested that increased upper esophageal sphincter pressure, gastroesophageal reflux and hypertonicity of esophageal body were possible etiologies. This study was to quantify the upper esophageal sphincter (UES) pressure, contractile front velocity (CFV), proximal contractile integral (PCI), distal contractile integral (DCI) and transition zone (TZ) in patient with globus gastroesophageal reflux disease (GERD) without globus, and normal controls to suggest the correlation of specific high-resolution manometry (HRM) findings and globus. Methods Fifty-seven globus patients, 24 GERD patients and 7 normal controls were studied with HRM since 2009. We reviewed the reports, and selected 5 swallowing plots suitable for analysis in each report, analyzed each individual plot with ManoView. The 5 parameters from each plot in 57 globus patients were compared with that of 24 GERD patients and 7 normal controls. Results There was no significant difference in the UES pressure, CFV, PCI and DCI. TZ (using 30 mmHg isobaric contour) in globus showed significant difference compared with normal controls and GERD patients. The median values of TZ were 4.26 cm (interquartile range [IQR], 2.30-5.85) in globus patients, 5.91 cm (IQR, 3.97-7.62) in GERD patients and 2.26 cm (IQR, 1.22-2.92) in normal controls (P = 0.001). Conclusions HRM analysis suggested that UES pressure, CFV, PCI and DCI were not associated with globus. Instead increased length of TZ may be correlated with globus. Further study comparing HRM results in globus patients within larger population needs to confirm their correlation. (J Neurogastroenterol Motil 2013;19:473-478)
