        운동교실 참가가 일본 중,고령자의 내장지방과 대사증후군 위험인자에 미치는 영향

        Metabolic syndrome is associated with increased risk of cardiovascular disease. Visceral obesity is closely associated with cardiovascular disease and the metabolic syndrome. We investigated the effect of health exercise program on the reduction in visceral fat and risk factors of metabolic syndrome in Japanese middle aged and old adults. Subjects were 41 living Japanese aged 57-71years(13males, 28females; 64±7years). Subjects participated in the health-exercise program that consists of 90min classes once in two weeks for six month. We assessed their clinical data and visceral fat measured by ultrasonography at baseline and then again after the health-exercise program. Visceral fat distance(from2.2±1.3cm to 1.9±1.0cm; p<.05)and metabolic syndrome risk factors(weight, waist circumference, BMI, systolic blood pressure, total cholesterol, LDL cholesterol and triglycerides; p<.05)were significantly decreased. These results suggest that an increased level of regular physical activity could reduction visceral fat mass and risk factors of the metabolic syndrome in middle-aged and old adults.

        여성 노인의 규칙적인 복합운동이 baPWV와 경동맥 혈류속도에 미치는 영향

        The purpose of this study was to investigate the effects of combined exercise training on common carotid artery blood flow velocities and brachial-ankle Pulse Wave Velocity (baPWV) in elderly women. Thirty eight elderly women (exercise group, n=20; control group n=18) were participated and their mean age was 66.4 ±4.5 years. Subjects participated in the combined exercise training which was consisted of 90min classes once in one weeks for six month. We assessed their systolic blood pressure (SBP), diastolic blood pressure (DBP), baPWV and Carotid artery blood flow velocities were measured at baseline and after the exercise training. SBP, DBP and baPWV (from 2.2±1.3cm to 1.9±1.0cm; p<.05) were significantly decreased in the exercise group compared to control group, respectively. Peak systolic velocity (PS) of common carotid artery(CCA) was significantly decreased in the exercise group than those of control group. These results suggest that regular combined exercise could reduce blood pressure, baPWV and carotid artery blood velocities in elderly women.

        소아 잠복 결핵 감염 진단에 있어서 투베르쿨린 피부반응 검사와 결핵 특이항원 자극 Interferon-γ 분비능 측정의 비교

        목 적 : 결핵 감염 관리에 있어서 잠복 결핵 감염을 찾아 내어 예방적 화학요법을 시행하는 것은 주요 핵심이다. 결핵에 노출된 소아에서 잠복 결핵 감염 진단을 위해 투베르쿨린 피부반응과 최근 널리 쓰이기 시작한 체외 검사법으로 결핵 특이항원 자극에 의한 T 세포의 $IFN-{\gamma}$ 분비능 측정 (QFT-G IT)이 있다. 잠복 결핵 감염 진단에 있어 두 검사법의 차이를 알아보고자 본 연구를 진행하였다. 방 법 : 2006년 10월 1일부터 2007년 4월 30일까지 6개월 간 총 111명의 소아를 대상으로 투베르쿨린 반응과 QFT-G IT 검사를 시행하였고, 동일 기간 내인 2개월 간 진단 된 성인 결핵 환자 29명에서 동일한 검사 결과를 후향적으로 알아보았다. 결 과 : 결핵 환자와의 가까운 접촉력을 주소로 내원한 무증상의 환자 33명 중 15% (5명)에서 QFT-G IT 검사 양성 소견을 보였고, 투베르쿨린 피부반응은 42% (14명)에서 양성결과를 보여 두 검사 간의 일치율은 낮았다(${\kappa}=0.39$). 성인 결핵 환자에서 QFT-G IT 양성율은 86.2%였고, 결핵균 배양 검사 양성율은 48.2%였다. 배양검사 양성이면서 QFT-G IT 음성이었던 환자는 12.5%였다. 결 론 : 소아에서 잠복 결핵 감염을 진단함에 있어 가족력을 근저에 둔 투베르쿨린 피부반응 검사를 시행하되 접촉력이 불분명하거나 지속적이지 않았을 경우 혹은 BCG 접종 효과로 의심되는 경우 결핵균 특이항원 자극 $IFN-{\gamma}$ 분비능 측정이 보조적 수단으로 사용 될 수 있을 지는 아직 확실하지 않으며 향후 수년 간 다 기관 연구가 필요하다. Purpose : Surveillance for detecting and managing latent tuberculosis infection (LTBI) is a key component of tuberculosis control. The classic surveillance tool, the tuberculin skin test (TST), may have some limitations when used in the Bacillus Calmette-$Gu{\acute{e}}rin$ (BCG)-vaccinated population. The object was to perform a blood test $QuantiFERON^{(R)}$-TB Gold In Tube (QFT-G IT) based on the detection of interferon-$\gamma$ ($IFN-{\gamma}$) released by T cells in response to Mycobacterium tuberculosis-specific antigens, and to compare the efficacy of this new diagnostic tool for LTBI with that of TST. Methods : For six months, between October 1, 2006 and April 30, 2007, data were collected from 111 patients under 15 years of age at Severance Children's Hospital. TST and QFT-G IT tests were performed with children with or without contact histories of tuberculosis. In addition to these tests, we examined comparative data from 29 adults who had tuberculosis, to detect false negative rates in the QFT-G IT method. Results : Thirty-three children had household contact histories. In this group, 15% and 42% of cases were found to be positive using the QFT-G IT assay and TST, respectively. Agreement was low between these two tests (${\kappa}=0.39$). In the adult active tuberculosis group, the QFT-G IT false negative rate defined as a positive culture and a negative QFT-G IT result was 12.5%. Conclusion : In diagnosing LTBI in children, the usefulness of a whole-blood $IFN-{\gamma}$ assay employing TB-specific antigens will be revealed only by examining additional longitudinal clinical data; this study serves as a starting point in that process.

        Rasch 모형을 활용한 U-19 축구 국가대표팀 선수들의 기능적 움직임 능력 검증

        본 연구는 기능적 움직임 검사(Functional Movement Screen: FMS)의 결과를 통해 U-19 축구선수 집단에 적용된 FMS 측정항목들의 적합성과 난이도를 분석하여 U-19 축구선수들의 기능적 움직임 특성 및 능력을 파악하고자 하였다. 파주 NFC U-19 축구 국가대표팀 선수들 30명(19.3±0.66세)의 FMS 7개 항목(Deep Squat: DS, Hurdle Step: HS, In-line Lunge: IL, Shoulder Mobility: SM, Active Straight Leg Raise: ASLR, Trunk Stability Push-Up: TSPU & Rotary Stability: RS)을 측정하였다. 그 결과를 바탕으로 Rasch 모형을 사용하여 선수들에 대한 측정항목의 적합성(goodness-of-fit)과 난이도(item difficulty)를 분석하였다. FMS측정 결과, 총 점수 평균은 10.02±1.79점이였으며, HS, SM, TSPU를 제외한 DS, RS, IL, ASLR가 적합한 측정항목으로 나타났다(0.5< x <1.5). 또한, 적합한 측정항목인 DS, IL, ASLR, RS의 난이도는 2.08, .26, -3.16, .83(logit)으로 나타났으며, 순서대로 난이도가 높은 것으로 밝혀졌다. 따라서 본 연구는 U-19 축구선수들의 기능적 움직임의 수준이 성인 선수들 보다 낮으며, FMS 측정항목들이 종목의 특성과 선수들의 연령에 따라 다른 결과를 나타낼 수 있음을 제시하고 있다. The primary purpose of the study was to identify the characteristics of Korean national youth soccer players’ functional movements. The secondary purpose was to examine whether certain tests of Functional Movement Screen (FMS) meaningfully achieve goodness-of-fit for the soccer-specific movements. Korean national youth soccer players (30 male players, 18.37 ± 0.67 yrs, 178.7±7.09 cm, 70.2±6.46 kg), performed FMS tests [deep squat (DS), hurdle step (HS), in-line lunge (IL), shoulder mobility (SM), active straight leg raise (ASLR), trunk stability push-up (TSP), and rotary stability (RS)]. The mean (±SD) FMS composite score and each test score were calculated. Rasch analysis, which was used to determine the goodness-of-fit for the tests, was applied to examine the item difficulty of the FMS tests. The mean FMS composite score was 10.2± 1.79; the mean DS, HS, IL, SM, ASLR, TSP, and RS score were 1.13±0.35, 1.27±0.45, 1.4±0.56, 1.6±0.77, 2.07±0.69, 1.43±0.82, and 1.3±0.47 respectively. According to the results of Rasch analysis, 4 tests (DS, IL, ASLR, and RS) were shown to be within the acceptable range (infit & outfit > 0.5 ~ < 1.5). The other 3 tests (HS, SM, and TSP) were shown to be out of acceptable range. The additional analysis revealed the DS (logit = 2.08) as the most difficult test and ASLR (logit = -3.16) the least. The results of the study showed that the players’ FMS composite score was lower (< 14) than the cut-off points used by previous studies for different athletes. The further study is warranted to examine the relationships between the scores of the tests appeared to be soccer-specific in the present study and the level of performance variables.
