Bojungbangdocktang (BJBDT), a formula of eight Oriental herbs, is a modified herbal prescription of Bangdoktang and Bojungbangamtang. Recently, BJBDT was demonstrated to inhibit angiogenesis induced by vascular endothelial growth factor in human umbilical vein endothelial cells, enhance hematopoiesis and protect cisplatin-induced cytotoxicity in normal MCF-10A breast cells. Nevertheless, there is no safety study of BJBDT before clinical trial so far. Thus, in the current study, we investigated the toxicity about ethanol-extracted BJBDT. Male and female Spraque Dawley (SD) rats were given orally by BJBDT at 250, 500, and 1000 mg/kg for 4 weeks. Mortality, clinical signs and measured change of body weight, food consumption and water consumption were observed. In addition, we performed ophthalmologic, urinary, hematological, blood serum biochemical and histopathological examination. Any general toxicity was not found in BJBDT treated group. Also, there were no significant differences in the parameters such as body weight, food consumption and water consumption, a lot of urine and blood factor levels except HCT, MCHC, Ca, TG, Glucose and T-Bilirubin level compared with control group. Although HCT was elevated and TG was decreased in male rats, and MCHC, Glucose and T-Bilirubin were elevated and Ca and HCT were decreased in female rats, these were within normal ranges. Finally, we determined that maximum tolerated dose (MTD) was 1000 mg/kg and no observed adverse effect level (NOAEL) was 500 mg/kg. Taken together, these results demonstrated that BJBDT is very safe to SD rats.

We previously demonstrated that the methylene chloride fraction of Scutellaria barbata suppessed human leukemic U937 cell proliferation by inducing apoptosis. In the present study, we have isolated luteolin from Scutellaria barbata and evaluated its apoptotic mechanism in Lewis lung carcinoma cells. Luteolin inhibited the proliferation of Lewis lung carcinoma cells in a concentration-dependent manner. Luteolin effectively increased the portion of $sub-G_1$ DNA content (apoptotic portion) and apoptotic Annexin-V positive cells in a concentration-dependent manner by FACS analysis. Caspase 9 and caspase 3 were activated and PARP was effectively cleaved by luteolin. It also increased the ratio of Bax to Bcl-2 through the decrease of Bcl-2 expression by Western blotting and reduced mitochondrial membrane potential following TMRE staining. These results suggest that luteolin can induce apoptosis through the mitochondrial mediated pathway.

목적 : 암환자에게 있어 통증은 삶의 질을 좌우하는 가장 중요한 증상이다. 현재의 통증 치료 방법 즉, 적절한 약물 치료 방사선, 마취, 신경차단 등의 방법으로 97%정도가 통증 조절이 가능함에도 불구하고, 여러 국내외 문헌에서는 암환자의 $60{\sim}70%$가 통증을 호소하고 있음을 보고하고 있다. 이에 이 연구에서는 의료진 측면에서 적절한 통증 치료의 장애가 되는 요인을 찾고, 진통제 및 암환자의 통증에 대한 잘못된 인식과 편견을 파악하여 보다 효과적인 교육프로그램을 개발하는 기초 조사를 마련하고자 한다. 방법 : 1998년 5월 1일부터 5월 31일까지 한달 동안 서울 시내에 있는 네개의 종합병원 내과에서 근무하고 있는 의사와 간호사를 대상으로 통증 관리에 대한 지식과 태도에 관한 도구를 수정한 32개 문항의 설문지를 통해 조사하였다. 설문의 내용은 통증 사정이 6문항, 진통제 투약이 5문항, 진통제 약물 작용이 8문항, 진통제 분류가 11문항, 그리고 진통제 투약 태도 2문항으로 이루어졌다. 결과 : 의사가 99명, 간호사가 152명이 참여하였으며, 전체 정답율은 의사가 71.6%, 간호사가 70.3%였다. 그러나, 그 중 진통제 분류에서는 80% 이상의 정답율을 보였으나 나머지에서는 대부분 $50{\sim}60%$정도의 정답율을 보이면서 통증 관리에 대한 낮은 지식 정도를 보여 주었다. 통증 사정에서는 간호사의 지식이 유의하게 높았으며, 진통제의 약리작용과 진통제 분류에서는 의사의 지식이 유의하게 높았다. 그러나, 전체 통증 관리에 대한 지식과 태도에 있어 의사 평균이 21.40이었으며, 간호사의 평균은 20.87로서 유의한 차이를 보이지 않았다. 결론 : 암환자 통증 관리에 대해 의료진의 지식 정도이 부족함이 나타났고, 이에 대한 적극적인 대안으로 의사와 간호사를 위한 교육프로그램 개발과 교육이 절실히 필요한데 일시적이고 일회적인 교육보다는 지속적이고 장기적이고 자동적으로 운영되는 체계가 필요하다. This survey was designed to evaluate knowledge and attitudes of physicians and nurses toward cancer pain management in South Korea and to compare physicians' knowledge and attitudes with nurses' Ninety-nine physicians and 152 nurses working at four major institutions in South Korea were included for the study. With the 30 items of the knowledge with true and false answers about cancer pain such as pain assessment(6 items), pharmacokinetics of opioids(8 items), analgesics classification(11 items), and drug administration(5 items), total score of knowledge answered by physicians was 21.40, which was not significantly higher than 20.87 answered by nurses. Rates of the correct answer were more than 70% in both physicians and nurses. Physicians were more knowledgeable in pharmacokinetics and analgesics classification than nurses, while nurses higher only in pain assessment than physicians. Since physicians and nurses could not effectively manage the cancer pain because of inappropriate knowledge, it is important to provide intensive education to physicians and nurses about cancer pain management.

The effects of bile salts (BS) on the stability of dioleoylphosphatidylethanolamine (DOPE) liposomes were investigated, observing apparent absorbance of vacant liposomes and calcein release from entrapped liposomes. Unilamellar liposomes were prepared by using a small quantity of palmitoly-immunoglobulin G(IgG) ($2.5{\times}10^{-4}$ mo1/lipid mol) to stabilize the bilayer phase of the unsaturated DOPE which by itself does not form stable liposomes. The destabilization of PE immunoliposomes by papain, clearly demonstrates that the IgG is essential for stabilization of PE bilayer. Approximately 4% of the entrapped calcein was released from the PE liposomes after 1 hr from liposome formation. Calcein release and absorbance of liposomes depended on the BS/lipid ratio because of the solubilization of lipid molecule in bilayer and the formation of mixed micelles. At very low BS concentrations, the incorporation of BS induced BS/lipid aggregates in the outer vesicles monolayer, while high BS concentrations, mixed micelles were formed. Chelate and its conjugates as $3{\alpha},\;7{\alpha},\;12{\alpha}-trihydroxy$ BS induce the concentration of the $3{\alpha}$, $12{\alpha}-dihydroxy$ BS at half-maximal solubilization of immunoliposomes to approximately 2.5-, or 5-fold. Conjugation of BS with glycine or taurine slightly enhanced their capacities to perturb membranes.
