        한약제제 무작위 대조군 연구에서 대조군 설계의 문제점과 대안

        Objectives : To discuss the types of control groups in randomized controlled trials (RCTs) of herbal medicine, and to provide suggestions for improving the design of control group in future clinical trials. Methods : We reviewed the 8 articles about clinical trial design of Chinese herbal preparation which were published from 2005 through 2008. We selected those articles from CNKI(中國知識基礎施設工程(http://www.cnki.net)). Results : It is necessary to have control group in randomized controlled trials(RCTs) of Korean herbal preparation. But there are problems in the selection of appropriate control group drug. This paper lists several problems about the choice of control drug and puts forward some proposals and countermeasures. There are problems such as ethics and manufacturing matching placebo and positive control herbal drug. Conclusion : To improve the quality of control group design, we introduce standard drug plus placebo drug method and add-on research for placebo control group design, double dummy technique, using negative control drug or composite control drug for active control group design.

        우울증 내담자의 가족치료에 관한 질적 메타분석연구

        Objectives: This study aimed to conduct a qualitative meta-analysis of domestic case studies on family therapy for clients with depression, focusing on the onset factors of depression, therapeutic interventions, and therapeutic outcomes. Methods: Thirteen Korean family therapy case studies were selected and analyzed using qualitative meta-analysis procedures. Through repeated comparative analysis, recurring patterns and structural themes were identified to provide an integrative understanding of the processes and outcomes of family therapy. Results: The analysis revealed three major categories of onset factors: (1) intrapersonal and psychological characteristics, (2) family relationships and interaction patterns, and (3) external environmental stressors. Therapeutic interventions were organized into three phases: (1) establishing a therapeutic relationship and preparing for intervention, (2) addressing problems and facilitating change, and (3) maintaining change and preparing for termination. The therapeutic outcomes were observed at two levels: systemic changes within the family, such as improved communication, boundary restructuring, and role readjustment; and individual changes in clients, including symptom reduction, cognitive restructuring, emotional independence, and personal growth. Conclusions: The findings indicate that family therapy functions not merely as a supplementary approach but as a central and effective intervention for depression. By demonstrating structured pathways of systemic and individual change, this study underscores the importance of incorporating family dynamics into clinical practice and provides foundational insights for future intervention strategies.

        패혈증이 동반된 아토피피부염 치험 1례

        Objective : It is well known that patients with atopic dermatitis are prone to cutaneous infections. Especially Staphylococcus aureus has been regarded as having an influence on atopic dermatitis. We observed and treated a case of an adult atopic dermatitis patient with sepsis by cooperation of east-west medicine. We present this case because there were no former reports of atopic dermatitis with sepsis treated by cooperation of east-west medicine. Method : We used herbal medication, acupuncture, herbal wet dressing combined with antibiotics and analgesics for 19 days. Atopic dermatitis grade was determined using SCORAD index. Result : The SCORAD index was lowered 32.3 at discharge than before of which the SCORAD index was 62.5 at admission. Other systemic inflammatory response such as fever, pus was also resolved. Conclusion : From this result, we suggest that herbal therapy associated with antibiotic therapy were shown to treat atopic dermatitis with secondary infection effectively.

        아토피피부염 환자들의 중증도별 치료 실태

        Objective : to investigate the modalities and treatments with atopic dermatitis. Methods : We made up a questionnaire consisting of symptoms, birth, family history, previous treatments of atopic dermatitis. We interviewed mild to severe 60 atopic dermatitis patients with questionnaire, from August to October, 2008 who were participating in clinical trials of Kyunghee University East-west neo medical center. Results : The results obtained are summarized as follows: 1. 49(81.7%) out of 60 patients reported previous or current use of medical treatment, and most common type of medical treatment was topical corticosteroid (95.9%). Oral antihistamine (61.2%), oral corticosteroid (61.2%) were also commonly used. 2. 31(51.7%) out of 60 patients reported previous or current use of oriental medical treatment, and most common type of oriental medical treatment was herbal medicine(100%). Acupuncture (41.9%), Herbal external supplements (22.6%) were also used. 3. 36(60.11% ) out of 60 patients reported previous or current use of folk remedies, and the most common type of folk remedy was extract of tree(Mokcho-ak)(75.0%). Aloe(61.1%), Charcoal(22.2%) were also commonly used. Conclusion : It seems the frequency of folk remedies used were related to severity of AD. However, there were no significant difference between medical treatment and oriental medical treatment.

        매선침법을 이용한 비순구 주름 개선 5례

        Objective : Needle embedding therapy is a newly induced therapy which uses specialized tools. The purpose of this study is to report the nasolabial folds correction effect of needle embedding therapy on 5 cases. Method : Five women with no disease were received needle embedding therapy four to six times over one to two months. The photos of each patient were taken before and after the treatment. Also, Glogau photoaging classification were assessed before and after the treatment. Result and conclusion : Needle embedding therapy may be an effective and safe treatment for the correction of nasolabial folds.

        아토피피부염의 한의학 진료 지침 개발을 위한 사전 연구

        Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.

        서울 지역을 대상으로 모집된 성인 아토피피부염 환자의 삶의 질과 경제적 비용 부담에 관한 조사

        Objective : Atopic dermatitis (AD) may profoundly affect patient's quality of life (QOL), and also cause economic impact. The aim of our study was to evaluate the quality of life and the economic impact of adult atopic dermatitis patient in Seoul. We also evaluate the relationship of QOL and economic impact with severity of AD. Methods : 30 adult atopic dermatitis patient were included and evaluated by using the SCORAD Index and EASI. Patients were asked to fill in the questionnaires about their quality of life and financial costs during the past year by AD. Data about sleep disturbance and pruritus were also obtained. Pearson's correlation were used for statistical analysis. Results : 1. Among 30 patients, women were 19(63.3%), men were 11(36.7%). The mean age of the patients were 27.3 years old, patients between the ages of 17 and 30 years were 23, over 30 were seven. 2. The mean score of Objective SCORAD was $32.89{\pm}7.30$, Subjective SCORAD was $8.13{\pm}3.53$ and EASI was $9.15{\pm}6.90$이었다. 3. The mean score of Skindex-29 was $28.26{\pm}7.58$, DLQI was $10.17{\pm}5.55$. 4. By analyzing the questionnaire, a monthly average cost of 583,200 won for each patient was determined. Direct cost was 236,800 won and indirect cost was 346,300 won. 5. By analyzing the correlation between the severity of AD and QOL, Objective SCORAD and EASI were positively correlated with QOL(Skindex-29, DLQI) but not significant, meanwhile subjective SCORAD were significantly and positively correlated with QOL(Skindex-29, DLQI). 6. By analyzing the correlation between the severity of AD and economic impact, Objective SCORAD were significantly and positively correlated with direct cost and indirect cost. Also, EASI were significantly and positively correlated with direct cost and oriental medical hospital visits. Conclusions : The above results show that the QOL of the patients with atopic dermatitis is significantly related to their disease severity. Atopic dermatitis patients pay 583,200 won a month, and the economic impact of the patients is significantly related to their disease severity.
