Objectives: This study assessed the application of Oryung-san (Wuling-san or Gorei-san), a common diuretic in traditional medicine, as an antihypertensive agent. Methods: Experimental studies of the pharmacological properties of Oryung-san, including diuretic action and lowering effects on blood pressure, and toxicology and clinical trials were reviewed. Results: In pharmacology, various, relatively safe diuretics are used to lower blood pressure and are the oldest and most studied antihypertensive agents. Despite many new drug approvals, antihypertensive diuretics are frequently used in Korea and Japan where high levels of sodium uptake are common. Oryung-san has been demonstrated to have diuretic, antihypertensive, and nephroprotective effects. Conclusions: Oryung-san might be effective for lowering blood pressure based on the results of this literature review. Further evaluations and large-scale clinical trials of Oryung-san to treat primary hypertension are warranted.

고혈압 한의 진료 실태조사

Objectives: This study surveyed doctors of Korean medicine on how they treat hypertension in their Korean clinical practices. Methods: An invitation to take an Internet-based questionnaire was sent to doctors who were registered in the Association of Korean Medicine in September 2016. Participants were first asked general profile questions, which included age, gender, work experience as a practitioner, and specialties. Second, they were asked whether they had experience with hypertension treatment and additional questions related to hypertension. Results: A total of 991 replies were received to the questionnaire. Most of the Korean medical doctors (95%) had blood pressure units and had monitored blood pressure. A majority of the Korean medicine doctors (55%) had provided hypertension treatment. Acupuncture and herbal medicine were commonly used for the treatment of hypertensive patients. In addition, bloodletting, moxibustion, pharmacopuncture, cupping, Chuna, Qigong exercise, meditation, and Tai Chi were used. Many doctors also recommended lifestyle modification. The respondents said that Korean medicine improves general health conditions (42.6%) and has low adverse effects (29.9%). One of the most common opinions about the revitalization measures of Korean medicine was expanding health insurance benefits (20.8%). Conclusions: Through this survey, we found out information about the present situation of hypertension treatment in Korean medicine. In the future, it is possible that hypertension treatment using Korean medicine will be demanded and used by practitioners if it is covered by public health insurance.

Objectives : One of major complications of diabetes, diabetic ulcer is also one of the main reasons for amputation, and the prevalence rate is 4-10%. Laser therapy is widely used for leg ulcer and diabetic ulcer, and it is known to improve wound epithelialization, cellular content, and collagen deposition. The purpose of this study is to investigate the properties of the laser and the spectrum of laser instruments for diabetic ulcer. Methods : We performed literature search using the PubMed, Cochrane, CINAHL and Web of science for the data in English. In addition, other databases were checked for different languages such as OASIS and NDSL for the literature in Korean, CNKI in Chinese, and CiNii and J-STAGE written in Japanese. We excluded all review article and experimental studies, and only clinical studies using laser or light emitting diode (LED) for diabetic ulcer were selected. Results : A total twenty papers were selected. Different light sources were used as follows: LED, HeNe, InGaAlP, GaAlAs, GaAs, CO2, and KTP. The number of LED studies was 9, and HeNe laser was 7, and InGaAlP and GaAlAs laser was 2, GaAs, CO2, and KTP laser was 1 for each. Various energy density of the clinical study were reported. Conclusions : It is suggested that to select appropriate laser type and give the adequate output power to treat diabetic ulcer. Further evaluation and research for the condition of laser therapy to treat diabetic ulcers are warranted.

당뇨병성 궤양의 레이저치료에 대한 효과 : 체계적 문헌고찰

Objectives : The purpose of this study is to investigate the effect of laser therapy for diabetic ulcer by using methods of systematic review. Methods : In this review, PubMed, Cochrane library, Web of Science, CNKI, CiNii, J-STAGE, NDSL and OASIS were used as the search engines. The search period is from the start date of the search engine to October 3, 2016. Randomized controlled trials(RCTs) using laser therapy for diabetic ulcer were searched and extracted by two independent researchers. Risk of bias(RoB) of Cochrane was used to assess methodological quality of studies. Results : Finally, five RCTs were selected. The follow-up period ranged from 15 days to 20 weeks. InGaAlP laser, GaAlAs laser and light emitting diode(LED) were used to treat diabetic ulcer. The clinical trials used sham laser irradiation or standard treatment as control in comparison to laser therapy. The endpoints included ulcer size, rate of healing and time to healing with follow-up period. The RCTs demonstrated therapeutic outcomes with no adverse effect. Most items of RoB were unclear and methodological quality was low. Conclusions : Our analysis suggests that laser therapy has therapeutic effects for diabetic ulcer. However, more systematic and stringent clinical trials will be required.

일개 한의원의 고혈압 한의치료 118례에 대한 고찰

Objectives: The aim of this study was to report a 118-case series demonstrating the treatment of hypertension in a Korean medicine clinic and to evaluate the effect of Korean medicine. Materials & Methods: From 2006 to 2018, patients who visited a Korean medicine clinic for hypertension were investigated by studying changes in blood pressure before and after treatment with herbal medicine, acupuncture, and Uwhangchungsim-won. The average treatment period was 81.6 days. Results: After treatment with Korean medicine, the mean systolic blood pressure (SBP) and diastolic blood pressure (DBP) of all patients decreased significantly (p<0.001). The SBP decreased from $148.9{\pm}10.3$ to $133.8{\pm}13.9$, and the DBP decreased from $91.3{\pm}7.5$ to $82.5{\pm}9.0$. After treatment with acupuncture or Uwhangchungsim-won, the patient showed decreased blood pressure but the differences were not statistically significant when compared with the non-treated group. Conclusions: This study shows the real situation of hypertension treatment in Korean medicine and indicates that Korean medicine could be one of the primary treatment for hypertension. However, this study had limitations, such as variations in the treatment periods, the frequency of acupuncture treatments, and the type of antihypertensive drugs. For further evaluation of the effectiveness of treatment for hypertension using Korean medicine, a well-designed study should be undertaken.

유비쿼터스 환경이 구축되고 사용자의 현재 주변 상황에 적합한 서비스를 제공하기 위해서는 실외 환경에서의 위치 기반 서비스뿐만 아니라 실내 위치 기반 서비스가 필요하다. 이를 위해 본 논문에서는 실내 환경에서 위치를 효과적으로 측위 할 수 있는 시스템을 개발하였고, 위치측위에 대한 정확도를 보정하기 위해 칼만필터와 확장칼만필터 보정알고리즘을 적용하여 이 두 방법에 대한 성능평가를 수행하여 정확도가 향상됨을 검증하였다.

More than 20 natural public parks including Mt.Buk-han, Mt.Kwan-ak, shall change into public park or natural public park zone by the Law on urban park and green space. Recent survey of park visitors showed that 23% of users wanted facility addition and liked the concept of theme park. Seoul has a total of 2,464 parks as of January 2009 and 29 parks among them are theme parks. However, the existing theme parks fall short of meeting the need of public expectation and most of them are of a sort that are showy amusement parks. There is nothing unique or with concrete theme but function disorders and restrictions such as difficult access, low awareness, and steep slants. Taking into consideration the institutional change from the existing natural public parks to public parks and leisure demand, this study analyzes 20 potential theme parks out of 82 projected areas regarding to renovation public parks in major mountains, suggests detailed park types and effective using plan on site. 1. Feasibility study on building theme park regarding to renovation public parks in major mountains This study performed physical evaluation and functional evaluation for theme parks. Physical evaluation criteria are size, topography, ecology / environmental value, accessibility of the park while functional evaluation criteria are mainly statuses of theme resources. According to the evaluation result, eco parks, cemetery parks, and amusement parks were influenced by size and in particular, eco parks were greatly influenced by high environmental value. Functional evaluation showed that theme parks, especially history parks were heavily affected by location of theme resources. Super size and large scale parks turned out to be adequate for eco park and parks with high accessibility for culture park. 2. Study on Building Theme Parks and encouraging their use History park is a gateway to a nation`s historical resources thus it can be designed to provide learning, education, experience on historical resources. Its tangible theme resources and intangible cultural resources are critical for cultural activity. Eco park can be designed for 5 functions: to provide Biotope, natural trail, and information/commentary, protect natural habitat, and facilitate research activities and dissemination on nature restoration. Theme parks should have diverse and clear themes to meet user needs providing incentives for visitors to participate such diverse programs. Also, well-trained volunteers from a nearby community center or civic groups can be a brilliant way to utilize applicable support. 3. Policy Directions Before building a new one, you need to understand regional characteristics of the existing park types and projected site. Future demands of the park also need to be predicted to choose an adequate theme for the park and promote use of the park. This facelift will help people in the local community experience varied theme parks and also contribute an improvement to the community. For a medium to large scale park, it is necessary to group varied themes and offer a broad variety of experiences of entertainment, play, and relaxation. When it comes to a park, green belt must come first rather than facilities installation. Most of all tranquil relaxation and dynamic trail with the view of forests and meadow is the base for a park. For this, modification of the relevant laws and regulations is a must since mandatory facilities rate often hinders promotion of theme park in selecting the existing park type.

서울시 용도복합 현상의 측정 및 기술에 관한 연구

본 연구를 통해 정의된 용도복합 개념은 세 가지 이상의 주요 도시기능의 복합을 의미하며 도시 공간의 경제적, 사회적, 환경적 가치를 동시에 실현하기 위한 수단을 의미한다. 이러한 정의 하에서 서울의 용도복합 현상을 탐구하였다. 상대적 엔트로피 개념을 사용하여 단위 지역인 법정동을 기준으로 복합도(R.E.)를 측정하였으며 도시기능의 유형은 용도복합의 정의에서 도출한 개념을 통해 건축물 층별 용도를 유형화하여 계량화하였다. 서울의 용도복합 현상은 복합도(R.E.) 0.631 이하를 ‘복합아님’, 복합도(R.E.) 0.631 초과 0.884 미만을 ‘불균형 복합’, 복합도(R.E.) 0.884 이상 0.991 미만을 ‘균형복합’, 복합도(R.E.) 0.991 이상을‘완전균형복합’의 단계로 구분하여 용도복합 현상을 해석하는 기준으로 지표화하였다. The mixed-use concept definition, this study reveals, is that the mixing three or more major types of urban uses implements for economical, social and environmental values in our urban space. With this definition the study explores Seoul’s mixed-use phenomenon. The quantification method, the study uses, is the relative entropy which calculate the balance of each urban use in a certain area. The relative entropy method, also known as the LUM(land-use mix score), uses three urban-use categories which is derived from the mixed-use concept definition. Hundreds of building-use types in the building regulations are categorized and calculate the LUM of Seoul’s legal-status neighborhoods. The result interpreted as the criteria of Seoul’s mixed-use phenomenon and categorize mixed land-use status in a certain value:‘non mixed-use’ category has a value 0.631 and below,‘unbalanced mixed-use’category has a value between 0.631 and 0.884,‘balanced mixed-use’category has a value between 0.884 and 0.991 and‘complete mixed-use’category has a value 0.991 and over.
