상구암종의 외과적 치료 -2례 보고-

Pancoast tumor was specific lung carcinoma that has been symptoms and signs according to locations. It was located in peripheral,and involved the extrathoracic structures more than parenchyme of the lung. At 1838, Hare reported it, and at 1932 Pancoast was first described it. Prior to 1950,superior sulcus tumor was considered uniformly fatal, but at 1961 Paulson and Shaw advocated the use of preoperative irradiation therapy and followed by an extended en bloc resection. Recently we were experienced 2 cases of pancoast tumor managed with same method. One was 60-years old man that has been recommended preoperative radiation therapy with dose of 3000 cGy to 20 fractions and followed resection after 4 weeks, the other was 53-years old man that has been recommended a dose of 4000 cGy to 20 fractions and followed resection after 4 weeks. On tumor histology first case was large cell carcinoma and second case was squamous cell carcinoma. all patients was complicated atelectasis. First patient was expired with brain metastasis after 17 months, second was expired after 6 months.

골절 후에 발생한 Checkrein 변형

Checkrein deformity has dynamic characteristics in which the degree of extension contracture of the metatarsophalangeal joint and flexion contracture of the interphalangeal joint change according to the movement of the ankle joint. Although the primary lesion is the flexor hallucis longus, several clinical features exist because of the accessory connection with the flexor tendon of other toes. After a physical diagnosis, a radiological examination should be performed to determine the cause and location of adhesion. Moreover, it is vital to determine if it is direct adhesion to the tendon tissue or muscle contracture due to ischemic muscle damage. Although there are no clear guidelines for surgical treatment, it can be divided broadly into two methods: soft tissue release and Z-plasty performed through direct access to the lesion site or indirect access through the tarsal tunnel or medial midfoot approach. Direct tendon tissue release surgery should be attempted if the tendon tissue is locally attached to the fracture callus or specific soft tissue. On the other hand, operation on the lesion site should be performed first if the checkrein deformity occurred due to an implant or bone fragments, followed by release surgery. If muscle contracture and movement are limited due to ischemic damage, surgery should be performed to remove adhesions and additional tendon connections around the flexor hallucis longus and digitorum longus by approaching through the tarsal canal and the medial side of the midfoot. The fixed contractures of the metatarsophalangeal and interphalangeal joints should be addressed if the limitations of tendon excursion are identified despite the release techniques.

하지 만성 허혈에 대한 동맥 우회술의 임상적 고찰

Arterial bypass for the chronic ischemia of the lower extremities underlying atherosclerotic obliterans has been performed with a number of alternative conduits from 1941 by Kunlin. It is indicated for the limb salvage of patients with threatened limb loss despite of several controversies in surgical treatment of atherosclerotic obliterans. From March 1991 to January 1995, 26 arterial bypasses were performed in 23 patients with the chronic ischemia of the lower extremities in our hospital. Their mean follow up period is 18.9 months ranging from 4 months to 44 months. Mean age is 60.9 years ranging 47 years to 76 years and the most prevalent incidence is the 6th decade. 21 patients are male and 2 patients are female. 19 of 23 patients are smokers. Clinical classifications by Fontaine are class II[21.7% , class III[34.8% and class IV[43.5% .Diabetes mellitus[47.8% , hypertension[43.5% , hyperlipid-emia[26% , tuberculosis[21.7% , cerebrovascular accident[13.0% and cardiac diseases[8.7% are associated. Aorto-single femoral bypass in 4 cases, aorto-bifemoral bypass in 5 cases, aortofemoral & femoropopliteal bypass in 2 cases, femoropopliteal bypass in 10 cases, popliteotibial bypass in 3 cases, femoropedal bypass[composite graft bypass in 2 cases were surgically approached. There are complicated early thrombosis in 4 cases those are required immediately reoperation, wound infection in 3 cases, hematoma in 3 cases, and so on. Postoperative complication rate is 53.8%.Postoperative patency rates are 84.6% at 6 months, 75.0% at 1 year, 70.0% at 2 years and 66.7% at 3 years. We usually used 6 mm & 8 mm graft for bypass, and the rate of thrombosis formation is 28.6%[2/7 in 6 mm graft and 12.5%[2/16 in 8 mm separately. In according to the graft materials, the rate of thrombosis formation is higher in the group using artificial graft than in that using autologous saphenous vein[16.6% vs 12.5% . Limb salvage rate is 76.9%. Postoperative mortality rate is zero %.

Tarsal coalition is an abnormal union between two or more bones of the hind- and mid-feet, which can occur at various rates from cartilaginous to osseous union. Talonavicular coalition is reported less frequently than calcaneonavicular or talocalcaneal coalition and has been associated with various abnormalities, including symphalangism, clinodactyly, ray anomaly, clubfoot, other tarsal coalitions, and a ball-and-socket ankle joint. Patients with talonavicular coalitions are usually asymptomatic and rarely require surgical treatment. We review the literature and report on a case of 59-year-old male patient with talonavicular coalition.

This paper presents a new numerical method to analyse tensegrity structures by using singular value decomposition and force method. The tensegrity system consisting of compressive and tensle elements are pin-jointed system. Tensegrity structures, unlike the general structure should be preceded by form-finding. Tensegrity structures form-finding of the self-equilibrium stress stability, seeking to have the process. In this study, tensegrity structures when subjected to external loads, find the optimal pre-stress values was studied.

대동맥 질환 수술의 임상적 고찰

From January 1991 to January 1995, 11 patients with aortic diseases underwent various surgical repairs. The age at operation ranged from 26 years to 63 years[ mean=50.9 years . The disease entities included 8 aortic dissections[ type I in 4, type II in 2 and type III in 2 cases , 2 Marfan`s syndrome with annuloaortic ectasia and 1 desecending thoracic aortic aneurysm The operative procedures we tried were 3 Bentall`s operation, 5 graft replacement of ascending aorta, and 3 graft interposition in descending thoracic aorta.Overall hospital mortality rate is 36.3%[4/11 . And causes of death are pump weaning failure in 2 cases and multiorgan failure in 2 cases. It was that 2 sternal dehiscence & mediastinitis, 1 acute renal failure, 2 hypoxic brain damages and 2 postoperative psychosis were complicated. Recently we tried surgical repair of aortic dissection five out of 6 cases using total circulatory arrest with deep hypothermia at 14$^{\circ}C$. Total circulatory arrest time ranged from 18 to 26 minutes[ mean 22.2 minutes , and mean aortic cross-clamping time was 48.2 minutes. One of 5 patient died on the 7th postoperative day due to multiorgan failure. Mortality of patients with TCA was 20%[1/5 , and it of remainders was 50%[3/6 . Our result for surgical repair using total circulatory arrest with deep hypothermia is satisfactory on the basis of our clinical data.

한정 생산된 원인과 처리 유창성이 프리미엄 가격 제품의 구매 의도에 미치는 효과: 가격-품질 휴리스틱과 노력 휴리스틱을 중심으로

Premium pricing is based on inferring high quality from the high price of a product, and often emphasizes the scarcity of the product. This study focused on verifying the effect of the production causes of limited products on the purchase intention of premium-priced products through a series of experiments. First, Experiment 1 classified limited production causes into scarce materials (i.e., material conditions) and difficult production processes (i.e., process conditions), and verified the effect of the level of fluency in processing advertisement messages (low vs. high) on purchase intention. As a result of Experiment 1, scarce material conditions had higher purchase intention than process conditions only under conditions with high treatment fluency. This can be seen as a result of the fact that price-quality heuristics are more easily induced when the processing fluency is high, and the material is more intuitive than the process. However, in the case of process conditions, the low processing fluency conditions had higher purchase intention than the high conditions. This is because the lower the fluency, the more higher the thinking is required in the process, which may be due to the effect of inferring the effort of the producer with high quality. Experiment 2 was conducted to verify this. As a result of dividing the process conditions into complexity (i.e., feasibility) and effort (desirability), the effort conditions had higher quality perception and higher purchase intention than complexity conditions. The implications of this study are as follows. First, the processing fluency of advertising messages that emphasize the scarcity of materials should be high. Second, a message that emphasizes the effort and sincerity spent in the production process is more effective than the difficulty of the production process. 고급화 가격 전략은 제품의 비싼 가격으로부터 높은 품질을 추론하는 것에 기초하며, 제품의 희소성을 강조하는 경우가 많다. 본 연구는 제품이 한정 생산된 원인이 프리미엄 가격 제품의 구매 의도에 미치는 효과를 일련의 실험으로 검증하는 데 초점을 두었다. 먼저, 실험 1에서는 한정 생산된 원인을 희소한 재료에 의한 것(즉, 재료 조건)과 생산공정의 어려움에 의한 것(즉, 공정 조건)으로 구분하고, 각 유형에 따른 광고 메시지의 처리 유창성 수준(저 vs. 고)이 구매 의도에 미치는 효과를 검증하였다. 실험 1의 결과, 처리 유창성이 높은 조건에서만 희소한 재료 조건이 공정 조건보다 구매 의도가 높았다. 이것은 처리 유창성이 높을 때 가격-품질 휴리스틱이 더 쉽게 유발되며, 이때 재료가 공정보다 더 직관적이기 때문에 나타난 결과로 볼 수 있다. 하지만 공정 조건의 경우, 오히려 낮은 처리 유창성 조건이 높은 조건보다 구매 의도가 높았다. 왜냐하면 공정은 유창성이 낮을수록 고차적인 사고를 필요로 하는데, 이 과정에서 생산자가 들인 노력을 높은 품질로 추론하는 효과 때문일 수 있다. 이를 검증하기 위한 실험 2를 진행하였다. 공정 조건을 복잡성(즉, 실행가능성)과 노력(바람직성)으로 구분하여 처치한 결과, 노력 조건이 복잡성 조건보다 품질지각이 높고 구매 의도도 높았다. 이 연구가 시사하는 바는 다음과 같다. 첫째, 재료의 희소성을 강조하는 광고 메시지의 처리 유창성은 높아야 한다. 둘째, 제조공정의 어려움보다 제조공정에 들인 노력과 정성을 강조하는 메시지가 더 효과적이다.

음주운전 처벌 강화법 실시전후에 따른 교통사고 발생양상변화

In order to compare the characteristics of traffic accident trends before and after the amendment of The Road Traffic Law, the author analyzed 541 accident victims for one month before and after the amendment of The Road Traffic Law. The findings of study were summerized as follow: 1. Traffic accidents were decreased 36.2% after amendment of The Road Traffic Law than before, which were decreased highly in the low education group and the drive career over ten years group. 2. Traffic accidents were most frequent during the time 12:00-20:00. For the comparison between before and after amendment of The Road Traffic Law, during the time between 20:00-24:00, the traffic accident was decreased with 54.3%, but during the time between 24:00-04:00, the traffic accident was increased with 70%. 3. Traffic accidents by the motorcar was most frequent, but the rate by bus and microbus was highly decreased after the amendment of traffic law. For the cause of accidents, unobeyed safety driving was most frequent cause, but that was decreased conspicuously among alcohol drinking driving and over speed driving. 4. Most patients were injured mildly, and traffic accident rate of it was far lower than injured severely or death after the amendment of the traffic law. Most traffic accidents were sprain and contusion. Fractures were decreased the highest rate after the amendment of the Road Traffic Law. The most patients was treated for 2 weeks, but treated over 4 weeks was decreased least. 5. For the state of traffic accidents, severe injuries, head traumas and fractures were higher rate among drinking driving, but mild injuries, sprains and contusions were higher rate among nondrinking driving. The duration of treatment was short for the accidents by non-drinking drive, but that was long among by the drinking drive.

원발성 우심방 횡문 근육종 -1례 보고-

Primary cardiac tumors are extremely rare and about 20-25% of primary cardiac tumors are malignant. Recently we have experienced a case of primary cardiac rhabdomyosarcoma originated on the right atrial wall. Patient was 41 years-old women and chief complaints were pitting edema and exertional dyspnea. Emergency operation was performed to relieve symptoms by pericardial effusion that hac been seen on the echocardiogram and large soft mass in right atrial cavity on the MRI. About 700ml amounts of bloody pericardial effusion was collected into the pericardial cavity. 4X5X5 cm sized ingrowing soft mass in the right atrial cavity and two small sized outgrowing masses were inspected. A tumor was invaded into the SA node and superior limbic portion of fossa ovalis. And then mass & right atrial free wall was resected but areas of SA node and fossa ovalis were not resected. Defect of right atrial wall was reconstruced with bovine patch. She was discharged after 2 weeks without any problems and she has been treated with chemotherapy.
