        건축분야 임대형 민자사업 협상단계 쟁점 분석에 관한 연구

        본 연구는 BTL사업 실시협약을 위한 협상단계에서의 쟁점이 되는 항목의 분석 및 그 발생 원인을 찾아보고 해결방안을 제시하고자 하였다. 이에 기 수행되었거나 또는 수행중인 협상에서 논의되고 쟁점화 되는 항목에 관하여 주무관청과 사업참여자를 대상으로 협상참여 주체들이 인식하고 있는 쟁점화정도를 분석하였다. 또한 협상단계 각 분야별 쟁점항목들의 계층 분석방법을 통한 쌍대비교를 통해 가중치를 산정하고 그 쟁점화정도를 정량적으로 도출하였다. 최종적으로 1차 계층의 6개 협상분야 중 4개 분야에 대한 발생원인 각 2개, 2개 분야에 대한 발생원인 각 1개를 도출한 후 그에 대한 해결방안을 제시하였다. This study aimed to find major issues on negotiations of the BTL project, to analysis the degree, and to investigate the underlying cause and propose solutions. Data were collected from authorities in charge and the companies which have participated in BTL projects. Also, the weights of major issues were analyzed using Analytic Hierarchy Process method(AHP), which uses pair-wise comparison between variables, and the degree of them were listed in the quantitative way. As a result, 4 part had each 2 reasons and others had one reason among 6 negotiation parts of first step. In addition, the solutions were indicated.

        한약의 약물상호작용 기전과 연구방향 - 독성학적인 측면을 중심으로 -

        Objectives : The mechanisms for korean traditional medicine-drug interaction has not been well reviewed in spite that the chance for co-administration with western drugs or diet supplements has been increased. Especially, it is well known that various cytochrome P450s play a major role in drug-drug interaction. Of course, Korean traditional medicines is not excluded in a view of metabolism or biotransformation by cytochrome P450. This article was focused on reviewing the possible roles of cytochrome P450 in Korean traditional medicine-drug interaction, Also, the directions for further studies were suggested in terms of Korean traditional medicine-drug interaction. Methods : New studies for korean traditional medicine-drug interaction were reviewed and summarized in terms of cytochrome P450 activities by various Korean traditional medicines and western drugs. Results and Conclusions : Even if a few studies related to Korean traditional medicine-drug interactions was carried out, almost no studies for Korean traditional medicine-drug interactions has been found in a view of cytochrome P450. It was suggested that Korean traditional medicines and their decoction should be analyzed that how they effects on cytochrome P450, expecially CYP 1, 2, 3 families and how they interact with western drugs.

        독성대사체를 생성하는 다빈도사용 한약재의 안전성등급화 - 천궁, 당귀, 감초, 숙지황을 중심으로 -

        The new formular for herbal medicine-safety classification in terms of evidence-based medicine was developed and applied to evaluate various herbal medicines in the previous study. This study is aimed to evaluate the frequently-used herbal medicines inducing toxic metabolites or reactive intermediates(RI), such as Ligusticum wallichii Franch, Angelica sinensis, Glycyrrhizae Radix, Rehmanniae Radix, based on 6 safety grades calculated from human equivalent dose(HED)-based MOS(margin of safety). HED-based MOS can be explained as the ratio of theoretical ALD(approximate lethal dose) of human as $LD_1$(lethal dose of 1%)/ clinical maximum dose as $ED_{99}$(Effective dose of 99%). The herbal medicine showing the ratio less than 1 belongs to Class 1, but the herbal medicine showing the ratio more than 500 belongs to Class 6 with the lowest toxicity. As a result, they were evaluated as class 2 for Angelica sinensis and Glycyrrhizae Radix, class 3 for Ligusticum wallichii Franch and Rehmanniae Radix. These resultant grades for 4 herbal medicines were lower than the grade expected under consideration that these herbal medicines are used very frequently in oriental clinics. These low grades would be due to their ingredients which is biotransformed to toxic metabolites.

        엽산 투여에 의한 니코틴-유도 혈장 homocysteine 농도에 대한 영향과 금단증상 완화를 위한 한의학적 처방에 대한 고찰

        Homocysteine is a sulfur amino-acid produced during the metabolism of the essential amino acid methionine. Moderately increased plasma total homocysteine concentration have been implicated as a risk factor for occlusive vascular disease. Smoking is known to be one of the most significant factors leading to elevated plasma homocysteine concentration. However, the main component of a cigarette, nicotine has been not studied whether it is linked directly to the increase of homocysteine concentration in blood. The metabolism of homocysteine is closely linked to that of its cofactors, folate. Here, the effects of nicotine and folic acid on amount of plasma homocysteine were studied. The concentration of homocysteine was increased by about 70% in rat plasma after nicotine treatment for one month. This increased concentration of homocysteine was reduced by about 60% at 6 hours later after folate treatment. Thus, nicotine should be directly involved in increasing the concentration of plasma homocysteine. Also it is suggested that these results can be and applied and used for controlling withdrawal symptoms after stopping smoking as one of oriental medicine formulas.

        당귀(Angelica sinensis)의 임상활용에 대한 효능 및 부작용연구

        Objectives : Angelica sinensis has been used extensively in Korean traditional medicine. Although thorough clinical trials are lacking, Various pharmacological actions for Angelica sinensis has been identified newly using animal models. In addition, it was reported that reactive intermediates, potentially causing toxic effects, was isolated from components in Angelica sinensis. In this article, it was purposed for explanation and introduction of new studies for Angelica sinensis. in terms of Pharmacological action and Toxicity. Methods : New studies for Angelica sinensis were reviewed and summarized in terms of pharmacological action and toxicity. Results and Conclusions : Angelica sinensis and its components including phthalide, phthalide dimers, polysaccharides, polyacetylenes, ligustilide와 butylidenephthalide as well as organic acid showed a variety of pharmacological actions on uterine, cardiovascular system and immune system. In addition, it was identified that three components of such as ligustilide, caffeic acid and safrole was biotransformed to reactive intermediates causing possibly toxicity.

        숙지황(Rehmannia glutinosa)의 약리작용과 독성

        Objectives : Rehmannia glutinosa has been used extensively in Korean traditional medicine. In addition, it was reported that reactive intermediates, potentially causing toxic effects, was isolated from one of components in Rehmannia glutinosa. In this article, it is purposed for explanation and introduction of new studies for Rehmannia glutinosa in terms of pharmacological action and toxicology. Methods : New studies for Rehmannia glutinosa were reviewed and summarized in terms of pharmacological action and toxicity. Results and Conclusions : Rhmannia glutinosa and its components including iridoids, saccharides, as well as amino acid, showed a variety of pharmacological actions on the blood system, immune system, endocrine system, cardiovascular system and the nervous system. In addition, it was identified that aucubin, one of major components of Rhmannia glutinosa was biotransformed to reactive intermediates by ${\beta}$-glycosidase and acid-hydrolysis, resulting in forming aucubigenin- albumin adduct. Even if a lot of new pharmacological actions has been identified, it should be considered for Rhmannia glutinosa to contain the material producing reactive intermediates which may induce the side effects.

        한약재의 대사 및 독성의 기전과 예방

        In recent years, there has been a globally increasing application of herbal medicines and dietary supplements to treat various chronic diseases and to promote health. However, there are increasing clinical reports on the organ toxicities associated with consumption of herbal medicines. In general, most xenobiotics are metabolized by Phase I reaction(the main enzyme : cytochrome P450) and Phase II reaction. However, reactive oxygen species, free radicals and electrophils are produced inevitably during xenobiotics metabolism. These toxic species and metabolites are increased whenever the endogenous substances and enzymes for Phase II reaction not available. In addition, herbal-drug interactions are pharmacokinetic, with most actually or theoretically affecting the metabolism of the affected product by way of the cytochrome P450 enzymes. This review updated the knowledge on metabolic activation of herbal components and its clinical and toxicological implications. Also, the possible way for preventing the side-effects by herbal-medicine use was suggested.

        한약의 안전성 등급화를 위한 evidence-based approach : Human equivalent dose-based the margin of safety

        This study was aimed to develop a new formula for herbal medicine-safety classification in terms of evidence-based medicine. Recently, human equivalent dose(HED)-based therapeutic index was developed for herbal medicine-safety classification by transforming $LD_{50}$ to HED. However, the use of the $ED_{50}$ and $LD_{50}$ to derive the therapeutic index may be misleading as to safety, depending on the slope of the dose-response curves for therapeutic and lethal effects. To overcome this deficiency, HED-based MOS(Margin of Safety)was developed and suggested in this study. The HED-based MOS developed by using $LD_1$, changing to ALD(approximate lethal dose), and $ED_{99}$. The HED-based MOS seems to be more useful and safer than HED-based therapeutic index since its values for several herbal medicines are basically two times less than the values from HED-based therapeutic index. Thus, HED-based MOS can be a good example of Evidence-based approach for herbal medicine-safety classification.

        천궁의 혈액순환과 항산화적 효과

        Ligusticum Chuanxiong and its components were reviewed in terms of pharmacodynamic mechanisms. Three components of about 40 chemical components in Chuanxiong tetramethylpyrazine, ferulinolol and Butylidenephthalide have mainly been studied for its pharmacodynamic mechanisms which are focused on the increase in blood flow and anti-oxidative stress. The mechanisms for the effects of Chuanxiong on the increase in blood flow can be summarized as four ways, 1) anti-coagulation 2) blocker of ${\beta}1$ adrenergic receptor 3) cellular control of Ca++ level 4) collagen synthesis. Chuanxiong also showed the effectiveness on free radical-induced injury. It seems that its effectiveness is also related to the mechanisms for the increase in blood flow.

        한약의 독물동태학적 특성

        Toxicokinetics of Korean Traditional Medicines(TKM) is the description of what rate TKM will enter the body and what happens to it once it is in the body in terms of toxicology. However, it is not easy to understand TKM toxicokinetics because of various factors such as a mixture of 2-30 kinds of herbal materials containing thousands of chemicals, and complex chemical properties. For these reasons, little is known about toxicokinetics of TKM. This study was aimed to characterize and review the absorption, distribution and metabolism of korean traditional medicines in a view of toxicokinetics. For this aim, some of korean traditional medicines were reviewed on a basis of drug-drug interaction, biotransformation and intestinal metabolisms by bacteria. As the factors affecting mainly on toxicokinetics of TKM, individual herbal material's degree of lipophilicity and metabolic rate, and decoction components according to different kinds of herbal materials were considered. Other factors such as intestinal pH and bacterial activity for metabolism affecting on TKM toxicokinetics, especially in small intestine. It would be a better way for improving the adverse or poor effects caused by TCM if the factors affecting on toxicokinetics of TKM is considered.
