        중(한)의사, 중(한)의의료기관 및 중(한)의학 관련 인식.태도 및 의료행태에 관한 연구 - 중국의 한족, 조선족과 한국인을 중심으로 -

        Over thousands of years oriental traditional medicine has developed a theoretical and practical approach to treat and prevent diseases and to promote people's health in China and Korea. In China, the integration of traditional Chinese medicine into the national healthcare system began in the late 1950s. This was in response to national planning needs to provide comprehensive healthcare services. On contrary to China, South Korea established the parallel operation of two independent medical systems in 1952. Hence there has been a political conflict between oriental and modern medicine over issues of fee, the ability to sell and prescribe herbal medicines, and the licensing of practitioners in traditional medicines. Given this background. This study is to compare peoples' attitudes and opinions for oriental traditional medicine by ethnicity (Chinese, Korean-Chinese and Korean). Chinese and Korean-Chinese were more used and satisfied with traditional medicine treatment and traditional practitioners compared with Koreans. The proportion of Koreans who reported the cost of traditional treatments was expensive was higher than those of Chinese and Korean Chinese. Most of Chinese, Korean-Chinese, and Koreans reported that they would use traditional medicine: 1) when they would have some disease to be treated best through traditional medicine; and 2) when traditional practitioner had a reputation and lots of experiences for those diseases. Most Korean people reported that oriental and western practitioners should cooperate each other to improve the quality of care. Therefore, policy framework including integration of traditional and western medicine, regulation, etc. is needed. In addition, research is needed to determine which diseases is treated best through traditional medicine.

        흰쥐에서 아급성 연독성에 대한 감두탕의 예방효과에 관한 연구(II) - 소변 및 혈액에 미치는 영향을 중심으로 -

        This study was performed to investigate the preventive effect of KDD against lead toxicity. KDD of 133, 266, 532 and 1,064 mg/kg were administered twice to the rats of Sprague-Dawley strain and then 300 mg/kg lead acetate was given to times, respectively. 1. The $\delta$-ALAD concentration in the urine showed 10.6 ~16.4 mg/kg in the control group indicated statistical significance for the experimental group II, III, IV, V (p<0.05). Also, the Coproporphyrin concentration had 0.119 ~ 0.226 $\mu$g/ml in the control group indicated statiscial significance for the experimental group V of 10 weeks (p<0.05). 2. The $\delta$-ALAD concentration in the blood showed 13.28 ~ 16.08 ALAD unit in the control group indicated statistical significance for the experimental group I (Pb 300 mg/kg) of 6 and 8 weeks, for the experimental group III, IV of 8 and 10 weeks, and for the experimental group V of 4 weeks (p<0.05). The $\delta$-ALAD concentration of experimental group I (Pb 300 mg/kg) group was inclined to decrease during the experiment period. The $\delta$-ALAD concentration of experimental group I (Pb 300 mg/kg) showed statistical significance for the experimental group II, III, IV, V of 6, 8 and 10 weeks. But, there was no statistical significance in the concentration change of hemoglobin, RBC, WBC, hematocrit, Ca, protein among the experimental groups. In conclusion, this study revealed the preventive effect of KDD against lead toxicity and its mechnism inferred to facilitate lead excretion in urinary following hinderance of lead absorption in the gastric-intestine and organs.

        전국 한방병원의 장기입원과 이에 영향을 미치는 요인 - 2014년 한방의료이용 및 한약소비실태조사(보건복지부)를 중심으로 -

        Objectives : It was to classify and its affecting factors to the patients of Korean medicine hospital with short term and long term hospitalization. Methods : I focused on long-term hospitalized patients. I was conducted on 344 hospitalized patients among the original data of usage and consumption of Korean medical report in 2014. Among those patients, I have classified them into long term inpatients(131 patients) and short term inpatients(213 patients) based on 16 days of hospitalization. Also multiple regression analysis was conducted to investigate the characteristics of the hospitalization, treatment satisfaction and dissatisfaction, the characteristics of long term hospitalization according to the sociodemographic of the subjects, the top 21 diseases and the distribution of human bodies, side effects and kinds of Korean medicine. Results : There was a statistically significant difference between the short term and long term hospitalized patients due to age, occupation, marital status, all 21 diseases and institutional fees, experience of Korean medical treatment due to traffic accidents. There was no significant difference in gender, education level, residence, income level, type of medical insurance, whether private insurance, type of medical treatment for Korean medicine, medical expenses for car accidents, reason for dissatisfaction with treatment. The number of long term patients at Korean medicine hospitals increased by a statistically significant by age in model 1 where confounding factors were differently controlled. In model 2, the number of long term patients at Korean medicine hospitals increased by a statistically significant by age, among those who earned 5,000,000 Korean won or more, and among those with nerve diseases. The number of long term patients at Korean medicine hospitals decreased by a statistically significant amount among the unemployed and others in model 2. In model 3, the number of long term patients at Korean medicine hospitals increased by a statistically significant by age, among those who earned 5,000,000 Korean won or more, and among those with nerve diseases, while the number decreased by a statistically significant amount among the married. Conclusions : These results suggest that the factors affecting the short term and long term hospitalization of patients with Korean medicine hospital are different from each other. Especially it was significant by age, over 5,000,000won Income per month, nerve disease, but decrease significant married.

        전향적 전후비교환자군과 다중회귀분석방법을 활용한 변증연구 - 117명 건선환자의 양허증과 혈열증를 중심으로 -

        Objectives: To study symptom differentiation for yang deficiency and blood heat based on 117 psoriasis patients. Methods: Obtained frequency analysis (%), mean (%), and OR, CL(P-value) with prospective pre-post intervention design and multiple regression analysis. Results: There were no statistically significant differences as to sex, BMI, smoking and marital status, but there were statistically significant differences in variables of average age, family history, and alcohol consumption (P<0.05). There were no statistically significant differences in psoriasis symptoms as to initial age of onset, morbidity span, area of the initial onset, and the progression of psoriasis during the last three months of each patient. However, the type of psoriasis showed a statistically significant difference (p=0.011). While there were no statistically significant differences as to common cold, condition of psoriasis after recovering from the cold, skin condition, exercise, and seasonality, irregular perspiration showed significant difference (p<0.00). When confounding factors have been controlled the blood heat patient group as the comparison group, multiple regression analysis showed OR, CI(95%) of 1.06(0.31-3.63) for men, 0.28(0.08-1.06) for aged 30 to 49,0 and 0.18(0.04-0.80) for aged 50 and older. it was 0.06(0.01-0.7) for family history, 1.06(0.29-3.88) for drink alcohol, 19.90(2.53-156.7) for seasonality, and 10.28 (3.19-33.11) for perspiration problems. In these variables, Sex, age, smoking, and alcohol consumption showed no statistically significant results, but family history(p=0.049), seasonality(p=0.005), and irregular perspiration (p=0.017) were statistically significant. Conclusion: Family history, seasonality and irregular perspiration are the determining factors for yang deficiency and blood heat in psoriasis.

        Current Korean medical system is evaluated as inefficient and the government is planning a new medical development plan to provide guaranteed life-long medical service and more efficient medical system Korean medicine also needs to participate as the primary medical provider and strengthen public recognition. The needs for active participation are essential to prevent irrational medical policies and Korean medicine can exercise medical merits. When the doctors of Korean medicine are recognized as the primary medical provider, not only social rights are served but also provide medical service to less privileged sector of the society. This calls for the establishment of system for attending physician and public health doctor from Korean medicine. Another important issue is the establishment of public medical service at the government level. Doctors of Korean medicine should be posted at the national health center and other public hospitals. Revision of the current policies on Korean medicine must be made to yield concrete outcome of the public Korean medicine service as well as cooperation between the conventional allopathic medical community and the Korean medical community

        한의사인력의 수급전망과 대책

        This paper estimated the demand and supply of physicians for oriental medicine for the period of $2009{\sim}2019$. Two equation models were used in the estimation of manpower. In 2004, the total number of physicians of oriental medicine was amounted to 13,662 registered and 10,532 available in clinical practice, respectively. According to estimates in the study, overall excess supply of physician manpower in oriental medicine was expected in the period, such as $5,300{\sim}5,700$ persons in 2009 and $900{\sim}1,700$ persons in 2019. However, the excess supply would be mitigated after 2019 mainly due to an increase in demand for oriental medical services. Specially, opening medical service market to overseas could be an exogenous variable in physician supply. An alternative manpower policy for oriental medical doctors is needed in a way of controlling oversupply.

        Metal problems in herbal medicine are not regulate properly by law and public management around world country until this time. General people belief as safety about herbal since natural material. And almost all persons can also purchase by their-self and use by self-prescription in reality. As this result herbal medicines can always occur acute and chronic toxicity by not proper use problems, side-effect and overdose. Heavy metal toxic diseases in historical view point was big accidents that didnot forget including minamata and itai-itai in Japan. These accident's teach to us must not use toxic metal level and not include or at least Pb Hg As Cd in all kind material use and intake by people, especially herbal. Herbal contamination research is beginner state that had not many papers until nowadays. Even if this pan had some papers, it had negative result and bigger and larger than problems level because of one way research trend of not many sample case-report and screening test of dried herbal form in chiefly. Many persons have afraid and risk thinking about herbal, animal and minerals since these cause. Further research related this subject will be needed at fact of epidemiology including case-control and cohort study for more precision research affecting in short and long term intake of oriental medicines

        생명현상의 다양성과 특징에 관한 고찰 - 보건의료에 미치는 영향을 중심으로 -

        This research was intended to delve into the diversity of life phenomenon and its characteristics. First of all, this research gave real examples to compare the differences in men's health, disease, and longevity in order to confirm the existence of diversity of life phenomenon. In addition, it also studied the process and mechanism of manifestation of life phenomenon, as well as the influence and problems of existing studies' results and implications. The results are as follow. 1. Differences in health, diseases, and longevity were very big and diverse in researches on different races, nations, ages, socioeconomic status, positions, and even (monozygotic) twins. 2. The basic foundation of all organisms is DNA, and environmental factors change DNA methylation and the structure of chromatin by constantly influencing DNA. Due to this, the manifestation, control, and phenotype of DNA change, resulting in diversified life phenomenon. Therefore, it is the environmental factors, not DNA, that has more influence on the diversity of life. 3. Looking at available studies, the most reasonable perspective on human requires focusing on the diversity of life phenomenon, holistic thinking, and reversible change instead of irreversible determinism. Considerable differences in life phenomenon between entities require a change in malformed perspective on life. Public health and medicine deals with live human beings, a more precise and accurate perspective on life is very important. Because management methods of health and disease, such as structure and approaches of medical research, prevention and cure, must be different by life perspectives.
