        In the HVDC Cable Accessory connection, there are a lot of space charges at the interface between the cable insulation and the connection part insulation, and at the interface between metals and polymers. It is also reported that insulation and accessory insulation have a large difference between the ratio of the dielectric constant and electrical conductivity of the cable and accessory insulation. And this space charge can strengthen and distort the local electric field near the interface. For that reason, the surface of SiC (Silicon Carbide), a semiconductive inorganic material based on EPDM (ethylene propylene diene terpolymer), was modified to strengthen the chemical reaction and interface. EPDM/Nano SiC/Micro SiC Mixture Composites were prepared and the properties of electrical insulation, electrical conductivity, and thermal conductivity were studied by mixing nano SiC and micro SiC.Moreover, in order to increase the mechanical tensile strength in the optimal state of electrical conduction and electrical insulation, the electrical and mechanical properties were studied by mixing Akitifit (Silica Kolinite), a surface-modified inorganic material.

        정신장애인 직업재활 사례연구

        본 사례연구는 사회복귀시설에 입소한 30세 남자 정신장애인을 대상으로 2010년 8월부터 10월까지 총 5회에 걸친 직업재활 사례 개입 연구이다. 본 사례연구의 목적은 직업재활 사례관리를 통해 직장 내 적응과 사회적응능력을 향상시켜 경제적으로 자립할 수 있도록 하며 이와 더불어 가족들의 보호부담을 줄이고 클라이언트의 직업에 대한 만족도를 높여 지속적으로 직업유지를 할 수 있도록 하는 것이다. 이를 위해서 다음과 같은 목표를 설정하여 5회에 걸쳐 개입하였다. 첫째, 클라이언트의 직업재활에 대한 욕구를 파악하여 적성에 맞는 직업을 탐색하였다. 둘째, 취업 전 직무지도를 통해 직무능력 습득 및 의욕을 강화하였다. 셋째, 취업 후 지속적인 직무지도를 통해 직무능력을 향상시켜 직업만족도를 높인다. 본 사례연구를 통해서 도출된 결론으로, 정신장애인의 직업재활을 위해 첫째, 직업재활에 대한 욕구를 파악하고 적성에 맞는 직업을 탐색하는 것이 매우 중요하다. 둘째, 사회복지사는 직무지도에 대한 전문성을 갖추어야 한다. 직무지도원의 전문성이 취업률, 직업유지에 영향을 주므로 전문성을 향상시킬 수 있는 현장중심의 교육이 필요하다. 셋째, 정신장애인의 직업적응능력을 향상을 위한 지원 프로그램이 필요하다. 개별상담, 사회기술훈련, 대인관계기술훈련, 직무역량강화프로그램 등이 필요하다. 넷째, 취업 후 사회복지사는 지속적인 직무지도를 통해 정신장애인의 직업적응능력을 향상시키고 직업만족도를 높여서 성공적인 직업유지를 할 수 있도록 노력해야 한다. 따라서 정신장애인의 성공적인 직업재활을 위해 지속적인 사례관리를 통한 직업유지와 자립생활에 대한 지원 계획도 필요함을 인식하게 되었다. The subjects of this case study are mentally injured men in their thirties who are housed in facilities intended for helping them to return to normal life in society, and this study looked at the facilities` vocational rehabilitation programs for a total of five times from August, 2010 till November of the same year. This study has its aim at coming up with ideas on effective vocational rehabilitation programs for the mentally injured people that can help them stand their own feet financially and that help them adapt themselves to their work places and their society despite their physical handicaps. And this study also aims to devise effective ways that can ease their family members` financial burdens and that can increase their satisfaction with their jobs in the hope that their satisfaction with their jobs will surely help them remain on payrolls of their companies. In order to study these, the following goals were established to look at the facilities` vocational rehabilitation programs for a total of five times. First, whether or not the facilities try to develop types of jobs that are suitable with the subjects` aptitudes after finding out their desires regarding their jobs. Second, whether or not the facilities educate them to perform their jobs better before they find employment, and develop any effective education programs that include how to improve their job performance skills and do anything to strengthen the subjects` will to perform their jobs successfully. Third, whether or not they implement any programs that can help the mentally injured people improve their job performance skills by offering job coaching programs continuously even after they find employment and what the facilities do to increase their satisfaction with their jobs. In conclusion, a continual management of cases regarding vocational rehabilitation programs for people with mental disability is absolutely needed and establishment of support programs intended for helping them stand on their own feet financially by helping them stay on their payrolls is also necessary.

        증례 : 순환기 ; 독사조신 복용 후 발생한 여성형유방증 1예

        Doxazosin은 알파아드레날린길항제의 대표적인 약으로 전립선비대증에 아주 흔하게 사용되고 있다. 비슷하게 사용되는 finasteride, dutasteride와 같은 5-알파환원효소억제제는 여성형유방증, 유방통과 같은 부작용이 흔하게 보고되고 있으나 알파아드레날린길항제는 보고된 바가 국내에 아직 없다. 또한 알파아드레날린길항제는 약리 기전으로 보아 그러한 부작용을 의심하는 것이 쉽지 않다. 이에 doxazosin 사용 후 일측성의 여성형유방증과 유방통이 발생한 1예를 경험하였기에 국내 첫 번째 사례로 보고하는 바이다. Doxazosin is an adrenergic alpha-1 receptor antagonist used to treat lower urinary tract symptoms that are common in prostatic hyperplasia. To our knowledge, few cases of gynecomastia and mastodynia, as a complication of adrenergic alpha-1 receptor antagonist, have been reported to date; no cases have been reported in Korea. We describe a case involving a 78-year-old man treated for prostatic hyperplasia with 13 months of doxazosin. He complained about unilateral gynecomstia and mastodynia. Five months after the discontinuation of doxazosin, the gynecomastia was significantly improved. This is the first reported case of gynecomastia and mastodynia associated with doxazosin use in Korea.         N/A Background: Five-day regimen of FP (5-fluorouracil, cis-platinum) combination chemotherapy for advanced gastric cancer is one of the popular regimens with approximate 40% of response rate in many series of clinical trials. This study was designed to assess the therapeutic effect and toxicity of three days regimen of FP combination chemotherapy instead of five days regimen for disseminated or recurrent gastric cancer. Methods: From July 1996 through July 2000, 32 patients with inoperable or recurrent gastric cancer were enrolled in the study. The regimen consisted of continuous 12-hour infusion of 5-FU 1000 mg/m2/day plus bolus infusion of cisplatin 25 mg/m2/day for 3 days. The treatment was repeated every 3 weeks. Results: Thirty patients were evaluable for response with a median follow up of 27 months. No one entered complete remission. There were 9 partial responses with the response rate of 30%. Median overall survival duration was 9.1 months. It was longer in responders than that in non-responders .(20.5 months vs 8.0 months, p<0.01) Toxicity was acceptable with no treatment related mortality. Toxicities in grade 3 or more included neutropenia in 5% and vomiting in 10% throughout a total of 160 treatment courses with no febrile episode. Conclusion: These results revealed that the toxicity of 3 day regimen of 5-FU and cisplatin is acceptable and the efficacy comparable to that of 5 day regimen.(Korean J Med 63:61-68, 2002)

        하악골골절시 각종 고정법의 응력분산에 대한 광탄성학적 연구

        The primary goals of treatment of the mandibular fracture are to reestablish proper occlusion, adequate function, and esthetic harmony. The main surgical objective is to provide sufficient immobilization of the fragments to allow normal bony healing. This was most often achieved by closed reduction using intermaxillary fixation. However, oral and maxillofacial surgeons involved in treatment of the mandibular fracture have long been aware of complicating situations that can limit or preclude the intermaxillary fixation. As a result, a number of techniques have been introduced to provide a rigid internal fixation to minimize or eliminate the need for intermaxillary fixation. The purpose of this study is to compare various fixation methods of mandibular fracture by photoelastic analysis. The results obtained from this study were as follows; 1. Under the vertical load, tensile stress at the upper border and compressive stress at the lower border were widely produced. 2. Distraction of the upper border was noticed in multiple loop wiring. 3. The gap of interosseous wiring was wider than that of miniplate fixation. 4. In the miniplate osteosynthesis group, the middle positioned miniplate fixation was superior to the others. 5. In all the models without intramaxillary fixation, the gap was opened. 6. The combined fixation of intramaxillary arch bar and the middle positioned miniplate was excellent in mechanical aspect.

        신축공동주택에서 플러쉬아웃 실시에 따른 실내 VOCs 농도저감 효과에 관한 연구

        The purpose of this study was to confirm the effects of flush-out on the reduction of Volatile Organic Compounds(VOCs) concentration in newly built residential buildings. The field measurements were conducted in two complexes of multi-residential buildings which are located suburban areas of Seoul. About three samples of residential buildings were selected in the field measurements. Two samples, B and C, had been flush-out with outdoor air more than seven days. The sample A was maintained at same condition during the flush-out because of the comparison. The indoor concentrations of VOCs in the three samples were measured at three times, before, during ,and after flush-out. From the field measurements, it was confirmed that two samples’ indoor VOCs’ concentrations were considerably decreased after the flush-out comapred with those of the sample A in which the flush-out was not conducted. The decreases of indoor VOCs concentrations were corresponding to about 50% ~ 92% of the concentrations of before. Through the reviews for the previous studies, it was also found that the reduction effects of the flush-out is similar with the those of the bake-out which were reported in the previous studies.

        대퇴경부골절에서 시행한 Unipolar Arthroplasty와 bipolar arthroplasty의 비교

        54 unipolar pmstheses and 61 bipolar prostheses with same femoral stem were examined clinically and radiologically at an average of 34 months postoperatively. The average age of the patients in unipolar group was 74 years and 71 years in bipolar group. All prostheses had been inserted due to femoral neck fractures. There were two cases of dislocations, two cases of acetabular erosion in unipolar group and bipolar group, respectively. Dislocated prosthetic head was reduced with closed method in unipolar group, meanwhile open reduction was required in bipolar group. One case of early failure was found in cemented bipolar arthroplasty at post-op 2 years follow-up. Functionally, the results were excellent and good in 70.4% of unipolar group and in 73.8% of bipolar group. There was no significant difference between unipolar and bipolar arthoplasty.

      • PET/CT 3차원 영상 획득에서 부분용적효과 감소를 위한 재구성법의 최적화

        PVE는 PET/CT 3D 영상 획득에서 발생되는 것으로 평가값이 저평가되어 영상의 정확도를 떨어뜨리는 현상이다. 특히 이는 병소의 크기가 작고 분해능이 저하될수록 더 큰 오차를 초래하여 검사 결과에 영향을 줄 수 있다. 본 연구는 PVE에 영향을 줄 수 있는 매개변수의 변화를 이용하여 최적의 영상 재구성법을 알아보고자 한다. GE Discovery STE16 장비에서 NEMA 2001 IEC phantom을 이용하여 각기 다른 크기의 구체(직경 37, 28, 22, 17, 13, 10 mm)에 $^{18}F$-FDG를 열소와 배후방사능비 4:1로 주입하여 10분간 영상을 획득하였다. 재구성은 반복재구성법(iterative reconstruction)을 사용하였으며, 반복 횟수(iteration) 2~50회, 부분집합 수(subset number) 1~56개로 변화를 주었다. 분석은 영상의 구체부분에 관심영역(ROI)을 설정하고 최대 표준섭취계수($SUV_{max}$)를 이용하여 백분율 차이(% difference)와 신호대잡음비(SNR)를 산출하였다. 반복 횟수 2, 6, 13, 30, 50회 변화를 준 10 mm 구체의 $SUV_{max}$는 2.32, 3.60, 3.88, 3.88, 3.90이고, SNR은 0.36, 0.49, 0.47, 0.43, 0.41이었으며, 백분율 차이는 58.9, 38.5, 34.8, 35.7, 35.4%로 측정되었다. 또한 6회로 고정한 반복 횟수에 2, 5, 8, 20, 56으로 부분집합 수를 변화시킨 평균 $SUV_{max}$는 10mm의 구체에서 1.46, 3.10, 3.10, 3.48, 3.78로 측정되었으며, SNR은 0.19, 0.30, 0.40, 0.48, 0.45로 나타났다. 또한 각 구체의 SNR의 합은 2.73, 3.38, 3.64, 3.63, 3.38로 측정되었다. 반복 횟수 6회부터 20회까지는 평균 백분율 차이($73{\pm}1%$)와 평균 SNR ($3.47{\pm}0.09$)은 비슷한 값을 나타내었으며, 20회 이상에서는 noise의 영향으로 SUV가 저평가되는 현상이 증가하였다. 또한 동일한 반복 횟수의 경우에 부분집합 값의 변화에서 SNR은 8회부터 20회가 높은 구간($3.63{\pm}0.002$)으로 나타났다. 따라서 작은 병소의 PVE를 줄이기 위해서는 재구성 시간을 고려하여 반복 횟수 6회, 부분집합 수 8~20회에서 PVE를 가장 저감할 수 있다. Purpose: Partial volume effect (PVE) is the phenomenon to lower the accuracy of image due to low estimate, which is to occur from PET/CT 3D image acquisition. The more resolution is declined and the lesion is small, the more it causes a big error. So that it can influence the test result. Studied the optimum image reconstruction method by using variation of parameter, which can influence the PVE. Materials and Methods: It acquires the image in each size spheres which is injected $^{18}F$-FDG to hot site and background in the ratio 4:1 for 10 minutes by using NEMA 2001 IEC phantom in GE Discovey STE 16. The iterative reconstruction is used and gives variety to iteration 2-50 times, subset number 1-56. The analysis's fixed region of interest in detail part of image and compute % difference and signal to noise ratio (SNR) using $SUV_{max}$. Results: It's measured that $SUV_{max}$ of 10 mm spheres, which is changed subset number to 2, 5, 8, 20, 56 in fixed iteration to times, SNR is indicated 0.19, 0.30, 0.40, 0.48, 0.45. As well as each sphere's of total SNR is measured 2.73, 3.38, 3.64, 3.63, 3.38. Conclusion: In iteration 6th to 20th, it indicates similar value in % difference and SNR ($3.47{\pm}0.09$). Over 20th, it increases the phenomenon, which is placed low value on $SUV_{max}$ through the influence of noise. In addition, the identical iteration, it indicates that SNR is high value in 8th to 20th in variation of subset number. Therefore, to reduce partial volume effect of small lesion, it can be declined the partial volume effect in iteration 6 times, subset number 8~20 times, considering reconstruction time.

        방사선 조사 기술을 이용하여 제조한 멸균 우유죽(타락죽)의 품질 특성

        우유를 첨가하여 제조한 죽(우유죽)을 멸균 환자식으로 개발하기 위해 0, 1, 3, 5, 7 및 10 kGy의 흡수선량으로 감마선조사 후 미생물 초기오염도 측정, 멸균여부 확인, 제조 단계별 isolated spore의 방사선 감수성 및 색도를 측정하였으며, 저장 중 점도에 대한 변화를 관찰하였다. 감마선 조사된 우유죽의 미생물 초기 오염도는 2.60 log CFU/g이었으며, 대장균군, 포자형성균과 효모 및 곰팡이군은 검출한계 내에서 나타나지 않았다. 저장 기간 중 10 kGy 조사구에서는 미생물이 검출되지 않았으며, 동일 선량을 조사 시 우유죽의 멸균을 확인할 수 있었다. 우유죽에서 분리된 spore의 D10 value는 2.21 kGy(saline solution) 및 2.71 kGy(milk porridge)로 각각 나타났다. 감마선 조사선량의 증가에 따라 CIE color value는 증가하였으나, 색에 대한 관능검사에서는 변화가 나타나지 않았다. 점도는 감마선 조사선량이 증가할수록, 저장기간이 길어질수록 유의적으로 감소하였다. 감마선 조사 직후 우유죽의 관능검사 결과 3 kGy 조사구까지는 모든 항목들이 4.0 이상으로 높게 평가되었다. 따라서 방사선 조사 기술을 이용하여 우유죽을 제조할 경우, 전분의 농도가 높은 멸균 환자식으로의 제조가 가능하였으며, 또한 멸균된 우유죽을 경관급식이 필요한 의식불명 환자 등에게도 적용시킬 수 있어 결과적으로 고농도의 멸균 우유죽을 환자에게 제공할 수 있으리라 사료된다. We conducted this study to determine the optimum dose of gamma irradiation needed for the sterilization of milk porridge for patients. Milk porridge, known as Tarakjuk, was irradiated with gamma ray at doses of 0, 1, 3, 5, 7, or 10 kGy. The microbial contamination, D10 values of isolated microbe spores, color, and viscosity were measured during storage at 35℃. The initial count of total aerobic bacteria was 2.60 log CFU/g in the non-irradiated milk porridge, but coliforms, spore-forming bacteria, yeast, and molds were not detected. The total counts of aerobic and spore-forming bacteria in the non-irradiated and 1 kGy irradiated milk porridge increased with storage period. These microbes were not detected in the milk porridge irradiated with 10 kGy. The D10 values of isolated spores from milk porridge were 2.71 kGy (in milk porridge) and 2.21 kGy (in saline solution). All CIE color increased with gamma irradiation, but the sensory value of color did not significantly change. The viscosity of the milk porridge decreased with gamma irradiation and storage period, and the decrease in viscosity with storage period became smaller as the radiation doses increased. Sensory evaluation scores of the milk porridge were above normal (4.0) when irradiated with less than 5 kGy. These results indicate that gamma irradiation could be beneficial for preparing food with higher nutrient density and lower viscosity, especially for gastric tube-fed patients.

        수동시료채취기를 이용한 대기 및 수계 유기화학물질의 비표적 검색 모니터링 방법 적용성 검토

        The purpose of this study was to compare a few passive samplers that may be used for non-target screening of organic chemicals in air and water. In this study we customized passive sampling and subsequent analytical methods. The methods were applied to three sites (S sites) as potential pollution hotspots and three reference sites (H sites). The number of the chemicals accumulated in polyurethane foam (PUF) in the pollution hotspots was greater than those in the control sites by more than a factor of two. Passive sampling was found to be less affected by the seasonal variations. In S3 site, the results of grab sampling showed significant monthly variations, suggesting the necessity of passive sampling. Up to 90,000 peaks were detected for each site as a result of analyzing of pre-treated samples with two-dimensional gas chromatography time of flight mass spectrometer (2D-GC/TOF-MS) after passive sampling. Especially, there were 800 to 1,800 chemical compounds screened under the condition that the similarity was over 700 and the signal-to-noise ratio was over 1,000. These results suggest that passive samplers would be useful in non-target screening of residual organic chemicals in air and water although further research is needed.
