An Infantile Case of Sandhoff Disease Presenting With Swallowing Difficulty

Infants with Sandhoff disease typically appear normal until 3-6 months of age. As the disease progresses, they present with symptoms such as loss of motor skills, exaggerated startle response to loud noise, seizures, visual loss, and paralysis. We encountered a rare case of a 22-month-old girl with Sandhoff disease characterized by progressive motor weakness and dysphagia, who initially showed signs of aspiration at 20 months of age. The major problems related to dysphagia were oromotor dysfunction and abnormal feeding posture. Within 3 months of identification of difficulty in swallowing, the patient showed a significant decrease in food intake, with rapid deterioration of nutritional status. We report our case with a review of the literature.

최근 많은 사물들의 센싱 정보를 인터넷을 통해 수집하고 가공 및 분석하는 사물인터넷 (Internet of Things, IoT) 서비스를 제공하고 있다. 2021년 기준 전세계 사물인터넷디바이스 수는 123억개로 사물인터넷 디바이스 수는 무서운 속도로 증가하고 있다. 사물인터넷 디바이스는 대체로 전력 및 비용의 문제로 저사양 디바이스를 사용하고 있고 다양한 구성요소를 가지고 있는 만큼 다양한 보안 취약성을 가지고 있다. 기존 IT 분야의 네트워크, 플랫폼, 서비스에서의 취약성은 모두 가지고 있으며, 사물인터넷 디바이스의 자원 제약성으로 인한 보안 결여 다양한 공격루트를 통한 공격자의 쉬운 접근 가능성으로 많은 보안 취약성과 높은 공격 가능성을 가지고 있다. 본 논문에서는 사물인터넷 하드웨어 보안 관점에서 살펴보고, 최근 오픈소스 하드웨어로 각광받고 있는 RISC-V를 활용한 사물인터넷 디바이스 보안 적용 방안을 보도록 한다.
하드웨어 기반 암호화와 메모리 격리를 통한 경량 RISC-V 신뢰 실행 환경

A trusted execution environment (TEE) is a technology that isolates sensitive data and code from untrusted environments to provide resistance to external tampering and ensure safe execution. However, the existing trusted execution environment technologies lack support for functions such as mitigation of physical attacks on external memory in a lightweight embedded environment, protection of memory isolation rules for safely isolating real-time tasks, and multiple trusted domains. In this paper, based on Ibex, a low-power RISC-V-based open-source processor implementation that has been widely researched in recent years, we present a trusted execution environment structure that can safely isolate and execute multiple trusted domains at the same time. Our structure can ensure confidentiality and integrity of data and code efficiently through on-chip hardware cipher accelerator and has high resistance to advanced software and physical attacks. 신뢰 실행 환경(trusted execution environment)은 민감한 데이터와 코드를 비신뢰 환경으로부터 격리하여 외부 탬퍼링에 대한 저항성을 제공하고 안전한 실행을 보장하기 위한 기술이다. 하지만, 기존의 신뢰 실행 환경 기술은 경량 임베디드 환경의 외부 메모리에 대한 물리적 공격 완화, 실시간 작업을 안전하게 격리하기 위한 메모리 격리 규칙에 대한 보호, 다중 신뢰 영역과 같은 기능의 지원이 미비하다. 본 논문에서는 최근 많은 연구가 이루어지고 있는 저전력 RISC-V 기반 오픈 소스 프로세서 구현인 Ibex를 기반으로 다중 신뢰 영역을 안전하게 격리하여 동시에 실행할 수 있는 신뢰 실행 환경 구조를 제시한다. 본 구조는 온 칩(on-chip) 하드웨어 암호 가속기를 통해 데이터 및 코드의 기밀성과 무결성을 효과적으로 보장하며 고도화된 소프트웨어 및 물리적 공격에 대한 높은 저항성을 가진다.

Objective To evaluate the clinical features that could serve as predictive factors for improvement in gait speed after robotic treatment.Methods A total of 29 patients with motor incomplete spinal cord injury received 4-week robot-assisted gait training (RAGT) on the Lokomat (Hocoma AG, Volketswil, Switzerland) for 30 minutes, once a day, 5 times a week, for a total of 20 sessions. All subjects were evaluated for general characteristics, the 10-Meter Walk Test (10MWT), the Lower Extremity Motor Score (LEMS), the Functional Ambulatory Category (FAC), the Walking Index for Spinal Cord Injury version II (WISCI-II), the Berg Balance Scale (BBS), and the Spinal Cord Independence Measure version III (SCIM-III) every 0, and 4 weeks. After all the interventions, subjects were stratified using the 10MWT score at 4 weeks into improved group and non-improved group for statistical analysis.Results The improved group had younger age and shorter disease duration than the non-improved group. All subjects with the American Spinal Injury Association Impairment Scale level C (AIS-C) tetraplegia belonged to the non-improved group, while most subjects with AIS-C paraplegia, AIS-D tetraplegia, and AIS-D paraplegia belonged to the improved group. The improved group showed greater baseline lower extremity strength, balance, and daily living function than the non-improved group.Conclusion Assessment of SCIM-III, BBS, and trunk control, in addition to LEMS, have potential for predicting the effects of robotic treatment in patients with motor incomplete spinal cord injury.
