Bilateral trans-scaphoid perilunate fracture dislocations are uncommon and have been rarely reported in the literature. Furthermore, it is more difficult to manage in the case of Fenton’s syndrome (scaphocapitate fracture syndrome, trans-scaphoid trans-capitate fracture dislocation). These injuries occur after a high-energy trauma caused by fall from height or vehicular accidents. These fracture dislocation patterns have very few reported cases in the literature and little information as to the diagnosis, management, and surgical approach for treatment. We present a case of scaphocapitate fracture syndrome on the right hand and trans-scaphoid trans-triquetral perilunate injury on the left hand at the same time.
중심성 척추관 협착증 환자의 감압술이 요통 감소에 미치는효과

Study Design: A retrospective study of prospectively-collected data. Objectives: To investigate whether decompression surgery can improve low back pain and symptoms of lower limb pain in patients with one-level central lumbar spinal stenosis Summary of Literature Review: Patients with central lumbar spinal stenosis complain of not only lower leg symptoms, but also simultaneous low back pain in many cases. Therefore, patients who undergo decompressive surgery expect recovery from low back pain as well as lower leg symptoms, and surgeons who perform decompression surgery make efforts to improve both symptoms. Materials and Methods: The present study included 39 patients who had findings of central lumbar spinal stenosis and underwent decompression surgery due to claudication and lower leg radiating pain. Their pain, functional outcomes, and Short Form-36 scores were evaluated before surgery and at 6 and 12 months after surgery. Results: Mean lower leg radiating pain showed a steady and statistically significant improvement up to 12 months after surgery (p<0.05, p=0.003). Mean low back pain scores showed a statistically significant improvement (p<0.05) up to 6 months after surgery, after which no further improvement was noted. Conclusions: Decompression surgery for patients with central lumbar spinal stenosis showed significant improvements in lower leg radiating pain and low back pain (visual analogue scale >4) up to 6 months after surgery and continuing improvements in lower leg radiating pain up to 12 months, but low back pain did not continue to improve after 6 months. Key words: Lumbar spinal stenosis, Radiating pain, Low back pain, Decompression 연구 계획: 후향적 선행 연구목적: 한 분절의 중심성 척추관 협착증으로 진단받은 환자에서의 감압술은 하지 증상 뿐만 아니라 요통의 호전도 기대할 수 있는지에 대해 알아보고자 하였다. 선행 연구문헌의 요약: 척추관 협착증의 감압술은 대부분 하지 증상의 호전을 위해 시행되며, 이러한 감압술이 요통까지 호전될 수 있다는 연구는 많지않은 실정이다. 대상 및 방법: 2013년에서 2018년까지 본원에서 한 분절 중심성 척추관 협착과 그에 합당한 하지 방사통이나 파행으로 인해 수술적 치료가 필요하였던 총 39예의 환자를 대상으로 하였다. 하지 방사통과 요통을 시각 통증 척도를 사용하여 평가하였고, 척추의 기능적 결과는 Oswestry Disability Index나Rolland Morris Disability Questionnaire로 평가하였으며, 삶의 질은 Short Form 36을 이용하였으며, 술 전 술 후 6개월, 12개월에 평가하였다. 결과: 감압술 후 평균 하지 방사통은 술 후 1년까지 통계적으로 의의 있는 호전을 보였다(p<0.05). 요통은 술 전 4.72±3.40, 술 후 6개월 2.33±2.27, 술후 1년 2.21±2.02로 술 후 6개월까지는 통계적으로 의의 있는 호전을 보이지만(p<0.05) 이후의 호전은 없었다. 결론: 중심성 척추관 협착증 환자에서 감압술만으로 술 후 6개월까지 하지 방사통이나 심한 요통(VAS >4) 모두에서 통계적으로 의의있는 회복을 기대할수 있으며, 그 이후에는 하지 방사통은 통계적으로 의의 있는 호전을 보이지만, 요통의 호전은 없었다.
