        담낭암의 성차 이슈: 국내 환자의 임상적 특성 및 치료, 예후를 중심으로

        Gallbladder cancer (GBC) is a relatively uncommon malignancy of the gastrointestinal tract, but it has a poor prognosis. Most cases of GBC are initially diagnosed in advanced stage and the efficacy of currently available treatments have been disappointing. Sex and gender medicine investigates the clinical impact of sex and gender difference on diseases and it may provide a good starting point for the personalized medicine. In this article, sex and gender differences in GBC are explained based on an overview of epidemiology and risk factors focusing on recent researches. (Korean J Gastroenterol 2021;78:27-30)

        개정된 대한췌장담도학회 급성췌장염 임상진료지침: 초기 치료, 영양지원, 회복기 치료

        급성췌장염의 초기 치료와 영양 및 회복기 치료에 대한 권고사항을 요약하면 다음과 같다. 초기 치료로 적절한 정맥내 수액 공급을 위해서 목표 지향적 치료(goal-directed therapy)를 권장하고, 치료 초기에 통증 조절을 적극적으로 고려해야 하며 예방적 항생제의 일률적인 사용은 권고되지 않는다. 그리고 급성 담석성 췌장염 환자에서 담도염이 있거나 담도 폐쇄가 지속되는 경우에는 조기에 내시경역행담췌관조영술(ERCP)을 시행해야 한다. 영양지원에서는 가능한 조기에 경구로 식이를 시작할 것을 권고하고, 경구 식이가 어려운 경우 경장관 식이를 고려할 수 있으며 경구 식이의 조성이나 형태는 환자가 식이 가능한 형태의 저지방 식이를 권장한다. 마지막으로 회복기 치료에서 담낭담석이 있는 경증의 급성 담석성 췌장염은 담낭절제술을 가능한 같은 입원 기간 내에 시행하고, 중증의 경우에는 염증반응이 충분히 해소된 후에 시행하는 것을 권장한다. 그리고 반복하여 재발하는 알코올성 급성췌장염에서는 금주 치료를 고려할 것을 권고한다. Initial and convalescent treatment of acute pancreatitis (AP) is important in order to improve the prognosis and prevent the recurrence in the patients with AP. Initial intensive treatment includes fluid therapy, pain control, antimicrobial therapy, endoscopic retrograde cholangiopancreatography (ERCP), and nutritional support. Goal-directed therapy is recommended for fluid therapy, and the routine use of prophylactic antibiotics is not recommended. In acute gallstone pancreatitis, urgent ERCP should be performed only in patients with cholangitis or persistent cholestasis. Early oral feeding is advisable as tolerated and enteral feeding via nasogastric or nasojejunal tube appear comparable. In convalescent treatment, cholecystectomy during the initial admission is advisable for mild biliary pancreatitis with gallstone as possible, and treatment against alcohol dependence is considerable for recurrent acute alcoholic pancreatitis.         악성 담관 폐쇄에서 내시경적 담관내 고주파 소작술의 유용성

        Malignant biliary tract obstruction (MBTO) is caused by a group of neoplasms that compromise bile duct flow, and the clinical presentation includes obstructive jaundice. The optimal treatment depends on both the type of malignancy and the stage of disease. Surgical resection may be the first choice of treatment. However, an operation is often impossible because of locally advanced disease or a high metastatic potential at the time of diagnosis. Considering the unfavorable prognosis of unresectable MBTO, endobili-ary radiofrequency ablation (EB-RFA) has emerged as a palliative therapeutic modality that directly ablates malignant tissue in the bile duct. To date, some reports have suggested that EB-RFA is possibly beneficial, but it remains unclear whether EB-RFA prolongs biliary stent patency or overall survival. Nevertheless, EB-RFA is regarded as a promising loco-regional therapy for MBTO. This review focuses on the clinical application of the technique and its appropriate use, along with the benefits afforded and the complications encountered. (Korean J Med 2017;92:437-442)

        참나무 5종의 생태 복원 적지 추정을 위한 경제림 육성단지의 기후와 입지 요인 분석

        The most important thing to successfully restore an oak forest is finding suitable climatic conditions and topographic factors for the oak species to be introduced. In this study, in order to find suitable environmental conditions for the five dominant oak trees on the Korean Peninsula, we carried out analysing the information on the location of forest vegetation on the Korean Peninsula. The range of annual mean temperature of the five oak trees was narrow in the order of Q. mongolica (7.7~14.3℃), Q. variabilis (9.2~13.8℃), Q. acutissima (10.5~14.3℃), Q. serrata (11.4~13.7℃), Q. aliena (11.0~12.9℃). The range of annual precipitation of oaks was narrow in order of Q. mongolica (1072.7~1780.9 mm), Q. variablis (1066.6~1554.9 mm), Q. acustissima (1036.5~1504.8 mm), Q. serrata (1062.6~1504.7 mm). The range of altitude was in order of Q. mongolica (147~1388m), Q. serrata (93~950m), Q. variabilis(90~913m), Q. acustissima (60~516m), Q. aliena (55~465 m). The range of slope was in the order of Q. mongolica (8~56°), Q. variabilis(5~52°), Q. serrata (11~45°), Q. aliena (15~38°), Q. acustissima (16~37°). These results are considered to be very useful in the case of ecological restoration using deciduous oak trees on the Korean Peninsula.

        기후변화 지표종 구상나무(Abies koreana E. H. Wilson)의 생태학적 반응

        기후변화조건에서 구상나무의 생태적 변화를 파악하기 위해 5년 동안 대조구(야외)와 RCP 4.5에 근접하게 온도 및CO2를 상승시킨 기후변화처리구에서 생육 및 생리적 반응을 분석하였다. 그 결과, 기후변화처리구에서 구상나무의년생별 가지 길이는 시간이 지날수록 증가하는 경향이 나타났다. 그리고 기후변화 조건은 구상나무 잎의 형태적 차이에는 큰 영향을 주지 않지만, 잎의 생물량에는 영향을 주어, 기후변화가 진행될수록 구상나무의 잎 생산성이 저하될것으로 예상되었다. 한편, 기후변화 조건에서 구상나무의 엽록소 함량은 기후변화처리구에서 높은 반면, 광합성률, 기공전도도, 증산율, 그리고 수분이용효율은 대조구가 더 높게 나타났다. 이는 온도와 CO2가 상승된 환경이 기공밀도증가에 영향을 줄 수 있으나, 광합성 반응에는 부정적 영향을 주는 것으로 예상된다. 향후 이를 확인하기 위해 각각의환경 처리에서 기공 밀도에 대한 추가적인 연구가 필요할 것으로 판단된다. 한편, 본 연구에서는 잎의 생물량 변화만관찰하였기 때문에 기후변화 조건 하에서 구상나무의 생물량 변화에 대한 추가적인 실증 연구가 고려되어야 할 것으로 사료된다. 결과적으로 장기간 상승된 온도와 CO2에서 구상나무의 환경 적응력은 약화될 것으로 판단된다. To assess the ecological changes of Korean fir (Abies koreana E. H. Wilson) under climate change conditions, growth and physiological responses were analyzed over a 5-year period in a control group (outdoors) and in a treatment group where the temperature and CO2 levels were elevated to closely resemble RCP 4.5 conditions. The results showed an increasing trend in annual branch length of A.koreana in the climate change treatment group over time. While climate change conditions did not significantly impact the morphological differences of A.koreana leaves, they did influence the biomass of the leaves, suggesting that as climate change progresses, the productivity of A.koreana leaves may decline. On the other hand, the chlorophyll content in A.koreana under climate change conditions was higher in the climate change treatment group, whereas the photosynthesis rate, transpiration rate, water use efficiency and stomatal conductance was higher in the control group. This suggests that an environment with elevated temperature and CO2 could influence an increase in stomatal density, but having a negative impact on photosynthetic reactions. Further research on stomatal density under each environmental treatment will be required to confirm this hypothesis. Additionally, as this study only observed changes in leaf biomass, further empirical research should be considered to understand the changes in biomass of A.koreana under climate change conditions. In conclusion, the environmental adaptability of A.koreana is expected to weaken in the long term under elevated temperatures and CO2.

        한국 서·남해안 연안생태계의 염생식물군락과 염생식물상의 특성에 관한 연구

        한국은 삼면이 바다로 되어 있어 염습지와 이에 인접한 해안사구가 발달되어 있다. 본 연구는 한국 서·남해안의 염습지와 사구에서 나타나는 염생식물의 군집과 식물상의 특성을 48개 지소의 571 식분에서 조사하고, 분석하여 연안 생태계의 특성을 밝히고자 시도하였다. 그 결과, 염습지에 분포하는 식물군락은 39개이었으며, 갈대군락의 분포면적이 가장 넓었고, 버들명아주군락이 가장좁았다. 나문재군락이 해안선으로부터 가장 근접하여 분포하였고, 해홍나물군락이 해양쪽으로 가장 멀리까지 분포하였다. 해안사구에 분포하는 식물군락은 29개이었으며, 갯그령군락의 분포면적이 가장 넓었고, 수송나물군락이 가장좁았고, 갯메꽃군락이 해안선으로부터 가장 근접하여 분포하였고, 해당화군락이 내륙쪽으로 가장 멀리까지 분포하였다. 염습지의 식물상은 6과 14속 17종 1변종 18분류군 이였으며(식물구계학적 점수; 16점), 해안사구는 11과 18속20종 1변종 21분류군(식물구계학적 점수; 34점)으로 해안사구가 염습지 보다 약간 높게 나타났다. 이러한 결과는 한국 서남해의 연안에서는 해안사구가 염습지보다 해수의 영향을 간접적으로 받기 때문에 염도에 약한 식물도 생육할수 있기 때문으로 해석된다. Korea is surrounded by the sea on three sides, salt marshes and coastal sand dunes adjacent to them are developed. This study attempted to reveal the characteristics of the coastal ecosystem by investigating and analyzing the characteristics of plant communities and flora of 571stands of 48 sites in the salt marshes and coastal sand dunes on the western and southern coasts of Korea..As a result, in the salt marshes, 39 vegetation units appeared, with Phragmites australis community having the widest area and Chenopodium virgatum community having the narrowest area. Suaeda glauca community was distributed closest to the coastline, while Suaeda maritima community extended the farthest seaward. In the coastal sand dunes, 29 vegetation units were distributed with Elymus mollis community having the widest area and Salsola komarovii community having the narrowest area. Calystegia soldanella community was showed closest to the coastline, while Rosa rugosa community extended the farthest landward. The flora of the salt marshes consisted of 6 families, 14 genera, 17 species, 1 variant, and 18 taxa (floristic regional indicator score; 16) and the coastal sand dunes consisted of 11 families, 18 genera, 20 species, 1 variant, and 21 taxa (floristic regional indicator score; 34) appeared slightly higher than that of salt marshes. This result is interpreted that halophyte that are sensitive to salinity can grow on the coast of the west-south coast of Korea because coastal sand dunes are more indirectly affected by sea than salt marshes.

        Cameron ulcers are gastric ulcers or erosions located at mucosal folds in patients with large hiatal hernias. Mechanical trauma, gastric acid injury, or ischemia can cause their formation. Cameron ulcers are frequently seen as incidental findings during upper gastrointestinal endoscopy. About half of patients with these lesions are asymptomatic. However, Cameron ulcers are related to iron deficiency anemia and gastrointestinal bleeding in some cases. Here, we report a case of Cameron ulcers presenting with melena, which was successfully treated with proton pump inhibitors and iron supplementation. (Korean J Med 2012;82:585-588)

        골퍼들의 경력 및 핸디가 브랜드 충성도에 미치는 영향

        This article investigates that how golfer's career and handy influence brand loyalty in the increase an enterprise of golf products & saturation market. Data collection was made by utilizing random sampling of field(10), practice club(21). from field club & practice club, which were located in Daegu area, Korea. A total of 300 questionnaires were completed. 66 surveys were excepted and 234 were usable. The result of This article were as follows: First, golfer's career and handy showed negative effects to brand loyalty. Second, the perceived quality moderated between golfer's career & handy and brand loyalty. The perceived quality was consist of price, design, technical ability, material and popular item. Third, the price showed moderating effect negatively on relation between golfer's career and brand loyalty, but in the case of golfer's handy, price showed moderating effect. Fourth, the technical ability was strengthen moderating about relation of golfer's career to brand loyalty. In conclusion, companies for golf-products have to anticipate customer's needs. They have to make differentiated marketing, segment, targeting strategy according to career of golfer and degree of handy. These are important factors to let people perceived quality and differentiate about their products. Because people decide to buy products according to their perceived quality. It will be give the opportunity that establish the strategy of discrimination marketing, segment targeting.
