        A Study on the Seropositivity of HBsAg among Biennial Health Examinees ; A Nation-wide Multicenter Survey

        목적 : 1998년 한국인 성인에서의 혈청 HBsAg의 양성률을 추정하는 것을 일차적 목적으로 하며, HBsAg 양성률의 연령별로 분포, 지역적 차이, 과거 간질환력, 만성간질환 가족력 및 예방접종과의 관련성을 파악하며 또한 양성자를 6개월간 추적후 재검사 하여 B형 간염의 만성보균율을 파악하고자 하는 것을 이차적 목적으로 하였다. 방법 : 1998년 전국 10개 지역에서 의료보험관리공단의 정기건강검진 수진자를 대상으로 하여 HBsAg의 혈청유병률을 조사하였다. HBsAg 양성인 사람들을 대상으로 6개월 이상 추적조사하여 B형간염 만성보균율을 파악하였다. 총 1,816명에 대한 혈청과 설문서가 수집되었다. HBsAg는 RIA로 측정하였다. 결과 : HBsAg의 혈청유병률은 5.5%(95% CI-4.5-6.6)였으며 남자에서 7.4%(95% CI=5.8-9.4), 여자에서 3.6%(95% CI=2.5-5.0)로 나타났다. 급성간질환 과거력과 만성간질환 가족력을 HBsAg 혈청검사결과와 비교한 결과 유의한 관련성이 있는 것으로 나타났다. HBsAg 양성자에서 6개월 후에 음성으로 전환한 사람은 3.2%(95% CI=0.1-16.7)였으며 따라서 B형간염의 만성보균율은 5.3%(95% CI=3.7-6.6)로 추정되었다. 결론 : 본 연구결과에서는 HBsAg의 양성률이 1980년대의 연구결과들에 비하여 비교적 낮게 추정되었으며 이는 특히 여성과 젊은 연령층에서 두드러지게 나타났다. 그러나 우리나라에서의 간암 및 만성간질환의 공중보건학적 중요성을 고려하면 지속적인 간염발생의 예방대책이 필요하다고 할 수 있다. Objective : The primary objective of this study was to estimate the prevalence of HBsAg-positives in the late 1990's among Korean adults. In addition, we evaluated the association of age, a residential area, a vaccination rate, a family history of chronic liver diseases and a past history of acute liver disease with the seropositivity of HBsAg, and estimated the prevalence of chronic HBV infection by follow-up for 6 month or more. Methods : A total of 10 areas, six metropolitan and four small cities, were selected. In each cities, one health screening center was selected for recruitment of study subjects. The study subjects were enrolled from a general health examination program that is provided by medical insurance companies. Questionnaires on various risk factors were administered to the study subjects. Sera was drawn and tested for HBsAg by radioimmunoassay. HBeAg and ALT were also tested for those of HBsAg positive. The HBsAg positives was retest for HBsAg 6 months later Results : Among the study subjects (n= 1816), the seroprevalence of HBsAg was 5.5% (95% CI=4.5%-6.6%), 7.4% in men (95% CI=5.8-9.4) and 3.6% in women (95% CI=2.5-5.0). A past history of acute liver disease and a family history of chronic liver diseases was shown to be risk factors for HBsAg positivity. Among the 31 HBsAg-positives, negative seroconversion rate was estimated to be 3.2%, Thus, prevalence of chronic HBV infection was estimated to be 5.3% (95% CI=3.7-6.6). Conclusion : In this study, the HBsAg seroprevalence rate was lower than that of the other studies in 1980's, particularly in young adult and women. Considering the public health importance of liver cancer and chronic liver diseases, the further effort is needed to prevent and reduce the HBV infection.

        In most dynamic manufacturing environments today, systems and processes are constantly changing. Simulation tools are required that can accurately model the system in detail, but still be easy to use, and allow rapid model redevelopment to react quickly for system changes. An object-oriented simulation modeling environment is presented to provide flexible modeling capabilities for simulation. Also, when simulating an assembly system, a large number of factors must be considered. Because of such complexities, simulation has been used as the primary method in designing, planning and analyzing. In this paper, the dynamic manufacturing environment is discussed for the assembly system. Also, an application method of simulation tools is presented with the experimental data from the automobile manufacturing shop to improve the productivity effectively.

        Maximal oxygen debt, lactate and excess lactate were measured in 13 men with low hematocrit ratio before and after maximal exercise. Maximal exercise run was performed on a treadmill and the duration of run was 2 minutes 45 seconds in each subject. Hematocrit ratio ranged between 35 and 47%, the mean being 39.8%. The following results were obtained. 1. Maximal oxygen debt expressed on basis of body weight increased as the hematocrit ratio decreased. The correlation coefficient between the two was r= -0.770. 2. The time necessary for decreasing to 50% of total maximal $O_2$ debt(half time) became longer as the hematocrit ratio decreased. In normal men the half time was about 4 minutes and at the longest it was 12 minutes in men with the lowest hematocrit ratio. 3. The lactate concentration reached its peak value after 3 minutes of recovery. Thereafter, the time course of decrease in lactate concentration coincided roughly with that of respiratory oxygen debt curve. To reach to the resting level, however, it took longer time than that of respiratory oxygen debt. 4. Resting concentrations of lactate was 1.28 mM/l, pyruvate 0.13 mM/l and L/P ratio was 9.8. Peak value of ${\Delta}L$ after exercise reached to the value of 10.4 mM/l and ${\Delta}L/P$ reached 26.0. Peak excess lactate after exercise was 6.34 mM/l. 5. The part of oxygen debt accounted for by the oxygen equivalent of excess lactate was only 38.4%. A better relationship between lactate and oxygen debt was observed and the part of oxygen debt accounted for by the oxygen equivalent of lactate was 63.3%. 6. Peak value of lactate after maximal exercise increased as the hematocrit ratio decreased. 7. Respiratory oxygen debt of 100 ml/kg was accounted for by lactate more than 60% and only 30% was by excess lactate. 8. Excess lactate was not a good index of respiratory oxygen debt.

        서울시 중년남성에서 육체적 활동량이 총 사망률에 미치는 영향에 관한 코호트 연구

        Although previous studies revealed the association of physical activity with mortality rate, it is unclear whether there is a linear trend between physical activity and mortality rate. In this study, the association of physical activity with the risk of all-cause mortality was analysed using Cox's proportional hazard model for a cohort of 14,204 healthy Korean men aged 40-59 years followed up for 4 years(Jan. 1993-Dec. 1996). Physical activity and other life style were surveyed by a postal questionnaire in December 1992. Total of 14,204 subjects were grouped into quartiles by physical activity. Using death certificate data, 123 deaths were identified. The second most active quartile had a lowest mortality .ate with relative risk of 0.44(95% C.I. : 0.23-0.84) compared with most sedentary quartile, showing a J-shape pattern of physical activity-mortality curve. By examining the difference in proportion of cause of the death between most active quartile and the other quartiles, there was no significant difference of proportional mortality from cardiovascular deaths, cerebrovascular deaths or deaths from trauma. The covariates were stratified into two group between which the trend of RR was compared to test the effect modification. There was no remarkable effect modification by alcohol intake, smoking, body mass index, calorie consumption, percent fat consumption. In conclusion, moderate activity was found to have more protective effect on all-cause mortality than vigorous activity and that the J-shape pattern of physical activity-mortality curve was not due to the difference of mortality pattern or effect modification by alcohol intake, smoking, body mass index, calorie consumption and percent fat consumption.

        소양인(少陽人) 형방패독산(刑防敗毒散)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(硏究)

        We have studied some effects of the SOYANGIN-HYEONGBANGPAEDOK-SAN (S.H.P.). Several empirical remarks, depending on the different treatments, are investigated through this study as follow; 1. The Analgesic effects of S.H.P. are remarked by Acetic acid method in mice, 100 and 300mg/kg (Sample II, III), 2. The Antipyretic effect of S.H.P. are remarked by yeast method in rats, 100 and 300mg/kg (Sample II, III). 3. The inhibitory effects of S.H.P. on strychnine induced convulsion in mice is remarked. However the inhibitory effects of S.H.P. on picrotoxin induced convulsion in mice is not observed. 4. The effects of S.H.P. on muscle relaxation in mice, using the Rota-Rod test, is remarked on 1,2,3 hours by 300mg/kg (Sample III).

        부직포의 열적 특성과 보온성에 관한 연구(II) -풍속에 따른 충전 용도 변화에 대하여-

        Effects of air velocity and packing density under controlled heating and constant heat flow on thermal insulation of nonwoven fabrics were studied. Results obtained can be summarized as follows; 1. Thermal insulation should be considered in terms of air velocity, packing density, time required to reach thermal equilibrium and thermal behavior. 2. Time for thermal equilibrium decreases with increasing air velocity and decreasing packing density, respectively, while it increases with increasing packing density. 3. Thermal insulation is fairly sensitive to increasing packing density at the air velocity of 0.5m/sec while it becomes less sensitive to increasing air velocity, representing almost constant values at the air velocity of more than 2.0m/sec. 4. Thermal insulation effeciency of nonwoven fabrics makes than 10 percent, providing unfavorable thermal insulation effect.
