        Fentanyl 을 사용한 마취유도

        We evaluated the efficacy as an anesthetic induction agent and cardiovascular effects of fentanyl in sixty patients with myocardial ischemia on electrocardiogram, who were scheduled for elective operations of more than two-hour duration. In addition, the effects of pretreatment with pancuronium and/or lorazepam prior to anesthetic induction were evaluated on the induction speed, the incidence and severity of skeletal muscle rigity, heart rate, and systolic blood pressure. All the patients were given 30μg/kg of fentanyl and divided into four equal groups of fifteen patients according to pretreatment drugs administered before fentanyl. The results were as follows: 1) Eighty-seven percent of the patients in control group (pretreated with saline)were rendered unconscious during or one minute after infusion of 30μg/kg of fentanyl. 2) Pretreatment with lorazepam before fentanyl significant#ly increased the induction speed (p$lt;0.025), but was not significantly correlated with the incidence and severity of skeletaluscle rigidity. Pancuronium pretreatment significantly decreased the incidence and severity of skeletal muscle rigidity (p$lt;0.01) but was not significantly correlated with the induction speed, Pretreatment with lorazepam plus pancuronium significantly increased the induction speed (p$lt;0.025) and significantly decreased the incidence and severity of skeletal muscle rigidity (p$lt;0.01). 3) Saline pretreatment and pancuronium pretreatment did not significantly change heart rate or systolic blood pressure, but both lorazepam poretreatment and lorazepam-pancuronium pretreatment decreased heart rate or systolic blood pressure (p$lt;0.05), which were within thirty percent of preanesthetic values. 4) All the patients did not exhibit significant changes in heart rate or systolic blood pressure immediately after, in comparison to, immediately before tracheal intubation or skin incision (p$gt; 0.05). From the above results, it is concluded that anesthetic induction with 30 μg/kg of fentanyl produces profound analgesia and amnesi, stable cardiovascular dynamics, and a minimum of problems, and pretreatment with small doses of lorazepam and pancuronium before fentanyl facilitates not only a rapid loss of consciousness but also a reduction in the incidence and severity of fentanyl-induced rigidity.

        The importance of setting advertising budgets using careful analysis, rather than industry rules of thumb or gut feel, cannot be overemphasized. Various studies have repeatedly shown that the average advertising elasticity-the expected percentage change in volume sales when the amount spent on advertising rises 1 percent-is small, about only 0.22 percent. Clearly, this is only an average across brands, that ignores long-term effects on consumers and effects on the trade, and has various other shortcomings as a measure of the value of advertising spending. However, it does suggest that at least some brands may be spending too much on advertising. When you combine this observation with the fact that for many brands, the advertising budget is the single largest discretionary expense, running into tens of millions of dollars, you should realize quickly that fine-tuning the advertising budget is an activity that is worth a fair amount of a brand manager's time.

        Videofluoroscopic swallowing study (VFS) is accepted as a gold standard tool for evaluating dysphagia. The realtime video radiographic image provides visualization of the structures, movement, and coordination of swallowing. Abnormal oropharyngeal and esophageal anatomy can be readily identified. There are several scoring systems foranalyzing VFS; A Penetration-Aspiration Scale (PAS), The Dysphagia Outcome and Severity Scale (DOSS), NationalInstitutes of Health Swallowing Safety Scale (NIH-SSS) and Videofluoroscopic Dysphagia Scale (VDS). They can providereliable quantification of selected events observed during VFS and be used as powerful outcome measures toinvestigate the efficacy of various swallowing treatments.         생약 추출물 함유 정제 제조를 위한 이산화규소 함유 분말의 제조 및 평가

        The purpose of this study is to optimize the powder formulation and manufacturing conditions for the solidification of an extract of the herb Bangpungtongseong-san (BPTS). To develop BPTS-loaded particles for the tablet dosage form, various BPTS-loaded particles composed of BPTS, dextrin, microcrystalline cellulose (MCC), silicon dioxide, ethanol, and water are prepared using spray-drying and high shear granulation (high-speed mixing). Their physical properties are evaluated using scanning electron microscopy and measurements of the angle of repose, Hausner ratio, Carr's index, hardness, and disintegration time. The optimal BPTS-loaded particles exhibit improved flowability and compressibility. In particular, the BPTS-loaded particles containing silicon dioxide show significantly improved flowability and compressibility (the angle of repose, Hausner ratio, and Carr's index are 35.27 ± 0.58°, 1.18 ± 0.06, and 15.67 ± 1.68%, respectively), hardness (18.97 ± 1.00 KP), and disintegration time (17.60 ± 1.50 min) compared to those without silicon dioxide. Therefore, this study suggests that particles prepared by high-speed mixing can be used to greatly improve the flowability and compressibility of BPTS using MCC and silicon dioxide.

      • 생활체육 참여 지체장애인의 성별, 장애등급, 가족 참여 유무에 따른 지각된 유능감과 신체적 자기개념 차이분석

        본 연구는 국내 지체장애인의 신체활동 참여에 있어 지각된 유능감과 신체적 자기가치감이 인구사회학적 특성 (성별, 장애등급, 가족 신체활동 참여 유무)에 따라 어떠한 차이가 있는지 알아보고 지체장애인의 신체활동 참여에 도움을 줄 수 있는 기초자료를 제공하는데 그 목적이 있다. 이를 위해 부산에 거주하며 장애인 협회에 등록되어 있는 지체장애인을 모집단으로 설정하고, 이들 중 생활체육 단체 종목에 참여하고 있는 지체장애인을 유목적 표집방법(purposeful sampling)을 통해 표집된 210명의 자료를 최종 자료처리에 이용하였다. 자료분석 방법은 SPSS 18.0을 이용하여 다변량 분산분석(MANOVA)을 실시하였다. 이를 통해 나타난 결과는 다음과 같다. 첫째, 성별에 따른 지각된 유능감과 신체적 자기가치감은 유의한 차이가 있는 것으로 나타났다. 둘째, 장애등급에 따른 지각된 유능감과 신체적 자기가치감은 유의한 차이가 있는 것으로 나타났다. 셋째, 가족의 신체활동 참여 유무에 따른 지각된 유능감과 신체적 자기가치감은 유의한 차이가 있는 것으로 나타났다.

        장애노인의 신체활동 참여를 위한 사회인지적 결정요인: 건강행동접근과정(HAPA)을 중심으로

        Purpose The purpose of this study was to analyze the physical activity participation process of the elderly with disabilities and how social cognitive determinants affect physical activity participation based on the Health Action Process Approach model proposed by Schwarzer and Ruszczynska (2008). Methods The manuscript presents a quantitative analysis of primary data gathered through a survey of a convenience sample of elderly people with disabilities (n=273). Data were analyzed using by SPSS 23.0 and Amos 23.0 program. Result As a result of the confirmatory factor analysis, it was confirmed that the Health Action Process Approach model was suitable. As a result of path analysis, most of them showed significant influence relationships except for the relationship between risk perception and intention, maintenance self-efficacy, and planning. Conclusion The Health Action Process Approach model is suitable for analyzing and explaining the process of physical activity and social cognitive determinants of the elderly with disabilities. 연구목적 본 연구의 목적은 Schwarzer와 Luszczynska(2008)가 제안한 건강행동접근과정(HAPA) 모형을 바탕으로 장애노인들의 신체활동 참여과정 분석 및 사회인지적 결정요인이 신체활동 참여에 어떠한 영향을 미치는가를 분석하는데 있다. 연구방법 본 연구는 설문조사로 실시되었으며 편의표본추출법을 이용하여 신체활동에 참여하고 있는 장애노인 273명의 자료를 수집하였다. 수집된 자료는 SPSS 23.0과 AMOS 23.0 프로그램을 이용하여 분석하였다. 결과 확인적요인분석결과 건강행동접근과정 모형은 적합한 것으로 확인되었다. 경로분석결과 위험지각과 의도, 자기효능감 유지와 계획의 관계를 제외하고는 대부분 유의한 영향관계를 보였다. 결론 건강행동접근과정 모형이 장애노인들의 신체활동의 과정과 사회인지적 결정요인을 분석하고 설명하는데 적합한 모형임을 나타내었다.
