Autosomal dominant polycystic kidney disease(ADPKD) is one of most common hereditary disorder. It is potentially fatal and the major mobilities are hypertension and renal failure. To elucidate clinical characteristics and outcomes of ADPKD, 67 ADPKD patients were analyzed retrospectively. The male to female ratio was 48 years for males and 42 years for females. The factors leading to the dignosis of ADPKD were flank or abdominal pain (46%), palpable abdominal mass (24%), incidental findings (18%), gross hematuria and headache. Urinalysis result showed hematuria (31%), proteinuria (19%), pyuria (18%). Among renal complications hypertension (67%) was the most common. Other complications were urolithiasis (16%) upper urinary tract infection (9%), cyst infection (7%). Azotemia was observed in 24% of cases. Echocardiography showed left ventricular hypertrophy in 24% and valvular regurgitation in 18% of cases, but there was no mital valve prolapse. Cerebrovascular events occurred in 6 cases (2 cases of intervertebral hemorrhage and 4 cases of cerebral infarction). Hipatic cyst were observed in 64% of cases. Eighteen cases culminated in end stage renal failure (ESRF) at an of 47. According to this study, the most common complications of ADPKD was hypertension (67%), ESRF (27%), and cerebrovascular event (9%). Hypertension was more frequently observed in increased creatinine group than normal.
Rankine 반형체를 이용한 트랙터-트레일러의 항력감소장치에 관한 연구

트렉터-트레일러 차량의 공기역학적 항력을 감소시킬 목적으로 Rankine반형체를 이용하여 Modified roof-fairing (MRF)과 Sub-trailer(ST)를 개발하였다. 이 항력감소장치들이 차량의 항력특성에 미치는 영향을 조사하기 위하여 트랙터-트레일러의 1/30 축소모형에 대해 ??의 레이놀즈수 영역과 -10˚∼ +10˚의 yaw각 범위에서 풍동실험을 실시하였다. 실험결과 가장 효율적인 항력감소장치는 3차원 형상의 Modified roof-fairing 으로서 yaw 각이 0˚일 때 32%의 항력감소를 나타내었으며, Sub-trailer는 장치의 길이가 길수록 좋은 항력감소 효과를 얻을 수 있었다. 각 항력감소장치들은 레이놀즈수의 변화보다 yaw각의 변화에 더 큰 영향을 받음을 알 수 있었다. Modified roof-fairing and sub-trailer using the shape of Rankine half-body were developed to decrease the aerodynamic drag of a tractor-trailer truck. A wind-tunnel investigation was conducted to determine the influence of these drag reduction devices on the drag characteristics of a tractor-trailer. The measurement were made on a 1/30th scale model at Reynolds number range of ?? and yaw angle range of -10˚∼ +10˚. The 3-dimensional modified roof-fairing which was found to be the most effective drag reduction device results in 32% reduction at zero yaw drag coefficient. Sub-trailer is more effective as the length of the sub-trailer increases. The drag reduction devices were more affected by the variation of yaw angles and the variation of Reynolds numbers.

Membranous nephropathy (MN) is a glomerular disease characterized by diffuse thickening of the glomerular basement membrane without significant mesangial proliferative change and is usually manifested by the nephrotic syndrome. To evaluate the clinical and pathologic characteristics of this disease in adults, we analyzed 65 patients with primary membranous nephropathy who were diagnosed at Department of Internal Medicine, Chungnam National University Hospital from February 1986 to February 2001. The results are as follows: 1) Of total 96 patients with membranous nephropathy, 67.7% was primary MN. Hepatitis B-associated MN and lupus MN were 22.9%, and 9.4%, respectively. Median duration of follow-up was 35.1 months(0.3∼177) in primary MN, 49.6 months(2.5∼103) in lupus MN, and 35.6 months(0.5∼108) in hepatitis B-associated MN. 2) Mean age of the patients with primary MN at the time of diagnosis(43.3 years) was significantly higher than that of lupus MN(30.4 years) and HBV-associated MN(36.2 years). Male to female ratio of primary MN was 1.03:l. 3) At the time of diagnosis, 89.2% of primary MN was presented with nephrotic syndrome. Twenty seven percent of patients showed hypertension. Hematuria was found in 58.5% of primary MN patients. Azotemia was noted in 3.1 %. 4) Global sclerosis, tubular atrophy and interstitial fibrosis were noted in 39.8, 16.1 and 11.3% of primaty MN, respectively. IgG and C3 were deposited on the glomerular capillary loop in 92.7% and 29% of primary MN patients. There was no significant correlation between the electron microscopic pathologic stage and clinical findings. 5) Of 56 patients with primary MN followed more than 6 months, 50(90.9%) patients were treated with prednisolone, cyclophosphamide or cyclosporin. Complete remission was obtained in 29 patients(51.8%), partial remission in 6 patients(10.7%), and no response in 18 patients(32.1%). Of 6 patients who were not treated with immunosuppressive drugs, 3 patients showed spontaneous complete or partial remission. Three patients who did not show clinical improvement progressed to end-stage renal failure. There were no significant clinical difference including renal pathology at the time of diagnosis between the patients who showed complete or partial remission and patients who did not. In conclusion, it was difficult to predict the long-term prognosis of primary MN at the time of diagnosis, clinically. The response to therapeutic modality would be the most important to predict long-term prognosis of primary MN.
광섬유 방사선 센서와 EBT 필름을 이용한 심부선량 백분율 측정 및 비교

In this study, fiber-optic sensor is fabricated to measure PDD of CLINAC. As a scintillating light generated by a high energy X-ray from the medical LINAC is measured using photo diode amp system, It is expected that a high-resolution, real-time fiber-optic dosimeter for medical LINAC can be developed.

In this study, we have measured scintillating light generated in a fiber-optic dosimeter by high energy photon and electron beams using a charge-coupled device. The intensity of Cerenkov lights are also measured and characterized as functions of incident angles and energies of the high energy beams from a clinical linear accelerator. To minimize or remove Cerenkov light, a simple subtraction method using a background optical fiber and wavelength discrimination method using optical filters are investigated. Also, the intensities of Cerenkov light induced by high energy photon and electron beams are compared.

In this study, a fiber-optic bundle sensor has been developed using water-equivalent organic scintillating fibers for photon beam measurement. Beam penumbra and percent depth doses(PDD) are measured according to the depth of the photon beam. This sensor has many advantages such as high resolution, real-time measurement and ease of calibration over conventional radiation measurement devices.

The Process Safety Management (PSM) by the Law of Industry, Safety and Health has been performed for preventing major accidents of chemical plants since 1996. In terms of preventing chemical accidents more precisely, it is essential to develop a tool for quantitative risk assessment. For this, KOSHA (Korea Occupational Safety and Health Agency) developed an Integrated Risk Management System (IRMS) . The system is designed to assimilate data on chemical plant hazards from external database, to integrate these data with location information (topographic and demographic), and to make them user-friendly accessible. The system consists of several main functions: display of five major Korean petrochemical complex layout, display of equipment layout with its information utilizing the external database, zonation of the hazard effected area with consequence analyses, the most probable accident scenario generation, accident/incident database and calculation of frequency of accident using equipment reliability database, etc. The highlight of IRMS is to provide the risk contours using GIS(Geographical Information System) technology. IRMS is intended to manage hazardous installation more systematically and effectively, to reduce the number of accident remarkably, further minimizing production loss in the plant. The system is now under application to about 500 PSM sites as well as and emergency authorities in Korea by KOSHA (Korea Occupational Safety and Health Agency)
