향사양위탕(香砂養胃湯)의 기원(基源), 변천과정(變遷過程) 및 구성원리(構成原理)

1. Objectives: The purpose of this study is to understand the origin, changes and the constructive principles of Hyangsayangyitang (XiangShaYangWeiTang). 2. Methods: Hyangsayangyi-tang and other related prescriptions were analyzed in terms of pathology, based on "Donguibogam(東醫寶鑑)", "Donguisusebowon Chobongwon(東醫壽世保元 草本卷)", "Donguisusebowon Gabobon(東醫壽世保元 甲午本)", "Donguisusebowon Sinchukbon(東醫壽世保元 辛丑本)". 3. Results and Conclusions: (1) The origin of Hyangsayangyi-tang is Hyangsayukgunja-tang from "Gongshin Uigamseo(?信 醫鑑書"). Hyangsayangyi-tang inherited some of the principles from"Euihangangmok(醫學綱目)" Jeonssiyigong-san's (全氏異功散) application. (2) "Gabobon(甲午本)"'s prescription emphasizes the effect of lossen up by using Pogostemonis Herba(藿香). "Sinchukbon(辛丑本)"'s prescription emphasizes the effect of warm up stomach by using Zingiberis Rhizoma(乾薑). (3) Ginseng Radix(人蔘), Atractlodis Rhizoma White(白朮), Glycyrrhizae Radix(甘草), Paeoniae Radix Alba(白芍藥) support Soeumin's spleen root (脾元), and Zingiberis Rhizoma(乾薑), Myristicae Semen(肉豆?) warm up stomach, and Pinelliae Rhizoma(半夏), Citri Pericarpium(陳皮) circulate Qi, Crataegi Fructus(山?), Amomi Fructus(砂仁), Cyperi Rhizoma(香附子) encourage digestion to maintain descending Yin.

Objectives: Hyangsayangyi-tang (香砂養胃湯) is a polyherbal formula, has been used as one of the representative So-eumin (少陰人) prescriptions in Sasang-medicine (四象醫學), a famous Korean medicinal theory. The aqueous extract of Hyangsayangyi-tang (HSYYT) was evaluated for its possible ameliorative effect in the regulation of hypothyroidism related reproductive organ damages in propylthiouracil (PTU)-induced rat model. Methods : HSYYT aqueous extracts were administered, once day for 42 days from 2 weeks before start of PTU treatment as an oral dose of 500, 250 and 125 mg/kg (bodyweight), and hypothyroidism was induced by daily subcutaneous treatment of PTU 10 mg/kg for 28days. Results : PTU induced hypothyroidism and related male reproductive organ (testis, epididymis and prostate) damages were favorably and dose-dependently inhibited by treatment of HSYYT 500 and 250 mg/kg, and they also effectively regulated the PTU-induced abnormal antioxidant defense system changes in the testis. No effective or any harmful changes on the PTU induced hypothyroidism and related male reproductive organ damages were observed in HSYYT 125 mg/kg treated rats as compared with PTU control in this experiment. Conclusions : The results obtained in this study suggest that oral administration of 500 and 250 mg/kg of HSYYT showed favorable effects on the hypothyroidism and related reproductive organ damages through augmentation of antioxidant defense system in the testis, and it is considered that HSYYT may be help to ameliorate the hypothyroidism and related organ damages in clinics.

미후등식장탕으로 오치하여 복통(腹痛), 자한(自汗), 오심(惡心)이 발생한 소음인 직장암 환자 1례 증례보고

In this case, we report a case of Soeumin rectal cancer patient who was mistreated with herb medication. This patient was 72 years old female diagnosed rectal cancer stage IV with multiple liver, lung, and LN metastases. She administered our cancer center in Sep. 2011 for cancer treatment, we diagnosed this patient as Taeyangin at first, and treated with Mihuedungsikjang-tang. After 1 week, she complained upper abdominal discomfort, severe abdominal pain, nausea and sweating. Considering several symptoms and conditions, the change of symptoms seemed to a adverse events caused by mistreatment. We re-diagnosed this patient as Soeumin, and treated with Soeumin herbal medication, Baekhaoleejung-tang(白何烏理中湯) and Hyangsayangyi-tang(香砂養胃湯). Several symptoms including abdominal pain were improved 2 weeks later. Therefore, we report this case as a reference that Soeumin misdiagnosed as Taeyangin and the improvement of adverse symptoms.

태음병 황달증으로 진단한 소음인 황달 환자 치험 1례

Objectives This case study is about a Soeumin patient with jaundice identified as Greater-Yin accompanied Jaundice pattern. In this study, we report significant improvements of anorexia, dyspepsia, fatigue and weight loss after treatment with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture.Methods The patient was identified as Greater-Yin accompanied Jaundice pattern and treated with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture. The Global Assessment Scale (GAS) was used to assess the improvements of anorexia, dyspepsia, fatigue and pelvic pain after the treatment. And the liver function test was performed to measure the overall function of the liver.Result and Conclusion The symptoms of anorexia, dyspepsia, fatigue and pelvic pain were improved from GAS 100 to GAS 0-10 for four months. And The liver function test showed improved the response to treatment. The levels of AST (aspartate aminotransferase)/ALT (alanine aminotransferase) decreased from 43/42 to 29/14 IU/L. The level of total bilirubin was decreased from 2.7 to 1.9 mg/dL. Furthermore total weight gain was 8 kg from starting weight of 36.5 kg for four months. In conclusion, this study shows that Sasang constitutional medicine can be effective and safe treatment for the Soeumin patient with jaundice.

향사양위탕(香砂養胃湯) 전탕액이 노화주의 비장, 췌장, 위장 세포의 항산화능에 미치는 영향

1. Objectives The purpose of this study was to observe the anti-oxidant effects of Hyangsayangyi-tang(HY) in SD rats. 2. Methods This experiment was used the tissue of stomach, spleen and pancreas cells of 6, 52 and 68 weeks old SD rats. Each age group was again divided into three groups. One group, as normal group, was not-treated cells, another group, as control group, was saline-treated cells, and the last group, as experimental group, was HY-treated cells. After culture for 48 hours, each groups measured the level of SOD, GSH, MDA and NO in the tissue of stomach, spleen and pancreas cells. 3. Results and Conclusions The activity of SOD were significantly increased in spleen cell of 52, 68 w-HY group, pancreas cell of 68 w-HY group and in stomach cell of 52, 68 w-HY group compared with those of the normal and the control groups. The level of GSH were significantly increased in spleen cell of 52, 68w-HY group and in pancreas cell of 68w-HY group compared with those of the normal and the control groups. The level of MDA were significantly decreased in pancreas cell of 68 w-HY group compared with those of the normal and the control groups. The level of NO were significantly decreased in spleen cell of 68 w-HY group, pancreas cell of 52, 68w-HGD groups compared with those of the control groups. According to results, HY showed anti-oxidant effect. Investigation into the clinical use of the HY is suggested for future research.

태음인 소음인 환자의 여드름 치험 2례

1. Objective To review the effectiveness affecting to acne patients by Yeoldahanso-tang(熱多寒少湯) and Hyangsayangyi-tang(香砂養胃湯) who were constitutionally typed as Taeeumin and Soeumin 2. Methods The patient's subjective and objective symptoms were observed. After treatment's we compared the photos before and after, and estimated by Korean Acne Grading System(KAGS-2) 3. Results After we treated two cases of acne patient with constitutional prescription, their symptoms and result of KAGS-2 improved 4. Conclusions This case study showed an effectiveness of using Sasang Constitutional prescription in Taeeumin and Soeumin to Acne.
