        백서(白鼠) 편측(片側) 신장절제(腎臟切除) 후(後) 좌신(左腎)과 우신(右腎)의 조직학적(組織學的) 및 전자현미경적(電子顯微鏡的) 관찰(觀察)과 형태계측학적(形態計測學的) 비교(比較) 연구(硏究)

        This study examined the histomorphomeric and histological changes of the left and right kidney in uninephrectomized rat. The results were as follows: 1. In the control, the right kidney was more prominent than the left in the basement membrane of glomerular capillaries. The podocyte had well developed Golgi apparatus in the left kidney and rough endoplasmic reticulum in the right kidney. 2. At the 30 days after unilateral nephrectomy, the basal lamina of glomerular capillaries was prominently thickened in the right kidney. The cytoplasm of the podocyte of the left kidney was markedly increased and had free ribosomes, developed Golgi apparatus and rough endoplasmic reticulum. 3. At the 30 days, the section of the glomeruli were more enlarged in the left kidney than in the right. 4. At the 20 day, the nuclear section of the podocytes were markedly enlarged in the right kidney, but those of the left kidney were diminished. The mitochondrial section of the podocytes were prominently increased in the right kidney. 5. The nuclear section of the parietal layer lining cells was no significant change in the right kidney. That of the left kidney was increased at the 20 days and decreased at the 40 days. The nuclear section of glomerular endothelium of the left kidney increased earlier than the right. 6. In the morphometry of the control kidney, the section areas, long and short diameters, the nuclear section, the mitochondrial section of the proximal tubule cells, and the changes of those were more large in the right kidney than in the left. 7. The luminal secretory vesicles and peroxisomes of the left kidney were more than the right at the 20 days. The increase of mitochodrial section in the proximal tubule cells of the left kidney was more prominent than the right. The large cytoplasmic vacuoles were more prominent in the left kidney than in the right. 8. The thickness of cytoplasm and brush border was more thick in the control left kidney than in the control right. The change of cytoplasmic thickness of the left kidney was increased earlier than in the right and both kineys were increased in the thickness of brush border at the 30 days.

        토끼 신장의 근위곱슬세관과 근위곧은세관에 존재하는 Na-α-ketoglutarate Cotransport System의 생화학적 특성

        Purpose: alpha-Ketoglutarate (αKG), a Krebs cycle intermediate, is extensively used in the kidney as a fuel substrate and as a counter anion for organic acid secretion. It is known to be taken up by the proximal tubule cells via the brush-border as well as basolateral membranes. We explored biochemical characteristics of the brush-border and basolateral αKG transport systems in pars convoluta and pars recta of the proximal tubule, respectively. Methods: Brush-border and basolateral membrane vesicles (BBMV and BLMV) were isolated from rabbit renal outer cortex and outer medulla by Percoll gradient centrifugation. Vesicular uptake of αKG was determined by rapid Millipore filtration method using α-14[C]KG as a substrate. Results: Both BBMV and BLMV showed a Na-gradient dependent uphill transport of αKG. The systems in both membranes were similarly inhibited by Li and activated by Na (Hill coefficient of 1.4). Kinetic analyses indicated that the Na-αKG cotransporters in the BBMV had a lower substrate affinity as compared with those in the BLMV. The transport systems in BLMVs showed a similar Km but different Vmax between the outer cortex (Km: 34 uM, Vmax: 3.3 nmol/mg protein/10s) and outer medulla (Km: 37, Vmax: 1.8). On the other hand, the systems in BBMVs were different in both Km and Vmax between the outer cortex (Km: 194, Vmax: 3.3) and outer medulla (Km: 89, Vmax: 1.7). Conclusion: The findings suggest that both axial and apical to basolateral heterogeneity of the Na-αKG cotransport system in proximal tubules may be due to a physiological adaptation to efficiently utilize αKG in the kidney.

        CUBN mutation: a benign genetic cause of proteinuria?

        Proteinuria is an important risk factor for renal and cardiovascular disease. It is associated with a risk for glomerulonephritis, chronic kidney disease, and end-stage renal disease. Therefore, if persistent proteinuria is detected, kidney biopsy is considered to diagnose and treat the underlying disease. Recently, variants in the cubilin (CUBN) gene, which is associated with albuminuria, have been reported. This gene encodes cubilin, a membrane glycoprotein receptor expressed in the renal proximal tubules. Cubilin is a component of the megalin and cubilin-amnionless complex that mediates albumin reabsorption into the proximal tubules through endocytosis. A defect in cubilin leads to a reduction in albumin reuptake, resulting in albumin-dominant proteinuria. Although numerous controversies exist, several reports suggest that cubilin defects lead to proteinuria with a high portion of albuminuria but may not impair renal filtration function. If albuminuria due to reduced cubilin function is confirmed as a benign condition, we can consider using genetic studies to detect CUBN mutations in patients with proteinuria and they may not require any treatment or kidney biopsy. Here, we review recent papers on CUBN mutations and discuss the prognosis and management of individuals with this mutation.

      • Testosterone이 토끼 근위 세뇨관 상피세포의 성장에 미치는 영향

        Testosterone이 serum-free medium에서 배양한 토끼의 신장 근위세뇨관 상피세포의 세포성장과 기능에 미치는 영향을 관찰한 바 다음과 같은 결과를 얻었다. 1. 토끼의 신장 근위세뇨관 상피세포는 testosterone 1 nM의 농도에서 유의한 세포 성장 촉진 효과를 나타내었고, testosterone 10 nM이상의 농도에서는 세포성장이 억제되었다. 2. Testosterone은 serum-free medium에서 성장촉진인자의 하나인 hydrocortisone을 growth supplement로 넣어준 serum-free medium에서 토끼 신장의 근위세뇨관 상피세포의 성장을 촉진시키었다. 3. Testosterone은 hydrocortisone을 growth supplement로 넣어준 serum-free medium에서 토끼 신장의 근위세뇨관 상피세포의 성장을 촉진시키었다. 4. Testosterone은 Northern blot analysis에 의하여 확인한 토끼 신장의 근위 세뇨관 상피세포의 β-actin mRNA level은 증가되었다. 이상의 결과로 미루어 보아, serum-free 그리고 hormonally defined media에서 testosterone이 토끼의 신장 근위세뇨관 상피세포의 성장 및 기능에 대하여 촉진적으로 작용하는 것은 cellular mecrofilament의 중요한 구성단백의 하나로 밝혀진β-actin의 합성 증가에 기인하는 것으로 생각된다. In order to examine the effect of testosterone of the cell growth, using a primary rabbit kidney proximal tubule cell culture system, we observed the effect of 3 growth factors and testosterone supplementation on the growth of primary rabbit kidney proximal tubule cells in the serum-free medium. 1 nM of testosterone showed a potentiation of the effect on the growth of the proximal tubule cell in serum-free medium, but higher concentration (>10 nM) of testosterone indeed inhibited the growth. In the absence of hydrocortisone as a growth supplement in serum-free medium, testosterone caused to potentiate the growth of the cell. In the presence of hydrocortisone, testosterone also potentiated the grwoth of the proximal tubule cells. According to the Northern analysis, testosterone increased significantly the level of β-actin mRNA in proximal tubular cells of rabbit kidney. Consequently we may suggest that growth stimulatory effect of testosterone on the primary rabbit kidney proximal tubule cell in serum-free and hormonally defined media ascribed to increase the synthesis of β-actin, which is an important protein consisting of cellular microfilament.

        The recent WHO classification has recognized mucinous tubular and spindle cell carcinoma (MTSCC) as a distinct entity of renal cell carcinoma, exhibiting a mixed pattern of tubules and a surrounding spindle cell proliferation within a myxoid stroma, with low-grade nuclear features. A 51-year-old woman had an incidentally discovered renal mass. Radiologic examination revealed a large, well defined mass in the lower pole of the right kidney; a right radical nephrectomy was performed. Imprint cytologic smears from fresh surgical specimens showed cellular, cohesive clusters with thick, broad trabecular arrangements and branching structures. On high power fields, the tumor was composed of round-to-oval low-grade nuclei with vesicular chromatin and small nucleoli. The tumor cells had indistinct borders and pale, eosinophilic cytoplasm, In some areas, round-to-elongated tubular structures and spindle cell patterns were noted. Chronic inflammatory cell infiltration was noted, along with a mucinous back-ground and occasional psammoma bodies. Neither significant cytologic atypia nor mitosis was seen.

        Effects of Human Adipose-Derived Stem Cells in Regenerating the Damaged Renal Tubular Epithelial Cells in an Animal Model of Cisplatin-Induced Acute Kidney Injury

        Background: We conducted this experimental study to examine whether human adipose-derived stem cells (ADSCs) are effective in achieving a recovery of damaged renal tubular epithelial cells in an animal model of cisplatin-induced acute kidney injury using rats. Methods: To examine the in vitro effects of ADSCs in improving nephrotoxicity, we treated mouse renal tubular epithelial cells with both ADSCs and cisplatin mouse renal tubular epithelial cells. And we equally divided 30 male white Sprague-Dawley (SD) rats into the three groups: the control group (intraperitoneal injection of a sterile saline), the cisplatin group (intraperitoneal injection of cisplatin) and the ADSC group (intraperitoneal injection of cisplatin and the hADSC via the caudal vein). At five days after the treatment with cisplatin, serum levels of blood urine nitrogen (BUN) and creatinine were measured from each SD rat. We performed histopathologic examinations of tissue samples obtained from the kidney. Results: The degree of the expression of TNF-${\alpha}$ and that of Bcl-2 were significantly higher and lower respectively, in cisplatin group (P<0.05). Serum levels of BUN (P=0.027) and creatinine (P=0.02) were significantly higher in cisplatin group. On histopathologic examinations, there was a significant difference in the ratio of the renal injury between cisplatin group and ADSC group (P=0.002). Conclusion: The ADSCs might have a beneficial effect in regenerating the damaged renal tubular epithelial cells.

        Cryoablation for Endophytic Renal Cell Carcinoma: Intermediate-Term Oncologic Efficacy and Safety

        Purpose: To evaluate the efficacy and safety of laparoscopic renal cryoablation (LRC) of small endophytic renal cell carcinoma, for which surgical treatment is technically difficult. Materials and Methods: We enrolled patients with endophytic tumors from a prospectively collected database of 45 renal tumors in 39 patients who had undergone LRC from June 2005 to May 2009. An endophytic tumor was defined as less than 40% of the lesion extending off the surface of the kidney. We evaluated surgical and oncological outcomes. Results: Among the treated tumors, 17 tumors (37.8%) were defined as endophytic tumors and 15 tumors from 14 patients were confirmed as renal cell carcinoma (RCC) in the pathologic examination of the tissue biopsy that was conducted at the time of LRC. The mean American Society of Anesthesiologists (ASA) score of the whole patient group was 2.9 (range, 1-4), and 85.7% (12/14) of the patients had an ASA physical status score over 3. The mean tumor size was 2.8 cm (range, 1.7-3.7 cm). The layout of the cryoprobe was carefully planned preoperatively on the basis of radiologic evaluation in all tumors. Multiple cryoprobes (mean, 3.2; range, 2-5) were used. No major complications, including open surgical conversion and nephrectomy due to bleeding, occurred. No patient experienced clinical symptoms of collecting system injuries. During the mean follow-up of 32.6 months (range, 12-51 months), radiologic evidence of tumor recurrence was found in one patient (6.7% for RCC). With the exception of this patient, all other patients have remained free of recurrence or metastasis, as determined by periodic radiologic workups. Conclusions: In this series of patients with intermediate-term follow-up, LRC for endophytic renal cell carcinoma showed acceptable oncological and surgical outcomes without sequelae in the collecting system.

        Tubulopathy: the clinical and genetic approach in diagnosis

        Remarkable advances in genetic diagnosis expanded our knowledge about inherited tubulopathies and other genetic kidney diseases. This review suggests a simple categorization of inherited tubular disease, clarifies the concept of autosomal dominant tubulointerstitial kidney disease (ADTKD), and introduces novel therapies developed for tubulopathies. Facing patients with suspicious tubular disorders, clinicians should first evaluate the status of volume and acid-base. This step helps the clinicians to localize the affected segment and to confirm genetic diagnosis. ADTKD is a recently characterized disease entity involving tubules. The known causative genes are UMOD, MUC1, REN, and HNF1β. Still, only half of ADTKD patients show mutations for these four identified genes. Whole exome sequencing is a suitable diagnostic tool for tubulopathies, especially for ADTKD. Genetic approaches to treat tubulopathies have progressed recently. Despite the practical obstacles, novel therapies targeting inherited tubulopathies are currently in development.
