TSH-α is a subunit of the heterodimeric hormone TSH that is mainly secreted from the pituitary gland and acts on the thyroid glands. Although expression and promoter-methylation profiles of the gene have been observed in a few tissue types, no observation has been reported in the thyroid gland per se. To reveal the involvement of TSH-α in the thyroid cancer, we examined its expression in both the normal and cancer tissues of thyroid. As a result, TSH-α was repressed in the normal thyroid tissue and de-repressed in the cancer. Next, to identify the epigenetic mechanism of up-regulation the methylation status of the seven CpG sites in the TSH promoter between -238 and +59 was examined in sixty thyroid cancer tissues. The overall methylation percentages of the CpG sites were 51% in the thyroid cancer and 40% in the normal tissues indicating that DNA from cancer exhibited higher average methylation. These findings indicate that TSH was expressed in the thyroid cancer tissue per se in an autocrine manner and the expression can be affected by promoter methylation.

Objective: This study aimed to investigate the difference in micronutrient levels between oral feeding and enteralfeeding in chronic stroke patients to assess the risks of enteral feeding. Methods: Patients with chronic stroke who were admitted to the Department of Rehabilitation Medicine betweenJanuary 2011 and June 2012 were enrolled. The serum concentrations of iron, copper, zinc, folate, and vitamin B12,as well as the absolute CD4 and CD8 lymphocyte counts, were assessed. Results: Of the 73 patients enrolled in this study, 50 were fed orally, while the other 23 were fed through a percutaneousendoscopic gastrostomy (PEG) or nasogastric (NG) tube. The serum concentrations of vitamin B12 and folatewere significantly higher in the enteral feeding group than in the oral feeding group. However, the serum concentrationof zinc was significantly lower in the enteral feeding group. Conclusion: There is little difference between enteral feeding and oral feeding in terms of micronutrient provisionexcept that the serum concentration of zinc in the enteral feeding group was significantly lower than that in the oralfeeding group. Clinicians should recognize that chronic stroke patients who require tube feeding have a risk ofmicronutrient deficiency. Early detection of malnutrition and micronutrient deficiency is important for providing thenecessary nutrients.
Drug Reaction with Eosinophilia and Systemic Symptom Syndrome Induced by Lamotrigine

Drug reaction with eosinophilia and systemic symptom (DRESS) syndrome is a type of severe adverse drug-induced reaction. Dermatologists should make a quick diagnosis and provide appropriate treatment for DRESS syndrome to re-duce mortality rates, which can be as high as 10%. We pres-ent the case of a 47-year-old man with schizoaffective dis-order treated with lamotrigine who developed DRESS syn-drome to emphasize the importance of close observation of patients with drug eruption. He was consulted for eryth-ematous maculopapular rashes on the trunk that developed 3 weeks after starting lamotrigine. A few days later, he devel-oped generalized influenza-like symptoms. The skin rashes spread over his entire body, and the sense of itching was rap-idly aggravated within a few days. Increased liver enzyme levels and significant eosinophilia were found on laboratory test results. His condition was diagnosed as DRESS syn-drome, and he was treated with systemic and topical cortico-steroids for 2 weeks. (Ann Dermatol 29(2) 206∼209, 2017)
