        A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

        Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum.         빅데이터 기반 폭염 영향 지역의 공간적 탐색

        지속적인 기후변화와 도시공간의 변화로 인해 빈도와 강도가 강해지고 있는 폭염의 부정적 영향 저감을 위해서는 영향에 대한 진단과 관련 연구의 수행이 필요하다. 국내에서는 2018년 이례적인 극한기상의 발생 이후 폭염에 대한 관심의 증가로 많은 연구가 수행되었으나, 폭염이 도시민의 생활과 활동에 미치는 영향에 관한 연구는 매우 부족한 실정이다. 이에 본 연구는 폭염의 발생이 도시민의 활동에 미치는 영향을 실증하고, 공간적으로 탐색하고자 하였다. 여름철 인구 빅데이터를 활용하여 폭염 발생일과 대조일 간 활동인구 변화를 대응표본 검정을 통해 통계적으로 검증하였으며, 이들 변화의 지역적 차이를 GIS와 공간군집분석을 활용해 지역 간 차이를 시각화하여 분석하였다. 통계분석 결과 시기에 따라 차이는 있으나 폭염의 발생은 도시민의 활동을 감소시키는 경향을 보였다. 지역 간 활동인구의 변화는 차이를 보였으며, 군집분석 결과 폭염의 발생에도 활동인구가 증가하는 핫스팟은 평일 마포구와 송파구, 강남구 일대, 주말 관악구, 강서구, 은평구 일대에서 나타났다. 반대로 콜드스팟은 평일 서대문구, 강북구, 성북구, 종로구, 중구 일대에서 뚜렷하게 나타났으며, 주말 종로구와 중구, 서초구와 강남구 일대에서 넓게 도출되었다. 이러한 분석 결과는 지역의 특성을 반영한 폭염 대응정책 마련을 위한 기초자료로 활용될 수 있을 것이다. With the increase in frequency and intensity of heatwaves due to continued climate change and changes in urban spaces, research is needed to reduce their negative impacts. In Korea, several studies have been conducted on the subject since the unusual occurrence of extreme weather in 2018; however, as few have focused on the effects of heatwaves on the lives and activities of urban residents, this study investigates these effects and explores their spatial aspects. Using population big data, the changes in the active population on days when heatwaves occurred were statistically verified through a paired-samples test. Additionally, regional variations in changes were analyzed using GIS and spatial cluster analysis. Despite a slight temporal variation, the statistical analysis showed that the occurrence of heatwave is associated with decreased activity among urban residents. There was a difference in the active population between regions, and cluster analysis revealed that some hot spots saw increased activity in the population despite the occurrence of heatwaves, including Mapo-gu, Songpa-gu, and Gangnam-gu on weekdays and Gwanak-gu, Gangseo-gu, and Eunpyeong-gu on weekends. Conversely, the cold spots found included Seodaemun-gu, Gangbuk-gu, Seongbuk-gu, and Jongno-gu on weekdays and Jongno-gu, Jung-gu, Seocho-gu, and Gangnam-gu on weekends. These results can serve as basic data to prepare a heatwave response policy that reflects the characteristics of the region.

        양주 노고산(老姑山) 독재동(篤才洞)의 장소성과 역사문화경관 해석

        본 연구는 경기도 양주시 장흥면 삼하리 독재동 바위글씨와 노고산의 장소성에 주목하여, 이곳의 경관 및 장소특질을 고찰하고 바위글씨에 담긴 경관의미를 추론함으로써 노고산과 독재동 계곡의 역사문화경관적 함의를 밝히고자 한 것으로 연구 결과는 다음과 같다. 삼각산은 조선왕조 6백년 도읍지의 주산(主山)이며 오악(五嶽)을 이루는 중앙의 성산(聖山)으로, 노고산은 삼봉산(三峰山)즉 삼각산의 우람한 세봉우리를 향배하는 의미공간으로서 그 장소성이 획득된다. 더불어 노고산은 조선의 정기를 상징하는 삼각산을 가장 완벽하게 조망할 수 있는 ‘망삼각산(望三角山)’의 절대 시점장이다. 독재동 계곡에 산포(散布)된 다수의 바위글씨는 미수 허목의『고양산수기』의 내용을 현현(顯現)하기 위한 행위이자 경물로 재생산된 산물이다. 그리고 추사 필적의 몽재(夢齋)는 중의적(衆意的) 표현이 읽혀지는 바위글씨로, 먼저 글자 그대로의 뜻인 ‘꿈꾸는 집’ 이지만 내면적 의미는 “한낮 몽상(夢想)도 바탕을 깊이 헤아려 보면 정당성이 확보된다.”는 의미로 ‘몽재거사’로 알려진 성재(性齋) 허전(許傳)의 『몽재집(夢齋集)』서(序)의 내용으로 보아 성재의 독재동 거주와 관련된 징표로 추정된다. 또한 몽재라 쓴 바위 하단의 갑자(甲子)·경오(庚午)·무술(戊戌)·갑인(甲寅) 등의 간지(干支)는 추사와의 교유관계가 있는 1804년 5월생 유생(儒生)의 사주팔자로 판단된다. 노고산과 독재동 계곡은 삼각산을 조망하는 장소일 뿐 아니라 송월재·성재 등 미수의 학통을 계승한 기호지역의 남인학자의 공통된 장구지소로써의 응집력을 보여준다. 또한 이곳에 각자(刻字)된 추사체와 미수체(전서체) 바위글씨는 우리나라 서예사의 큰 흐름을 함축적으로 보여준다는 측면에서도 매우 의미 깊은 역사문화경관이다. This study was focused on the placeness of Nogosan Dokjae-dong and rock inscription located in Jangheung-myeon, Yangju-si, Gyeonggi-do. The purpose was to reveal the implication of Nogosan and Dokjae-dong Valley in terms of Historic cultural landscape by considering the characteristics of the place and its landscape, and inferring the significance of the letters engraved on the rock. The result is as follows.10)Samgaksan is one of the five famous mountains in Korea. It is the sacred mountain that stands behind the capital city of the Joseon Dynasty that lasted for 600 years. On the other hand, the placeness of Nogosan is significant since it was the place where people used to bow low to the three great summits of Samgaksan. Furthermore, Nogosan was the perfect place to view Samgaksan that represented the Joseon spirit.Many letters were engraved on the rocks around Dokjae-dong Valley by Misu(眉수) Heo Mok(許穆) as part of his memories to describe his experience in the mountain and became a scenery. Chusa(秋史) Kim Jung-Hee(金正喜) also engraved letters of ``Mongjae`` on the rock that contains a double meaning. Literally, it means ‘a dreaming house’ However, it also implies that even just a daydream is significant in a way. These letters are presumed to be the signs that are related to Sung-Jae(性齋) Heo Jeon(許傳)’s experience in Dokjae-dong since the letters were part of his famous book title. Below these two letters, Chusa also engraved letters that describe the sexagenary cycle including the year of the rat, horse, dog, and tiger. They are regarded as the letters that describe the fate of Chusa’s friend, a scholar born in May 1804. Nogosan and Dongjae-dong Valley are very significant in terms of historic cultural landscape as a viewpoint to see Samgaksan; and a place that cherishes the traces of Misu and shows common values and cohesiveness of well-known scholars of the Joseon Dynasty including Song Wol-Jae(松月齋) and Sung-Jae as a of Gihonamin(畿湖南人). The engraved letters of Chusa and Misu also implicitly demonstrates the major trends of Korea’s calligraphy history.

        The Effects of Heat Application on the Immune Activities of the Human Body

        The purpose of this study was to determine the effects of heat appli˗ cation on the immune activities of the human body. To exam, further˗ more, the immune effect from the healthy volunteer(male:15, female:15) by monitoring changes of immune substances such as various leukocytes[total white blood cell(WBC), eosinophil, neutrophil, basophil, monocyte, and lymphocyte], a comparative study with warm water immersion(40.8±0.3℃) and infrared(250W) was carried out. The plasma analysis showed that the count of white blood cell, eosinophil, and neutrophil were elevated in warm water immersion- or infrared˗ stimulated group compared with control group. However, the count of basophil was decreased in both warm water immersion- and infrared-stimulated group than control group. Therefore, these results suggest that the thermostimulation improved immune activity.

        Effects of a chemical chaperone on genetic mutations in α-galactosidase A in Korean patients with Fabry disease

        Fabry disease is an X-linked inborn error of glycosphingolipid catabolism that results from mutations in the gene encoding the α-galactosidase A (GLA) enzyme. We have identified 15 distinct mutations in the GLA gene in 13 unrelated patients with classic Fabry disease and 2 unrelated patients with atypical Fabry disease. Two of the identified mutations were novel (i.e., the D231G missense mutation and the L268delfsX1 deletion mutation). This study evaluated the effects of the chemical chaperones 1-deoxygalactonojirimycin (DGJ) on the function of GLA in vitro, in cells containing missense mutations in the GLA gene. Nine missense and a nonsense mutations, including one novel mutation were cloned into mammalian expression vectors. After transient expression in COS-7 cells, GLA enzyme activity and protein expression were analyzed using fluorescence spectrophotometry and Western blot analysis, respectively. DGJ enhanced GLA enzyme activity in the M42V, I91T, R112C and F113L mutants. Interestingly, the I91T and F113L mutations are associated with the atypical form of Fabry disease. However, DGJ treatment did not have any significant effect on the GLA enzyme activity and protein expression of other mutants, including C142W, D231G, D266N, and S297F. Of note, GLA enzyme activity was not detected in the novel mutant (i.e., D231G), although protein expression was similar to the wild type. In the absence of DGJ, the E66Q mutant had wild-type levels of GLA protein expression and approximately 40% GLA activity, indicating that E66Q is either a mild mutation or a functional single nucleotide polymorphism (SNP). Thus, the results of this study suggest that the chemical chaperone DGJ enhances GLA enzyme activity and protein expression in milder mutations associated with the atypical form of Fabry disease.         The Effects of NEES on PARP Expression and Cell Death in Rat Cerebral Cortex After Ischemic Injury

        The majority of strokes are caused by ischemia and result in brain tissue damage, leading to problems of the central nervous system including hemiparesis, dysfunction of language and consciousness, and dysfunction of perception. The purpose of this study was to investigate the effects of Poly(ADP-ribose) polymerase(PARP) on necrosis in neuronal cells that have undergone needle electrode electrical stimulation(NEES) prior to induction of ischemia. Ischemia was induced in male SD rats(body weight 300g) by occlusion of the common carotid artery for 5 min, after which the blood was reperfused. After induction of brain ischemia, NEES was applied to Zusanli(ST 36), at 12, 24 and 48 hours. Protein expression was investigated using immuno-reactive cells, which react to PARP antibodies in cerebral nerve cells, and Western blotting. The results were as follows: In the cerebral cortex, the number of PARP reactive cells after 24 hours significantly decreased(p<.05) in the NEES group compared to the GI group. PARP expression after 24 hours significantly decreased(p<.05) in the NEES group compared to the GI group. As a result, NEES showed the greatest effect on necrosis- related PARP immuno-reactive cells 24 hours after ischemia, indicating necrosis inhibition, blocking of neural cell death, and protection of neural cells. Based on the results of this study, NEES can be an effective method of treating dysfunction and improving function of neuronal cells in brain damage caused by ischemia.

        The Synergism between Belotecan and Cisplatin in Gastric Cancer

        Purpose: We wanted to demonstrate the anti-cancer effect and interaction between belotecan and cisplatin on gastric cancer cell line and we evaluated the mechanisms of this synergistic effect in vitro. Materials and Methods: The growth inhibitory effect of belotocan and cisplatin against several gastric cancer cell lines (SNU-5, SNU-16 and SNU-601) was estimated by tetrazolium dye assay. The effect of a combination treatment was evaluated by the isobologram method. The biochemical mechanisms for the interaction between the drugs were analyzed by measuring the formation of DNA interstrand cross-links (ICLs) and DNA topo-I activity.Results: Belotecan showed synergism with cisplatin for growth inhibitory effect on the gastric cancer cell lines SNU-5, and SNU-16, but this was subadditive on the SNU- 601 cell line. The formation of DNA ICLs in SNU-16 cells by cisplatin was increased by combination with belotecan, but this was not affected in SNU-601 cells. The topo-I inhibition by belotecan was enhanced at high concentrations of cisplatin in SNU-16, but not in SNU-601 cells.Conclusion: Belotecan and cisplatin show various combination effect against gastric cancer cells. The synergism between cisplatin and belotecan could be the result of one of the following mechanisms: the modulating effect of belotecan on the repair of cisplatin-induced DNA adducts and the enhancing effect of cisplatin on the belotecan-induced topo-I inhibitory effect.
