        혼수상태의 외상환자에서 초음파를 이용한 흉부 및 복부손상의 진단

        The need for rapid diagnosis and treatment of life-threatening thoracic and intra-abdominal injury result in controversy over the appropriate triage of unconscious blunt trauma patients. To aid in early decisions for these patients, a prospective analysis of 98 patients with glasgow coma scale(GCS) scores≤8 was undertaken. Although intra-thoracic and intra-abdominal injuries were frequently identified based on systolic blood ressure, the use of clinical signs alone resulted in more missed injuries than did using the emergency ultrasonography. In normotensive patients(n=34), intra-thoracic injuries was identified in one patient(2.9%), and intra-abdominal injuries were in 7 patients(20.6%). In shock(systolic blood pressure<90mmHg) Pa-tients(n=64), thoracic injuries and intra-abdominal injuries were diagnosed in 8(12.5%) and 27 patients(42.3%), thoracic and abdominal combined injuries were identified in 11 patients(17.2%). The sensitivity, specificity, and accuracy of emergency ultrasonography were 96.3%, 91.7%, 94.9%. This study suggests that all unconscious trauma patients undergo immediate emergency ultrasonography to prevent missing life-threatening injuries.

        다발성 외상시 응급 초음파검사의 의의

        The Evaluation of blunt abdominal trauma is clinically challenging. Abdominal computed to-mography(CT) and diagnostic peritoneal lavage(DPL) has become primary diagnostic modali-ty in advanced trauma life support(ATLS). But DPL is invasive study with potential for the patients. Monitoring and resuscitation are usually suboptimal during there examinations. We examined the efficacy and role of emergency ultrasonographic(US) studies in the initial abdominal evaluation of blunt trauma patients. A prospective study was carried out on 101 blunt trauma patients from 1994, June to September. 101 patients underwent real-time US ex-amination of the chest and abdomen. Indication for emergency US included the cases of shock (systolic blood pressure<90mmHg) or tachycardia (pulse rate/min.>100) in blunt trauma pa-tients. US showed a sensitivity of 96.2%, specificity 98%, accuracy 97% in regard to indica-tion for surgery in case of blunt abdominal or thoracic trauma. US saves time, and it can be performed during the emergency care in the emergency department. US showes high sensitivity and specificty, and is the method of first choice in the evaluation of blunt trauma.

        This paper describes a simple and straightforward method for determining the minimum capacitance required to self excitation in a three-phase induction generator with Steinmetz connection. Using the input impedance of the induction generator and the self-excitation condition, the capacitance is computed by real and imaginary function in an application software. This solution method is applied to determine the capacitance required to maintain the specified terminal voltage when the generator is supplying load. The computed results are compared with Chan and Lai's results to verify the proposed method.

        As the most 0f informations have time constraints, user must acquire necessary information within the time constraints. To provide information service with time constraints, it is required to support realtime information system including the retrieval of distributed information and the update of the distributed information whenever authorized users try to do in realtime. In this paper. We propose internet information retrieval system model which is applicable on regional information. And we design and implement a realtime controlled search engine for the regional information network. Also, an intelligent robot gathering and updating widely distributed information is implemented. In order to improve the performance of the whole system, a realtime robot control policy is applicable by monitoring the loads of network and a web server on the basis of a statistical model.

      • 최적 컴파일러를 위한 Loop 구조내의 벡터화

        본 논문에서는 루프내의 문장들을 벡타화 시키기 위해 그에 대한 자료종속 테스트를 거쳐 문장들간의 자료종속 싸이클 제거를 위해 종속 제거 알고리즘을 구성 실험하였다. 종속제거를 위한 방법으로는 문장재배열(Statement Reordering)방법, 문장 치환(Statement Substitution)방법, 루프분해(Loop Fission)방법을 적용시켰다. 위에서 살펴 본 기법을 적용하더라도 자료종속 싸이클이 완전히 제거되는 것은 아니지만 루프 내의 자료종속 관계를 많이 감소시키는 역활을 하였고 수행속도에도 많은 영향을 미침을 알 수 있었다. 그리고 벡터화에도 많은 기여를 하게 되리라 믿는다. 위의 기법 외에도 자료 종속 싸이클이 자료흐름, 역, 출력종속의 조합으로 나타날 때 적용할 수 있는 기법이 있다. 자료흐름과 역종속 싸이클이 존재할 때, "인덱스 집합 분활(index set splitting)"을 적용할 수 있고 자료 역종속으로 생긴 싸이클에는 "변수 복사(varialble copying)"등을 적용할 수 있다. In this study a dependence checker is designed witch checks dependence relationship between sentences for the process to vectorize the senctences in the loop. Vectorizing translator accepts programs written in a Fortran language for serial computer and translates them into using the vector instructions of the target machine. If there exists a data dependency relation in the loop of a program, then the translator can attept several transformation on the statement in the loop to vectorize the Loop The data dependence graph can used to decide with transformation should be applied in the vectorization process. This paper presents the vectorization of the statenents in the loop through the transformation by data dependency cycle elimination methods.

      • 객체지향 방법론을 이용한 자동차부품기업의 영업관리시스템 설계 및 구현

        According to the increase of organizational complexity and the change of rapid information technology environment, many firms have shifted their information technology(IT) strategy from developing information systems in-house to purchasing application software such as Enterprise Resource Planning(ERP) Systems. Marketing and Sales functions within a Korean automotive part company include developing new assembly products, determining pricing, taking customer's orders, and shipping assembly products to customers. Marketing and Sales Information System(MSIS) in ERP system plays an important role in next Production Planning process. MSIS also makes management reporting and decision making faster and more uniform throughout an organization. MSIS promotes thinking about corporate goals, as opposed to thinking only about the goals of a single department or functional area. This paper intends to design and implement a MSIS in ERP systems for Korean automotive part company using object-oriented methodology. In order to accomplish the implementation of MSIS in ERP system, we employed UML as its standard modeling language. In this study, four diagramming techniques such as use case diagram, sequence diagram, class diagram, component diagram among eight modeling techniques are used for analyzing hierarchical business process. In traditional marketing and sales function, a company with an unintegrated information system can have marketing and sales data that is data redundant or inaccurate. MSIS integrated in ERP system can solve the sales forecast problem, which minimizes the total costs of production, inventory, and transportation under constraints of production capacity. Also, the use of UIVIL methodology makes S/W programmers shorten the phase of analysis and design in the implementation of MSIS system, and increase the reuse of software and the interoperability with corporate internal information system.

      • 진성 수소화된 비정질 규소에서의 급냉효과

        In this work, the rapid cooling effects were investigated for the hydrogenated amorphous silicon (a-Si : H) films prepared by decomposition of pure silane. In general, the rapid cooling of doped a-Si : H usually produced more conductive states(σ_(FC)) than slowly cooled(annealed) state(σ_(FC)). Undoped a-Si : H, however, showed even the state of σ_(FC)<σ_(SC) as well as the state of σ_(FC)>σ_(SC). The equilibrium temperature, T_(E), in which samples are reached to thermal equilibrium within few minutes linearly increased with T_(s) which is the most importtant parameter to control the hydrogen content(C_(H)) of samples. In order to interpret our data in undoped a-Si : H, we proposed a model based on the hydrogen-glass and weak bond-dangling bond conversion model which had already been proposed for doped a-Si : H.

      • 차량의 GPS 동적 측량에 의한 데이터 정확도 비교

        GPS developed for the strategic equipment by U.S. Deparment of Defence is a navigation satellite system alternative of Navy Navigation Satellite System. So it is not introduced to public. Otherwise, it is used variously in many fields as well as high accuracy surveying network and acquirment of geo-spatial data. The GPS system has been used in static surveying and specialist used its equipment. It is very difficult for unspecialist to use. The GPS system is used to the Car Navigation system and being changed easy equipment for everyone. The GPS system of dynamic surveying is known about 100m accuracy. In this study, it is compared to three GPS system (Pathfinder, Garmin, Svee six) navigated along the road from Pusan Univ. to Haeundae. According to the study, expensive equipment was surveyed to DGPS principle and was very good to acquracy. But, it is able to be processed to real time DGPS. It is difficult to survey by using expensive equipment in the Car Navigation System. Thus, accuracy of the GPS equipment is not higher than it can process datum to real time, the development of software is suitable to make Car Navigation System GPS는 종래의 위성항법체계였던 NNSS와 교체된 새로운 항법시스템으로써 미국방성의 전략장비로 개발되었다. 일반인들에게는 공개되지 않았으나, 현재는 전세계적으로 고정밀 측지망 구성 및 지형정보의 취득에 필수적 장비인 동시에 다양한 분야에서 범용적으로 사용하고 있다. 또한 GPS장비는 정적의 측량에 사용되어져왔기 때문에 일반인이 이용하기에는 상당히 어려웠다. 그러나, 현재에는 GPS장비가 Car Navigation System의 소개로 일반인이 쉽게 이용할 수 있는 장비로 바뀌고 있다. 일반적으로 동적 측량에 의한 GPS장비는 약 100m의 정확도를 가지고 있는 것으로 알려져 있다. 본 연구에서는 부산대학교를 출발하여 해운대까지의 도로를 따라 주행한 세 개의 GPS장비인 pathfinder와 garmin 그리고 SVee Six의 정확도를 서로 비교 고찰하였다. 연구에 따르면, 값비싼 장비는 DGPS를 원리로 측량이 되고 그 정확도도 상당히 좋았다. 그러나, 상당히 고가의 장비는 실시간으로 DGPS가 가능하다. 차량주행을 위한 시스템은 값비싼 장비로는 불가능하다. 따라서, 정확도는 조금 떨어지지만, 값싸고 실시간으로 자료를 처리할 수 있는 GPS장비와 소프트웨어의 개발이 차량주행 시스템 구성에 적당하였다.

      • 수소화된 비정질 탄소박막(a-C:H)에서의 준안정상태

        This paper presents experimental results on the electrical properties of hydrogenated amorphous carbon films(a-C:H) prepared by PECVD. DC conductance of a-C:H was measured as a function of temperatures in the range of 100 to 423K. We studied two a-C:H films: one was well explained by the Mott’s Variable Range Hopping(VRH) rule but the other sample did not follow it. However, the conduction data of second sample were well fitted to Shimakawa's Multi-Phonon Hopping(MPH) model according to which conductivity is proportional to T^(m) with m=15-17 but, in our samples, m was 10-12. Also a-C:H showed several bias effects like relaxation of conductance, bias-dependent conductance and the change of conductance slope in ln(α√T) vs. T^(-1/4) plot. In this study we interpret these data by bias-dependent detrapping model.

      • 기관지천식 환자에서 Furosemide, Disodium cromoglycate 및 Heparin 흡입이 고장액 식염수 기관지유발검사에 미치는 영향

        목적 : 기관지천식 환자에서 고장액 식염수의 흡입은 기도수축을 유발할 수 있으며 이는 운동유발성 천식반응과 밀접한 관계가 있는 것으로 알려져 있다. 저자들은 알레르겐 흡입이나 운동에 의해 유발되는 천식에 예방효과가 있는 것으로 알려진 disodium cromoglycate(DSCG), furosemide 및 heparin 흡입이 4.5% NaCl 기관지유발검사(BPT)에 미치는 영향을 조사하였다. 대상 및 방법 : 4.5% NaCl BPT에서 양성반응을 보이는 기관지천식 환자 13명을 대상으로 하였으며 사용된 약물은 furosemide 40㎎, DSCG 40㎎ 및 heparin 1,000μ/㎏이었다. 먼저baseline 4.5% NaCl BPT를 시행한 다음 이들 약물로 전처치후 다시 4.5% NaCl BPT를 시행하여 약물의 효과를 관찰하였다. 결과 : Furosemide 40㎎, DSCG 40㎎및 heparin 1,000μ/㎏의 흡입 전처치는 고장액 식염수에 의한 기도수축 반응에 뚜렷한 예방효과를 보였다. Furosemide와 DSCG로 전처치한 군(n=6)에서 이들의 기도수축 방어율은 각각 100.6±6.6%, 91.1±17.2%였으며 furosemide와 heparin으로 전처치한 군(n=7)에서는 각각 58.7±29.2%, 59.0±51.1%로서 각 군에서 이들 약제간의 방어율에는 유의한 차이가 없었다. 결론 : Furosemide(40㎎), DSCG(40㎎) 및 heparin(1000μ/㎏)의 흡입 전처치는 고장액 식염수에 의한 기도수축 반응에 뚜렷한 예방효과를 보였으며, 적어도 이 용량에서 기도수축 예방 정도에는 유의한 차이가 없음을 알 수 있었다. Background: Hypertonic saline (4.5% NaCl) inhalation is known to induce broncho-constriction by affecting mast cell, epithelial cell and vagal afferent pathway in some asthmatics. Disodium cromoglycate (DSCG) is known to have a preventive effect on allergic asthma and exercise induced asthma, and recently it was reported that furosemide and heparin had similar effect. The purpose of this study was to evaluate the effects of furosemide, DSCG and heparin on hypertonic saline provocation test in asthmatics. Methods: Thirteen asthmatics with a positive response to hypertonic saline challenge were enrolled. Hypertonic saline and test drugs were generated by ultrasonic nebulizer. After taking baseline 4.5% NaCl challenge, subjects were rechallenged with 4.5% NaCl after inhalation of furosemide 40㎎, DSCG 40㎎ or heparin 1,000μ/㎏. Results: 1. There was a significant positive relationship between PC_20-methacholine and PTM-4.5% NaCl(r=0.5575, p = 0.024). 2. Furosemide, DSCG and heparin had no direct bronchodilating effects. 3. Premedication of furosemide and DSCG(n=6) showed significant protective effects on 4.5% NaCl induced broncho-constriction. The average protection rate were 100.6±6.6% and 91.1±17.2%, respectively. 4. Premedication of furosemide and heparin(n=7) showed significant protective effects on 4.5% NaCl induced broncho-constriction. The average protection rate were 58.7±29.2% and 59.0±51.1%. respectively. Conclusions: Furosemide(40㎎), DSCG(40㎎) and heparin(1.000μ/㎏) had significant protective effects on hypertonic saline induced broncho-constriction in asthmatics, and there were no significant differences in their potency of protection rate.
