요로결석 환자의 체외충격파쇄석술 치료경과와 결석 특성과의 관련성

요로 결석에 대한 비 침습적 치료 방법인 체외충격파쇄석술에 의한 치료경과와 결석 특성과의 관련성을 파악키 위해 2002년 12월부터 2008년 3월까지 G시 H병원에서 요로 결석으로 진단되어 체외충격파쇄석술을 받은 33l례를 대상으로 의무기록 자료를 이용하여 분석한 결과는 다음과 같다. 1. 조사 대상자 331례중 남자는 68.6%, 여자는 31.4%이었으며 평균연령(표준편차)은 44.2 (13.9)세 이었고 84%가 G시내 거주자였다. 2. 조사 대상자에게 시행된 체외충격파쇄석술의 평균 시술횟수(표준편차)는 1.9(1.5)회 이었으며 평균 통증제거기간(표준편차)은 3.2(2.1)일 이었고 요로결석 진단 시로부터 치료가 완료되었다고 의사가 판정했을 때까지의 치료기간의 평균(표준편차)은 10.3(8.4)일 이었다. 3. 시술횟수에 통계적으로 유의하게(P<0.01) 관련된 요인은 결석크기, 결석의 방사선 투과성, 요관 폐색여부 이었으며 모형의 설명력은 13.5%이었다. 4. 통증제거기간에 통계적으로 유의하게(P<0.01) 관련된 요인은 결석의 방사선 투과성 이었으며 모형의 설명력은 7.4%이었다. 5. 치료기간에 통계적으로 유의하게(P<0.01) 관련된 요인은 결석크기와 결석의 방사선 투과성 이었으며 모형의 설명력은 13.5%이었다. 따라서 결석의 크기가 크거나 결석이 방사선에 비 투과성일 때, 요관이 폐색되어 있을 때 치료경과가 늦게 됨으로 이러한 사실을 참고 하여 추후 진료지침을 작성 시 보다 효과적이고 효율적인 치료법을 적용할 필요가 있다고 판단되며 보다 체계적인 평가와 연구가 필요할 것이다. Objectives & Methods: To find the treatment process by extracorporeal shock wave lithotripsy (ESWL) and its related stone characteristics in urinary tract stone patients, an analysis was done from medical records of 331 cases who were treated by ESWL from December 2002 to March 2008 at H hospital in G city. Results: Among 331 study subjects 68.6% were male and 31.4% were female. Their mean age (standard deviation, SD) was 44.2 (13.9) years old and 84.0% lived in G city. Average frequency of ESWL done toward study cases were 1.9±1.5 times. Mean duration of disappearance from pain was 3.2±2.1 days and total treatment period for a stone was 10.3±8.4 days. Statistically significant (p<0.01) stone and urinary tract characteristics related with frequency of ESWL were stone size, radio-opacity of the stone, and obstruction of urinary tract. The model composed of these 3 independent variables explained 13.5% of frequency of ESWL. Statistically significant (p<0.01) variable related with disappearance from pain was radio-opacity of the stone, and this model explained 7.4%. Statistically significant (p<0.01) variables related with treatment duration due to stone were stone size and radio-opacity of the stone. These two variables explained 13.5%. Conclusion: The bigger the size of stone are, the slower the treatment process will be. And when the stone is radio-opaque and urinary tract is obstructed, the treatment process will be slower than stone is radio-luscent and urinary tract is not obstructed. So to make effective and efficient treatment manual for ESWL, above fact must be considered and continuous systematic evaluation and research toward ESWL are necessary.
Distribution of the Fairy Pitta (Pitta nympha) in the South Korea: A focus on protected areas

국내 팔색조(Pitta nympha) 분포 현황을 알아보고자 전국 국립공원 등 보호지역 중심으로 총 19개 지역을 조사하였고, 전문가 네트워크를 통해 자료를 보강하였다. 전국 19개 지역을 조사한 결과 북한산, 지리산 등 내륙지역 7개 국립공원에서는 팔색조가 확인되지 않았고, 경남 거제, 남해, 해남, 장흥 고흥 등 남해안과 인접한 12개 지역에서 팔색조 서식이 확인되었다. 또한 전문가 네트워크를 통해 수집한 팔색조 출현 위치 정보 82건을 분석한 결과, 전국 8개 광역지자체 모두 팔색조 서식이 확인되었고, 내륙지역(26.8%)보다 해안지역(73.2%)에서 더 많이 출현하였다. 연구 결과를 종합해보면, 우리나라의 팔색조는 주로 제주도와 남해안 지역을 중심으로 많이 분포하며, 그 보다 적은 개체수가 내륙 지역에 넓게 분포하고 있는 것으로 나타났다. The present study investigated the current distribution of Fairy Pitta (Pitta nympha) in South Korea. A total of 19 study areas, including protected areas such as national and provincial parks in South Korea were surveyed, and additional data obtained from an expert network. According to the results, Fairy Pitta was absent in seven inland national parks, including Mt. Bukhan and Chiri parks. Fairy Pitta inhabited 12 areas near the southern coast of South Korea, including Geoje, Namhae, Haenam, Jangheung, Goheung, and Gyeongnam Province. The analysis results of 82 datasets obtained from experts with regard to the locations where Fairy Pitta have been spotted showed that the bird inhabits all the eight metropolitan regions in South Korea, and is more frequently spotted in coastal areas (73.2%) than in inland areas (26.8%). The findings demonstrate that Fairy Pitta is mainly distributed in Jeju Island and the southern coast of South Korea, and is distributed in inland areas at relatively lower numbers.

To predict the influence of volcano eruption on agriculture in South Korea we evaluated the dispersal ranges of the volcanic ashes toward the South Korea based on the possibilities of volcano eruption in Mt. Baekdu. The possibilities of volcano eruption in Mt. Baekdu have been still being intensified by the signals including magmatic unrest of the volcano and the frequency of volcanic earthquakes swarm, the horizontal displacement and vertical uplift around the Mt. Baekdu, the temperature rises of hot springs, high ratios of N₂/O₂ and ₃He/₄He in volcanic gases. The dispersal direction and ranges and the predicted amount of volcanic ash can be significantly influenced by Volcanic Explosivity Index (VEI) and the trend of seasonal wind. The prediction of volcanic ash dispersion by the model showed that the ash cloud extended to Ulleung Island and Japan within 9 hours and 24 hours by the northwestern monsoon wind in winter while the ash cloud extended to northern side by the south-east monsoon wind during June and September. However, the ash cloud may extent to Seoul and southwest coast within 9 hours and 15 hours by northern wind in winter, leading to severe ash deposits over the whole area of South Korea, although the thickness of the ash deposits generally decreases exponentially with increasing distance from a volcano. In case of VEI 7, the ash deposits of Daejeon and Gangneung are 1.31 × 10<SUP>4</SUP> g m<SUP>-2</SUP> and 1.80 × 10<SUP>5</SUP> g m<SUP>-2</SUP>, respectively. In addition, ash particles may compact close together after they fall to the ground, resulting in increase of the bulk density that can alter the soil physical and chemical properties detrimental to agricultural practices and crop growth.

Aims: To identify imaging characteristics that differentiate small (£2 cm) HCCs from small (£2 cm) benign nodules in cirrhotic liver on gadoxetic acidenhanced and diffusion-weighted (DW) magnetic resonance (MR) images. Methods: On gadoxetic acid-enhanced and DW MR images, we analysed signal intensity of 222 small HCCs and 61 benign nodules (diameter, 0.5-2 cm) at each sequence and rim enhancement during portal or equilibrium phases. Univariate and multivariate logistic regression analyses identified predictors of HCC. Combinations of significant MR findings in multivariate analysis were compared with American Association for the Study of Liver Disease (AASLD) practice guidelines. Results: In multivariate analysis, arterial enhancement (adjusted odds ratio [aOR], 8.6), T2 hyperintensity (aOR,5.8), and hyperintensity on DW images (aOR, 3.8) were significant for differentiating small HCCs from benign nodules (p £ 0.004). When two or all three findings were applied as diagnostic criteria for differentiating small HCCs from benign nodules, sensitivity and accuracy were significantly higher compared with AASLD practice guidelines (91% vs. 78% and 89% vs. 81%, respectively;each P<0.0001). Conclusions: On gadoxetic acid-enhanced MR imaging, arterial enhancement and hyperintensity on T2-weighted and DW MR images are helpful for differentiating small HCCs from benign nodules in liver cirrhosis.

A diazatetrathia crown ether derivative that has two appended nitrobenzoxadiazolyl moieties showed selective OFF-ON type fluoroionophoric signaling properties toward Hg2+ ions over other transition metal ions. The compound also exhibited a pronounced chromogenic behavior toward Hg2+ ions by changing the solution color from light orange to yellow, which can easily be detected with naked-eye. The detection limit for the analysis of Hg2+ ions in 90% aqueous acetonitrile was found to be 4.8??10-6 M, which suggests that the compound may be used as a chemosensor for analyzing sub-millimolar Hg2+ ions in aqueous environments.
