For small capacity rectifier circuits such as these for consumer electronics and appliances, capacitor input type rectifier circuits are generally used. Consequently, various harmonics generated within the power system become a serious problem. Various studies of this effect have been presented previously. However, most of these employ switching devices, such as FETs and the like. The absence of switching devices makes systems more tolerant to over-load, and brings low radio noise benefits. We propose a power factor correction scheme using a LC resonant in commercial frequency without switching devices. In this method, it makes a sinusoidal wave by widening conduction period using the current resonance in commercial frequency, Hence the harmonic characteristics can be significantly improved, where the lower order harmonics, such as the fifth and seventh order are much reduced. The result are confirmed by the theoretical and experiment implementations.

Background/Aims: There are few available data regarding the association between the single nucleotide polymorphisms (SNPs) of the gene encoding interleukin 28B (IL28B) and a sustained virologic response (SVR) to peginterferon (PEG-IFN) plus ribavirin (RBV) therapy in Korean chronic hepatitis C patients. Methods: This was a retrospective cohort study of 156 patients with chronic hepatitis C virus (HCV) infection who received combination treatment of PEG-IFN plus RBV. Blood samples from these patients were analyzed to identify the IL28B SNPs at rs12979860, rs12980275, rs8099917, and rs8103142. Association analyses were performed to evaluate the relationships between each IL28B SNP and SVRs. Results: Seventy six patients with HCV genotype 1 and 80 with genotype non-1 were enrolled. The frequencies of rs12979860 CC and CT genotypes were 90.4% and 9.6%, respectively; those of rs12980275 AA and AG genotypes were 87.2% and 12.8%, respectively; those of rs8099917 TT and TG genotypes were 92.3% and 7.7%, respectively; and those of rs8103142 TT and CT genotypes were 90.4% and 9.6%, respectively. Among the patients with HCV genotype 1, the SVR rates were 69.7% and 80.0% for rs12979860 CC and CT, respectively (P=0.71). Among the HCV genotype non-1 patients, SVR rates were 88.0% and 100% for rs12979860 CC and CT (P=1.00), respectively. Conclusions: Genotypes of the IL28B SNP that are known to be favorable were present in most of the Korean patients with chronic hepatitis C in this study. Moreover, the IL28B SNP did not influence the SVR rate in either the HCV genotype 1 or non-1 patients. Therefore, IL28B SNP analysis might be not useful for the initial assessment, prediction of treatment outcomes, or treatment decision-making of Korean chronic hepatitis C patients. Feasibility of Red Tide Detection Around Korean Waters Using Satellite Remote Sensing

The monitoring activities at the National Fisheries Research and Development Institute (NFRDI) in Korea have been extended to include all the coastal waters of Korea after the outbreak of Cochlodinium polykrikoides blooms in 1995. We used several alternative methods including climatological analysis, spectral and optical methods which may offer potential detection of the major species of red tide in Korean waters. In the climatological analysis, NOAA, SeaWiFS, 0CM satellite data was chosen using the known C. polykrikoides red tide bloom data and the area was mapped by helicopter reconnaissance and ground observation. The relationship between the distribution of sea surface temperature to C. polykrikoides bloom areas was studied. The anomalies of SeaWiFS chlorophyll a imageries against the imageries of non-occurring red tide for August, 2001 showed where the C. polykrikoides occurred. The anomalies of chlorophyll a concentrations from the satellite data during red tide outbreaks showed a similar distribution of C. polykrikoides in the red tide in August, 2001. The distribution between differences in sea surface temperatures during the day and at night also showed a possibility for red tide detection. We used a corrected vegetation index (CVI) to detect floating vegetation and submerged vegetation containing algal blooms. The results of from the optical absorption of C. polvkrikoides in the ultraviolet band (340 nm) showed that if we use the optical characteristics from each red tide, we will be able to establish the feasibility of red tide detection.

Background: Current guidelines suggest the criteria for discontinuation of nucleos(t)ide analogues (NA) in selected patients. However treatment induced virological response is not permanent. Aim of our study is to evaluate the clinical significance of HBsAg titer in predicting sustained virologic response after NA therapy discontinuation. Methods: From Jun 1998 to Dec 2010, medical record of 81 chronic hepatitis B patients who discontinued NA was analyzed retrospectively. Sustained virologic response (SVR) was arbitrarily defined as undetectable HBV DNA by real-time PCR(with lower limit of detection of 116 copies/mL, 20 IU/mL) persisted more than 12 months after treatment discontinuation. Results: Median age was 51 years, 54 (67%) patients were male, and 50 (62%)patients were HBeAg positive. Median baseline ALT, HBV DNA and HBsAg were 292 IU/mL, 7.1log10 IU/mL and 3.3log10 IU/mL. NA were lamivudine (n=53), adefovir (n=15), lamivudine combined with adefovir (n=4), and entecavir (n=9). Median treatment duration and follow-up period were 26 and 27 months. 11/81 (14%) patients had SVR. The cumulative relapse rates were 37/81 (46%) at 6 months and 42/81 (52%) 12 months after treatment discontinuation. The baseline ALT, HBV DNA and presence of HBeAg were not different between patients with or without SVR. In univariate analysis, age, treatment duration and HBsAg level at treatment discontinuation were different in patients with or without SVR; 51 vs. 43 years, p=0.033; 53 vs. 25 months, p=0.011; 2.1 vs. 3.3log10 IU/mL, p=0.003. In multivariate analysis, only HBsAg level at treatment discontinuation remained as an independent factor associated with SVR (p=0.019). The cutoff value of HBsAg level <2log10 IU/mL was predictive of SVR [(AUROC, 0.991; 95% confidence interval[CI], 0.000-1.000; p<0.05); sensitivity, 100%; specificity, 93%; positive predictive value, 69%; negative predictive value, 100%]. Conclusions: Large proportion of patients treated with oral antivirals relapsed after the treatment discontinuation. In the decision of the treatment discontinuation, HBsAg level <2log10 IU/mL at treatment discontinuation can predict sustained viral suppression in selected patients.
Is propofol safe when administered to cirrhotic patients during sedative endoscopy?

Background/Aims: In patients with liver cirrhosis, drugs acting on the central nervous system can lead to hepatic encephalopathy and the effects may be pro-longed. Recently, misuse of propofol has been reported and the associated risk of death have become an issue. Propofol is commonly used during sedative endoscopy; therefore, its safety in high-risk groups must be further investigated. We performed a pilot study of the safety and effi cacy of propofol during endos-copy in Korean patients with cirrhosis. Methods: Upper gastrointestinal endoscopy was performed under sedation with propofol along with careful monitoring in 20 patients with liver cirrhosis and 20 control subjects. The presence or development of hepatic encephalopathy was as-sessed using the number connection test and neurologic examination. Results: Neither respiratory depression nor clinically significant hypotension were observed. Immediate postanesthetic recovery at 5 and 10 minutes after the procedure was delayed in the cirrhotic patients compared with the control group; however, at 30 minutes, the postanesthetic recovery was similar in both groups. Baseline psychomotor performance was more impaired in cirrhotic patients, but propofol was not associated with deteriorated psychomotor function even in cir-rhotic patients with a minimal hepatic encephalopathy. Conclusions: Sedation with propofol was well tolerated in cirrhotic patients. No newly developed hepatic encephalopathy was observed.

This paper proposed that an Analysis of a partial resonant AC-DC converter for high power and power factor operates with four choppers connecting to a number of parallel circuit. To improve these, a large number of soft switching topologies included a resonant circuit have been proposed And, some simulative results on computer are included to confirm the validity of the analytical results The partial resonant circuit makes use of an inductor using step-down and a condenser of lose-less snubber. The result is that the switching loss is very low and the efficiency of system is high And the snubber condenser vised in a partial resonant circuit makes charging energy regenerated at input power source for resonant operation.<br/> The proposed conversion system is deemed the most suitable for high power applications where the power switching devices are used.<br/>

BACKGROUND:: Therapeutic drug monitoring (TDM) of posaconazole is usually performed 1 week after starting the drug because of its long half-life. However, previous studies showed that measuring the posaconazole plasma concentration (PPC) on day 3 is effective for predicting steady-state levels. The purpose of this study was to evaluate the relevance of early TDM (day 3) of posaconazole for achieving an optimal PPC. METHODS:: This prospective study was conducted from September 2014 to August 2016. A total of 148 patients with acute myeloid leukemia or myelodysplastic syndromes received a 200 mg posaconazole oral suspension 3 times daily for fungal prophylaxis. During the period from September 2014 to December 2015 (control group), no dose adjustment was performed on day 3. During the period from January 2016 to Aug 2016 (early TDM group), the frequency of posaconazole 200-mg administration was increased to 4 times daily in patients whose PPC on day 3 was <400 ng/mL. The cutoff value for optimal PPC on day 8 was defined as 500 ng/mL. RESULTS:: In 21 of 107 patients (20%) in the control group, PPC was <400 ng/mL on day 3. In 15 (71%) of these 21 patients, the PPC was suboptimal on day 8. In the early TDM group, the PPC was <400 ng/mL on day 3 in 4 of 41 patients (10%). After increasing the posaconazole administration frequency in these 4 patients, PPC was suboptimal on day 8 in 1 patient (25%). In both groups, 104 patients had a PPC of ≥500 ng/mL on day 3, but 7% (7/104) of these had a suboptimal level on day 8. CONCLUSIONS:: Early TDM on day 3 for posaconazole suspension may help more patients achieve optimal drug levels on day 8, although TDM on day 8 is needed even in patients with optimal levels on day 3.
