Phase I/II study of S-1 combined with weekly docetaxel in patients with metastatic gastric carcinoma

We designed a phase I/II trial of S-1 combined with weekly docetaxel to determine the maximum tolerated dose (MTD) and recommended dose (RD) and to evaluate the efficacy and toxicity in metastatic gastric carcinoma (MGC). Patients with measurable disease received S-1 orally b.i.d. on days 1–14 and docetaxel intravenously on days 1 and 8 every 3 weeks. In phase I (n=30), each cohort received escalating doses of S-1 (30–45 mg m<SUP>−2</SUP> b.i.d.) and docetaxel (25–40 mg m<SUP>−2</SUP>); MTD was 45 mg m<SUP>−2</SUP> b.i.d. S-1/35 mg m<SUP>−2</SUP> docetaxel and RD was 40 mg m<SUP>−2</SUP> b.i.d. S-1/35 mg m<SUP>−2</SUP> docetaxel. Dose-limiting toxicities included grade 3 elevated liver enzymes, gastric perforation, grade 3 diarrhoea/fatigue, febrile neutropenia with grade 3 anorexia/fatigue, and neutropenic infection with grade 3 stomatitis/anorexia. In phase II (n=52), the overall response rate was 66.7% (95% confidence interval (CI): 53.8–79.6%) and the median time to progression and overall survival were 6.5 months (95% CI: 4.9–8.1) and 13.7 months (95% CI: 9.9–17.5), respectively. The most common grade 3/4 toxicity was neutropenia (29.4%), and febrile neutropenia/neutropenic infection occurred in 19.6% of patients. Non-haematological toxicities were generally mild. There was one treatment-related death due to pneumonitis. <P><B>Background:</B></P><P>We evaluated the association between polymorphisms of cytochrome P450 2A6 (<I>CYP2A6</I>)/excision repair cross-complementation group 1 (<I>ERCC1</I>)/X-ray repair cross-complementing group 1(<I>XRCC1</I>) and treatment outcomes of metastatic gastric cancer (MGC) patients treated with S-1/cisplatin.</P><P><B>Methods:</B></P><P>Among MGC patients (<I>n</I>=108), who received S-1 (40 mg m<SUP>−2</SUP> b.i.d., days 1–14) and cisplatin (60 mg m<SUP>−2</SUP>, day 1) every 3 weeks, we analysed the wild-type allele (<I>W</I>) and variants (<I>V</I>) of <I>CYP2A6</I> (<I>*4</I>, <I>*7, *9, *10</I>), and the polymorphisms of <I>ERCC1</I> (rs11615, rs3212986) and <I>XRCC1</I> (rs25487).</P><P><B>Results:</B></P><P>Patients having fewer <I>CYP2A6</I> variants had better response rates (<I>W</I>/<I>W vs W</I>/<I>V</I> other than <I>*1/*4 vs V</I>/<I>V</I> or <I>*1/*4</I>=66.7 <I>vs</I> 58.3 <I>vs</I> 32.3% <I>P</I>=0.008), time to progression (TTP) (7.2 <I>vs</I> 6.1 <I>vs</I> 3.5 months, <I>P</I>=0.021), and overall survival (23.2 <I>vs</I> 15.4 <I>vs</I> 12.0 months, <I>P</I>=0.004). <I>ERCC1 19442C</I>><I>A</I> (rs3212986) was also associated with response rate (<I>C/C</I>, 46.7% <I>vs C/A</I>, 55.3% <I>vs A/A</I>, 87.5%) (<I>P</I>=0.048) and TTP (4.4 <I>vs</I> 7.6 <I>vs</I> 7.9 months) (<I>P</I>=0.012). Patients carrying both risk genotypes of <I>CYP2A6</I> (<I>V</I>/<I>V</I> or <I>1/*4</I>) and <I>ERCC1 19442C</I>><I>A</I> (<I>C/C</I>) <I>vs</I> those carrying none showed an adjusted odds ratio of 0.113 (<I>P</I>=0.004) for response, and adjusted hazard ratios of 3.748 (<I>P</I>=0.0001) for TTP and 2.961 (<I>P</I>=0.006) for death.</P><P><B>Conclusion:</B></P><P>Polymorphisms of <I>CYP2A6</I> and <I>ERCC1 19442C</I>><I>A</I> correlated with the efficacy of S-1/cisplatin.</P>

New seventeen imazapyr derivatives, 2-(4-isopropyl-4-methyl-5-oxo-2-imidazolin-2-yl)-3-(N-methyl-N-X(sub.)-phenylaminooxoacetyl)pyridine, 6 were synthesized and their pre-emergence herbicidal activity(pI_(50) in vivo against Corn (Zea mays L.) and Pigweed (Amaranthus viridis L.) were studied by the pot test under paddly conditions. (quantitative structure activity relationships (QSARs) were analyzed using the physicochemical parameters of substituent(X) on the phenyl ring of 3-(N-methyl-N-X(sub.)-phenylaminooxoacetyl) group and regression analysis. The herbicidal activities were related to the steric effect of X-substituent. The effect was rationalized by paraholic function of MR and L₁, where the optimal values were MR=5.56 (Zea mays L.) and L₁=3.34 Å (Amaranthus viridis L.). Among them, 2,5-difluoro substituted compound, 6i showed good herbicidal activity against Pigweed with excellent tolerance to Corn.

Purpose: The purpose of this study was to investigate correlation among sexual knowledge, attitude and Barriers to patient’s sexual health of nursing undergraduate students and to provide objective data to promote sexual health-care. Method: Using a descriptive correlation, 304 nursing undergraduate students were recruited through convenience random sampling from August 1st to December 20th, 2016. Data were analyzed using average, standard deviation, percentage, t-test, ANOVA, Scheffe test, Pearson’s correlation and Multiple linear regression analysis using the SPSS 21.0 program. Results: The correlation between Sexual knowledge and Sexual attitude is statistically significant(r=.289, p<.01). As the level of sexual knowledge is higher, the sexual attitude is more open. And, Sexual knowledge(r=-.307,p<.01) and Sexual attitude(r=.180,p=.002) had a negative relationship with Barriers to patient’s sexual health. Also, Sexual knowledge was the greatest influencing factor on Barriers to patient’s sexual health. Conclusion: This study showed as the level of Sexual knowledge is higher, the Sexual attitude is more open. As the degree of Sexual knowledge is lower, the Sexual attitude is more conservative, and Barriers to patient’s sexual health is more higher. Also, Sexual knowledge is the most influential factor on Barriers to patient’s sexual health.
Stimulated N-doping of reduced graphene oxide on GaN under excimer laser reduction process

Graphene oxide coated on GaN was simultaneously reduced and doped with nitrogen via excimer laser irradiation. Nitrogen dopant was originated from the GaN during the laser-induced dissociation at high energies. This phenomenon was confirmed by the absence of C-N bond formation in laser irradiated graphene oxide on SiO<SUB>2</SUB>. A top-gated field-effect transistor based on laser reduction of graphene oxide channel on GaN showed n-type behavior via the gate voltage modulation. The present findings indicate a paradigm for the formation of graphene-nitride semiconductor interfaces.

Since early 1990s reclamation projects in Busan and its vicinity in Nakdong River plain have been performed for industrial and residential complexes. As well it has started to build a new port (Busan Newport) due to increase in overloads of Busan port since late 1990s. In the area the sedimentary deposit is composed of soft clay of about 30m thick, thin sand layer of 3-4m thick, hard clayey soil, and then basal conglomerates and sandy sediments, total thickness of which is about 70m at the maximum under seawater. For the huge project many contract companies have simultaneously performed geotechnical investigations to get design parameters. Based on the harmful experiences gained by the previous projects, it was also required to pay careful attention to sampling and laboratory and field tests for the project. Nevertheless the results of laboratory tests were so scattered and then resulted in difficulty to evaluate the design parameters. Existing data has been reviewed to evaluate the validity or cause of their variation. In addition geological study, sampling and soil tests have been carefully carried out at a few locations. Using the results the clay would be effectively characterized for depositional environment and its relation to soil properties.

<P><B>Abstract</B></P> <P>Foodborne pathogens, including enterohemorrhagic <I>Escherichia coli</I> (EHEC) O157:H7, may enter from the farm environment and foods via several different vectors and influence human health. Here, we employed <I>Caenorhabditis elegans</I> as a host model system and compared specific host responses during EHEC O157:H7 infection using whole-transcriptome analysis. To elucidate the immune pathways stimulated by EHEC O157:H7, we employed quantitative real-time polymerase chain reaction (qRT-PCR), transgenic worms, and RNAi. Whole-transcriptome analysis revealed that genes encoding the cytochrome P450 (CYP450) family were induced more than 10-fold during EHEC O157:H7 infection in <I>C. elegans</I> host models. Importantly, <I>C. elegans</I> mutants lacking CYP450 genes were highly susceptible to EHEC O157:H7 infection compared with wild-type N2 worms. Consistent with susceptibility tests, qRT-PCR results indicated that CYP450 loss-of-function mutations significantly affected the transcriptional induction of antimicrobial peptide genes, such as <I>clec-60</I>. Together, our results provide critical insights into host strategies for avoiding EHEC O157:H7 pathogenesis in the gastrointestinal tract via the cytochrome P450 family and highlights potential molecular targets for preventing the virulence of EHEC O157:H7 in foods.</P>

Base-catalyzed domino reaction between N-methylisatin and methyl propiolate afforded four diastereomeric dispiro-1,3-dioxolane bisoxindoles. The reaction mechanism involved sequential 1,2-addition of acetylide anion to the carbonyl group of isatin, 1,2-addition of the resulting oxyanion to a second molecule of isatin, and the final 5-exo-dig cyclization process.
