        우측 수신증을 동반한 골반내 방선균증 1예

        Pelvic actinomycosis is a chronic granulomatous suppurative disease caused by an anaerobic grampositive organism Actinomyces israelii. It is com-monly associated with an intrauterine device(IUD) and can mimick pelvic or intra-abdominal malignant neoplasm. Ureteral obstruction leading to hydronephrosis is a rare complication of tubo-ovarian abscess. We experienced a case of hydronephrosis as a complication of pelvic actinomycotic abscess. The patient was a 46-year-old women presenting with fever and right flank pain. Leukocytosis and pyuria were present and a hydronephrosis was diagnosed by intravenous pyelography. Ultrasonography and a computerised tomography revealed a mass in right adnexum compressing the right ureter. Removal of retroperitoneal abscess and salphingo-oophorectomy were done and the diagnosis of actinomycosis was made by pathologic finding of resected mass. Postoperatively, the patient was treated with second-generation cephalosporin successfully. (Korean J Nephrol 2002;21(2):337-340)

      • 이동형 생체계측을 위한 임베디드 컴퓨팅

        연구목적: 초소형 정밀 기계 기술로 제작된 2차원 가속도 센서와 916 MH2 단거리 고주파 데이터 통신 기술을 응용하여 인체의 운동상태를 자동으로 감지하고 분류하는 기법을 제안 하고자 하였다. 재료 및 방법: 인체의 신호를 획득하기 위하여 미국 버클리대에서 제작한 무선 센서를 등 산용 조끼의 전면상단에 부착해서, 대상자의 흉골 부위에 위치하도록 하고, 앉은 상태에서 일어서거나 눕거나 하는 동작 및 그 역동작이 일어날 때, 가속도 센서에서 감지되는 4가지 종류의 파형 현화에 해당되는 데이터를 무선센서 네트웍을 통해 획득하였다. 결과: 획득한 신호를 주성분분석(Principal Component Analysis)과 지원벡터머신(Support Vector Machine)을 사용하여, 2차주성분까지를 도표화하고 클러스터링 함으로써, 4개의 영역으로 분할하는 것이 가능함을 말 수 있었다 결론: 소형 센서들이 작동되는 임베디드 컴퓨팅 환경에서 일상생활 중에 발생하는 인체의 4가지 동작을 자동으로 감지하고 분류하는 것이 가능함을 보였다. Purpose: New monitoring and classifying method of human motion contort was pro- posed by using 2-axial MEMS accelerometer and 916 MHz short-range data communica-tion technology. Materials and Methods: The wireless sensor of MICA mote was attached to a woman mountain vest and located close to the subject's sternum. During four types of sub-ject's motion, that is, sit-to-stand, stand-to-sit, sit-to-tying, and tying-to-sit, waveform changes of the accelerometer data were acquired by wireless sensor network. Results: The acquired signal was analyzed by Principal Component Analysis(PCA) and Support Vector Machine(SVM) technique. By clustering the first and second principal components, four regions corresponding to the four types of motion, were clearly segment-ed. Conclusion: Human motion context during daily life could be monitored and classified automatically by using small sensors in embedded computing environment

      • 예비 및 현직 초등교사의 경력과 성별에 따른 STS 상호작용에 대한 인식조사

        The purpose of this study is to consider pre-service and in-service elementary school teachers' perceptions about STS (Science-Technology-Society) STS interactions by gender and career. To consider their perceptions and develop the proper teaming methods/materials to it, the next matters were established ; 1. What difference is there between their perceptions about the nature of science and technology and those about STS interactions by genders? 2. What difference is there between their perceptions about the nature of science and technology and those about STS interactions by careers? 222 of the seniors in Jinju National University of Education, namely pre-service elementary school teachers, and 185 of the active teachers in Kyung- Nam Province were selected as the objects for researching these matters. The results were analyzed with a percentage as an examining tool of TBA-STS (Teachers' Beliefs about Science-Technology-Society). The results were as follows ; First, the difference of opinions by genders was not clear in contents. In some of the questions which asked the interactions among science, technology, and society, actual and simple views were a little bit different. Second, in active teachers' recognition of STS interactions by careers, there was a meaningful difference. In other words, most teachers thought of the influence of science on technology as 'technology is an application of science', but the teachers wish 6 to 15 years careers told mostly consider the influence of technology on society as actual opinions. It showed that the teachers with more than 25 years careers had simple thoughts that technology makes our lives more convenient. Moreover, on the STS interactions, those with more than 25 years careers recognized with simplicity that one thing has influence on only another thing. From these above results, the following suggestions are made for the succeeding studies. First, the basic principles of STS should be introduced in the curriculums for pre -service teachers, and also the instruction of it to in-service teachers should be intensified in order that they can have firm views of it. Second, studies should be made to investigate what factors have influence on teachers' recognition of STS. I hope that the succeeding studies on it will be made actively and they will be helpful to settle the education of STS in elementary science education.

      • Ultimate Strength Formulation for Steel Plated Structure

        A ship structure is basically an assembly of stiffened panel elements and an estimation of the aximum load carrying capacity or the ultimate strength of these members is of the highest mportance task for the safety assessment and rational design of the structure. In general, thin lates are often used in cruise ship/high speed vessel structures, for example at deck plating, side shell plating and inside structure. It has been known that reducing the thickness of plating dramatically reduces the buckling strength of a steel plate subjected to in-plane compression and shear loading, and the ultimate strength of it is also expected to decrease. In the present paper, to clarify and examine the fundamental buckling and progressive collapse behaviors of thin plates subjected to in-plane compression and in-plane edge shear load, a series of elastic and elasto-plastic large deflection analyses are performed applying nonlinear FEM codes, MSC Patran/Nastran. On the basis of the numerical results, influences of yielding stress, thickness of plating, slenderness ratio, loading condition on the characteristics of the buckling/ultimate strength and progressive collapse behavior of steel plates, are discussed. It is shown that the ultimate strength formula developed for a flat plate can give a reasonable estimate of strength, when the newly defined double beta parameter considering the change of yield strength as well as slenderness ratio. It is our hope that the obtained ultimate strength formulation would be used as practical design guideline to predict ultimate strength of steel flat plates in the field of ships and offshore structures.

        Short and long-term prognostic value of hyponatremia in heart failure with preserved ejection fraction versus reduced ejection fraction: An analysis of the Korean Acute Heart Failure registry

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Hyponatremia is a well-known risk factor for worse outcomes in heart failure (HF) patients. The impact of hyponatremia according to the ejection fraction (EF) is unclear. We evaluated the prognostic value of hyponatremia according to HF type.</P> <P><B>Methods and results</B></P> <P>The Korea Acute Heart Failure (KorAHF) registry consecutively enrolled 5625 patients. Hyponatremia was defined as serum sodium level<135mmol/L at hospital admission. HF with preserved and reduced ejection fraction were defined as an LVEF ≥50% and LVEF ≤40%, respectively.</P> <P>Among 5103 patients with available EF, 2088 (60%) had HFrEF, and 1284 (25%) had HFpEF. There was no difference in serum sodium level between the groups (HFrEF: 137.4±4.7mmol/L vs. HFpEF: 137.5±5.0mmol/L, P=0.710).</P> <P>Hyponatremic patients had higher in-hospital mortality or urgent heart transplantation in all (11.3% vs. 4.5%, P<0.001), in HFrEF (13.1% vs. 4.9%, P<0.001), and in HFpEF (6.0% vs. 1.9%, P<0.001).</P> <P>After adjustment for significant covariates, hyponatremia was associated with 1.5 fold increased risk for 1-year post-discharge death in the HFrEF group (HR, 1.52; 95% CI, 1.24–1.86), but not in the HFpEF group (HR, 1.16; 95% CI, 0.84–1.61). During admission, the sodium status changed in 22% of the patients and the discharge sodium status had greater prognostic value.</P> <P><B>Conclusions</B></P> <P>Hyponatremia is independent of HF type. Hyponatremia is a significant risk factor for adverse in-hospital outcomes; however its long-term prognostic value is only limited to patients with HFrEF, but not for those with HFpEF.</P>

        Inhibitory Effects of Various Edible Plants and Flavonoids from the Leaves of Cedrela sinensis on Human Immunodeficiency Virus Type 1 Protease

        The inhibitory effect of extracts from 15 edible plants on the protease of human immunodeficiency virus (HIV) type 1 was investigated. Protease activity was determined by incubating the extracts in a reaction mixture containing protease and substrate His-Lys-Ala-Arg-Val-Leu-(p-NO$_2$-Phe)-Glu-Ala-Nle-Ser-NH$_2$ to inhibit proteolytic cleavage. Of various plants tested, the leaves of Cedrela sinensis inhibited the HIV-1 protease by 42% at a concentration of 100$\mu\textrm{g}$/ml. A major flavonoid isolated from the leaves of C. sinensis, quercetin 3-O-$\alpha$-L-rhamnoside showed inhibitory activity of 19% at a concentration of 100$\mu$M.

        Far-Infrared Radiation to Improve Clinical Outcomes after Arthroscopic Rotator Cuff Repair: A Prospective Randomized Comparative Clinical Study

        Background: The efficacy of far-infrared radiation (FIR) after rotator cuff repair has not been demonstrated yet. The aim of this study was to evaluate the effects of postoperatively applied FIR with regard to early pain, range of motion (ROM), and tendon-tobone healing after arthroscopic rotator cuff repair. Methods: A total of 64 consecutive patients who underwent arthroscopic rotator cuff repair with small- to medium-sized tears were enrolled in this prospective comparative study and randomly divided into an FIR group (n = 31) and a control group (n = 33). In the FIR group, FIR using a radiator device (Aladdin-H) was applied for 30 minutes per session twice daily from the first postoperative day. This application lasted for 10 weeks during the postoperative period. Clinical outcomes were assessed using a visual analog scale for pain (pVAS) at 5 weeks and ROM at 3 and 6 months postoperatively. Functional scores were evaluated at 6 months postoperatively. Healing of the repaired rotator cuff was also evaluated using ultrasonography at 3 months and magnetic resonance imaging at 6 months postoperatively. Results: In both groups, clinical and functional outcomes were improved up to 6 months compared with preoperative values. At 5 weeks and 3 months postoperatively, the average pVAS was significantly lower in the FIR group than in the control group (1.7 ± 1.0 vs. 2.8 ± 1.4; p = 0.002 at 5 weeks, 2.4 ± 1.3 vs. 3.2 ± 1.8; p = 0.041 at 3 months). However, there was no significant difference in ROM, functional score, or healing rate between two groups at each follow-up time point. Conclusions: The application of FIR after arthroscopic rotator cuff repair could be a safe and effective procedure to decrease postoperative pain, especially in the early postoperative period. This effective application of FIR can be considered to facilitate painless rehabilitation in the postoperative period after arthroscopic rotator cuff repair.

        <P>In this article, we are prepared that the liquid crystal dimers have aromatic-ester type mesogenic units or aromatic-Schiff base typemesogonic units and confirmed by H-1-NMR spectrometry. The mesomorphic and optical properties of the resultant dimers were studied by differential scanning calorimetry and polarizing optical microscopy.</P>
