        직장 카르시노이드 종양에서 Ki-67 발현의 예후적 중요성

        Background/Aims: Rectal carcinoid tumors can be resected with endoscopy, and it is important to assess their prognostic factors. We evaluated the potential of Ki-67 expression as a prognostic factor in rectal carcinoid tumors. Methods: We retrospectively reviewed the medical records of 37 patients with rectal carcinoid tumors who got endoscopic resection from January 2001 to January 2011 at Inje University Seoul Paik Hospital. We analyzed their endoscopic and histologic findings, Ki-67 expression, clinical outcome, and prognosis. Results: The mean age (±SD) of the patients was 56.3±10.7 years, and the male : female ratio was 3.6:1. The mean tumor size was 0.5±0.4 cm, 33 patients showed grade 1 tumors (89.2%) and the average Ki-67 expression was 0.7±1.2%. Thirty five patients underwent endoscopic mucosal resection, and two required endoscopic submucosal dissection. Eight patients had positive margins after resection, but no cases of lymphovascular invasion were identified. The median follow-up duration was 21.4±25.4 months, and no recurrences were observed. Conclusions: In low grade rectal carcinoid tumors which are lack of central depression on colonoscopy, the expression of a molecular marker of malignant potential, Ki-67, was low. Therefore, endoscopic resection seemed to be a safe and effective treatment for these tumors.

        유산양 유즙으로부터 분리된 세균의 분포 및 항균제 감수성 검사

        The aim of this study was to isolate microorganisms from half milk samples of dairy goats by California mastitis test (CMT) during the lactation period and to further investigate the susceptibility of isolated organisms to antimicrobial drugs. From a total of 235 half milk samples with CMT scores of 2 or above from 366 dairy goats distributed throughout Jeonnam province, microorganisms were isolated from 198 (83.5%) samples either singly (99.0%) or in combination (1.0%). The most prevalent microorganism was the coagulase-negative Staphylococcus spp., (44.4%, n=88) followed by Staphylococcus aureus (24.2%, n=48), Escherichia coli (11.1%, n=22) and Streptococcus spp. (7.6%, n=15). Isolated bacteria also included Bacillus spp. (2.5%, n=5), Pseudomonas spp. (2.5%, n=5), Micrococcus spp. (1.5%, n=3), Corynebacterium spp. (1.5%, n=3), Enterococcus facium (1.0%, n=2), Morganella morganii (0.5%, n=1) and Streptococcus agalactiae (0.5%, n=1). During the summer season, a high prevalence of all micro-organisms were observed in which Staphylococcus spp. (30.8%), Escherichia coli (8.6%), and Streptococcus spp. (5.6%) were among the most prevalent bacteria isolated. Staphylococcus spp. was also shown to be high in the winter (21.7%). In most samples, the presence of bacterial pathogens in goat milk led to the increase in the total somatic cell count (SCC). Most of the half milk samples of dairy goats with bacterial contamination showed SCC of ≥1×10(6) cells/ml (90.4%). Minor pathogens (11.4%) were more detected from milk samples with SCC of <1×10(6) cells/ml than major pathogens (4.1%), while the major pathogens tended to be higher from samples with SCC of ≥3×10(6) cells/ml. Susceptibility of these bacteria to 12 antimicrobial agents was tested by the Kirby-Bauer disc diffusion method. Results indicated that more than 90% of bacteria isolated from CMT 2+ dairy goat half milk samples were susceptible to trimethoprim/sulfamethoxazole, amoxicillin/clavulanic, enrofloxacin and cephalothin while they were resistant to tetracycline (44.7%).

        은침점전기자극이 $Na^+$, $K^+$ 이온과 $Ca^{2+}$ 이온변동에 미치는 효과

        The present study examined that in vivo test is investigated in sham-operated(control group) and aldosterone-analogue deoxycorticosterone acetate (DOCA)-salt hypertensive animals(experimental group) and that the antihypertensive effect was induced by silver spike point(SSP) low frequency electrical stimulation at meridian points(CV-3, -4, Ki-12, SP-6, LR-3, BL-25, -28, -32, -52), specifically, such as diuretic action in 24 hour urine analysis from normal volunteer. The $Na^+$ and $Ca^{2+}$ ions were significantly increased in aldosterone-analogue DOCA-salt hypertensive rats than that in sham-operated rats. However, the $K^+$ ions were significantly decreased in aldosterone-analogue DOCA-salt hypertensive rats than that in sham-operated rats. The current of 1 Hz continue type of SSP low frequency electrical stimulation significantly increased in excretion of urine $Na^+$ and $K^+$ ions from normal volunteer. However, the excretion of $Ca^{2+}$ ion were significantly decreased by SSP electrical stimulation in volunteer. These results suggest that the development of aldosterone analogue-DOCA-salt hypertension is associated with changed $Na^+$, $K^+$ and $Ca^{2+}$ ions of urine. which directly affects blood pressure. Therefore, the hypertension is a risk factor on cardiovascular disease. Moreover, These results demonstrate that the SSP low frequency electrical stimulation, especially current of 1 Hz continue type, significantly regulates $Na^+$, $Ca^{2+}$ and $K^+$ ions from volunteer. Therefore, the SSP low frequency electrical stimulation is a good regulator through a diuretic action of aldosterone-induced hypertension.

        Aldosterone 유도체-고혈압의 음성적 유해와 은침점전기자극의 aldosterone 억제

        The present study examined that in vivo/vitro test is investigated in normotensive sham-operated rats(NSR) and aldosterone-analogue deoxycorticosterone acetate (DOCA)-salt hypertensive rats(ADHR) and that the antiliypertensive effect was induced by silver spike point(SSP) electrical stimulation at meridian points(CV-3, -4, Ki-12, SP-6, LR-3, BL-25, -28, -32, -52), specifically, such as aldosterone in 24 hour urine analysis from normal volunteer. The heart weight, the tickness of vascular wall, collagen fiber and the systolic blood pressure were significantly increased in ADHR than that in NSR. The required time of PSS-induced resting tone and the phosphorylation of stress-activated protein kinase/c-Jun N-terminal protein kinase(SAPK/JNK) were significantly increased in ADHR than that in NSR. However, the Kv currents were significantly decreased in ADHR than that in NSR. The current of 1 Hz continue type of SSP electrical stimulation significantly decreased in excretion of urine aldosterone from normal volunteer. These results suggest that the development of aldosterone analogue-induced hypertension is associated with changed heart weight, content of collagen fiber, tickness of vascular wall, blood pressure, resting tone, voltage-dependent K+ current(Kv) and phosphorylation of SAPK/JNK, which directly affects blood pressure. Therefore the hypertension is a risk factor on cerebrovascular disease. Moreover, These results suggest that the SSP electrical stimulation, especially current of 1 Hz continue type, significantly regulates excretion of urine aldosterone from volunteer.

        Anti-allergic Components from the Peels of Citrus nushiu

        In a bioassay-guided search for anti-allergic compounds from higher plants of Korea, poly-methoxyflavones, 3',4',5,6,7,8-hexamethoxyflavone (I), 5-hydroxy-3',4',6,7,8-pentamethoxyflavone (II) and 3',4',5,7,8-pentamethoxyflavone (III) have been isolated from the immature peels of Citrus unshiu. Structures of these compounds were elucidated on the basis of spectroscopic techniques. Compounds I and II inhibited dose-dependent histamine release from the rat peritoneal mast cells activated by compound 48/80 or anti-DNP IgE.

      • KR‐31378 ameliorates atherosclerosis by blocking monocyte recruitment in hypercholestrolemic mice

        <P>The recruitment of monocytes into the artery wall is a crucial early step in atherogenesis. A novel compound, KR-31378, has been shown to be a neuroprotective agent for ischemia-reperfusion damage in rat brain via its potent antioxidant and antiapoptotic actions. Here, we report the effects of this compound on atherogenesis and possible mechanisms of action. In Ldlr knockout mice fed with a high-fat, high-cholesterol diet, treatment with KR-31378 significantly inhibited fatty streak formation and macrophage accumulation. To address the possibility that KR-31378 may influence the initial stages of atherogenesis, we examined its effect on the adhesion and migration of monocytes to endothelial cells stimulated with tumor necrosis factor-alpha. KR-31378 decreased the adhesion in a dose-dependent manner. The observed decreases in cell adhesion and migration correlated with KR-31378-mediated down-regulation of vascular cell adhesion molecule-1 (VCAM-1) and interleukin (IL)-8. Nuclear factor-kappaB (NF-kappaB) is known to regulate the expression of adhesive and chemotactic molecules including VCAM-1 and IL-8. Indeed, transient transfection experiments, electrophoretic mobility shift assay, and IkappaB degradation assay showed that KR-31378 decreased NF-kappaB activation. These results indicate that KR-31378 potently reduces fatty streak formation by inhibiting NF-kappaB-dependent cellular adhesion and chemotactic molecule expression, which are crucial to monocyte infiltration into the arterial wall during the early stages of atherogenesis.</P>

        Change in Water Contact Angle of Carbon Contaminated TiO<sub>2</sub> Surfaces by High-energy Electron Beam

        We studied change in water contact angle on $TiO_2$ surfaces upon high-energy electron-beam treatment. Depending on conditions of e-beam exposures, surface OH-content could be increased or decreased. In contrast, water contact angle continuously decreased with increasing e-beam exposure and energy, i.e. change in the water contact angle cannot be rationalized in terms of the overall change in the surfacestructure of carbon-contaminated $TiO_2$. In the C 1s spectra, we found that the C-O and C=O contents gradually increased with increasing e-beam energy, suggesting that the change in the surface structure of carbon layers can be important for understanding of the wettability change. Our results imply that the degree of oxidation of carbon impurity layers on oxide surfaces should be considered, in order to fully understand the change in the oxide surface wettability.

        A SIMPLE ANALYTICAL METHOD FOR NONLINEAR DENSITY WAVE TWO-PHASE INSTABILITY IN A SODIUM-HEATED AND HELICALLY COILED STEAM GENERATOR

        A simple model to analyze non-linear density-wave instability in a sodium-cooled helically coiled steam generator is developed. The model is formulated with three regions with moving boundaries. The homogeneous equilibrium flow model is used for the two-phase region and the shell-side energy conservation is also considered for the heat flux variation in each region. The proposed model is applied to the analysis of two-phase instability in a JAEA (Japan Atomic Energy Agency) 50MWt No.2 steam generator. The steady state results show that the proposed model accurately predicts the six cases of operating temperatures on the primary and secondary sides. The sizes of three regions, the secondary side pressure drop according to the flow rate, and the temperature variation in the vertical direction are also predicted well. The temporal variations of the inlet flow rate according to the throttling coefficient, the boiling and superheating boundaries and the pressure drop in the two-phase and superheating regions are obtained from the unsteady analysis.

      • Changes in the Hypothalamic Gonadotropin-Releasing Hormone Gene Expression and the Pituitary Luteinizing Hormone Immunoreactivity in Male Rats: Comparison of Clozapine with Typical Antipsychotics

        Evidence suggested that atypical antipsychotics (APs) such as clozapine show less side effects than those of typical APs such as haloperidol and sulpiride. However, little is known about chronic effects of these drugs on changes in gonadotropin releasing hormone (GnRH) mRNA expression and luteinizing hormone (LH) immunoreactivity. Male rats were divided into water-, haloperidol-, sulpiride-, and clozapine-treated groups, and these drugs were administered orally for 4 weeks. The changes in the expression of GnRH mRNA and the LH immunoreactivity were determined in the hypothalamus and pituitary, respectively, using in situ hybridization and immunohistochemistry. GnRH mRNAs were clearly expressed in the water-treated control vats. This was significantly reduced by the chronic treatments with the typical APs, especially with haloperidol, but not with atypical APs clozapine. Likewise, LH immunoreactivity was clearly stained in the control group. While its immunoreativity was significantly reduced by the chronic APs treatments, clozapine treatment showed only slight attenuation. The results show that the atypical APs clozapine has less side effects in the gonadal function than the typical APs haloperidol and the sulpiride. These results suggest that clozapine is a safer drug than the typical APs, at least in the reproductive system.

        Characteristics and management of patients with inflammatory bowel disease between a secondary and tertiary hospitals: a propensity score analysis

        Background/Aims: This study aimed to compare the clinical characteristics and management patterns of inflammatory bowel disease (IBD) patients in a secondary hospital (SH) with those in tertiary referral centers (TRC). Methods: Data from IBD patients in SH and 2 TRCs were retrospectively reviewed. The cumulative thiopurine use rate was compared between hospitals after controlling for different baseline characteristics using propensity score matching. Results: Among the total of 447 patients with IBD, 178 Crohn’s disease (CD) and 269 ulcerative colitis (UC) patients were included. Regarding initial CD symptoms, patients from SH were more likely to show perianal symptoms, such as anal pain or discharge (56.6% vs. 34.3%, P=0.003), whereas those from TRCs more often had luminal symptoms, such as abdominal pain (54.9% vs. 17.1%, P<0.001), diarrhea (44.1% vs. 18.4%, P<0.001), and body weight loss (9.8% vs. 1.3%, P=0.025). Complicating behaviors, such as stricturing and penetrating, were significantly higher in TRCs, while perianal disease was more common in SH. Ileal location was more frequently observed in TRCs. For UC, SH had a more limited extent of disease (proctitis 58.8% vs. 21.2%, P<0.001). The cumulative azathioprine use rate in SH was significantly lower than that in TRCs in both CD and UC patients after controlling for disease behavior, location, and perianal disease of CD and extent of UC. Conclusions: The clinical characteristics and management of the IBD patients in SH were substantially different from those in TRCs. Thiopurine treatment was less commonly used for SH patients.
