재발한 지주막하 포도낭미충증의 Albendazole 치험 1예

뇌실이나 뇌기조에 발생하는 포도낭미충증은 매우 드물며 유구조충 유충의 머리부분이 변성되면서 발생하는 낭성 막의 비정상적인 성장을 특징으로 한다. 이러한 유형의 병변은 지름술 후에도 진행적인 경과를 보이며 아직까지 항기생충제를 이용한 치료 사례들이 적고, 따라서 가장 적절한 치료 기간도 알려져 있지 않다. 본 증례 보고는 재발한 실비우스 틈새의 포도낭미충증 환자에서 4주간의 albendazole과 스테로이드로 효과적으로 치료하였던 예이다. 환자는 praziquantel과 뇌복막지름술로 치료를 받고도 포도상의 뇌실질외형의 병변이 악화되어, 뇌척수액 투과가 좋은 것으로 알려진 albendazole로 치료제를 바꾼 뒤 병변의 수와 크기가 감소였으며 이후 2년 간의 추척기간동안 다른 발작 없이 잘 지내고 있다. 지주막하의 포도낭미충증은 albendazole과 corticosteroid 치료에 잘 반응하는 것으로 보인다. The so-called racemose cysticercosis, a rare variety of neurocysticercosis occurring in ventricles or basal cisterns, is characterized by abnormal growth of cystic membranes with degeneration of Taenia solium heads (scolex). Although lesions of this type are known to follow a progressive course even after ventricular shunting, there are limitations of case series treated with antiparasitic drugs, and the optimal duration of the treatment is not yet known. We report a case of relapsed racemose cysticercosis in the Sylvian fissure, who has been successfully treated with albendazole and adjunct corticosteroid for 4 weeks. The patient had been previously treated with praziquantel and ventricul-operitoneal shunt, and maintained on the anticonvulsant drug for one year, but returned to the hospital due to seizure recurrence. The patient has been well in seizure-free state for the follow-up 2 years after albendazole therapy. The subarachnoid racemose cysticercosis seems to respond well to treatment of corticosteroid along with prolonged albendazole.
Si Bulk Micromachining을 위한 Wafer Rolling Etching 및 그 특성

A wafer rolling etching system for the silicon bulk micromachining has been designed and fabricated. The silicon diaphragms were anisotropically etched in a 24.5 weight percent KOH solution. Compared to the conventional KOH etching systems, pyramidal hillocks, and wave-shaped structures on the etched surfaces were greatly reduced by using this system. After etching for time of 438 minutes, the average etched depth and the etch-rate were measured to be 537μm and 1.22μm/min, respectively. The average etching uniformity of etching depth was 0.87% in 5-inch wafer. Our results showed that the wafer rolling method enhanced etch uniformity and etch rate.

A ten months old, female Yorkshire terrier weighing 2.88 kg referred to veterinary teaching hospital of college of veterinary medicine, Konkuk University because of syncope, cough and dyspnea. First hematological and serum chemical test revealed thrombocytopenia, mild anemia, and increase of concentration of ALP (195 U/L). On 57 days later, second hematological and serum chemical test revealed polycythemia, increase of concentration of ALP (211 U/L), and Tchol (387 mg/dl). Right atrium enlargement, main pulmonary artery bulge and cardiomegaly (VHS = 11.5) were observed in radiographic findings. Ultrasonographic images showed both right and left ventricular dilation and turbulent flow between the descending aorta and the main pulmonary artery in color Doppler imaging. ECG showed left ventricular enlargement, SA block, and electrical alternant. Thoracotomy was performed through left fourth intercostal incision under isoflurane anesthesia. Patent ductus arteriosus was double ligated with 1-0 silk. Cough and dyspnea disappeared on 5 days after operation. Turbulent flow was not found in color doppler imaging of ultrasonography on 10 days after operation. Ten months later after the operation, syncope could not exist any more.

<P>Two new cyclic octadepsipeptides, octaminomycins A (1) and B (2), were isolated from a microbial metabolite fraction library of Streptomyces sp. RK85-270 based on Natural Products Plot screening. Their structures were elucidated on the basis of HRESIMS, 1D and 2D NMR spectroscopic data, and MS/MS experiments for sequence analysis. The absolute configurations of the constituent amino acid residues were determined by a combination of single crystal X-ray diffraction and Marfey's methodology. Notably, octaminomycins A (1) and B (2) showed good in vitro antiplasmodial activity against chloroquine-sensitive as well as chloroquine-resistant strains with no cytotoxicity up to 30 mu M.</P>

<P>Two new cyclic peptides, pentaminomycins A (<B>1</B>) and B (<B>2</B>), were isolated from cultures of <I>Streptomyces</I> sp. RK88-1441. Based on the interpretation of the NMR, UV, IR, and MS data, the planar structures of <B>1</B> and <B>2</B> were elucidated as cyclic pentapeptides with a modified amino acid residue, <I>N</I><SUP>5</SUP>-hydroxyarginine (N5-OH-Arg). The absolute configurations of the constituent amino acid residues were determined by the advanced Marfey’s method. Localization of <SMALL>L</SMALL>- and <SMALL>D</SMALL>-amino acids in the sequence was ascertained by chiral analysis of the fragment peptide obtained from a partial hydrolysate; amino acids were identified by LC-MS. Pentaminomycin A (<B>1</B>) reduced α-MSH-stimulated melanin synthesis by suppressing the expression of melanogenic enzymes including tyrosinase, tyrosinase-related protein-1 (TRP-1), and tyrosinase-related protein-2 (TRP-2).</P> [FIG OMISSION]</BR>
[P278] A case of Fonsecaea monophora skin infection

Chromoblastomycosis is a chronic fungal infection of the cutaneous and subcutaneous tissue resulting from implantation of certain dematiaceous fungi through the skin. The genus Fonsecaea is a major dematiaceous fungus and accounts for 90% of chromoblastomycosis. The genus Fonsecaea has been reclassified using analysis of rDNA internal transcribed spacer (ITS) region sequence; Fonsecaea (F.) pedrosoi, F. monophora, and others. In Korea, F. monophora chromoblastomycosis is very uncommon and just 4 cases have been reported. F. monophora infection was confirmed using rDNA ITS sequence analysis. Additionally characteristic sclerotic cells on both KOH preparation and histopathologiy can be helpful in making a diagnosis. A 65-year-old immunocompetent male presented with a walnut-sized, scaly erythematous plaque on left forearm for 1 year. KOH preparation showed brown sclerotic bodies and fungal culture showed dark black, velvety colonies. Long, septate hyphae with numerous conidia were observed on lactophenol cotton blue staining. Histopathology demonstrated brownish sclerotic bodies and suppurative granulomatous infiltration in the dermis. Also, sequencing analysis of the ITS region of rDNA in GenBank identified F. monophora. The lesion was successfully controlled by oral itraconazole (100mg bid for 2 months). We report a case of F. monophora chromoblastomycosis which is very rare in Korea, identified with gene sequencing analysis.
RK-270D and E, Oxindole Derivatives from Streptomyces sp. with Anti-Angiogenic Activity

A chemical investigation of a culture extract from Streptomyces sp. RK85-270 led to the isolation and characterization of two new oxindoles, RK-270D (1) and E (2). The structures of 1 and 2 were determined by analyzing spectroscopic and spectrometric data from 1D and 2D NMR and High-resolution electrospray ionization mass spectrometry (HRESIMS) experiments. Compound 1 exhibited anti-angiogenic activities against human umbilical vein endothelial cells (HUVECs) without cytotoxicity. Results of Western blot analysis revealed that 1 inhibits VEGF-induced angiogenesis in the HUVECs via VEGFR2/ p38 MAPK-mediated pathway.

A new homocysteine thiolactone derivative, thiolactomide (1), was isolated along with a known compound, N-acetyl homocysteine thiolactone (2), from a culture extract of soil-derived Streptomyces sp. RK88-1441. The structures of these compounds were elucidated by detailed NMR and MS spectroscopic analyses with literature study. In addition, biological evaluation studies revealed that compounds 1 and 2 both exert neuroprotective activity against 6-hydroxydopamine (6-OHDA)-mediated neurotoxicity by blocking the generation of hydrogen peroxide in neuroblastoma SH-SY5Y cells.

RK88-1441 led to the isolation and characterization of two new glycosylated anthraquinones, aturanosides A (<B>1</B>) and B (<B>2</B>), and a new anthraquinone derivative, aturanocin (<B>3</B>). The structures of these compounds were elucidated by detailed NMR and MS spectroscopic analyses. The absolute configurations of the sugar units, based on the magnitudes of the coupling constants, ROESY correlations, and chemical derivatization, from <B>1</B> and <B>2</B> are 6-<I>O</I>-[<I>N</I>-acetyl-α-<SMALL>D</SMALL>-glucosamino-(1→2)-α-<SMALL>L</SMALL>-rhamnoside] and 6-<I>O</I>-α-<SMALL>L</SMALL>-rhamnoside, respectively. Compounds <B>1</B> and <B>2</B> showed no cytotoxicity against human umbilical vein endothelial cells (HUVECs), but significantly suppressed vascular endothelial growth factor (VEGF)-induced tube formation and invasion of HUVECs. The down-regulation of both the phosphorylation of VEGF receptor 2 and the expression of vascular endothelial cadherin at the protein level were also observed.</P> [FIG OMISSION]</BR>
