        임플란트 hexagon 높이에 따른 임플란트와 주위 조직의 응력분포 평가

        PURPOSE. To analyze the stress distribution of the implant and its supporting structures through 3D finite elements analysis for implants with different hexagon heights and to make the assessment of the mechanical stability and the effect of the elements. MATERIALS AND METHODS. Infinite elements modeling with CAD data was designed. The modeling was done as follows; an external connection type ∅4.0 mm×11.5 mm OsstemUSII (Osstem Co., Pusan, Korea) implant system was used, the implant was planted in the mandibular first molar region with appropriate prosthetic restoration, the hexagon (implant fixture’s external connection) height of 0.0, 0.7, 1.2, and 1.5 mm were applied. ABAQUS 6.4 (ABAQUS, Inc., Providence, USA) was used to calculate the stress value. The force distribution via color distribution on each experimental group’s implant fixture and titanium screw was studied based on the equivalent stress (von Mises stress). The maximum stress level of each element (crown, implant screw, implant fixture, cortical bone and cancellous bone) was compared. RESULTS. The hexagonal height of the implant with external connection had an influence on the stress distribution of the fixture, screw and upper prosthesis and the surrounding supporting bone. As the hexagon height increased, the stress was well distributed and there was a decrease in the maximum stress value. If the height of the hexagon reached over 1.2 mm, there was no significant influence on the stress distribution. CONCLUSION. For implants with external connections, a hexagon is vital for stress distribution. As the height of the hexagon increased, the more effective stress distribution was observed. 연구 목적: 본 연구는 hexagon 높이에 따른 임플란트 각 부위와 주위 지지조직의 응력분포를 3차원 유한요소 해석을 통해 평가하여 hexagon 높이가 기계적 안정성에 미치는 영향을 평가하고자 시행되었다. 연구 재료 및 방법: 외측 연결 형태의∅L4.0 mm×11.5 mm USII (Osstem Co., Pusan, Korea) 임플란트 시스템을 이용하여 하악 제 1대구치 부위에 임플란트를 식립하여 보철 수복한 경우를 연구 모델로 가정하고 임플란트 고정체의 외측 연결부인 hexagon의 높이를 각각 0.0 mm, 0.7 mm, 1.2 mm, 1.5 mm로 적용한 CAD data를 유한요소 모형화하였다. ABAQUS 6.4 (ABAQUS Inc., Providence, RI, USA)를 이용하여 산출된 응력 값 중에서 등가응력을 기준으로 각 요소(상부 치관, 지대주 나사, 고정체, 치밀골, 해면골)에서 나타나는 최대 응력 값을 비교 하였다. 결과:외측 연결을 갖는 임플란트의 hexagon의 높이는 고정체, 지대주 나사, 상부 보철물 그리고 주위 지지골에 대해 응력 분산에 영향을 주었다. Hexagon의 높이가 증가할 수록 임플란트의 응력 분산은 더 잘 이루어졌으며, 최대 응력 값의 감소를 보였다. Hexagon의 높이가 1.2 mm 이상이 되면 응력 분포에 더 이상 크게 기여하지 않았다. 결론: 외측연결을 갖는 임플란트에서 hexagon은 응력 분산에 필수적인 요소이며 그 높이가 증가할수록 더욱 효과적인 응력의 분산이 나타났다.

        국화 기형화 발생과 S-adenosyl-L-homocysteine hydrolase 유전자 발현

        본 시험은 국화에서의 기형화 발생과 DNA 메틸레 이션에 관여하는 S-adenosyl-L-homocysteine hydrolase (SAHH) 유전자 발현과의 관련성을 검토하기 위해 수행 하였다. 스프레이 국화 ‘Lerbin’은 단일후 14일에서 27일 사이에 고온과 장일 조건에서 기형화가 발생되어, 35/20oC 의 고온과 14시간의 장일 처리 할 경우 25/20oC(12 h/ 12 h)에 비해 설상화가 2배 이상 증가하는 양상을 보였다. 스프레이 국화 ‘Lerbin’에서 분리한 전장 cDNA (DgSAHH) 는 크기가 1455 bp로 다른 작물과 90%이상의 높은 상동 성을 나타내었다. 국화의 DgSAHH 유전자 발현은 기형화 발생을 유도하는 고온과 장일 조건에서 크게 감소하였다. 이러한 결과를 통해 국화의 화아 발달에 있어서 DgSAHH 유전자 발현은 온도와 일장의 영향을 받으며, 억제된 이 유전자의 발현이 기형화 발생에 관여할 수 있을 것으로 판단된다. This study was conducted to investigate the involvement of flower abnormality and S-adenosyl- L-homocysteine hydrolase (SAHH), which is one of the key enzyme in the maintenance of methylation. Plants exposed to high temperature (HT) and long day (LD) condition from 14-27days after short day (SD) produced abnormal flower, having numbers of ray florets. Numbers of ray florets were increased more than 2 folds by HT of 35/20oC and LD of 14 hour comparing those of 25/20oC (12 h/12 h). Full-length cDNA clone of S-adenosyl-L-homocysteine hydrolase (DgSAHH) in spray chrysanthemum ‘Lerbin’ contained an 1455 bp open reading frame coding for 485 amino acids. It showed highly conserved coding sequences among the different plant species with over 90% homology. DgSAHH expression was decreased in abnormal flower inducing treatment of HT and LD, while DgSAHH transcripts accumulated in flower bud of non abnormality inducing condition. This result implicate that DgSAHH expression is affected by temperature and photoperiod during flower development and suppression of DgSAHH is a one of the cause of abnormal flower under HT and LD condition.

        Background: Anemia is commonly observed in elderly population, and the prevalence of anemia increases with advanced age. Anemia is a sign of poor health state and increase adverse outcomes in the elderly. Therefore, assessment of anemia is important in elderly patients. Objectives: The aim of this study was to classify the type of anemia and to assess the current state of anemia evaluation in the elderly population. Methods: Medical records-based retrospective study was performed for patients who were more than 80 years (>80) old and visited Dongguk University Ilsan Hospital from April 2005 to February 2014. Results: Total 548 patients who had anemia according to WHO criteria were identifi ed. The median age was 85 years old (range, 82-99) and median hemoglobin level was 11g/dL (range, 2. 7-12. 9). Twenty eight, 468, and 52 patients were classifi ed as microcytic anemia, nomocytic anemia, and macrocytic anemia, respectively. Among them, 397 patients (72. 4%) did not underwent proper the evaluation for the cause anemia. In detail: 8 (28. 5%) of microcytic anemia, 361 (77. 1%) of normocytic anemia, and the 28 (53. 84%) of 52 macrocytic anemia patients. The remaining 151 patients (27. 6%) had completed the evaluation, and 24 patients (15. 9%) were diagnosed as solid or hematologic malignancies. Conclusions: This fi nding showed physicians often neglected anemia in elderly over age 80. Though the patients have limited life expectancy, physicians need to discriminate the sub-population who will be benefi t from adequate evaluation and treatment.

      • Microsatellite Instability Status in Gastric Cancer: A Reappraisal of Its Clinical Significance and Relationship with Mucin Phenotypes

        <P><B>Background</B></P><P>Gastric cancers with microsatellite instabilities (MSI) have been reported to be associated with favorable prognosis. However, the significance of the effect of MSI on the clinicopathological features, as well as its association with mucin phenotype, remains unclear.</P><P><B>Methods</B></P><P>MSI status was assessed in 414 cases of gastric cancer using polymerase chain reaction analysis of five microsatellite loci, as recommended by National Cancer Institution criteria. The expression of mucins (MUC5AC, MUC6, MUC2, and CD10) was assessed.</P><P><B>Results</B></P><P>Out of 414 total cases of gastric cancer, 380 (91.7%), 11 (2.7%), and 23 (5.6%) were microsatellite stable (MSS), low-level MSI (MSI-L), and high-level MSI (MSI-H), respectively. Compared to MSS/MSI-L, MSI-H gastric cancers were associated with older age (p=0.010), tumor size (p=0.014), excavated gross (p=0.042), intestinal type (p=0.028), aggressive behaviors (increase of T stage [p=0.009]), perineural invasion [p=0.022], and lymphovascular emboli [p=0.027]). MSI-H gastric cancers were associated with tumor necrosis (p=0.041), tumor-infiltrating lymphocytes (≥2/high power field, p<0.001), expanding growth patterns (p=0.038), gastric predominant mucin phenotypes (p=0.028), and MUC6 expression (p=0.016). Tumor necrosis (≥10% of mass, p=0.031), tumor-infiltrating lymphocytes (p<0.001), intestinal type (p=0.014), and gastric mucin phenotypes (p=0.020) could represent independent features associated with MSI-H gastric cancers. MSI-H intestinal type gastric cancers had a tendency for poor prognosis in univariate analysis (p=0.054) but no association in Cox multivariate analysis (p=0.197).</P><P><B>Conclusions</B></P><P>Our data suggest that MSI-H gastric cancers exhibit distinct aggressive biologic behaviors and a gastric mucin phenotype. This contradicts previous reports that describe MSI-H gastric cancer as being associated with favorable prognosis.</P>

        외상에 의한 뇌사자에서의 장기기증 : 3년간의 경험

        Organ transplantation has become a viable treatment for an increasing number of patients suffering from irreversible organ failure. But organ donation has not kept pace with steeply rising demand, many patients die each year while awaiting donor organ. The authors have analysed the 115 patients who contacted the our transplantation center for organ donation from May, 1995 to April, 1998. Successful donation was done in 62 cases(55.4%) and in 53 cases(44.6%), it was not possible due to non brain death state in 26 cases(49.1%), refusal of the relatives in 13 cases, improper state of the patients in 6 and cardiac arrest and death during the process in 3 cases. In successful 62 cases, 42(67.7%) of them was due to trauma. The main cause was car(42.8%) and the motor cycle was in the next position(33.3%). The motor cycle victims were younger than the victims by car. The refferal was mainly from the medical staffs(69.1%). Transplanted organs were 70 cases of kidney from 36 donors, comea 32 cases, heart valve 23 cases, liver 15 cases, heart 15 cases, pancreas 6 cases, bone 6 cases and one case of lung. The authors considered that to overcome recent organ shortage of transplantation, frequent commucation between transplantation team and trauma team was important as well as education of of the public.
