Background: As the global pandemic of severe acute respiratory syndrome coronavirus-2 continues, so does the need for molecular tests with simple, painless, and easy sample collection. We aimed to investigate the potential use of saliva and anterior nasal swab samples as a noninvasive and selfcollectable specimen for the molecular diagnosis of coronavirus disease 2019 (COVID-19). Methods: We prospectively collected the saliva, anterior nasal swab, and conventional nasopharyngeal swab samples at the same time of patients hospitalized in a public hospital from March 2021 to September 2021. Patients were assessed for symptoms, and real-time polymerase chain reaction (RT-PCR) was performed on the samples. The results and cyclethresholds (Ct) of the three sample types were compared. Results: Three types of samples per patient were collected, and a total of 135 samples were collected. The mean age of the patients was 39.5 years (range, 9–71 years). The positive rates were 82.2% for nasopharyngeal swab samples, 35.6% for anterior nasal swab samples, and 55.6% for saliva samples. No significant differences in the Ct values between samples (P =0.256) were observed when comparing only the sets which were positive in all three sample types. Using nasopharyngeal swab RT-PCR as the reference standard, the sensitivity and specificity of the combined saliva and anterior nasal swab RT-PCR were 70.3% and 62.5%, respectively. Conclusions: Saliva and anterior nasal swab samples could not be used as alternative samples for COVID-19 diagnosis. Further studies with a larger number of patients are needed in the future.

Background: Chryseobacterium indologenes is ubiquitous in nature and rarely causes infections. However, the clinical impact of C. indologenes has increased in recent years, especially in immunocompromised patients, and has resulted in high mortality rates. We aimed to investigate the clinical and microbiological characteristics of C. indologenes bacteremia. Materials and Methods: We retrospectively reviewed medical records of a 642-bed university-affiliated hospital in Korea, dating from January 2001 to December 2020, to investigate C. indologenes bacteremia. Results: A total of 22 C. indologenes isolates were identified from blood culture records. All patients were hospitalized at the time of bacteremia, and the most common manifestation was primary bacteremia. A sizable majority of the patients (83.3%) had underlying diseases, and all patients received intensive care unit care during their admission. The 14-day and 28-day mortality rates were 8.3% and 16.7%, respectively. Importantly, all C. indologenes isolates were 100% susceptible to trimethoprim-sulfamethoxazole. Conclusion: In our study, most of the infections were hospital-acquired, and the susceptibility pattern of the C. indologenes isolates showed multidrug resistance. However, trimethoprim-sulfamethoxazole is a potentially useful antibiotic for C. indologenes bacteremia treatment. More attention is required to identify C. indologenes as one of the most important nosocomial bacteria with detrimental effects in immunocompromised patients.

External quality assessment (EQA) of peripheral blood smear test for malaria was conducted as a pilot project in 2020 by the Korean Association of EQA Service. Two trials were performed with three different cases per trial. Scanned images for the whole slide with brief patient histories were distributed to the participating laboratories online. In total, 208 and 232 laboratories participated in the two trials, respectively. The acceptable rates for malaria detection were 96.1%–100.0% and those for identifying malarial species were 68.1%–97.6%. The acceptable rates for parasitemia count were 87.2%–94.6%. The EQA scheme assessing peripheral blood smear test for malaria can contribute to the improvement of the diagnostic ability of the participating laboratories.
Status of Pre-analytical Quality Management of Laboratory Tests at Primary Clinics in Korea

Background: The quality of laboratory test results is crucial for accurate clinical diagnosis and treatment. Pre-analytical errors account for approximately 60%–70% of all laboratory test errors. Laboratory test results may be largely impacted by pre-analytical phase management. However, primary care clinics currently do not have pre-analytical quality management audit systems. We aimed to understand the current status of pre-analytical quality management in laboratory medicine in Korean primary care clinics. Methods: Questionnaires were designed to focus on essential components of the pre-analytical process of primary care clinics. An online survey platform was used to administer the survey to internal medicine or family medicine physicians in primary care clinics. Results: A total of 141 physicians provided a complete response to the questionnaire. In 65.2% of the clinics, patient information was hand-labeled rather than barcoded on the specimen bottles; 14.2% of clinics displayed only one piece of patient information (name or identification number), and 19.9% of clinics displayed two pieces of information. Centrifuges were not available in 29.1% of the clinics. Institutions carrying out the National Health Screening Program (NHSP) used more barcode system and had more centrifuges than institutions that did not carrying out the NHSP. Conclusions: Pre-analytical quality management is inadequate in many primary clinics. We suggest implementation of a mandatory management system, allowing for a pre-analytical quality management to be carried out in primary care clinics.

Objectives: This study is a practice based research conducted to investigate the effectiveness and safety of a 5-day short-term diet program ('Oil-rescue' program) designed to reduce the adverse events and initial dropout rate in obesity treatment. Methods: We retrospectively analyzed 4 Korean medicine clinic patient data who completed 'Oil-rescue' program which is consisting of Gambi-hwan, Bium-hwan, Butgiban-hwan and Jayoon Kyungokgo. The weight change before and after participating 'Oil-rescue' program was primary outcome of our study. Changes in body mass index, body fat mass, skeletal muscle mass, body water content, lean body mass, protein, and minerals were also measured. Results: A total of 35 patients who satisfied the eligible criteria were finally included. The body weight decreased from 69.45±11.86 kg to 67.43±11.58 kg, a total of 2.02±1.03 kg (P<0.001). Body fat mass decreased from 25.77±7.45 kg to 24.98±7.26 kg, a total of 0.78±1.21 kg (P<0.001). Body mass index decreased from 26.39±3.64 kg/㎡ to 25.64±3.49 kg/㎡, a total of 0.75±0.41 kg/㎡. (P<0.001). A total of 15 patients had side effects such as diarrhea, abdominal pain, and insomnia, but all were mild. 16 out of 35 people switched to long-term obesity treatment programs. Conclusions: Through this retrospective practice based research, it was found that the 'Oil-rescue' program effectively reduced body weight, body fat, and body mass index, and the other obesity related parameters. It was a relatively safe and effective short-term obesity treatment program.

A single spray plume is the basic unit of the entire spray plume and is an important factor in understanding the spray characteristics. However, since the multi-hole GDI injector has a narrow spray angle, the superposition of the spray plumes occurs severely. Therefore, the spray uniformity and the spray atomization characteristics of a single spray plume were analyzed in this study using a single-hole GDI injector. Five single-hole GDI injectors with different nozzle hole diameters were used in the experiment. The uniformity of the spray was evaluated through the analysis of the spray pattern images. In addition, the atomization characteristics were compared using the diameter distribution of the spray droplets obtained using PDPA. As a result, the larger diameter of the nozzle hole, the less uniformity of the spray, and the injection pressure did not have a significant effect on the spray uniformity. It is judged that the surface roughness of the injector has a greater effect on spray uniformity than the diameter of the nozzle hole. Also, the size of the spray droplets increased sharply when the diameter of the nozzle hole was 230 μm.

The purpose of this study is to compare the injection and spray characteristics of single-hole GDI injectors using injection rate and mie-scattering spray images. Five types of single-hole injectors with different nozzle hole diameters were used, and the spray rate, spray tip penetration, spray area, and spray width were analyzed. As a result, the diameter of the nozzle hole had a direct effect on the injection and spray characteristics. It was confirmed that the larger the diameter of the nozzle hole, the higher the injection quantity, the spray tip penetration, the spray area, and the spray width. In addition, it was confirmed that the near-field spray, which has little influence of ambient air, has a great correlation with the injection rate.
