        The role of FGF-2 in smoke-induced emphysema and the therapeutic potential of recombinant FGF-2 in patients with COPD

        <▼1><P>Although the positive effects of recombinant fibroblast growth factor-2 (rFGF-2) in chronic obstructive pulmonary disease (COPD) have been implicated in previous studies, knowledge of its role in COPD remains limited. The mechanism of FGF2 in a COPD mouse model and the therapeutic potential of rFGF-2 were investigated in COPD. The mechanism and protective effects of rFGF-2 were evaluated in cigarette smoke-exposed or elastase-induced COPD animal models. Inflammation was assessed in alveolar cells and lung tissues from mice. FGF-2 was decreased in the lungs of cigarette smoke-exposed mice. Intranasal use of rFGF-2 significantly reduced macrophage-dominant inflammation and alveolar destruction in the lungs. In the elastase-induced emphysema model, rFGF-2 improved regeneration of the lungs. In humans, plasma FGF-2 was decreased significantly in COPD compared with normal subjects (10 subjects, <I>P</I> <I>=</I> 0.037). The safety and efficacy of inhaled rFGF-2 use was examined in COPD patients, along with changes in respiratory symptoms and pulmonary function. A 2-week treatment with inhaled rFGF-2 in COPD (<I>n</I> = 6) resulted in significantly improved respiratory symptoms compared with baseline levels (<I>P</I> <I><</I> 0.05); however, the results were not significant compared with the placebo. The pulmonary function test results of COPD improved numerically compared with those in the placebo, but the difference was not statistically significant. No serious adverse events occurred during treatment with inhaled rFGF-2. The loss of FGF-2 production is an important mechanism in the development of COPD. Inhaling rFGF-2 may be a new therapeutic option for patients with COPD because rFGF-2 decreases inflammation in lungs exposed to cigarette smoke.</P></▼1><▼2><P><B>Lung disease: Inhaling a protein might help</B></P><P>Studies on the role of the protein ‘fibroblast growth factor-2’ (FGF-2) in chronic obstructive pulmonary disease (COPD) suggest that inhaled FGF-2 could help treat the emphysema linked to smoking. Researchers in South Korea led by Young-Koo Jee at Dankook University, Cheonan, and Yeon-Mok Oh at the University of Ulsan, Seoul, studied the role of the reduced FGF-2 levels found in mice with lung inflammation caused by exposure to cigarette smoke. They also uncovered details of a protective effect of inhaled FGF-2, identifying specific cellular and lung structure changes attributed to the administered FGF-2. Reduced FGF-2 levels were also found in patients with COPD. Initial trials revealed some improvement in patients treated with FGF-2, but not at a statistically significant level. Nevertheless, the authors suggest their results justify further investigation of the protein’s therapeutic potential.</P></▼2>

        복수과 진료 환자의 중복 처방에 대한 분석 및 평가

        In an aging society, there is an increasing possibility of the duplication of the drugs given to patients because they take many kinds of drugs. Among the prescriptions given to the patients who was treated at multiple ambulatory clinics in St. Mary's hospital for one month of September, 2002, in which drugs that has identical or similar effects are prescribed, we analysed and evaluated them by patients' characteristics, kinds of medication, severity of side effects on a case by case basis. More, we assessed the potential additional costs. As a result of this study, the duplication rate turned out to be 6.69% and it was shown that the proportion of the elderly patients over 60 was high. The gastro-intestinal medications took up a large part and most of the cases showed that the danger caused by double taking of medicine was slight, but there were some examples in which the serious side effects were predicted. Expected additional costs were the average 11.349 won and it ranges from the minimum of 56 won to the maximum of 135,720 won. In conclusion, the management of drug histories of the patients who need the plural treatments is very important and the necessity of the individual and professional guidance of taking medicines for the elderly patients is emerged, too.

        Backgrognd: Bronchial hbyperresponsiveness (BHR) is a key feature of asthma, and may precede the development of asthma. Genetically determined and acquired factors may contribute to development of BHR. Objective .' To evaluate expression of bronchial responsiveness to methacholine according to age, sex, smoking habit, and atopy in general population without bronchial asthma, a cross sectional study was performed. Method: A total of 1,190 general population who composed of 408 subjects with age less than 19 years (young age group), 621 subjects with age from 20 to 40 years (middle age group), and 161 subjects with age more than 41 years (old age group) were enrolled. Evaluations were made by a questionnaire, serum IgE level and skin prick test to common inhalant allergens, and methacholine bronchial provocation test (MBPT). Bronchial responsiveness were evaluated by positive rate of MBPT (PC,p-methacholine 4 25mg/ml), and slope of dose- response curve (slope, %fall of FEV, / log[last concentration of methacholine, mg/ml]). Result: Positive rate of MBPT was 11.0%, and slope (mean+SE) was 10.6+0.2 %/mg/ml. Postive rate of MBPT was more prevalent in the young age group than in middle and old age groups (19.6% vs. 6.6% vs. 6.2%, p<0.05), and slope was higher in young age group than in other groups (14.4+0.4 vs. 8.6+0.3 vs. 8.9+0.5 %/mg/ml, p<0.05). No significant differences in positive rate of MBPT and slope were noted according to sex in young and old age groups. However, in the middle age group, slope was higher in females than in males (9.5+0.4 vs. 7.9+ 0.3 %/mg/ml, p<0.05). No significant differences of slope was observed according to smoking habit in males of middle age group, but in males of old age group, the slope was higher in subjects with smoking habit than those without it (9.6+0.8 vs. 6.5+0.9 %/mg/ml, p<0.05). Sig- nificant relationship was observed between geometric value of serum IgE level and slope(r=0. 152, p=0.009). The postive rate of MBPT and slope were significantly higher in subjects with positive skin rea,ctivity to common inhalant allergens than those without it (14.3% vs. 8.6%, p ( 0.05; 11.8+0.4 vs. 9.8+0.3 %/mg/ml, p<0.05). The difference of bronchial responsiveness according to skin reactivity was observed in young and middle age groups, but not in old age group. Conclusion : Bronchial responsiveness to methacholine is significantly higher in children than in adults, in middle-aged females than in middle-aged males. Atopy and smoking may have a dif ferent role to determine the bronchial responsiveness depending upon age and sex.

      • Heritability of muscle mass in Korean parent–offspring pairs in the Fifth Korean National Health and Nutrition Examination Survey (KNHANES V)

        <P><B>Abstract</B></P> <P><B>Objectives</B></P> <P>Decreased muscle mass is known to be associated with several serious medical conditions. We analyzed the Fifth Korean National Health and Nutrition Examination Survey (KNHANES V, 2010–2011) to estimate the heritability of muscle mass in Korean parent-offspring pairs.</P> <P><B>Study Design</B></P> <P>Cross-sectional.</P> <P><B>Main outcome measures</B></P> <P>A total of 1233 parents (average age 57.67 ± 8.50 years) and 917 offspring (average age 29.10 ± 7.57 years) from 743 families were included in the analysis. Muscle mass was estimated based on three different indices: appendicular skeletal muscle mass (ASM) measured with a dual-energy X-ray absorptiometry (DXA), weight-adjusted ASM (SMI), and height-adjusted ASM (RASM). The heritability was estimated by employing the maximum-likelihood variance components implemented in Sequential Oligogenic Linkage Analysis Routines (SOLAR). The best-fitting model was determined out of four polygenic models. Pearson’s partial correlation coefficient was also calculated using the muscle mass indices to further study the association between father or mother and son or daughter pairs.</P> <P><B>Results</B></P> <P>The heritability estimates of the muscle mass indices ranged from 55% to 80% (all p < 0.01). The correlation coefficient of father and offspring ranged from 0.11 to 0.40, while that of mother and offspring ranged from 0.23 to 0.43 (all p < 0.01).</P> <P><B>Conclusions</B></P> <P>The heritability estimates of muscle mass in Koreans are large and significant, suggesting that parental muscle mass is an important predictor of the offspring’s muscle mass. The result implies that there may be a genetic factor partly determining muscle mass.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The heritability of muscle mass in Korean parent–offspring pairs was estimated. </LI> <LI> All three indices of muscle mass examined showed significant heritability. </LI> <LI> This is the first study of the heritability of muscle mass using a nationwide cohort in an Asian country. </LI> </UL> </P>

        암세포 Bcl-2 family 유전자 군의 DNA 메틸화 연구

        목적: 본 연구는 암세포주와 난소암 조직에서 세포예정사의 핵심 조절 단백질인 Bcl-2 family 유전자의 DNA 메틸화 여부를 암과의 관련성을 밝히는데 그 목적이 있다. 연구 방법: 자궁경부암 세포주인 HeLa, CaSki 그리고 만성골수성 혈액암 세포주인 K562 세포에서 genomic DNA를 추출하여 sodium bisulfite를 통한 cytosine 염기를 uracil로 치환하였다. 치환된 염기서열은 methylation과 unmethylation을 특이적으로 확인할 수 있는 primer를 이용하여 MSP (Methylation Specific PCR)을 수행한 후 암세포에서 Bcl-2 family 유전자의 DNA 메틸화 여부를 판별하였다. 결과: 각각의 세포주에서 antiapoptotic Bcl-2 family 유전자 군인 Mcl-1과 Bcl-2 유전자는 DNA 메틸화가 이루어지지 않은 것으로 관찰되었으며, proapoptotic Bcl-2 family 구성원인 Harakiri 유전자는 DNA 메틸화가 이루어진 것을 확인하였다. 반면, 다른 proapoptotic Bcl-2 family 유전자인 Noxa 유전자는 DNA 메틸화가 이루어지지 않은 것으로 관찰되었다. 또한 난소암 조직에서의 DNA 메틸화 여부를 살펴본 결과 Mcl-1과 Noxa는 세포주에서와 같은 결과를 보였고, Harakiri 유전자의 경우 hypomethylation으로 관찰되었다. 결론: 본 연구는 암세포주와 난소암 조직에서 proapoptotic Bcl-2 family 유전자인 Harakiri와 Noxa의 경우, 유전자 특이적인 DNA의 메틸화가 나타났으며 antiapoptotic 유전자는 DNA의 메틸화가 나타나지 않았다. 이로써 Bcl-2 유전자군과 암세포의 세포사멸을 억제하는 기작을 DNA의 메틸화를 통한 특이 유전자 발현 억제에 의해서 이루어질 수 있다고 사료된다. Objective: Promoter methylation of Bcl-2 family genes in cancer cells were studied to verify possible correlation between DNA methylation pattern of Bcl-2 family members and cancer. Methods: The genomic DNAs were extracted from different cancer cell lines, HeLa, CaSki and K562, and ovarian cancer tissue from patients. The cytosine residues were converted to uracil by sodium bisulfite treatment. MSP (methylation specific PCR) was performed to determine the methylation status of Bcl-2, Mcl-1, Noxa, and Harakiri promoters. Using primers that distinguish methylated DNA from unmethylated DNA after bisulfite modification of DNA, MSP was conducted to observe the methylation pattern of Bcl-2 family genes in different cancer cells. Results: The promoter regions of Bcl-2 family genes including Mcl-1, Bcl-2, and Noxa were not methylated in cancer cells, whereas the proapoptotic Bcl-2 family gene Harakiri was detected as methylated in the cancer cell lines and hypomethylated in the ovarian cancer tissue. Conclusion: The present study demonstrated the differential methylation profiles of Bcl-2 family genes in cancerous cells, which suggests a possible connection between the methylation pattern of some of Bcl-2 family genes and ovarian cancer.

        Regulation of interleukin-11 expression in ovulatory follicles of the rat ovary

        <P> The aim of the present study was to examine the regulation of interleukin (IL)-11 expression, as well as the role of IL-11, during ovulation in gonadotropin-primed immature rats. Injection of equine chorionic gonadotropin (eCG), followed by human CG (hCG) to induce superovulation stimulated expression of the Il11 gene in theca cells within 6 h, as revealed by northern blot and in situ hybridisation analyses. Real-time reverse transcription-polymerase chain reaction analysis showed that the IL-11 receptor, α subunit gene was expressed in granulosa and theca cells and that injection of hCG had no effect on its expression. IL-11 protein expression was stimulated in theca cells by hCG. LH-stimulated increases in Il11 mRNA levels in cultured preovulatory follicles were inhibited by protein kinase A and mitogen-activated protein kinase kinase inhibitors. Toll-like receptor (TLR) 2 and TLR4 were detected in preovulatory follicles, and the TLR4 ligand lipopolysaccharide, but not the TLR2 ligand Pam3Cys, increased Il11 mRNA levels in theca cells, but not in granulosa cells. Treatment of preovulatory follicles with IL-11 stimulated progesterone production and steroidogenic acute regulatory protein (Star) gene expression. Together, these results indicate that IL-11 in theca cells is stimulated by mitogen-activated protein kinase signalling and TLR4 activation, and increases progesterone production during ovulation. </P>

        자기효능감과 자아탄력성이 초등학생의 학습된 무기력에 미치는 영향

        본 연구는 초등학생의 자기효능감과 자아탄력성이 학습된 무기력에 미치는 영향을 밝히고자 하였다. 연구대상은 초등학교 4, 5, 6학년 809명이었고 학습된 무기력, 자기효능감, 자아탄력성 설문지를 실시하였다. 자료분석은 SPSS 14 통계처리프로그램으로 연구대상의 일반적 특성을 확인하였고, 학습된 무기력, 자기효능감, 자아탄력성 간의 Pearson 상관계수, 학습된 무기력, 자기효능감, 자아탄력성 간의 중다회귀분석을 실시하였다. 자기효능감과 자아탄력성이 초등학생의 학습된 무기력에 미치는 영향을 분석한 결과, 첫째, 자기효능감과 자아탄력성은 학습된 무기력과 부적 상관이 있는 것으로 나타났다. 둘째, 자기효능감의 하위변인인 자신감과 자기조절 효능감은 초등학생의 학습된 무기력에 영향을 미치는 것으로 나타났다. 셋째, 자아탄력성의 하위변인인 친구관계, 활력성, 감정통제는 초등학생의 학습된 무기력에 영향을 미치는 것으로 나타났다. 넷째, 학습된 무기력의 하위변인에 대한 자기효능감과 자아탄력성의 하위변인의 영향력을 분석한 결과, 자기효능감의 하위변인인 자신감과 자아탄력성의 하위변인인 활력성, 친구관계가 학습된 무기력의 하위변인에 많은 영향을 미치는 것으로 나타났다. 이러한 연구결과가 학습된 무기력 수준이 높아서 다양한 정서, 행동적 문제를 가진 초등학생들을 지도하는 교사나 상담사에게 주는 시사점을 논의하였으며, 후속연구를 위한 제언을 하였다. The purpose of this study was to analyse the effects of self-efficacy, self-resilience on learned-helplessness of Elementary school students. The Participants were 809 4th~6th elementary school students and learned-helplessness, self-efficacy, self-resilience were measured with questionnaire based on several studies. The data analyzed by using frequence, percentage, Pearson correlation, multiple regression. The research findings were as follows. First, There were negative correlations between learned-helplessness with self-efficacy and self-resilience. Second, Subordinate concept, confidence and efficacy of self-adjustment of self-efficacy have effects on learned-helplessness of elementary school students. Third, Subordinate concept, relationship of friend, vitality, emotion-control of self-resilience have effects on learned-helplessness of elementary school students. Forth, Subordinate concept, confidence of self-efficacy and vitality, relationship of friend of self-resilience have effects on subordinate concept of learned-helplessness. The results of the study showed that learned-helplessness of elementary school students is decreased by increasing self-efficacy, self-resilience to theaters and therapists. Finally, implications for future studies and the limitations of this study were descussed.
