Bolus Transition During Oropharyngeal Swallowing after Unilateral Cortical Stroke

Purpose: Bolus transition during swallowing represents neuromuscular coordination between oropharyngeal structures and bolus movement. The purpose of this study was to examine the difference between stroke patients with left and right hemisphere lesion in terms of oral and pharyngeal transitions using oral transit time (OTT), pharyngeal transit time (PTT), and duration of upper esophageal sphincter opening (DUESO) during swallowing. Methods: Thirty-four unilateral cortical stroke patients were divided into two groups depending on the hemispheric site of the lesion. Means and standard deviations of OTT, PTT, and DUESO were measured on 2 mL and 5 mL of thin liquid swallows from videofluoroscopic swallowing examinations (VFSEs). Statistical comparison was performed by repeated measurements of analysis of variance (ANOVA). Significance level was set at <0.025. Results: In OTT, PTT, and DUESO, there was no significant difference between left and right hemispheric stroke groups. There was a significant bolus volume effect on OTT and DUESO. Conclusions: The site of the lesion may not affect the transition of bolus during oropharyngeal swallowing. Clinicians should not only examine physiological swallowing disorders of individual patients regardless of hemispheric site of the lesion, but also investigate various volumes during VFSEs in order to find the proper diet recommendation.

Objective To examine the relation between the presence of penetration or aspiration and the occurrence of the clinical indicators of dysphagia. The presence of penetration or aspiration is closely related to the clinical indicators of dysphagia. It is essential to understand these relationships in order to implement proper diagnosis and treatment of dysphagia. Methods Fifty-eight poststroke survivors were divided into two groups: patients with or without penetration or aspiration. Medical records and videofluoroscopic swallowing examinations were reviewed. The occurrence of clinical indicators of dysphagia between two groups was analyzed with Cross Tabulation and the Pearson chi-square test (p<0.05).Results Poststroke survivors with penetration or aspiration had significantly high occurrences of delayed initiation of the swallow (p=0.04) and reduced hyolaryngeal elevation (p<0.01) than those without penetration or aspiration.Conclusion The results of this study indicate that delayed initiation of the swallow is a strong physiological indicator of penetration or aspiration during the oral stage of swallowing in poststroke survivors. For the pharyngeal stage of swallowing, hyoid and laryngeal elevation is a key event related to occurrence of penetration or aspiration. Clinical indicators should be investigated further to allow appropriate implementation of treatment strategies for stroke survivors.

The goal of the present study was to examine the design patterns of multimodal information representation that elementary students used to create a project display board, and to gain insights into their emerging identities as multimodal text authors. The study was conducted at an elementary English dual-immersion school located in an urban city, Korea. Year-long inquiry-based instruction was offered for the entire body of fourth- to six-grade students. Students created their own project display boards as the culminating product of their inquiry learning, which were collected as the data for the examination of multimodal design patterns. Descriptive data analysis was conducted to identify the array of design elements, features, and layouts used in the display boards and to classify these student authors according to their different multimodal engagement. The analysis resulted in three related but distinctive profiles of student authors, including (a) emergent authors mainly using images for aesthetical purposes, (b) conventional authors using written text as the dominant mode of information representation, and (c) developing multimodal authors integrating written texts and images into a spatial framing of messages. The result indicated different stages of multimodal literacy development with respect to use of design features, genre awareness, and audience consideration. Implications are discussed in relation to literacy research and practice.

자연을 모티브로 한 소재는 에너지, 전자, 생물의학 등 다양한 분야에 걸쳐 활용되고 있다. 그중 식물이나 해양 생물에서 주로 발견되는 폴리페놀을 화학적으로 모사한 바이오 소재가 생체 의학 분야에서 그 두각을 드러내고 있다. 자연계 폴리페놀은 특이적인 카테콜 및 갈롤 작용기의 존재로 인해 수중 접착력, 자가 조립 능력, 분자 가교 능력 등의 생리학적 특성을 나타내는데, 위 화학적 원리를 기반으로 설계된 인공폴리페놀 소재 역시 자연에서 발현되는 특이적인 성질을 그대로 나타낸다. 그뿐만 아니라, 자연 유래 폴리페놀은 기본적으로 뛰어난 생분해성과 생체 적합성을 나타낸다. 때문에, 폴리페놀에서 영감을 얻은 기능성 소재는 조직 공학, 재생 의학 등의 응용 분야에서 자주 차용되었다. 본 글에서는 그중에서도 가장 활발하게 연구가 진행되고 있는 폴리페놀 기반의 바이오 접착제, 바이오 코팅 소재, 하이드로젤 기반의 바이오 프린팅 재료, 약물 전달 시스템의 최근 연구 동향을 중점적으로 소개하여, 의료 기술 분야에서의 혁신을 도모하는 폴리페놀의 다재다능함과 잠재력을 홍보하고자 한다.

We report on the influence of water as an electrolyte on the photovoltaic performances. The photovoltaic performance was shown to be quite sensitive to the substituent on the donor group. An optimized efficiency of 4.41% in the presence of 100% water content using I−/I3 − redox couple was obtained using the D21L6 organic dye.
교사의 평가 경험을 통한 초등 영어 성취도 평가의 방향 탐색

This study explores future tasks and challenges of the school-based English achievement test in elementary schools. Ten elementary school teachers who had been teaching for over five years and teaching English for over two years were selected as participants. This study examined the procedures and the usefulness of the school-based English achievement test by means of a survey and interviews. The results indicate some problems associated with the test: loss of assessment purpose, lack of convenience in item development, and an absence of feedback after the test. In terms of test usefulness, the teachers mostly thought that the school-based achievement test was valid, but the results were not reliable. Both the authenticity and practicality of the achievement test were considered low because, according to the teachers, it did not reflect the goals of elementary English education; and because, in teachers’ opinions, it takes a relatively long time to develop and conduct the test compared to its importance. The backwash effects were controversial. Some teachers thought the achievement test provided a positive effect such as an increase in student motivation and an improvement in learning attitude. Others, however, thought that it could make students fear and be less interested in English. Last, the teachers had not received any training or been provided with educational programs for testing in English education; they felt that there were no training or educational programs for teachers to learn about what to test and how to develop the tests. In conclusion, this study suggests that the achievement test needs to be developed in different ways with variable methods, and there should be instructions for teachers with respect to how to provide students with appropriate feedback. It is also important to give teachers the opportunity to learn about testing in English education. 본 연구의 목적은 교사들의 평가 경험을 통한 초등학교 교내 영어 성취도 평가의 분석을 통해 실제 초등학교 현장에서 시행되고 있는 성취도 평가의 문제점을 살펴보고, 초등영어 성취도 평가의 과제에 대해 탐색하고자 하는 것이다. 이를 위해 교사들을 대상으로 서술형 설문 및 추가 면담을 실시하였다. 본 연구결과, 교내 영어 성취도 평가의 시행절차에서는 평가의 목적 상실, 편의주의적 문항 제작, 평가 시행 후 피드백 부재와 같은 문제점이 드러났다. 또한 타당성과 신뢰성의 측면에서 대부분의 교사들은 성취도 평가는 타당하다고 인식하였으나 평가 결과는 신뢰롭지 못하며, 초등영어교육의 목표를 반영하고 있지 못한다는 점 때문에 성취도 평가는 진정성이 낮다고 응답하였고, 평가의 실용성에 대해는 성취도 평가의 중요도에 비해 상대적으로 필요 이상의 시간과 노력이 많이 들어간다고 지적하였다. 성취도 평가가 학생들에게 주는 긍정적인 역류 효과로는 학습동기유발과 학습태도 개선 등이 있다고 보았으나, 부정적인 역류 효과로 학생들의 영어에 대한 두려움 및 흥미저하 등이 언급되었다. 마지막으로 교사들의 영어 평가 관련 연수 및 교육 경험은 거의 없는 것으로 나타났는데, 현재 영어 평가와 관련하여 수강가능한 연수가 없기 때문이었다. 결론적으로, 성취도 평가는 초등영어교육과정에서 제시하는 성취 수준의 도달 여부뿐 아니라 수행평가나 포트폴리오 평가 같은 수시평가를 통해서는 평가할 수 없는 전체적인 의사소통 능력의 성취에 대한 평가가 가능하다는 점에서 주목할 필요가 있다. 따라서 교내 영어 성취도 평가의 방법을 다양화하는 방향과 더불어 평가 결과를 활용하는 방법에 대한 지침을 마련해야 하며, 영어 평가와 관련한 연수기회 제공 및 성취도 평가에 대한 학생과 교사의 인식 개선이 필요하다.
