Proceedings of international Symposium on East-West Medicine, Seoul. 44-64, 1999.-Cordyceps sinensis is a major parasitic fungus on larva of Lepidoptera. It is one of the well know fungi used in traditional Chinese medicine for treatment asthma, lung and kideney diseases. Many fungi belonging to the genus Cordyceps have been demonstrated to produce natural products with various biological activities. Reported biological activities of genus cordyceps include:(a)inhibition of DNA and RNA synthesis;(b)enhancement of cell differentiation;(c)restruction of cytoskeleton;(d)inhibition of protein kinase activity;(e)antitumor activity on bladder, colon, lung carcinoma as well as fibroblastoma;(f)inhibition of the infection and revers transciptase activity of human immunodeficiency virus type 1; and(g) inhibition of methylation of nucleic acid. Thus, it is suggested that the extracts of Cordyceps sp. Contain antitumor, antiviral, and many other biological activities. In our previous studies, we proved that C. sinensis contains the antitumor and immunomodulatory agents. In the present studies, effects of C. sinensis on bronchoalveolar lavage fluids(BAL)cells were demonstrated. The crude methanolic extracts of C. sinensis were fractionated into 15 fractions by silica gel column chromatography. Effects of various fraction on BAL cells proliferation and lipopolysaccharide(LPS) stimulated interleukin-1β(IL-8),platelet activating factor(PAF), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8) production on BAL cells were determined. The result indicated that the CS-19-22 fraction suppressed BAL cells proliferation activated by LPS. The median inhibitory concentration(IC50) was 6㎍/ml. The CS-19-22 fraction also decreased IL-1β,PAF,IL-6, TNF-αand IL-8 production.The results of revers transcription-polymerase chain reaction(RT-PCR) demonstrated that the CS-19-22 fraction did not affect IL-1β,PAF,IL-6, TNF-α,and IL-8 mRNAs expression in BAL cells activated by LPS. By contrast, it inhibited the IL-10 mRNA expression but enhanced IFN-γand IL-12 mRNAs expression in activated BAL cells. Moreover, the CS-19-22 fraction blocked PAF-induced platelet aggregation. It is unlikely that cytotoxicity was involved. Because no cell deths were observable. We hypothesize that inhibitory mechanisms of CS-19-22 on BAL cells proliferation may be related to the impairments of gene expression and production of cytokines in BAL cells. The explanation of C. sinensis antiasthma function may involved the following(1)C.sinenesis contained immunomodulatory agents that stimulated IFN-γand IL-12 expression in TH1cells.(2)Enhanced secretion of IFN-γand IL-12 will inhibit the Tγcells immune responses Ex. Decreasing of IL-6 and IL-10 production.(3) The impairments of cytokines production in TH2 cells suppressed th BAL cells proliferation then more cytokines production Ex.IL-1β,PAF,IL-6,TNF-αand IL-8 in activated BAL cells were decreased. (4)The suppression of TH2cells immune responses will inhibit the growth and differentiation of B cells then IgE production will be decreased. (5)Reduced production of IgE will decreased asthma attack occurring in individuals. However, the results of the present study suggested that C. sinensis may also have acted to treat asthma in part by inhibiting BAL cells proliferation and cytokine gene expression and production. Future, experiments with treatment of animals with asthma with C. sinensis will be necessary to defined whether C. sinensis can reduce asthma injury. Plans are underway for the isolation of pure principal growth inhibitors and their mechanisms of action are subjected for further study.

        안면신경마비에 대한 안륜근 미소안면침 증례보고

        Objective : This study was designed to evaluate the effect of Miso Facial Acupuncture on Orbicularis Oculi muscle. Methods : Two men and four women with Facial nerve palsy were recruited. Miso Facial Acupuncture on Orbicularis Oculi muscle was performed two times a week for three weeks. The effects of the treatment were evaluated by H-B scale, Y-system and clinical symptom. Results : Before treatment with Miso Facial Acupuncture, three patients showed H-B scale improvement and the other three patients had no change. and four patients showed Y-system improvement and the other two patients had no change. In six cases, the patients had improvement in clinical symptom related with Orbicularis Oculi muscle function. Conclusion : Miso Facial Acupuncture on Orbicularis Oculi muscle showed significant improvement in facial nerve palsy.

      • Comparison of postoperative changes in the distal and proximal segments between conventional and sliding mini-plate fixation following mandibular setback

        Objective: The purpose of the present study was to evaluate the postoperative three-dimensional (3D) changes in the proximal segments after mandibular setback sagittal split ramus osteotomy and to compare the changes between the conventional mini-plate fixation and semi-rigid sliding plate fixation. Methods: Cone-beam computed tomography (CBCT) images were used to evaluate the postoperative 3D changes in the proximal segments during the healing process. CBCT images were superimposed using the symphysis and the lower anterior mandible as references. Results: There were no statistically significant differences between the conventional mini-plate and semi-rigid sliding plate groups (p > 0.05). With respect to the distribution of changes greater than 2 mm in the landmarks, the right condylion, right coronoid process, and left condylion showed ratios of 55.6%, 50.0%, and 44.4%, respectively, in the semirigid sliding plate group; however, none of the landmarks showed ratios greater than 30% in the conventional mini-plate group. Conclusions: There were no statistically significant differences in postoperative changes in the segments between the conventional mini-plate and semi-rigid sliding plate groups. Nevertheless, while selecting the type of fixation technique, clinicians should consider that landmarks with greater than 2 mm changes were higher in the semi-rigid sliding plate group than in the conventional mini-plate group.

        Increase in Plasma Glucose Lowering Action of Rosiglitazone by Electroacupuncture at Bilateral Zusanli Acupoints (ST.36) in Rats

        Objectives: Hypoglycemia induced by electroacupuncture (EA) is due to an increase of insulin secretion and/or mediation of β-endorphin. We applied EA at the Zusanli (ST.36) acupuncture point (acupoint) in combination with rosiglitazone (TZD) administration to evaluate their effect on plasma glucose and to explore possible mechanisms of action. Methods: Thirty six normal adult Wistar rats were randomly divided into four groups: the 0.1 mg/kg TZD group (0.1TZD), 0.1 mg/kg TZD and EA group (0.1TZD + EA), EA group, and control group. In other experiments, streptozotocin was used to induce type 2 diabetes mellitus in neonatal rats; these were then randomly divided into a 0.1TZD group, 0.1TZD + EA group, and EA group and changes in plasma glucose and insulin concentrations evaluated. Results: A marked hypoglycemic response was observed in the normal rat 0.1TZD, 0.1TZD + EA and EA groups, with the response more significant in the 0.1TZD + EA group than in the 0.1TZD group. Among the diabetic animals, the hypoglycemic responses in the 0.1TZD + EA and EA groups were greater than in the 0.1TZD group. In both the normal and diabetic rats, insulin secretion was increased by EA or 0.1TZD + EA treatment, but not by 0.1TZD. Conclusions: The plasma glucose lowering action of rosiglitazone was increased by EA in both normal and diabetic rats, indicating that the application of EA may enhance the hypoglycemic action of this insulin sensitizer.         Objectives : This study is designed to investigate the effect of oriental medical therapy on Gu-Chang. Methods : We experienced one case of Gu-Chang treated with oriental medical treatment, such as acupuncture, herbal medication and medicines used externally. Results : After treatment, we were able to observe that the sore and the pain disappeared. Also, the general condition of the patient grew better. Conclusions : Oriental medical treatments were very effective for Gu-Chang.

        Comparison of Cortical Bone Trajectory to Pedicle-Based Dynamic Stabilization: An Analysis of 291 Patients

        Objective: Pedicle-based dynamic stabilization (DS) has gained popularity outside of America. Although pedicle screw (PS) loosening has always been a concern, it is reportedly innocuous. Cortical bone trajectory (CBT) screw is an emerging option with less invasiveness and similar effectiveness to PS in short-segment lumbar fusion. This study aimed to verify the use of CBT for DS by comparing the outcomes between pedicle- and CBT-based DS. Methods: Consecutive patients with lumbar spondylosis or low-grade spondylolisthesis who underwent 1- or 2-level DS between L3–5 with a minimum follow-up of 24 months were reviewed. Screw loosening was determined by computed tomography and the incidences were compared. Results: A total of 291 patients who underwent Dynesys DS (235 pedicle- and 56 CBT-based, respectively) were compared. The demographics and preoperative conditions were similar. All the clinical outcomes improved at 24-month postoperation, while the CBT-based group had less operation time and blood loss than the pedicle-based group. The rates of screw loosening were lower in the CBT-based (5.4% per screw and 12.5% per patient) than the pedicle-based group (9% per screw and 26.4% per patient). Furthermore, there were no differences in the clinical outcomes and complication profiles. Conclusion: The CBT-based DS for 1- or 2-level lumbar degeneration demonstrated equivalent clinical improvement as the pedicle-based DS. The adaption of CBT-based screws for DS could be a less invasive approach (shorter operation time and less blood loss), with lower chances of screw loosening than the conventional PS-based DS.

        Measurement of Deformity at the Craniovertebral Junction: Correlation of Triangular Area and Myelopathy

        Objective: Diseases of the craniovertebral junction (CVJ) are commonly associated with deformity, malalignment, and subsequent myelopathy. The misaligned CVJ might cause compression of neuronal tissues and subsequently clinical symptoms. The triangular area (TA), measured by magnetic resonance imaging/images (MRI/s), is a novel measurement for quantification of the severity of compression to the brain stem. This study aimed to assess the normal and pathological values of TA by a comparison of patients with CVJ disease to age- and sex-matched controls. Moreover, postoperative TAs were correlated with outcomes. Methods: Consecutive patients who underwent surgery for CVJ disease were included for comparison to an age- and sex-matched cohort of normal CVJ persons as controls. The demographics, perioperative information, and pre- and postoperative 2-year cervical MRIs were collected for analysis. Cervical TAs were measured and compared. Results: A total of 201 patients, all of whom had pre- or postoperative MRI, were analyzed. The TA of the CVJ deformity group was larger than the healthy control group (1.62 ± 0.57 cm2 vs. 1.01 ± 0.18 cm2 , p < 0.001). Moreover, patients who had combined anterior odontoidectomy and posterior laminectomy with fixation had the greatest reduction in the TA (1.18 ± 0.58 cm2 ). Conclusion: In CVJ deformity, the measurement of the cervical TA could indicate the severity of brain stem compression. After surgery, the TA had a varying degree of improvement, which could represent the efficacy of surgery.

        The Evaluation of Otilonium Bromide Treatment in Asian Patients With Irritable Bowel Syndrome

        Background/Aims Antispasmodics including otilonium bromide (OB) are recommended to treat irritable bowel syndrome (IBS). However, reports about OB experience in Asia is sparse. The purpose of present study was to provide the efficacy of OB in treating Asian IBS patients. Methods Overall, 117 IBS patients meeting Rome II criteria were enrolled in an 8-week, double-blind, active-controlled and single center trial. Randomized participants received either OB 40 mg or mebeverine 100 mg 3 doses daily. The primary endpoints were to evaluate the net changes of abdominal pain/discomfort frequency score (APDFS) and safety profile, while the secondary endpoints were to assess the changes in abdominal pain/discomfort intensity, flatulence, abdominal bloating, satisfied stool frequency etc. Results Finally, 49 OB and 52 mebeverine subjects were eligible for efficacy analysis. Compared to baselines in per protocol populations, the reduced APDFSs in OB and mebeverine were 0.55 ± 1.20 (P = 0.011) and 0.37 ± 1.11 (P = 0.042), respectively, to show similarly reduced scores. The most reported side effects included dry mouth, nausea and dizziness. Besides, the improved APDFSs at 4th week visit, final alleviations in abdominal pain intensity, flatulence, abdominal bloating and satisfied stool frequency with global assessments filled by both patients and investigators were significantly achieved by both treatments, and OB was not inferior to mebeverine in treating these parameters. Conclusions In Orientals, OB is as effective as mebeverine for alleviating IBS symptoms in terms of abdominal pain, flatulence, abdominal bloating etc. However, obvious side effects are also observed. A large-scaled trial and post-marketing surveillance are recommended to confirm its efficacy and safety. (J Neurogastroenterol Motil 2011;17:402-410)
