정당한 세상에 대한 믿음과 학교생활적응의 관계에서 사회적 관심과 대인관계 유능성의 매개효과

While Belief in a Just World (BJW) is an influential variable in adolescent mental health and adaptation, there is insufficient research on this topic. The purpose of this study was to investigate the mediation effects of social interest and interpersonal competence on the relationship between BJW and school adjustment. In order to examine the effect of BJW on school adjustment, survey data from 382 high school students (210 males, 172 females) was analyzed. The results indicated that social interest had a mediating effect on the relationship between BJW and school adjustment, whereas interpersonal competence did not. Additionally, the results of structural equation modeling indicated that social interest and interpersonal competence were mediated sequentially in the process of BJW affecting school adjustment. The research results were analyzed for differences and characteristics. Implications for the use of BJW in counseling practice are discussed. 정당한 세상에 대한 믿음(Belief in a Just World; BJW)은 청소년의 정신건강과 적응에 정적 영향을 미치는 변인으로 알려졌으나 국내에서의 학교현장과 관련된 연구는 부족한 실정이다. 본 연구는 정당한 세상에 대한 믿음과 학교생활적응의 관계에 영향을 미치는 사회적 관심과 대인관계 유능성의 매개효과를 검증하고자 하였다. 학교생활적응에 미치는 정당한 세상에 대한 믿음의 영향을 알아보기 위하여 고등학교에 재학 중인 고등학생 382명(남 210명, 여172명)의 자료를 기반으로 분석하였다. 자료 분석 결과, 정당한 세상에 대한 믿음과 학교생활적응의 관계에서 사회적 관심은 매개효과를 보였다. 그러나 대인관계 유능성은 정당한 세상에 대한 믿음과 학교생활적응의 관계에서 매개효과를 보이지 않았다. 또한 구조모형 검증결과 정당한 세상에 대한 믿음과 학교생활적응에 영향을 미치는 과정에서 사회적 관심과 대인관계 유능성이 순차적으로 부분 매개하는 것으로 나타났다. 마지막으로 연구결과에 대한 차이 및 특성을 해석하고 상담 장면에서 정당한 세상에 대한 믿음의 실제 활용방안을 논의하였다.

Some cases of Middle East Respiratory Syndrome-Coronavirus (MERS-CoV) infection presented renal function impairment after the first MERS-CoV patient died of progressive respiratory and renal failure. Thus, MERS-CoV may include kidney tropism. However, reports about the natural courses of MERS-CoV infection in terms of renal complications are scarce. We examined 30 MERS-CoV patients admitted to National Medical Center, Korea. We conducted a retrospective analysis of the serum creatinine (SCr), estimated glomerular filtration rate (eGFR), urine dipstick tests, urinary protein quantitation (ACR or PCR), and other clinical parameters in all patients. Two consecutive results of more than trace (or 1+) of albumin and blood on dipstick test occurred in 18 (60%) (12 [40%]) and 22 (73.3%) (19 [63.3%]) patients, respectively. Fifteen (50.0%) patients showed a random urine ACR or PCR more than 100 mg/g Cr. Eight (26.7%) patients showed acute kidney injury (AKI), and the mean and median durations to the occurrence of AKI from symptom onset were 18 and 16 days, respectively. Old age was associated with a higher occurrence of AKI in the univariate analysis (HR [95% CI]: 1.069 [1.013-1.128], P = 0.016) and remained a significant predictor of the occurrence of AKI after adjustment for comorbidities and the application of a mechanical ventilator. Diabetes, AKI, and the application of a continuous renal replacement therapy (CRRT) were risk factors for mortality in the univariate analysis (HR [95% CI]: diabetes; 10.133 [1.692-60.697], AKI; 12.744 [1.418-114.565], CRRT; 10.254 [1.626-64.666], respectively). Here, we report renal complications and their prognosis in 30 Korean patients with MERS-CoV.
Prediction of Patients Who Can Benefit from Oral Intestinal Sorbent AST-120

Indoxyl sulfate (IS) increases the expression of fibrogenic genes while it impairs anti- oxidative systems and down-regulates Klotho expression in the kidney. AST-120, which adsorbs uremic toxins and precursors including indole, can decrease serum and urinary levels of IS, the levels of advanced glycation end products (AGEs), and 8-hydroxydeoxyguanosine while it increases the renal expression of Klotho. In addition to the improvement of glomerular sclerosis, serum creatinine levels, and the survival in chronic kidney disease (CKD) animal models, AST-120 alleviated uremic symptoms and slowed the progression of renal dysfunction through small clinical investigations. There have been large clinical studies using AST-120.1-3 But they only recruited patients with advanced renal dysfunction and failed to prove the effectiveness of AST-120 except that Carbonaceous oral Adsorbent's effectiveness on Progression of CKD study showed less decrease of the estimated glomerular filtration rate (eGFR) in the AST-120 group.1
A Case Report of a Middle East Respiratory Syndrome Survivor with Kidney Biopsy Results

A 68-year old man diagnosed with Middle East Respiratory Syndrome-Coronavirus (MERSCoV) presented with multiple pneumonic infiltrations on his chest X-ray, and the patient was placed on a mechanical ventilator because of progressive respiratory failure. Urinary protein excretion steadily increased for a microalbumin to creatinine ratio of 538.4 mg/g Cr and a protein to creatinine ratio of 3,025.8 mg/g Cr. The isotope dilution mass spectrometry traceable serum creatinine level increased to 3.0 mg/dL. We performed a kidney biopsy 8 weeks after the onset of symptoms. Acute tubular necrosis was the main finding, and proteinaceous cast formation and acute tubulointerstitial nephritis were found. There were no electron dense deposits observed with electron microscopy. We could not verify the virus itself by in situ hybridization and confocal microscopy (MERS-CoV costained with dipeptidyl peptidase 4). The viremic status, urinary virus excretion, and timely kidney biopsy results should be investigated with thorough precautions to reveal the direct effects of MERS-CoV with respect to renal complications.
