Recent progress in laboratory diagnosis of thalassemia and hemoglobinopathy: a study by the Korean Red Blood Cell Disorder Working Party of the Korean Society of Hematology

Genetic hemoglobin disorders are caused by mutations and/or deletions in the -globin or -globin genes. Thalassemia is caused by quantitative defects and hemoglobinopathies by structural defect of hemoglobin. The incidence of thalassemia and hemoglobinopathy is increased in Korea with rapid influx of people from endemic areas. Thus, the awareness of the disease is needed. -thalassemias are caused by deletions in -globin gene, while -thalassemias are associated with decreased synthesis of -globin due to -globin gene mutations. Hemoglobinopathies involve structural defects in hemoglobin due to altered amino acid sequence in the - or -globin chains. When the patient is suspected with thalassemia/hemoglobinopathy from abnormal complete blood count findings and/or family history, the next step is detecting hemoglobin abnormality using electrophoresis methods including high performance liquid chromatography and mass spectrometry. The development of novel molecular genetic technologies, such as massively parallel se-quencing, facilitates a more precise molecular diagnosis of thalassemia/hemoglobin-opathy. Moreover, prenatal diagnosis using genetic testing enables the prevention of tha-lassemia birth and pregnancy complications. We aimed to review the spectrum and classi-fication of thalassemia/hemoglobinopathy diseases and the diagnostic strategies includ-ing screening tests, molecular genetic tests, and prenatal diagnosis.

Activity of Lactobacillus crispatus isolated from vaginal microbiota against Mycobacterium tuberculosis

Tuberculosis, an infectious disease, is caused by Mycobacterium tuberculosis. It remains a significant public health issue around the globe, causing about 1.8 million deaths every year. Drug-resistant M. tuberculosis, including multi-drug-resistant (MDR), extremely-drug-resistant (XDR), and totally drugresistant (TDR) M. tuberculosis, continues to be a threat to public health. In the case of antibiotic-resistant tuberculosis, the treatment effect of conventional antibiotics is low. Side effects caused by high doses over a long period are causing severe problems. To overcome these problems, there is an urgent need to develop a new anti-tuberculosis drug that is different from the existing compound-based antibiotics. Probiotics are defined as live microorganisms conferring health benefits. They can be potential therapeutic agents in this context as the effectiveness of probiotics against different infectious diseases has been well established. Here, we report that Lactobacillus crispatus PMC201 shows a promising effect on tuberculosis isolated from vaginal fluids of healthy Korean women. Lactobacillus crispatus PMC201 reduced M. tuberculosis H37Rv under co-culture conditions in broth and reduced M. tuberculosis H37Rv and XDR M. tuberculosis in macrophages. Lactobacillus crispatus PMC201 was not toxic to a guinea pig model and did not induce dysbiosis in a human intestinal microbial ecosystem simulator. Taken together, these results indicate that L. crispatus PMC201 can be a promising alternative drug candidate in the current tuberculosis drug regime. Further study is warranted to assess the in vivo efficacy and confirm the mode of action of L. crispatus PMC201.

정약용(丁若鏞)의 정의론(正義論)

다산 정약용의 정의론의 근본적인 문제의식은 인간의 보편적 존엄성의 현실적 담보 즉 인권의 보장에 있다. 다산의 정의론은 官吏를 비롯한 지배계층의 권리 문제에 집중하기 보다는 일반 백성들의 민생과 권리 문제에 초점을 둔다. 다산은 백성들이 현실적으로 봉착하고 있는 생명, 재산, 신분상의 不平等과 不公正의 문제를 주목하면서 이의 해결을 지향했다. 즉 다산은 노비의 신분으로 인해 초래되는 불평등 문제, 人情에 치우쳐서 공정성을 훼손하는 문제, 재산 처리 과정에서 야기되는 공정성 문제, 왕과 관련된 사안의 법적용상에서 야기되는 공정성 문제 등을 중심으로 정의론을 전개한다. 다산의 정의론의 입장은 엄격한 공정성만을 지향하는 것이 아니라. 일반 백성들의 어려움을 안타깝게 여기는 ‘欽恤’이라는 惻隱之心의 정신을 강조하는 정의론이다. 그러므로 다산의 정의론은 ‘愛民의 정의론’이라고 규정할 수 있다. 또한 다산은 정의 실현 문제에 있어서 人倫의 중요성과 의미를 강조하면서 정의와 인륜의 조화와 갈등 양상을 문제삼았다. 다산은 義理를 위한 것이라고 자임하면서 행한 살인의 문제와 조선의 烈女 추앙 분위기에서 인륜이라는 명분으로 恣行되는 復讐의 문제를 거론하면서 정의는 인륜의 가치를 초월할 수 없음을 역설한다. 또한 다산은 왕의 지위가 일반 백성과는 다르다는 점에 입각하여 왕의 親屬에게 법을 적용하는데 있어서 예외를 용인하는 차별적인 입장을 표명하고 있으며, 원수를 죽여 하늘을 같이 하지 아니한다는 입장에 근거하여 인륜에 어긋나는 행위를 한 사람을 私的으로 응징하는 復讐를 용인하고 있다. 이렇게 보면 다산의 정의론은 儒家의 애민의 정의론적 특성을 기저로 인륜을 강조하는 도덕주의적 양상이 두드러진다고 할 수 있다. 또한 정의문제에 있어서 왕에 대한 특수한 입지를 인정함으로써 인권과 왕권 사이에 미묘한 차별성을 현시하고 있다. The fundamental thoughts of the justice theory on the Jeong yak-yong is the respect of human dignity. In this context, Jeong yak-yong asserts that the realistic guarantee of human universal dignity is based on human rights. And Jeong yak-yong focus on peoples' problem of life, rights, and discrimination rather than concentrates on the rights of social leading class. Jeong yak-yong aims to resolve the problem which the people encountered discrimination and damage on the life, property, status. Therefore, the justice theory of Jeong yak-yong be called the justice of charity to the people. In order to resolve the injustice, Jeong yak-yong debates the issues that emotion causing injustice problem, property causing injustice problem, social state causing injustice problem, king related injustice problem. But Jeong yak-yong's theory of justice not only stress strict fairness but also attach greate importance to benevolence. And Jeong yak-yong emphasizes the importance of moral, at the same time he argues the conflict between justice and moral. The theory of justice on the Jeong yak-yong express progressive consciousness, but it includes the some point of era's paradigm. Those are as follows ; First, Jeong yak-yong rests on the basis of recognize the king's supreme state, so he accepts discrimination on the punishment of king's father's crime. Second, Jeong yak-yong recognizes the private homicide for a revenge conduct. The limitation on the Jeong yak-yong's justice theory reveals that he esteem the traditional culture.

한강수계 물이용부담금을 둘러싼 이해당사자 간 갈등분석

본 연구의 목적은 한강수계 물이용부담금을 둘러싼 이해당사자들의 갈등해결을 위한 최적방안을 제시하는 방법론적 틀을 설계하고, 분석결과를 통해 한강수계 물이용부담금 갈등해결을 위한 정책 방향을 제시하는 것에 목적을 두고 있다. 이를 위해 한강수계 물이용부담금을 두고 서로 다른 입장을 취하고 있는 의사결정자들 간의 다면적인 갈등의 메커니즘을 이해하는 과정이 선행되었으며 갈등해결을 위한 그래프모형을 적용하여 한강수계 물이용부담금 갈등 해결을 위한 최적방안을 도출하였다. 분석결과에 따르면 한강수계 물이용부담금 갈등의 해결점을 찾을 수 있는 최적의 상태는 서울·인천 지역이 지속적으로 물이용부담금 인하를 요구하는 것과 동시에, 서울·인천·경기 지역이 기존 부과요율에 따른 부과금을 납부하는 것이다. 그리고 한강수계관리위원회는 갈등 해결을 위해 물이용부담금 부과요율 결정에 대한 의사결정구조 합리화, 서울·인천 지역에 대한 한강수계관리기금 지원근거 마련, 수혜 지역 및 주민의 특성을 고려한 한강수계관리기금의 투명한 운용 등을 포함하는 서울·인천 및 경기 지역이 원하는 정책을 수립할 때 갈등의 해결점을 찾아갈 수 있었다. 본 연구는 한강수계 물이용부담금 갈등해결 관련 환경정책 방향을 수립하는데 객관적인 의사결정 정보 제공을 할 수 있고, 환경의 공동 사용에 따른 갈등과 관련된 유사한 후속연구에 방법론적인 기반을 제공할 것으로 기대된다. This research purposes to design a methodological framework to suggest the optimal method to resolve the conflicts of stakeholders surrounding the water use charge of the Han River region, and to use the analysis results to provide the direction of policy. For this, it was preceded that the process of understanding the mechanisms of the multi-faceted conflict between decision makers taking different positions over water use charge of the Han River region, and an optimal method to resolve the conflict of water use charge of the Han River region was derived by using a graph model for conflict resolution(GMCR). According to the analysis results, the optimal state to find a solution to the water use charge of the Han River is that the Seoul-Incheon-Gyeonggi region pays the charge according to the original rate while Seoul-Incheon requesting discount the water use rate. In addition, the Han River management committee should establish policies desired by Seoul-Incheon-Gyeonggi region including rationalization of the decision-making structure to determine the rate of water use charge, making the basis to support the Han River management fund system for the Seoul-Incheon region, and transparent management of the Han River management fund system considering the characteristics of beneficiary regions and residents. This study is expected to provide objective decision-making information in establishing environmental policy directions related to conflict resolution in the water use charge of the Han River region and to offer a methodological basis for similar follow-up studies related to conflicts derived from sharing nature environment.
