SN 기준선과 FH 기준선이 안모의 심미에 종종 맞지 않아서 모든 치열 교정 및 악교정 수술 증례를 이 기준선을 이용하여 반복적으로 정확히 진단하기가 매우 어렵다. 따라서, 이 연구의 목적은 한국 성인 환자를 대상으로 안정위에서 자연 두부 위치의 개인내 재현성을 확인하고 진성 수평선에 대해 SN 기준선과 FH 기준선의 개인간 다양성을 증명하는 것이다. 그리고, 상기 두 연구를 바탕으로 Na 기준 진수선에서 A점 및 B점까지의 거리가 SNA 및 SNB 각도와 상관 관계가 있는지 여부를 조사하는 것이다. 부정교합 및 악골의 부조화를 치료받기 위해 강남성심병원과 한강성심병원 교정과에 내원한 116명의 환자(남자 23명, 여자 93명)를 조사하였다. 중심위 왁스 바이트는 모든 대상들로부터 채득하였고 1주일 간격으로 자연 두부 위치와 중심위 상태에서 모든 대상자들의 측모두부방사선사진을 2번 촬영하였다. 3개의 변수에서 위치 선정의 방법 오류는 다른 연구에 비해 낮은 수치를 보였다. 그리고 6개의 변수에서 군내 상관계수를 이용하여 자연두부 위치의 개인간 재현성을 증명하였다 (p < 0.001). 각 환자에서 진성 수평선에 대한 FH 기준선간의 각도는 통계적으로 기준치 0도와 다르지 않았다 (p > 0.05). 하지만 임상적으로 다양한 수치를 보였다 (SD: 3.89˚). 반면 각 환자에서 진성 수평선에 대한 SN 기준선간의 각도는 기준치 7도와 매우 다르게 나타났다 (p < 0.05). Na 기준 진수선에서 A점 및 B점까지의 거리와 SNA 및 SNB 각도와의 피어슨 상관 계수에서 유의하지만 매우 낮은 수치를 보였다 (p < 0.01). 이에 자연 두부 위치는 치열 교정 환자 및 악교정 수술 환자의 진단을 위해 재현적이고 믿을 만한 위치이며 자연두부 위치 및 중심위 상태에서 Na 기준 진수선은 SN과 FH 기준선을 대신하여 한국인 성인 환자에서 상악골과 하악골의 전후 관계를 결정하는데 매우 유용한 기준선으로 이용될 수 있을 것으로 생각한다. Objective: This study aimed to verify the intra-individual reproducibility of the natural head position (NHP) in adult Korean patients in the centric relation (CR) position and to prove the inter-individual variability of the Frankfurt horizontal (FH) plane and sella-nasion (SN) line compared to the true horizontal line (THL). In addition, the study aimed to investigate the correlations between linear measurements from A-point and B-point to the nasion true vertical line (NTVL) and angular measurements from A-point and B-point to the SN line. Methods: Two lateral cephalograms were taken of 116 subjects (23 males, 93 females) with CR wax bites in a NHP at a one-week interval. Results: Method errors of three variables and intraclass correlation coefficients of six parameters proved the intra-individual reproducibility of NHP (p < 0.001). The angle of the FH to the THL was not significantly different from 0˚ (p > 0.05), but it was clinically variable (SD 3.89o) on the inter-individual level. Conversely, the angle of the SN line to the THL was significantly different from 7˚ (p < 0.05). Very low correlation was found between the linear measurements and angular measurements of A-point and B-point (p < 0.01). Conclusions: The NTVL could be a useful reference line for assessing the antero-posterior position of the maxilla and mandibleof Korean adult patients in NHP and CR. Botryoid Odontogenic Cyst on Mandibular Anterior and Both Body Area: a Case Report

Botryoid odontogenic cyst (BOC) is considered a rare multilocular variant of the lateral periodontal cyst. In this report, a 67-year-old male visited with chief complaint of severe mobility on mandibular incisors. Multilocular radiolucent lesion was seen from the right premolar to the left premolar area, involving almost the whole mandible. Histologically, the botryoid odontogenic cyst showed focal nodular thickening of the lining epithelium. These thickening often showed swirling appearance of the cells. Cyst enucleation and bone graft on mandible anterior and both body area were performed under general anesthesia, and postoperative healing was favorable without recurrence.

Basal cell nevus syndrome is a rare inherited disorder characterized by mulitple cutaneous basal cell carcinoma, pits of the palms and soles, cysts of the jaws, skeletal abnormalities and ectopic calcifications. Currently there are new lines of investigation based on biomolecular studies, which aim at identifying the molecules responsible for these cysts and thus early allowing an early diagnosis of these patients. We report a case of a 9-year-old boy with the various manifestation of basal cell nevus syndrome, which are multiple odontogenic keratocysts, pits of the soles, bifid ribs, ectopic calcification, macrocephaly, and hypertelorism, etc. Total five odontogenic keratocysts were found. For the reduction of the size of the odon-togenic keratocysts, following preoperative marsupialization, there were surgically enucleated. And the impacted upper right lateral incisor and canine are tracted orthodontically.

Purpose: The purpose of this experiment was to evaluate the clinical effect of cultured autoglogous osteoblasts as a way to treat the defect of mandible in rabbits. Materials and Methods: Twelve rabbits were used to determine the rate of osteogenesis. The osteoblasts were obtained from the iliac crest of rabbits using aspiration. They were then cultured in Dulbecco's Modified Eagles's Medium (DMEM) with beta-glycerophosophatate, L-ascorbicacid, and dexamethasone to proliferate and differentiate osteoprogenitor cells. The expression of osteogenic markers were detected by reverse transcription-polymerase chain reaction (RT-PCR) and silver nitrate staining techniques. Five, 10-mm holes were placed in each rabbit mandible to simulate defective regions with the use of a low speed trephine bur. In the experimental group, the previously cited defects were grafted with both activated osteoblastic and autogenous bone. The control group, however, was only grafted with autogenous bone. Both groups were then analyzed at 2, 4, and 8-week intervals using bone histomorphometric analysis. Results: According to histomorphologic analysis, the rates of new bone formation at the 2, 4, and 8-week intervals were 36%, 51%, and 23% for the control group, respectively; 52%, 39%, and 28%, for the experimental group, respectively. The experimental group showed higher rates of new bone formation compared to the control group at both the 2-week and 8-week interval. Conclusion: Bone marrow-derived osteoblasts seems to be a promising bone graft material.
하안면과 경부의 심미성 증진을 위한 지방흡입술을 동반한 Submentoplasty 술식

Purpose: The purpose of this study is to evaluate the clinical availability of submentoplasty for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle. Materials and methods: Case 1. We evaluate the changes of submental line length and chin-neck angle of 35-year-old woman with skeletal Class Ⅲ and mandibular excess with excessive submental fat before and after surgery: Bilateral sagittal split ramus osteotomy(BSSRO) setback(5㎜), Mandibular Angle Reduction, Reduction Malarplasty and Submentoplasty. In this case, It was done simultaneously with orthognathic surgery. Case 2. The changes of submental line length and chin-neck angle of 20-year-old man with skeletal class Ⅲ and maxillary defiency were evaluated before surgery, at first surgery: Lefort Ⅰ osteotomy(6㎜ posterior Impaction), BSSRO setback(9㎜), Paranasal Augmentation and at second surgery: genioplasty(6㎜ advanced) with submentoplasty, In this case, submentoplasty and advancement genioplasty were done after 2 months post-operative periods. Results : Case 1. In case of the Skeletal Class Ⅲ mandibular excess with submental fat deposit, It showed the improvement of submental angle and length of submental line after simultaneous submentoplasty. Submental angle is changed from 177° (pre-op) to 151° (post-op) and submental line length is changed from 8㎜(pre-op) to 36㎜(post-op). Case 2. The improvement of submental angle and length of submental line after delayed submentoplasty was aquired in case of the skeletal class Ⅲ maxillary defiency, Submental angle is changed from 154° (pre-op) to 161° (first surgery) and to 153° (second surgery) and submental line length is changed from 25㎜(pre-op) to 19㎜(first surgery) and to 23㎜(second surgery). Conclusion : The results suggest that Submentoplasty is useful surgical procedure for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle.

Purpose: The short dental implant is considered as possible solution in the alveolar bone height deficient cases. The aim of this study was to evaluate clinical availability of short implants by measuring the marginal bone loss of short length implants and comparing with that of conventional length implants. Materials and Methods: The groups were composed of patients who had received at least one implant. The samples of this study were selected from patients who with functional loading after prosthetic treatment for 1 year follow up period. The implants with a length of 5.7㎜ and 6㎜ were considered short. (Bicon Dental implants, USA). The experiment group was composed of 4.5×6㎜, 5×6㎜, 6×5.7㎜ implants (total 18 implants were placed in 14 patients, 8 on maxilla, 10 on mandible). The control group was composed of 4.5×8㎜, 5×8㎜, 4.5×11㎜, 5×11㎜. All implants were selected only by implants placed on molar area. We evaluated marginal bone loss in radiographic images at baseline (implant loading) and 3, 6, 12 months after loading. Additionally, crown-to-implant ratio was evaluated, and marginal bone loss according to crown-to-implant ratio after functional loading was analyzed. Results: The short implant group had a mean marginal bone level of -0.52±0.69 ㎜; the 8㎜ group, -0.22±0.82㎜; and the 11㎜ group, -0.10 ± l.09㎜ after 1 year of functional loading. But significant differences were not detected between three groups at every follow-up period. Crown-to-implant ratio in short implant group was 1.55±0.23; 8㎜ group was 1.15±0.18; and 11㎜ group was 0.92±0.15. Additionally, significant differences between three groups were founded. (P<.0001) The greatest marginal bone loss after 1 year follow-up was founded at crown-to-implant ratio 1-1.49 range in short implant. Conclusion: The marginal bone loss of short implants was comparable to that of long implants. So, the short implants can be a clinically acceptable option.

This article presents a modified palatal pedicled incision design in anterior segmental maxillary osteotomy. This method includes the circumcrevicular incisions and two vertical relaxing incisions. Then, it is different from the Couper’s method with vestibular horizontal incision and other anterior segmental osteotomy techniques. The main advantages of this method are an improved visibility of surgical field and favorable preservation of palatal pedicle. A patient who had been operated by anterior segmental maxillary and mandibular osteotomy without pre operative orthodontic treatment was analyzed for advantages and complications during the intraoperative and early post operative period. There are no specific major complications such as infection, gingival recession and dehiscence. And the patient had a pleasing esthetic facial appearance.

The goal of this treatment is the surgical-orthodontic eruption of impacted permanent teeth associated with a large dentigerous cyst in a preadolescent patient. Although enucleation of the entire cyst and the extraction of impacted teeth are common treatments, missing permanent teeth cause several problems in young patients. In this report, an 11-year-old female visited with the chief complaint of a large radiolucent lesion from the mandibular anterior area to the left mandibular posterior area. The permanent left canine and premolars were displaced toward the mandibular inferior border area. The extraction of infected deciduous teeth and marsupialization were performed. After 4 months, orthodontic buttons for forced eruption were applied to the impacted permanent teeth. The teeth emerged into the oral cavity 3 months after the orthodontic treatment. Although the root form was abnormal, there were no other pathogenic signs. The alveolar bone had a normal trabecular pattern and the teeth appeared to be well maintained at postoperative 24 months.
