Objective : Functional brain mapping study on acupuncture stimulation using the [14C]2-deoxyglucose([14C]2-DG) autoradiography provides quantitative data and visualized pathway in central nervous system(CNS). We aimed to investigate the neural pathway and spatial distribution of metabolic activity elicited in CNS on electroacupuncture stimulation using [14C]2-DG autoradiography. Methods : The study were divided into three groups by stimulation times. 45-mins stimulation group according to Sokoloffs method, 5-mins stimulation group according to Duncun's method, and 15-mins stimulation group. ;A venous catheter was equipped into right jugular vein. The rats (Sprague-Dawley rats, 230-260g) were kept fastened loosely on a holding platform without anesthesia. Electroacupuncture stimulation (5 ms, 2 Hz, 1~3 mA) were applied on the left Zusanli (ST36) acupoint and [14C]2-DG ($25{\mu}Ci/rat$) injection was performed through the catheter. After sacrifice, the brain and the spinal cord were made to sections for film image. The film images were digitalized as the isotope concentration based upon comparison of optical densities with that of the standards and normalized by the optical density of corpus callosum. Results : 1. 15-mins stimulation group was most effective among 3 experiments. 2. On 15-mins stimulation group, medial geniculate nucleus, intetpeduncular nucleus intermedius, ventral periolivary nucleus, caudal periolivary nucleus, medial superior olive, lateral paragigantocellular nucleus, including hypothalamic arcuate nucleus were increased by more than 25% (at least, p<0.05) by electroacupuncture stimulation. 3. Especially, the metabolism in hypothalamic arcuate nucleus was increased by 90% (p<0.05). 4. The fact that arcuate nucleus of hypothalamus might play a role of interconnection area between ascending and descending pathway of acupuncture stimulation was demonstrated visually. Conclusions : Advanced study on electroacupuncture stimulation elicited significant increase of metabolic activity in various nuclei of hypothalamus will provide the important experimental basis in research of the relationship between electroacupuncture stimulation and internal visceral functions.
노령 흰쥐의 뇌허혈 손상시 양격산화탕(凉膈散火湯)이 뇌해마의 c-Fos 및 c-Jun 발현에 미치는 영향

This study investigated the effect of Yanggyuksanhwa-tang on cerebral ischemia of the rats. Considering age-related impact on cerebral ischemia, aged rats (18 months old) were used for this study. Ischemic damage was induced by the transient occlusion of bilateral common carotid arteries(BCAO) under the hypotension. Yanggyuksanhwa-tang was administered twice orally. Then changes of immunohistochemical expression of c-fos and c-jun in ischemic damaged hippocampus were observed. The BCAO in aged rats led significant increase of c-fos expression in CA1 and DG of hippocampus. While the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of c-fos expression in CA1 hippocampus following BCAO ischemia. Depending on changes of the normalized optical density(NOD) of immunohistochemical c-fos expression, the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of NOD in CA1 and DG of hippocampus. And there was not changes in CA2 and CA3 hippocampus with respect to the control BCAO group. The BCAO in aged rats led significant increase of c-jun expression in CA1 hippocampus. While the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of c-jun expression in CA1 hippocampus following BCAO ischemia. Depending on changes of the NOD of immunohistochemical c-jun expression, the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of NOD in CA1 hippocampus. And there was not changes in CA2, CA3 and DG of hippocampus with respect to the control BCAO group.
노령 흰쥐의 뇌허혈 손상시 뇌대사활성의 변화 및 량격산화탕의 영향에 대한 [$^{14}C$2-Deoxyglucose Autoradiography 연구

Objective : This study investigated the alteration of neural activity and effect of Yanggyuksanhwa-tang (Lianggesanhuo-tang) on cerebral ischemia of rats. Methods : Considering age-related impact on cerebral ischemia, aged rats (18 months old) were used for this study. Ischemic damage was induced by the transient occlusion of bilateral common carotid arteries (BCAO) with hypotension. Yanggyuksanhwa-tang (Lianggesanhuo-tang) was administered twice a day orally. Then alterations of neural activities in the brain of aged BCAO rats were measured by the [$^{14}C$]2-deoxyglucose autoradiography method. Results : The BCAO in aged rats led to significant decrease of neural activity in the whole brain. Treatment with Yanggyuksanhwa-tang (Lianggesanhuo-tang) significantly attenuated the decrease of neural activity in the whole brain following BCAO ischemia. Treatment significantly attenuated the decrease of neural activity in the CA1, CA2, CA3, dentate gyrus of the hippocampus, activated barrel, barrel cortex, somatosensory cortex, cingulate cortex, caudate putamen, and medial septal nucleus following BCAO in aged rats. Treatment with Yanggyuksanhwa-tang (Lianggesanhuo-tang) also significantly attenuated the decrease of neural activity in the anteroventral thalamic nucleus, ventral anterior thalamic nucleus, arcuate nucleus, posterior hypothalamic area, medial mammillary nucleus, lateral periaqueductal gray, dorsal raphe nucleus, interpeduncular nucleus, median raphe nucleus, and medial pontine nucleus. Conclusion : It can be suggested that Yanggyuksanhwa-tang (Lianggesanhuo-tang) has a neuroprotecuve effect on cerebral ischemia through the control of glucose metabolic rate and cerebral blood flow.
양격산화탕(凉膈散火湯)이 뇌해마 조직배양의 허혈손상에 따른 신경세포손상에 미치는 영향

Objectives : We can find out the experimental reports of Yanggyuksanhwa-tang, which has the function of regulating blood pressure related with cerebral disease, and increasing local cerebral blood stream volume, also has the recoveries for the damage of vessel endothelium, and endothelium hypertrophy caused by angiospasm after subarachnoid hemorrhage, and reduces the contraction of smooth muscle, so simultaneously improves necrosis. The aim of this study is to investigate effect of Yanggyuksanhwa-tang protecting neuronal cells from being damaged by brain ischemia through using organotypic hippocampal slice cultures. Methods : We caused ischemic damage to organotypic hippocampal slice cultures by oxygen and glucose deprivation, and Yanggyuksanhwa-tang extract was added to cultures. Thereafter we measured area percentage of propidium iodide (PI)-stained neuronal cell, lactate dehydrogenase (LDH) levels in culture media and terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL)-positive cells. Results : Area percentage of PI-stained neuronal cells and count of TUNEL-positive cells in CA1 and DG area of organotypic hippocampal slice culture were significantly decreased in pertinent density level of Yanggyuksanhwa-tang extract. LDH levels in culture media of organotypic hippocampal slice culture were significantly decreased in pertinent density level of Yanggyuksanhwa-tang extract. Conclusions : Within pertinent density level, Yanggyuksanhwa-tang has cell protection effect that prevents brain ischemia damaging neuronal cells and apoptosis increasing.

Objectives: A safety issue on acupuncture and moxibustion treatment during pregnancy is as important as effectiveness. To establish a rationale and research strategy for future studies, a traditional literature review was performed to summarize how and for what conditions acupuncture and moxibustion treatment was given during pregnancy. Methods: An extensive traditional literature search for acupuncture and moxibustion treatment during pregnancy was conducted in texts on acupuncture and moxibustion, obstetrics and gynecology, and comprehensive medical texts. Treatment conditions, methods, and contraindications were summarized and tabulated. Results: Twenty-eight books were included in our review. Most frequent description of acupuncture and moxibustion treatment use during pregnancy was for difficult delivery including breech presentations; commonly used acupuncture points for difficult labor included LI4, SP6, BL67, BL60, KI6, ST30, SP12, LR4, LR3, PC6, CV3, CV14, KI13, and GB21, indicating that they may have to be avoided during pregnancy. Descriptions of other symptoms or conditions were sparse. For habitual abortion or recurrent miscarriage, moxibustion on GV4, BL23, CV3, KI8, and KI2 was indicated. A combination of LI4 and SP6, and CV4 were contraindicated during pregnancy consistently across the reviewed books. Conclusions: Our traditional literature review has shown that the use of acupuncture and moxibustion treatment during pregnancy has been limited. Given that more and more pregnant women are interested in safe and effective treatment, further research of acupuncture`s safety and efficacy during pregnancy is urgently needed.
임신 시 침 치료에 관한 인식 조사 -한의사, 환자 대상으로

Objectives: To investigate how Korean Medicine Doctors (KMDs) and pregnant women understand acupuncture treatment during pregnancy with a focus on safety. Methods: A survey was conducted in 163 KMDs and 157 women who were hospitalised in postnatal care clinics after childbirth. KMDs were asked regarding experience in acupuncture treatment for pregnant women and condition/disease for acupuncture treatment, intention to give acupuncture treatment to pregnant women and reason for yes/no, and consideration for acupuncture treatment for pregnant women. Patients were asked regarding their experience in acupuncture treatment before and during pregnancy and willingness to receive acupuncture treatment if necessary during future pregnancy. Results: 78% of KMDs have given acupuncture treatment to pregnant women if necessary, and it was mainly for pain. 63% of KMDs were willing to give acupuncture treatment for pregnant women with consideration of patient`s condition, specific acupuncture points, and stimulation methods. Pregnant women, however, were reluctant to receive acupuncture treatment during pregnancy (58%) in fear of miscarriage or needling itself. KMDs` clinical experience, patients` income Level and Level of education were not associated with greater use of acupuncture treatment during pregnancy (P>0.05). Conclusion: While many KMDs are willing to give acupuncture treatment to pregnant women if necessary, Little evidence on its safety and effectiveness prevents patients from seeking acupuncture treatment during pregnancy. Further research on this issue is urgently needed.
총명탕, 원지, 석창포가 베타아밀로이드로 유발된 학습과 기억장애에 미치는 영향

Objectives : This study investigated the protective effects of the water extracts of Chongmyung-tang, Polygalae Radix, and Acori Graminei Rhizoma in an in vivo Alzheimer's disease (AD) mouse model. Methods : Memory impairment was induced by an intraventricular injection of $A{\beta}25-35$ peptides and subsequently Chongmyung-tang, Polygalae Radix, or Acori Graminei Rhizoma extract were administered orally for 14days. Results : In the water maze task, Chongmyung-tang, Polygalae Radix, and Acori Graminei Rhizoma extracts improved learning ability during the acquisition period and significantly increased memory scores during the retention period versus $A{\beta}-injected$ controls. Furthermore, the toxicity of $A{\beta}25-35$ on hippocampus was assessed immunohistochemically (Tau, MAP2, TUNEL, Bax) and by in vitro study. Chongmyung-tang, Polygalae Radix, and Acori Graminei Rhizoma demonstrated significant neuroprotective effects against oxidative damage and apoptotic cell death of hippocampal neurons damaged by $A{\beta}25-35$. Conclusions : These results suggested that Chongmyung-tang, Polygalae Radix and Acori Graminei Rhizoma extract improve memory impairment and reduce Alzheimer's dementia via anti-apoptotic effects and by modulating the expressions of Tau and MAP2 protein in the hippocampus.
