Extramedullary plasmacytoma is a rare disease in which a neoplastic proliferation of plasma cells originates from the outside of the bone marrow. The jejunum is the most common site of extramedullary plasmacytoma of the gastrointestinal tract and its common complications are intestinal obstruction due to mass, gastrointestinal bleeding, and intussusception. A 68-year-old woman presented with intermittent postprandial abdominal pain, vomiting, and weight loss. Abdomen and pelvis CT revealed diffuse wall thickening of the proximal jejunum. Small bowel series and enteroclysis revealed a short segment stricture of the proximal jejunum. A 2 cm long jejunal stricture was detected on peritoneoscopic surgery. A pathology showed a solid mass beneath the submucosa grossly and malignant plasma cell infiltration microscopically. A immunochemical staining showed monoclonal kappa chain expression. A bone marrow examination showed only mild reactive plasmacytosis. Serum protein electrophoresis, skull and spine X-ray showed no specific abnormality, and urinary Bence Jones protein was negative. Afferent loop syndrome (ALS) accompanying acute pancreatitis is characterized by abdominal pain, vomiting and elevation of serum amylase. Thus, it is difficult to differentiate ALS from other causes of acute pancreatitis. However, the history of gastrectomy can be an important clue for diagnosing ALS. We experienced two cases of ALS accompanying acute pancreatitis. The laboratory findings of the patients showed elevation of serum amylase and they were initially diagnosed as only acute pancreatitis. However, the diagnosis were modified as ALS with the aid of computed tomography. After appropriate management, the abdominal pain disappeared and serum amylase level decreased. In conclusion, ALS should be considered as a cause of acute pancreatitis in patients who have the elevated level of serum amylase and a history of gastrectomy. Endometriosis is defined as the presence of endometrial tissue outside the uterus. We experienced 51-year-old woman with symptom of epigastric pain and sign of a palpable mass on the left quadrant abdomen. A cystic mass in the tail area of the pancreas was found on CT scan and was removed surgically. It was a cystic mass originated from the mesentery and separated from the pancreas. It was diagnosed as endometrial cyst histopathologically. (Korean J Gastroenterol 2001;37:389-391)

N/A PFD is effectively treated by biofeedback therapy. For the definite diagnosis of PFD, defecography, colon transit time study, balloon expulsion testing, and anorectal manometry are needed. However, these methods are of high cost and cause discomfort to patients. Moreover, definite diagnosis cannot be made by a single test due to a high false positive rate. In general, several symptoms linked with problems in defecating, including excessive straining, the sensation of incomplete evacuation, and applying pressure around the anus or the vagina to facilitate defecation, are known to be frequently associated with PFD. The aim of this study was to evaluate whether specific clinical parameters could differentiate patients with PFD from other constipated patients. Methods: An organized questionnaire including 47 questions that contained subjective symptoms, past medical history, and eating habits was designed. The questionnaire was distributed to 132 patients who fulfilled Rome II criteria for functional constipation. Digital rectal examination was conducted by a single gastroenterologist. Results: Among 132 patients, 45 patients were categorized as PFD, 26 patients as slow transit constipation (STC) and 17 patients as normal transit constipation. Among specific questions about constipation, hard stool was more frequently noted in patients with STC than PFD (p<0.05), and the frequency of defecation was lower in patients with STC than PFD (p<0.05). However, the symptoms suggesting difficult defecation were not different between the two groups. The percentage of paradoxical contraction by digital rectal examination was not different between the two groups (PFD: 57.1% vs. STC: 48.0%). Conclusion: The symptoms and signs suggesting difficult defecation could not differentiate PFD from STC and normal transit constipation, although several parameters were different among the three subgroups. Therefore, anorectal physiologic tests are needed for the diagnosis of PFD.(Kor ean J our nal of Gastrointestinal Motility 2002;8:167-176)
한국산야생등줄쥐(Apodemus agrarius coreae)의 해마복합체에서 neuropeptide Y 면역반응세포에 관한 연구

Neuropeptide Y(NPY) was first isolated from porcine brain. This discovery has lead some workers to study the distribution of this peptide in the central nervous system of various mammals. In this study the distribution pattern of neuropeptide Y-immunoreactive neurons was examined in the hippocampal complex of Striped field mouse in Korea(Apodemus agrarius coreae) Experimental animals were anesthesized with thiopental sodium and perfused with 4% paraformaldehyde through the left ventricle and the ascending aorta. The brains were removed and transferred 10%, 20% and 30% sucrose solution. Sections were cut on a cryostat into 40㎛-thick. The tissue was immunostained with free-floating and avidin-biotm complex(ABC) method. The results obtained in this study were summarized as followings. 1. The most of NPY-IR neurons in hippocampus proper were found in the polymorphic layer and a few was found in the pyramidal layer. 2. The process run vertically in the deep polymorphic layer and run parallel to corpus callosum in superficial polymorphic layer. 3. The shapes of neurons in hippocampus proper were varied of polymorphic, oval or round, and the nerve fibers were found in the molecular laver and the hippocampal fissure. 4. The highest density in number of NPY-IR neurons on dentate gyrus were found in hilar area of dentate gyrus. 5. The most of NPY-IR nerve fibers were found in molecular cell layer.
