        담즙울체 쥐에서 간 라이소솜의 Cathepsin B 및 D와 Acid Phosphatase 활성도에 미치는 Taurocholate의 정맥 내 투여의 영향

        이 연구는 담즙울체간에서 라이소솜의 cathepsin B 및 D와 acid phosphatase의 활성도가 증가되는 기전의 일부를 알아내기 위하여 쥐에게 총담관 결찰로 담관을 폐쇄시킨 직후 TCA또는 TUDCA를 정맥 내에 주입시킨 다음 간 라이소솜의 cathepsin B 및 D와 acid phosphatase 활성도를 측정하여 이 효소들의 활성도 변동에 대한 이들 담즙산의 효과를 알아 보았다. 쥐에게 총담관 결찰을 시킨 직후 TCA를 주입시켜 1일 및 2일 경과시켰을 때 간 라이소솜의 cathepsin B 및 D와 acid phosphatase 활성도는 대조군인 총담관 결찰만 시킨 군보다 통계학적으로 유의한 증가를 나타내었다. 총담관 결찰을 시킨 직후 TUDCA를 주입시켜 1일 및 2일 경과시켰을 때 간에서 이들 효소의 활성도는 모두 대조군과 별 차이가 없었다. 이상의 결과로 보아 담즙울체간에서 라이소솜의 cathepsin B 및 D와 acid phosphotase의 활성도 증가는 담즙산 중 TCA에 의해 이들 효소의 합성이 유도되어 나타난 결과로 생각된다. The effects of intravenous administration of high concentration of taurocholate on cathepsin B and D, and acid phosphatase activities in rat liver lysosome were studied. These liver lysosomal enzymes were determined in experimental rats with common bile duct ligation (CBDL). The activities of liver lysosomal cathepsin B and D, and acid phosphatase were found to be significantly increased in the CBDL plus taurocholic acid (TCA) injection group than in control group, such as CBDL alone group. However, these hepatic enzyme activities did not change in the CBDL plus tauroursodeoxycholic acid injection group. The above results suggest that TCA induces biosynthesis of the lysosomal cathepsin B, D and acid phosphatase in the liver.

        A case of di Guglielmo syndrome in 9 year old boy was presented. This syndrome is characterized by excessive erythroid precursors with abnormal morphology both in the bone marrow and blood, and rare in childhood. He visited with the chief complaints of general weakness, weight loss, epistaxis and abdominal distension. Physical examination showed pallor, grade Ⅰ-Ⅱ/Ⅵ systolic murmur on apical area and marked abdominal distension with hepatosplenomegaly. we confirmed the patient by the peripheral smear and bone marrow aspiration.

        흰쥐 담즙울체간의 Superoxide Dismutase의 활성

        The activity change of superoxide dismutase and its mechanism in rat cholestatic liver were evaluated. The activity of superoxide dismutase was measured in the liver which obtained from common bile duct ligated rats (CBDL) and sham operated rats, respectively. And mRNA expression of this enzyme was also measured by reverse transcription-polymerase chain reaction in each group. The activity of superoxide dismutase was higher in the CBDL group, but mRNA expression of superoxide dismutase was similar to the sham operated group. These results indicate that increased activity of superoxide dismutase in the cholestatic liver was due to the increased catalytic activity for this enzyme. The increase of superoxide dismutase activity may result in hepatic damage in the cholestasis by activating hydrogen peroxide release.

        이 연구는 담즙울체간에서 라이소솜의 α-D-glucosidase와 β-D-glucuronidase의 활성도 증가 기전의 일부를 알아내기 위하여 쥐에게 총담관 결찰로 담관을 폐쇄시킨 직후 TCA 또는 TUDCA를 정맥 내에 주입시킨 다음 경시적으로 간 라이소솜의 α-D-glucosidase 및 β-D-glucuronidase와 혈청의 라이소솜 α-D-glucosidase, 혈청의 총 β-D-glucuronidase의 활성도를 측정하여 이 효소들의 활성도 변동에 대한 이들 담즙산의 효과를 알아 보았다. 쥐에게 총담관 결찰을 시킨 직후 TCA를 주입시켜 1일 및 2일 경과시켰을 때 간 라이소솜의 α-D-glucosidase 및 β-D-glucuronidase 활성도와 혈청의 라이소솜 α-D-glucosidase 및 혈청의 총 β-D-glucuronidase 활성도는 대조군인 총담관 결찰만 시킨 군보다 통계학적으로 유의한 증가를 나타내었다. 이상의 결과로 보아 담즙울체간에서 활성도가 증가되는 라이소솜의 α-D-glucosidase 및 β-D-glucuronidase 활성도 증가는 담즙산 중 TCA에 의해 이들 효소의 합성이 증가되어 나타난 결과로 생각되며 아울러 담즙울체시 라이소솜 α-D-glucosidase 및 총 β-D-glucuronidase의 혈중 활성도 증가는 TCA에 의한 간의 괴사로 간 세포막의 투과성이 항진되어 나타난 결과로 생각된다. The above results suggest that TCA induces biosynthesis of the lysosomal α-D-glucosidase and β-D-glucuronidase in the liver and that the elevated lysosomal α-D -glucosidase and total β-D-glucuronidase activities in the serum are most likely due to increased hepatocyte membrane permeability caused by TCA mediated liver cell necrosis.

        담즙산 부하에 의한 흰쥐 간의 Cholinesterase의 합성 억제

        The possible mechanism of decrease of cholinesterase activity in cholestatic liver and serum was studied. Rats were divided into eight groups: Normal, sham operated control, bile duct obstruction (BDO) alone (BDO group), BDO plus taurocholic acid (TCA) injection (BDO plus TCA group), BDO plus tauroursodeoxycholic acid (TUDCA) injection (BDO plus TUDCA group). choledocho-caval shunt (CCS) operation (CCS group), CCS plus TCA injection (CCS plus TCA group), and CCS plus TUDCA injection (CCS plus TUDCA group). Cholinesterase activity was determined in the serum and liver cytosolic, mitochondrial and microsomal preparations isolated from the above experimental rats. The values of Km and Vmax in this hepatic enzyme were measured. The activity of liver microsomal cholinesterase showed a significant decrease in both the CCS and the BDO groups. And the activity of serum cholinesterase showed a marked decrease in both the CCS and the BDO groups. However, the cholinesterase activity in the serum and liver microsomal preparation fell more rapidly in the BDO group than the CCS group. Cholinesterase activities in liver cytosolic and microsomal preparations, and its Vmax values decreased significantly in both the CCS plus TCA group, and the BDO plus TCA group than that of each control group, such as CCS and BDO groups. On the other hand, the values of Km of the liver cytosolic and microsomal cholinesterase did not change in all the experimental groups. Serum cholinesterase activity decreased significantly in both the CCS plus TCA group and the BDO plus TCA group than that of each control group. However, these serum and hepatic enzymes activities did not change in both the CCS plus TUDCA group and the BDO plus TUDCA group. The above results suggest that TCA repress biosynthesis of cholinesterase in the liver. And the lowering of the serum enzyme activity is thought to be caused by a decrease of biosynthesis of this hepatic enzyme, which causes the enzyme to leak into the blood in small quantity.

        Thromboelastographic Evaluation in Patients with Severe Sepsis or Septic Shock: A Preliminary Analysis

        Purpose: Thromboelastography (TEG) was investigated for the diagnosis of coagulopathy compared with traditional coagulation tests, in association with disease severity in patients with severe sepsis or septic shock. Methods: Retrospective data was collected from a single center between January 25th to March 24th, 2016. There were 18 patients with severe sepsis or septic shock admitted to intensive care units included in this study. Laboratory tests including TEG were performed at admission. Disease severity was measured using the Simplified Acute Physiology Score III, Sequential Organ Failure Assessment score, and the level of lactate. Results: There were 18 patients (61% males; median age, 60.5 years) who were diagnosed with severe sepsis, or septic shock requiring a norepinephrine infusion (n = 10, 55.6%). Of these, 4 patients had traditional coagulation tests, and TEG profiles which confirmed hypercoagulability. Eight patients had follow-up tests 48 hours post-admission with a Sequential Organ Failure Assessment score of 6.5 (3-9.5) at admission, decreasing to 4 (2-11) after 48 hours (although not significantly lower), however, the lactate level decreased statistically significantly from 2.965 at admission, to 1.405 mmol/L after 48 hours (p < 0.05). The TEG profiles tended to normalize after 48 hours compared with admission, but there was no statistically significant difference. Conclusion: Coagulopathy with severe sepsis or septic shock patients can be life-threatening, therefore it is important to diagnose coagulopathy early and precisely. TEG can be a feasible tool to confirm coagulopathy with traditional coagulation tests.

        Hand-Schuller-Christian disease is reactive granulomatous lesion with histiocytic proliferation, the cause of which is unknown. Recently, we experienced a case in a 3 years old female with polydipsia and polyuria. We established the di agnosis with clinical feaures, radiologic examination and bone biopsy Histiologic examination of frontal bone showed mild to moderate infiltration of eosinophils lymphocytes, plasma cells and large histicytes and fibrosis. Similiar cases reported in literature are briefly reviewed.
