목적: 견관절 역행성 인공관절 전치환술 후 합병증을 분석하고, 임상 결과를 보고하고자 한다. 대상 및 방법: 견관절 역행성 인공관절 전치환술 후 최소 6개월 이상(평균, 22개월) 추시한 98예의 환자(남 22명, 여 76명; 평균연령, 75세)를 대상으로 최종 추시 시 합병증 및 임상결과를 분석하였다. 회전근 개 관절병증 43예, 회전근 개 거대 파열 33예, 골절 및 후유증 16예, 감염 4예, 기타 2예였다. 결과: 총 98예 중 18.4%에서 합병증이 발생하였다. 견관절 탈구(2예)에 개방적 정복술을, 삽입물 주위 골절(2예)에 금속판 내고정술을, 감염, 상완골 및 관절와 기구 해리 각각 1예에 재치환술을 시행하였다. 견봉 골절(2예)과 이소성 골화증(8예)에서 보존적 치료를 시행하였다. 합병증의 최종 추시 임상 점수는 의미있는 향상을 보였다(p<0.05). 견갑골 절흔은 43.9%에서 발생하였다. 합병증과 절흔의 유무 비교 시, 추시 임상 점수상 유의한 통계적 차이는 없었다(p>0.05). 결론: 18%의 합병증이 발생하였고, 3예에서 재치환술을 시행하였으나, 최종 추시 시 임상 결과는 유의하게 호전되었다. 견갑골 절흔이 44%에서 발생하였으나 단기 추시상 임상 결과에서 의미있는 영향을 미치지는 않았다. Purpose: The purpose of this study was to analyze complications after reverse total shoulder arthroplasty and report the clinical outcomes with review of previously reported studies. Materials and Methods: Complications after reverse total shoulder arthroplasty were analyzed for 98 patients who underwent reverse total shoulder arthroplasty and were followed-up for at least 6 months. Of 98 patients, 22 were men and 76 were women. The mean age was 75.0±6.5 years (range, 59-92 years) with a mean follow-up period of 22±19 months (range, 6-74 months). The types and time of occurrence of complications, methods of treatment, and clinical outcomes at the final follow-up were analyzed. Results: Complications occurred in 18 of 98 patients (18.4%). Seven of them received operative treatment and 11 were treated conservatively. Two cases had postoperative dislocations and were addressed with open reduction. In 2 cases, periprosthetic fractures occurred and were treated with open reduction and plate fixation. Two acromial stress fractures and 8 cases of heterotopic ossification were managed conservatively. Infection and humeral component loosening occurred in one case, respectively, and were treated with revision arthroplasty. Glenoid component loosening occurred in 2 cases, one of which was treated with revision arthroplasty and the other was managed conservatively. At the final follow-up, clinical outcomes showed a statistically significant improvement. Compared to groups with no complications, there were no significant differences in final clinical outcomes. Scapular notching occurred in 43.9% (43/98 cases). No statistically significant differences of clinical outcomes were observed between the scapular notching group and the no notching group (p>0.05). Conclusion: In our study, 18% of complications occurred after reverse total shoulder arthroplasty and final clinical outcomes of these complications showed significant improvement. Scapular notching developed in 44% and did not provide a significant clinical effect at short term follow-up.

Trivalent ions (Ga, Al, In) doped ZnO films were deposited by DC magnetron sputtering on non-alkali glass substrate at substrate temperature of 300℃. We used the different three types of high density (95%~) ceramic sintered disks (doped with Ga₂O₃; 6.65 wt%, Al₂O₃; 3.0 wt%, In₂O₃; 9.54 wt%). This study examined the effect of different dopants (Ga, Al, In) on the electrical, structural, and optical properties of the films. The lowest resistivity of 5.14×10?⁴ Ω㎝ and the highest optical band gap of 3.74 eV were obtained by Ga doped ZnO (GZO) film. All the films had a preferred orientation along the (002) direction, indicating that the growth orientation has a c-axis perpendicular to the substrate surface. The average transmittance of the films was more than 85% in the visible range.
Photoluminescence Characterization of Al-Doped ZnO Films Deposited by Using DC Magnetron Sputtering

We investigated the structural and the optical properties of Al-doped ZnO (AZO) films deposited by DC magnetron sputtering without substrate heating as a function of the Al2O3 content. The resistivity of the AZO films decreased from 2.6×10-2 Ωcm to 1.9×10-3 Ωcm and the optical band gap energy increased from 3.34 eV to 3.38 eV when the Al2O3 content in the AZO targets was increased from 0 wt% to 3.0 wt%. The AZO films showed a highly c-axis preferred orientation which was enhanced with increasing Al2O3 concentration. In order to investigate the effects of the crystallinity and the defect number on the emitting properties of the AZO films, we estimated photoluminescence excitation (PLE) spectra were estimated by using photoluminescence (PL) measurements. The PLE spectra of the AZO films revealed a near-band-edge (NBE) emission at 3.43 eV (UV region) and a deep-level emission centered at around 2.64 eV (blue-green region). The NBE intensity tended to increase markedly with increasing Al2O3 addition in the AZO targets. The doping efficiency of Al3+ ions is important to determine the correlation between the electrical and the structural properties.

The paper presents the fault detection and isolation of two DC-DC converters parallel operation by ZCT method. Two experimental prototype converters were designed and implemented for evaluation of fault tolerant system. The experimental results show that fault detection and isolation circuit works very well.

Moreover, this improve power factor.<br/> In this paper, we will use the state average methode and small signal analysis at the Interleaved dual boost converter.<br/> 4-type controllers were designed by using control transfer function. The result of these controller simulations is analyzed and proposed a proper controller at IDB.
고령의 대퇴전자간 분쇄골절 치료 시 내고정술과 인공관절 반치환술간의 임상결과 비교

목적: 고령 환자의 불안정성 대퇴전자간 골절에서 무작위로 전향적으로 시행한 내고정술과 고관절 부분 치환술간의 연령별 임상 결과를 비교 분석하고자 하였다. 대상 및 방법: 2004년 1월부터 2007년 2월까지 70세 이상 고령에서 대퇴 전자간 불안정성 골절로 내고정이나 고관절 부분 치환술을 시행한 62예에 대해 최소 12개월(12개월~36개월) 추시 관찰하여 SF-36을 통해 임상적 평가를 시행하였다. 내고정을 시행한 군은30명, 부분 치환술을 시행한 군은 32명이다. 임상 결과 분석은 SF-36을 이용하였으며 SPSS를 통해 통계 분석하였다. 결과: 70대에서는 동통, 파행, 보조기 사용 여부 항목에서 부분 치환술 군이 유의하게 좋은 결과를 보였고, 80대에서는 파행 항목에서만 부분 치환술 군이 좋은 결과를 보였으나(P<0.05), 나머지 항목에서는 각 군간에 유의한 차이가 없었다. 내고정군에서 고정물 실패가 4예 있었고, 부분 치환술 군에서 감염에 의한 실패가 2예, 탈구로 인한 재치환술이 2예 있었다. 결론: 고령의 불안정성 대퇴 전자간 골절 시 내고정술보다 초기 안정성으로 동통이 적고 조기 기동이 가능한 고관절 부분 치환술이 더 유용할 것으로 사료되었다. Purpose: We wanted to analyze the clinical outcomes of prospectively randomized surgeries between internal fixation and hemiarthroplasty for treating unstable intertrochanteric hip fracture in elderly patients. Materials and Methods: From January, 2004 to December, 2007, 62 cases of unstable intertrochanteric fracture that underwent internal fixation or hemiarthroplasty were analyzed retrospectively for the clinical outcomes with using the SF-36. Thirty cases were treated with internal fixation and 32 cases were treated with hemiarthroplasty. The clinical outcomes, as assessed by using the SF-36, were statistically analyzed with using SPSS for Windows. Results: Hemiarthroplasty show a better result than internal fixation for pain, limping and the support scales in the 70~79 years old group, and the limping scale was also better in the over 80 years old group (P<0.05). There were no differences between the two groups according to age. Four cases of internal fixation failed due to loss of fixation (3) and nonunion (1), and 4 cases of hemiarthroplasty failed due to infection (2) and revision for dislocation (2). Conclusion: Primary hemiarthroplasty should be more beneficial than osteosynthesis, such as performing internal fixation, for treating unstable intertrochanteric fracture in elderly patients because of the reduced pain and early ambulation that are due to the early stabilization.
비구 골절의 내고정 수술 후 발견된 관절면 사이에 낀 골절편의 관절경적 제거

Osteochondral fragements incarcerated in the hip joint during fracture-dislocation have been reported to be the cause of posttraumatic arthritis, and open reduction of the fracture or removal of the fragments during surgery is recommended. Locking symptoms developed during hip joint movement in a patient with a history of hip joint fracture-dislocation (Thompson and Epstein (T-E) type III) after undergoing internal fixation of the acetabular fracture. Computer tomographic (CT) scan revealed a bony fragment impinging on the acetabular weight-bearing portion. This bony fragment was removed using hip arthroscopy. We recommend that CT evaluation be performed after surgery for acetabular fractures. We also recommend arthroscopic removal as a treatment option for posttraumatic loose bodies in the hip joint.
