Lumbosacral radiculoplexus neuropathy (LRPN) can involve various parts of nerve roots and plexus and present with various symptoms and signs. LRPN can be caused by metabolic, inflammatory, traumatic, oncologic and autoimmune disorders. Electrodiagnostic study (EDX) is important to localize the involvement of lumbosacral lesion. In addition to EDX, additional tests including serum, cerebrospinal fluid and magnetic resonance imaging (MRI) are also required to clarify the cause of lesion. In this case, a 63-year-old man with diabetes and history of traffic accident suffered from severe pain and motor weakness on the left lower extremity. Initially, he was diagnosed with lumbar polyradiculopathy based on EDX. However, final diagnosis was confirmed as diabetic LRPN by second EDX and MRI, which means progressive pattern of disease from polyradiculopathy to plexopathy.

Objective To compare the effects of neuromuscular electrical stimulation (NMES) to abdominal muscles and back muscles on postural balance in post-stroke hemiplegic patients.Methods Thirty post-stroke hemiplegic patients were prospectively enrolled and randomly assigned to one of the three groups: core muscle-strengthening exercise (CME) with NMES to abdominal muscles (group A), CME with NMES to back muscles (group B), and CME alone (group C). All subjects underwent their targeted interventions for 30 minutes each day, 5 days per week for 3 weeks under a conventional stroke rehabilitation program. Subjects were evaluated using Korean version of Berg Balance Scale (K-BBS), Trunk Impairment Scale (TIS), Korean version of Modified Barthel Index (K-MBI), Weight Distribution Index (WDI), and Stability Index (SI) just before and 3 weeks after intervention.Results Changes in K-BBS (p<0.05) and TIS (p<0.05) were significantly higher in group A (18.5±8.10, 6.6±1.90) and group B (19.9±5.44, 7.0±2.26) than in group C (8.4±4.14, 3.1±0.99). However, K-MBI, WDI, and SI failed to show any significant difference. No significant difference in all outcomes was observed between groups A and B.Conclusion The effect of NMES to the abdominal muscles was similar to the effect on back muscles in terms of postural balance. This finding indicated that the NMES to the abdominal muscles may be an alternative for post-stroke hemiplegic patients contraindicated for NMES to the back muscles. Additional studies investigating the effects of NMES on abdominal and back muscles are needed.

대장의 원발성 복막수염은 복통을 일으키는 드문 원인 중의 하나로서 특별한 치료 없이도 치유되는 질환이다. 저자들이 경험한 원발성 복막수염 4예 중 1예는 상행결장에, 3예 는 하행결장에 발생하였으며 그 중 1예는 수술을 실시하였다. 바리움 이중종영 대장검사상 대장의 외측벽에 점막의 병변을 동반하지 않는 종괴에 의해 국소적 압박이 있었다. 초음파 검사에서는 대장에 붙어 있는 압박이 되지 않는 고에코성 종괴로 관찰되었다. 복부 전산화 단층촬영상 (이하 CT) 난형의 지방성종괴가 대장의 장막하조직에 부착되어 있었으며, 주변 의 얇은 고음영의 테두리와 중심부의 결절형 고음영부위가 관찰되었다. 특별한 치료없이 13-25일 후 실시한 추적 복부 CT상 병변의 호전을 볼 수 있었다. Primary epiploic appendagitis of the colon is a rare cause of abdominal pain and is a self-limiting disease. Double contrast barium enema showed a smooth extrinsic compression located anterolateraly to the involved colon. Ultrasonography revealed a non-compressible echogenic ovoid mass attached to the colon wall. On abdominal CT, all lesions were seen as pedunculated ovel fatty masses with surrounding streaky densities connected to the serosal surface of the adjacent colon. Follow up CT performed 13-25(mean 18) days later showed that during this interval, the volume of the mass and extent of surrounding infiltrations had decreased.
