        류마티스 질환에서의 이차성 아밀로이드증

        Objective: To study the clinical features and outcomes in 10 cases of secondary amyloidosis with rheumatic disease. Methods: The secondary amyloidosis associated with rheumatic disease was studied clinically in 10 patients (4 men and 6 women), who have suspicious tissue dysfunction. The diagnosis of amyloidosis was established by histological examination of biopsy materials. We evaluated them by medical record review; clinical features at onset, organ involvement, cause of death. Results: The mean age and mean duration of rheumatic disease were 52.9±17.27 years and 10.9±10.33 years respectively. The common initial clinical features were generalized edema, diarrhea and abdominal pain. The mainly involved organs were kidney and colon. Causes of death were DIC due to sepsis and renal failure. Conclusion: Renal and gastrointestinal symptoms are cardinal features of secondary amyloidosis with rheumatic disease in Korea.

        Schizophyllum commune에 의한 Cellulase 생산 및 섬유소계 바이오매스의 당화를 위한 효소적 특성

        본 연구에서는 Schizophyllum commune외 당 분해효소 생산을 위한 최씩 배양 조건과 목질바이오매스에 대한 당화 특성에 대하여 연구하였다. S. commune 균체 외 효소에는 endo-β-1.4-g1ucanase (EG). cellobiohydrolase (CBH) β-glucosidase (BGL)와 같은 cellulase와 β-l,4-xylosidase (EXL)이 함유되어 있고 그 중에서 EG 및 BGL활성이 비교적 높은 활성을 나타낸 것으로 밝혀졌다 S. commune에서 생산된 EG. BOL. 및 CBH의 최적 온도는 50°C이었으나. 열안정성을 기지는 온도범위는 30~40°C였다. 그리고 최적 pH는 5.5이었으며 열안정성을 나타내는 온도범위에서의 적정 pH는 동일한 pH 5.5이었다. Cellulase 생산을 위한 S. commune9.1 최적 배양 조건은. 탄소원으로 천연 cellulose. 질소원으로는 corn steep. 또는 peptone/yeast extract 혼합을 비타민 은 첨가하지 않는 것이 cellulase 효소활성 증가에 적절한 것으로 밝혀졌던. 또한 탄소원의 최적 첨가 농도는 2% (w/w). 적정 배양 pH 및 온도는 5.5~6.0과 25~30°C로 밝혀졌다 본 연구에서 도출된 최적 배양 조건으로 S. commune를 배양시키고 40배로 농축한 절과. EG가 3670.5 U/㎖. BOL과 CBH가 각각 631.9 u/㎖. 398.5 u/me. BXL이 15.2 U/Me로 매우 높은 효소 활성을 나타냈다 동일한 효소의 Filter Paper Unit도 11 FPU/㎖로 상당히 높았다. 최적배양조건에서 얻어진 S. commune 효소로 다양한 기질에 대해 당화 시험을 실행한 결과, 전처리를 하지 않은 공시 활엽수에 대하여 낮은 당화율을 나타냈으나 천연 cellulose (Aldrich. ~20 micron) 및 볏짚의 경우에는 각각 50.5% 및 33.1%의 높은 당화성능을 나타냈다. 이 같은 당화 수준은 동일 효소농도 (30 FPU/g.glucan)로 비교했을 때 Trichoderma reesei 유래 상용화 효소인 Celluclaset 1.5L의 약 110% 수준을 나타냄으로써. 대량생산기술 개발과정을 통해 목질계 당화 효소로의 사용화 가능성이 높은 균주로 평가되었다. The optimum culture condition of Schizophyllum commune for the cellulase production and its enzymatic characteristics for saccharification of cellulosic biomass were analyzed 5 commune secrets .β-l,4-xylosidase (BXL) and cellulases, including endo-.β-l,4-glucanase (EG), cellobiohydrolase (CBH), and . β-glucosidase (BGL). The optimum reaction temperature for all cellulases was 50℃ and the thermostable range was 30~40℃. The optimum reaction pH for all cellulases was 5.5 in a range of temperature from 0℃ to 55℃. The best nutritions for the cellulase production of s commune among tested nutrients were 2% cellulose for the carbon source and corn steep liquor or peptone/yeast extract for the nitrogen source without vitamins. The environmental culture condition for the cellulase production was 5.5~6.0 for pH at 25~30℃. The enzyme activities of EG, BGL, CBH, and BXL were 3670.5, 631.9, 398.5, and 15.2 U/㎖, respectively, after concentration forty times from the culture broth of 5 commune which was grown at the optimized culture condition. Alternative filter paper unit assay showed 11 FPU/㎖ enzyme activity. The saccharification tests using cellulase of 5 commune showed the low saccharification rate on tested hardwoods but a high value of 50.5% on cellulose, respectively. The saccharification rate (50.5%) of cellulose by cellulase produced in this work is higher than 45.7% in the commercial enzyme (Celluclast 1.51, 30 FPU/g, glucan).

        Fomitopsis palustris의 균체 외 효소에 의한 볏짚 당화에 관한 연구

        갈색부후균인 Fomitopsis palustris에서 균체 외 cellulase 생산 특성과 이 효소를 이용하여 목재와 볏짚의 당화특성, mediator 첨가 효과, 목질기질의 미세 표면구조나 결정화도가 효소 당화에 미치는 영향 등에 대해 연구하였다. F. palustris의 균체 외 효소의 생산에 혼합목분을 탄소원으로 이용 시 endo-β-1,4-gulcanase (EG)는 12.0 U/㎖, β-glucosidase (BGL)는 116.68 U/㎖, cellobiohydrolase (CBH)는 18.82 U/㎖, 그리고 β-xylosidase (BXL)는 13.33 U/㎖의 활성을 보였다. 이러한 활성은 BGL, CBH, 그리고 BXL이 볏짚을 이용한 경우보다 약 2~4배 정도 높았다. 볏짚을 탄소원으로 이용하여 생산한 cellulase-RS의 효소 최적반응 온도 및 pH는 45°C와 pH 5.0이었으며, 혼합 목분을 탄소원으로 이용하여 생산한 cellulase-SW의 경우에는 50°C와 pH5.0이었다. Cellulase-SW는 볏짚을 기질로 사용할 때 40.6±0.6%로 가장 높은 당화율을 보였다. 또한 당화촉매제인 Tween 20의 첨가로 당화율이 44%로 상승하여 상용화 효소인 Celluclast 1.5L의 53.7%의 당화율 대비 약 82% 수준으로 상승되었다. 이는 본 실험에서 사용한 효소가 조효소 형태임을 고려하면 상용화 효소에 매우 근접한 당화율을 얻은 것으로 판단된다. 또한 볏짚의 낮은 조직 결정화도와 주사전자현미경을 이용한 볏짚 표면의 섬유화를 통한 표면적 증대 효과는 목재에 비해 높은 볏짚의 당화율에 대한 원인을 제시하였다. In the enzymatic hydrolysis of rice straw and wood meals using extra-cellular enzymes from Fomitopsis palustris, key factors which enhanced the sugar conversion yield were investigated in this work, such as enzyme production and enzyme reaction conditions, surfactant effects, and the surface structure of substrates. F. palustris cultured with softwood mixture produced 12.0 U/㎖ for endo-β-1, 4-gulcanase (EG), 116.68 U/㎖for β-glucosidase (BGL), 18.82 U/㎖ for cello-biohydrolase (CBH), and 13.33 U/㎖ for β-xylosidase (BXL). These levels of BGL, CBH, and BXL activities were two to four folds more than enzyme activities of F. palustris cultured with rice straw. The optimum reaction conditions of cellulase-RS which produced by F. palustris with rice straw and cellulase-SW which produced by F. palustris with softwood mixture were pH 5.0 at 45°C and pH 5.0 at 50°C, respectively. The sugar conversion yield of cellulase-SW had the highest value of 40.6±0.6% within 72 h when rice straw was used as substrate. By adding 0.1% Tween 20 (w/w-substrate), the sugar conversion yield of rice straw was increased to 44%, which was about four fifths sugar conversion yield of commercial enzyme, Celluclast 1.5L (Novozyme A/S). A low crystallinity and an intensive fibril surface observed by the scanning electron microscope may explain the high sugar conversion yield of rice straw.

        A 31-year-old woman had a history of fatigue and hypokalemia and metabolic alkalosis and hypocalciuria. The patient had a subtotal thyroidectomy and denied ingestion of diuretic medication. Her clinical and laboratory findings were consistent with Gitelman`s syndrome. Normal blood pressure, hypokalemic metabolic alkalosis, hypocalciuria were present. She confessed to us that she had been taking a pill due to constipation for 7 years. She was afraid that her husband know it. But we don`t know the reason why she had concealed it Surreptious ingestion of diuretics must be excluded in any adult patient in whom a diagnosis of Bartter`s or Gitelman`s syndrome is considered.

        Case Reports : A Case of Eosinophilic Polymyositis Treated with Immunosuppressants

        저자들은 사지의 부종, 근육통 및 근력저하를 주소로 내원한 환자를 호산구성 다발근염 진단 하에 고용량 글루코코르티코이드 치료를 시행하였으나, 이에 충분한 반응을 보이지 않아 면역억제제를 추가로 사용하여 성공적으로 치료한 호산구성 다발근염 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Eosinophilic polymyositis is a rare form of inflammatory muscle disease associated with peripheral blood and/or tissue eosinophilia. Most patients respond to glucocorticoids, however some show poor prognosis, leading to mortality. A 28-year-old female was admitted to our hospital because of myalgia and motor weakness of the upper and lower extremities. Laboratory findings showed significantly elevated levels of muscle enzymes and inflammatory lesions in both thigh muscles were demonstrated on magnetic resonance imaging. A diagnosis of eosinophilic polymyositis was based on histological findings, which showed diffuse eosinophilic infiltration into perivascular spaces in the endomysium and perimysium, and necrosis of myofibers. High-dose methylprednisolone treatment improved the clinical symptoms and muscle enzymes. However, on tapering the glucocorticoid dose, clinical and laboratory findings were exacerbated. After the addition of methotrexate and azathioprine, the symptoms and muscle enzymes recovered without relapse. Here, we report on a case of eosinophilic polymyositis, which responded to immunosuppressants. (J Rheum Dis 2015;22:308-312)

        수입된 호주산 야생 산양의 혈액상에 관한 연구

        The hematologic values were measured from 64 healthy goats imported from Australia to obtain data about the hematologic values of Australian wild goat. In paper 32 goats imported in March called group 1, 32 goats imported in April called group 2. The result obtained are summarized as follows;, 1 Mean$\pm$SD values and ranges of red blood cell(RBC) were $17.8$\pm$3.0{\times}10^6/${mu}ell$ and 9.9-24.3{\times}10^6/${mu}ell$,$ of hemoglobin(Hb) content 13.8$\pm$1.3 g/㎗ and 10.0-16.1 g/㎗, of packed cell volume(PCV) 37.0$\pm$4.1% and 28.0-47.0%, of mean corpuscular volume(MCV) 21.3$\pm$3.3 fl and 16.4-37.2 fl, of mean corpuscular hemoglobin(MCH) 7.9$\pm$1.1 pg and 6.0-13.3 pg, of mean corpuscular hemoglobin concentration(MCHC) 37. Of 1.9% and 34.0-41.9%, of white blood cell(WBC) $28.6$\pm$9.4{times}10^3/${mu}ell$ and 11.6-49.8{times}10^3/${mu}ell$,$ of neutrophil 38.0$\pm$8.2% and 23.0-61.0%, of lymphocyte 54.0$\pm$8.4% and 32.0-73.0%, monocyte 3.1$\pm$1.3% and 1.0-7.0%, of eosinophil 4.0$\pm$1.9% and 1.0-10.0%, of basophil 0.1$\pm$0.2% and 0-1.0% respectively. 2. There was no significant differences between Group l and Group 2, respectively. 3. There was no significant differences between sexes. 4. Comparing with the values of Korean domestic goat, e values of Australian wild goat were disserent from the values of Korean domestic goat. 5. These hematologic values might be accepted as the hematologic values of Australian wild goat.

        연구보문 : 적심정도가 감초의 생육 및 품질에 미치는 영향

        감초의 적심정도가 생육 및 품질에 미치는 영향을 구명하고 감초 재배의 기초 자료를 얻고자 적심정도를 경장의 20% 등 4처리를 두어 2008년부터 3년간 시험한 결과는 다음과 같다. 1. 적심 유무에 따른 감초 경엽 생육은 초장, 분지수, 절수 및 경엽중은 무적심에서 양호한 경향이었으나, 경태는 가늘은 경향이었고, 적심정도 간에는 경태를 제외하고 적심비율이 적을수록 양호한 경향이었다. 2. 포복경 생육은 무적심 처리에서 양호한 경향이었으며, 적심정도에 따라서는 적심비율이 적을수록 양호한 경향이었다. 3. 뿌리의 생육은 무적심에 비하여 적심처리에서, 적심정도 간에는 주근장과 지근장은 20% 적심에서 길었으나, 다른 근생육은 40% 적심에서 양호한 경향이었다. 4. 근수량은 40%>60%>20%>무적심 순으로 많았으며, 40% 적심시 상품 근수량이 무적심 300kg/10a에 비하여 25% 증수 되었다. 5. 글리시리진(Glycyrrhizinic acid)의 함량은 20%와 40% 적심에서 무적심 1.35% 대비 각각 0.31%, 0.09% 높았다. Growth and quality according to top-precision degree of Glycyrrhiza uralensis Fischer were tested by 4 treatments including 20% rate cutting of branch-top during 3 years from 2008. Growth above ground part of Glycyrrhiza uralensis Fischer with or without topping-plant height, branch number, node number, weight of stem and leaf-was good tendency without topping but stem diameter was less. growth above ground part among topping ratio was the less topping ratio the better except stem diameter. Growth of surface runner was good tendency without topping. growth of surface runner among topping degree was the less topping ratio the better. Root growth was higher without topping compare to topping. Length of main and supporting root among topping ratio was longer in 20% topping treatment but root growth except root length was good tend in 40% topping. Weight of root was high 40%>60%>20%>no topping the order, marketable yield in 40% topping of height was increased by 25% compared with no topping as 300kg/10a. content of Glycyrrhizinic acid with 20% and 40% topping of height was increased by 0.31% and 0.09% compared to no topping as 1.35% respectively.

        병리소견과 조직염색을 통한 흰기러기의 조류결핵과 Aspergillosis의 진단

        A 7-year-old, female snow goose (Anser caerulescens hyperboreus) with history of decreased activity for 2 month died in Daejeon Zoo Land in September 2006. At necropsy, granulomatous pneumonia and hepatomegaly with multiple cysts were observed. Small masses were found in the spleen. Microscopically, fibrinous pneumonia distributed in most of the lung lobe with pulmonary edema and congestion. Especially, granulomatous inflammation with numerous multinucleated giant cells was observed around the dilated bronchi. To confirm the diagnosis, acid-fast (Ziehl-Neelsen method) and periodic acid-Schiff (PAS) staining was performed. Acid-fast staining showed red bacterial colony indicating tuberculosis. PAS staining was also positive enough to diagnose aspergillus spp. co-infection that was an opportunistic fungi occurring in immuno-compromised animals. Based on the above results, we confirmed that the case submitted was diagnosed as avian tuberculosis.

        지역아동센터를 이용하는 학령초기 아동의 발달 특성에 따른 유형 분석과 영향요인 검증

        이 연구는 학령초기 아동의 방과 후 돌봄의 대안으로 주목 받고 있는 지역아동센터에 주목하여 지역아동센터 이용 아동의 인지, 언어, 사회정서 발달을 표준화된 검사 도구를 활용하여 측정하고, 발달 특성에 따라 유형을 분석한 후에 가족, 학교 및 지역사회를 포괄하는 환경요인의 영향력을 살펴보고자 하였다. 이를 위하여 지역아동센터를 이용하는 학령초기 아동과 주양육자 113쌍을 대상으로 발달검사 및 설문조사를 실시하고, 잠재프로파일 분석과 이항로지스틱분석을 적용하여 발달 유형을 분석하고 영향요인을 검증하였다. 연구결과, 지역아동센터를 이용하는 학령초기 아동의 발달수준은 전반적으로 정상분포에 속하나 개인차가 큰 것으로 나타났다. 둘째, 집단 내 발달 유형은 인지, 언어발달에서는 차이가 없었지만 사회정서 발달영역에서만 차이가 나타났으며, 집단은 ‘사회정서발달 부진’(43.6%)과 ‘사회정서발달 정상’(56.4%)의 두 집단으로 분류되었다. 마지막으로, 발달 유형에 영향을 미치는 환경요인을 분석한 결과 주양육자의 우울수준이 낮고 지각된 사회적 지지가 높을수록, 아동의 학교교우관계가 좋을수록, 주당 지역아동센터 이용시간이 길수록 ‘사회정서발달 정상’집단에 속할 확률이 높았다. 이러한 연구결과를 토대로 단순히 돌봄 공백을 해소하는 것에 집중하기보다는 아동의 발달역량을 강화할 수 있는 돌봄 환경을 마련하기 위해서 사회정서발달을 중점적으로 고려할 필요가 있으며 아동을 둘러싼 다양한 환경체계 간의 연계와 협업이 중요하다는 결론을 도출하였다. The aims of this study were (1) to examine the characteristics and profiles of the cognitive, language and socio-emotional development of children using community child center, (2) to identify the influence of environmental factors. For theses purposes, developmental tests and questionnaires were conducted on 113 pairs of children and caregivers. To solve the research questions, latent profile analysis was used for identifying distinct group and binomial logistic analysis was employed for finding the environmental predictors influencing developmental profiles. The results showed that the degree of child’s development showed a large individual difference and subgroups were classified into two groups, ‘underachievement of socio-emotional development’ and ‘normal achievement of socio-emotional development’. The difference between two groups was found to be only in a socio-emotional development. Finally, the results showed that as the depression level of the caregiver was lower, the followign effects were found: higher perceived social support, the children s greater ability to develop school friendship longer partronage of the other, and higher probability of belonging to the ‘normal achievement of socio-emotional development’ group. Based on these results, it is important to focus on socio-emotional development and the ecological environment-including family, school, and community-to go beyond addressing the absence of care and also strengthen children’ developmental potential.
