Virus associated hemophagocytic syndrome(VAHS), a class II histiocytosis syndrome, is characterized by high fever, liver dysfunction, coagulation abnormalities, and generalized histiocytic proliferation with marked hemophagocytosis in bone marrow and lymph nodes. VAHS is associated with several viral infections including Epstein Barr virus which has a relatively high mortality rate. We report a fatal case of Epstein Barr virus associated hemophagocytic syndrome and its diagnosis by mRNA in situ hybridization and polymerase chain reaction. A brief review of related literaure is also presented.

The bronchial cast formation is a rare phenomenon which occurs exceptionally in association with pneumonia, asthma, mucoviscidosis, bronchial mucoid impaction, alveolar proteinosis, and bronchiectasis. Such diseases can produce severe obstructive conditions with excessive mucous and mucopurulent secretions in the bronchi and alveoli leading to formation of variable size of cast, In this report, we describe a child with bronchial cast and subsequent atelectasis who was treated with flexible bronchofiberscope to remove it.

Surgical resection is the treatment of choice for hepatocellular carcinoma (HCC) in non-cirrhotic patients. The optimal indication for resection is a single tumor in a suitable location for resection. However, limit of the tumor size is not clear. We report a case of successful hepatic resection in patients with massive HCC sized more than 15 cm that did not respond to transarterial chemoembolization (TACE). A 49-year-old male patient had received TACE two times for massive HCC. However, the tumor size increased. Right hemihepatectomy was performed despite the extensive tumor size and underlying liver cirrhosis. Ascites and wound infection were developed after resection, but the patient`s general condition got recovered soon. Until 6 months after surgery, recurrence has not been detected. However, distant metastasis was noted at 7th month. Although recurrence with distant metastasis was noted, we think aggressive surgical approach prolonged this patient`s survival.

Helatocellular carcinoma (HCC) is uncommon in young adults, and young HCC patients is known to show poor prognosis than older HCC patients because they have a more advanced tumor stage at diagnosis. We describe a case of HCC in a 28-year old chronic hepatitis B virus carrier who showed multiple nodular HCC with bone metastasis at diagnosis. In spite of multidisciplinary treatment including transarterial chemoembolization (TACE) for liver mass and radiotherapy for metastatic bone lesion, the patient died of cancer progression and weakened general condition 15 months after diagnosis. Therefore, we need to consider periodic surveillance in young chronic hepatitis B virus carriers.
스테인레스 강 용접중 발생하는 망간의 발생량 및 함량변화에 관한 연구

Manganese has a role as both toxic and essential in humans. Manganese is also an essential component in the welding because it increases the hardness and strength, prevents steel from cracking of welding part and acts as a deoxidizing agent to form a stable weld. In this study, manganese generation rate and its content was determined in flux cored arc welding on stainless steel. Domestic two products and foreign four products of flux cored wires were tested in the well designed fume generation chamber as a function of input power. Welding fume was measured by gravimetric method and metal manganese was determined by inductively coupled plasma-atomic emission spectrophotometer. The outer shell of the flux cored wire tube and inner flux were analyzed by scanning electron microscopy to determine their metal compositions. Manganese generation rate(FGRmn) was increased as the input power increased. It was 16.3 ㎎/min at the low input power, 38.1 ㎎/min at the optimal input power, and up to 55.4 ㎎/min at the high input power. This means that FGRmn is increased at the work place if welder raise the current and/or voltage for the high productivity. The slope coefficient of FGRmn was smaller than that of the generation rate of total fume(FGR). Also, the correlation coefficient of FGRmn was 0.65 whereas that of FGR is 0.91. FGRmn was equal or higher in the domestic products than that of the foreign products although FGR was similar. From the electron microscopic analytical data, we concluded that outer shell of the wire was composed mainly of iron, chromium, nickel and less than 1.2 % of manganese. There are many metal ingredients such as iron, silica, manganese, zirconium, titanium, nickel, potassium, and aluminum in the inner flux but they were not homogeneous. It was found that both FGRmn and content of manganese was higher and more varied in domestic flux cored wires than those of foreign products. To reduce worker exposure to fumes and hazardous component at the source, further research is needed to develop new welding filler materials that improve the quality of flux cored wire in respect to these points. Welder should keep in mind that the FGR, FGRmn and probably the generation rate of other hazardous metals were increased as the input power increase for the high productivity.

Purpose: To emphasize the importance of considering tuberculosis in atypical cases of foot and ankle by reporting clinical results of those cases. Materials and Methods: Seven cases which were diagnosed as tuberculosis around foot and ankle from March 1996 to June 2007 were included. We reviewed initial impressions, the time to be diagnosed, clinical symptoms, laboratory findings, radiological findings and the clinical results and complications. Results: We followed up at least 6 months ($6{\sim}24$ months) after surgery in all cases. Initially 2 cases had been diagnosed as cellulitis, 4 cases as chronic osteomyelitis, and 1 case as an ankle instability. Tuberculosis was diagnosed after biopsy in all cases. Mean duration of symptom was 15 months ($6{\sim}36$ months) except in infants. There were various radiologic manifestations such as osteopenia, bony erosion or destruction and cystic changes. Symptoms were relieved in all cases within 4 months with chemotherapy followed by surgical biopsy, except one ankle which had been misdiagnosed as ankle instability and joint destruction was developed after modified Brostrom surgery. Conclusion: It is important to perform a surgical biopsy for diagnosis and proper management even with a faint suspicion on tuberculosis in foot and ankle. And in case of need, when surgical biopsy is performed, curettage procedure may help to improve clinical result.
