A 7-month old. castrated male Shih-tzu dog was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with history of depression. dyspnea. mild scale. tachypnea. exercise intolerance and severe leanness. On physical examination. cardiac arrhythmias was heard and heart worm ELISA test was positive. Microfilaria was found on the direct blood smear. Survey radiographs showed loss of diaphragmatic line and cardiac silhouette. displacement of lung fields and presence of gas filled intestines in the thoracic cavity. Rib fracture and pelvic fracture with callus formation were found incidentally. On the basis of physical examination and radiographic findings. it was diagnosed as traumatic diaphragmatic hernia and heart worm disease. Herniorrhaphy was performed. Postoperatively. it was treated for heart worm disease and scale. There is no evidence of recurrence of diaphragmatic hernia and heart worm disease over 1 year.
S-622 A Case of Invasive Hepatic Aspergillosis and Subsequent Breakthrough Pulmonary Mucormycosis

Invasive fungal disease (IFD) is one of the impacted complications which can affect the prognosis of patients with hematologic malignancies. Although the recent improvement of antifungal therapy have reduced the morbidity and mortality of IFDs, there are concerns regarding increase of breakthrough IFDs other than aspergillosis or azole resistance. Here, we report a case of transplantation breakthrough invasive pulmonary mucormycosis developed during voriconazole treatment for invasive aspergillosis. The patient recovered from two concomitant IFDs as well as reached to successful stem cell transplantation (SCT) for acute myeloid leukemia (AML). A 63-years-old female AML patient was diagnosed as proven invasive aspergillosis of liver based on a cytology of abscess drainage before second consolidation chemotherapy. She underwent chemotherapy with kept taking voriconazole (400 mg/day) After 10 weeks of voriconazole therapy, she was admitted for allogeneic SCT. Serum voriconazole drug level was within therapeutic range and abdomen CT showed remained but decreased size (from 8.6 cm to 4.8 cm) of liver abscess. On day 6 after SCT, first onset neutropenic fever was developed (up to 38.0℃). Absolute neutrophil count (ANC) was 0/mm3 and cefepime (4 g/day) and isepamicin (400 mg/day) was started empirically. Chest x-ray showed haziness in right middle lobe (RML), and CT revealed ill-defined consolidation with ground glass opacities. Repeated serum galactomannan tests were negative. Therefore, voriconazole was changed into liposomal amphotericin B (5 mg/kg/day) for galactomannan-negative breakthrough fungal pneumonia. On day 20 after SCT, ANC was revovered and follow-up chest CT showed typical, reverse halo sign. Therefore, she underwent RML lobectomy. Pathology showed non-septated hyphae with broad and right angle budding, which finally proved as mucormycosis. On day 56 after SCT, liposomal amphotericin B was discontinued. On post SCT 1-year follow-up, she completely recovered from both IFDs and AML. Breakthrough IFDs can be occurred during antifungal treatment. Clinicians should consider active diagnostic efforts, such as drainage or surgery, which can lead to successful outcome despite of immunocompromised state of patients.
The Association of Low Vitamin D Levels with Viral Response in Chronic Hepatitis B Patients

Aims: Vitamin D is an important immunomodulator and drawing attention in chronic hepatitis B. But the relationship between Vitamin D levels and viral response of chronic hepatitis B patients is controversial. Therefore, we investigated the effects of low vitamin D on viral response of chronic hepatitis B patients. Methods: We recruited a group of 356 CHB patients starting antiviral therapy (AVT). Vitamin D deficiency was defined as 25(OH)D3 concentration < 20 ng/mL. The rate of Hepatitis B e antigen (HBeAg) seroconversion, HBeAg loss, Hepatitis B Virus(HBV) DeoxyriboNucleic Acid(DNA) suppression (<2000 IU/mL) were compared between adequate vitamin D group and vitamin D deficiency group by Cox proportional hazard model. Results: HBeAg-positive patients were 173(48.6%) and 213 patients (59.8%) had vitamin D deficiency at the time of starting AVT. Vitamin D deficiency group was younger (P=0.001) and had higher Child-Pugh score(P=0.043) than adequate vitamin D group. In HBeAg-positive patients the 5-year cumulative incidence rates of HBeAg seroconversion of vitamin D deficiency group were significantly lower than that of adequate vitamin D group (P=0.019). And the 5-year cumulative incidence rates of HBeAg loss of vitamin D deficiency group were lower than that of adequate vitamin D group (P=0.079), but not significant. The 10-year cumulative incidence rate of HBeAg seroconversion (P=0.89) and HBeAg loss (P=0.2) did not reach statistical significance. In both HBeAg-positive and negative patients the 10-year cumulative incidence rates of HBV DNA suppression of vitamin D deficiency group showed lower tendency compared to that of adequate vitamin D group (P=0.051). Conclusions: In conclusion, our results showed vitamin D deficiency is common among CHB patients and is associated with early viral response, although further studies are needed to clarify the underlying mechanisms.

The present study numerically investigates the motion of a solid body suspended in the square enclosure with natural convection. A two-dimensional circular cylinder levitated thermally has been simulated by using thermal lattice Boltzmann method(TLBM) with the directforcing immersed boundary method. To deal with the ascending, falling or levitation of a circular cylinder in natural convection, the immersed boundary method is expanded and coupled with the TLBM. The circular cylinder is located at the bottom of a square enclosure with no restriction on the motion and freely migrates due to the Boussinesq approximation which is employed for the coupling between the flow and temperature fields. For different density ratio between the cylinder and the fluid, the motion characteristics of the circular cylinder for various Grashof numbers have been carried out. The Prandtl number is fixed as 0.7.

With a growing global population expected to reach over 9 billion in 2050 and the increasing impact of climate change, developing sustainable agriculture and aquaculture while at the same time ensuring food safety presents great challenges. Not only does it require a better understanding of how terrestrial and aquatic ecosystems function and their interrelationship with the availability of quality water, it requires a shift in the way we managethe ecosystems and the water within them to ensure food security. We have to safeguard and use our ecosystems wisely as they provide the basis of all the environmental services needed to achieve food security which are particularly important for the poor, women, and marginal groups. Agricultural and aquaculture production systems have to be recognized and managed as a landscape of integrated agro ecosystems with the potential for multiple functions. This presentation will review and examine the complexities involved in our ability to grow food sustainably in the midst of the degradation of the ecosystem services and examine the role policy makers can play in encouraging producers to retain or adopt more susceptible farming practices while safeguarding the benefits for local, national and global populations.

Trochlear nerve palsy associated with spontaneous subarachnoid hemorrhage (SAH) is known to be a rare malady. We report here on a patient who suffered with left trochlear nerve palsy following rupture of a right posterior communicating artery aneurysm. A 56-year-woman visited our emergency department with stuporous mental change. Her Hunt-and-Hess grade was 3 and the Fisher grade was 4. Cerebral angiography revealed a ruptured aneurysm of the right posterior communicating artery. The aneurysm was clipped via a right pterional approach on the day of admission. The patient complained of diplopia when she gazed to the left side, and the ophthalmologist found limited left inferolateral side gazing due to left superior oblique muscle palsy on day 3. Elevated intracranial pressure, intraventricular hemorrhage or a dense clot in the basal cisterns might have caused this trochlear nerve palsy.
Toxicoproteomic Analysis of Differentially Expressed Proteins in Rat Liver by DEHP

The endocrine disrupting chemical, di (2-ethylhexyl) phthalate (DEHP) is a plasticizer used in polyvinyl chloride products ubiquitous in our daily lives. DEHP has potentially adverse effects on the liver, kidney, lung, heart, reproductive organs and endocrine systems. Many toxicological data on the DEHP toxicity have been stated, but complete protein profiles have not yet been reported. In this study, DEHP-induced oxidative DNA damage in rat lymphocyte was evaluated by Comet assay (single-cell gel electrophoresis) for the first time. Moreover, DEHP-induced protein profile alterations were examined in rat liver by using toxicoproteomic tools. 34 protein spots in the liver were identified to be significantly deregulated by DEHP on the 2-dimensional gel. Among them, 20 spots were up-regulated and 14 spots down-regulated by DEHP.
