        3차 병원의 병동에서 시행된 심폐소생술의 분석

        Background: Although cardiopulmonary resuscitation(CPR) is a very effective therapy in cardiac arrest, it is hard to prove the true effectiveness of CPR. Several studies about out-of-hospital and emergency department CPR exist, but only a few reports about in-hospital CPR are available. This study was designed to investigate in-hospital cardiac arrest, to analyze the result of CPR, and to evaluate the problems associated with in-hospital CPR. Methods: A clinical analysis of 71 cases of in-hospital CPR announcement from January 2000 to August 2000 was performed. The initial rhythm on cardiac arrest, return of spontaneous circulation(ROSC), and the survivals were analyzed in the case of the 46 true cardiac arrest patients. Results: During 8 months, there were 71 cases of in-hospital CPR announcement. Among them, there were 46 cases of true cardiac arrest and 25 cases of non-cardiac arrest. Of the 46 true cardiac-arrest cases, 27(58.7%) experienced ROSC, 15(32.6) survived for over 24 hours, and 7(15.2%) survived to be discharged. The initial rhythms on cardiac arrest were 30 cases(65.2%) of asystole, 14(30.4%) of PEA(pulseless electrical activity), and 2(4.3%) of ventricular fibrillation, with ROSC being 17 cases(56.7%), 9(64.3%) and 1(50.0%) cases and discharged survivors being 4 cases(13.3%), 3(21.4%) and 0(0.0%) cases, respectively. Conclusion: Extraordinarily high proportions of asystole and PEA were seen in the initial rhythm of cardiac arrest, and those were associated with high survival rates. Although further study is needed to evaluate the course leading to this high proportion of asystole and PEA, this result suggests that if the EMS system in the hospital is activated promptly and systematically, a better outcome will be achieved in case of cardiac arrest with asystole and PEA.

      • 관상동맥질환의 중증도와 혈중지질치 및 아포지단백과의 상관관계

        목적 : 관상동맥질환의 위험인자로서 혈청지질 및 혈장 지단백에 대한 관심이 높아지고 있고 또 아포지단백이 관상동맥 질환을 일으키는데 다른 지질보다 더 중요하다는 연구가 많이 발표되고 있으나 우리나라에서는 아포지단백에 대한 연구가 그리 많지 않은 편이다. 이에 저자등은 혈중지질치와 새로운 위험인자로 알려지고 있는 아포지단백치를 측정하고 이들과 관상동맥질환 중증도와의 상관 관계를 알아보았다. 대상 및 방법 : 관상동맥질환이 의심되어 관상동맥조영술을 실시한 69명의 환자를 대상으로 하였으며 12시간 이상 공복후 혈청을 채취하여 총콜레스테롤, 중성지방, HDL-콜레스테롤, 아포지단백 AI (ape AI), 아포지단백 B (ape B)를 측정하였다. 관상동맥조영상 50%이상의 협착이 있는 혈관의 수 (lesion number), 최대협착을 합한 값 (lesion score), 각 분절의 최대협착을 합한 값 (total score), 가장 심한 협착 값 (peak stenosis) 등으로 관상동맥 중증도를 알아본 다음 이들과 혈청지질치, 아포지단백과의 상관 관계를 비교해 보았다. 상관 관계는 Spearman's correlation coefficient를 구하였다. 결과 : 관상동맥질환의 중증도와 각 지질치 간의 상관 관계를 비교해 보았을때 lesion number와 관계있는 것은 연령 (r=0.2789), 중성지방치 (r=0.2829)이며 lesion score와 관계있는 것은 연령(r=0.2911), 중성지방치0 (r=0.3316), 총콜레스테롤과 HDL-콜레스테롤의 비 (r=0.2361), apo B 치(r=0.2759), apo AI과 apo B의 비 (-0.3154)로 나타났다. Total score는 연령 (r=0.3100), 총콜레스테롤치 (r=0.2452), 중성지방치 (r=0.3022), 총콜레스테롤과 HDL-콜레스테롤의 비 (r=0.2770), apo B치 (r=0.2927), apo AI과 apo B의 비 (r=-0.3408)와 연관이 있었고 peak stenosis는 중성지방 치(r=0.3305), ape B 치 (r=0.2968), apo AI과 apo B의 비 (r=-0.3977)와 관계가 있었다. 연령으로 조정하여 partial correlation을 좌면, 관상동맥조영술상 중증도는 중성지방치 (r=0.3408), apo B 치(r=0.2877), apo AI과 apo B의 비 (r=-0.3460)와 유의한 상관 관계가 있었다. 이중 ape AI과 ape B의 비는 lesion score (r=-0.2640), total score (r=-0.3057), peak stenosis (r=-0.3460)와 유의한 상관관계를 보였다. 결론 : 이상의 결과로 보아 apo B치 그리고 apo AI과 apo B의 비가 관상동맥 중증도의 예견인자로 다른 혈중 지질 치보다 더 유용하다는 것을 알 수 있었다. Objectives : Recently, it was reported that the measurements of apolipoprotein levels may be valuable in the clinical assessment of coronary artery disease severity. However there are a few reports regarding to the relationship between coronary artery disease severity and apolipoprotein levels in Korea. Thus, we measured serum lipid and apolipoprotein levels and studied the relationship between coronary artery disease severity and serum lipids and apolipoproteins levels. Subjects and Methods : The 69 patients who underwent coronary angiography to evaluate chest pain were subjected to this study. We measured the levels of serum cholesterol, triglycerides, HDL cholesterol, apolipoprotein AI(apo AI) and apolipoprotein B(Apo B) and measured the severity of coronary artery disease by lesion number, lesion score, total score and peak stenosis. Then, we studied the relationship between coronary artery disease severity and apolipoproteins and calculated the correlation coefficient. Results : The results were as follows. There are significant correlation(r=0.3) between age, triglycerides, apo B levels, apo AI/B ratio and coronary artery disease severity. In controlling for age, tyiglycerides, apo B levels and apo AI/B ratio shows significant correlation(r=0.3) with coronary artery disease severity. Especially, apo AI/B ratio shows significant correlation(r=0.3) with lesion number, total score and peak stenosis. Conclusion : These results suggest that the apo B levels and the ratio of apo AI/B can be used as significant independent predictor for coronary artery disease severity rather than other serum lipid levels.

        급성심근경색증의 조기진단에 있어서 Myoglobin의 진단적 의의

        Background: Exact and early diagnosis of acute myocardial infarction (AMI) is essential for the subsequent routine management of this frequent cardiovascular disease. Currently, AMI has been diagnosed using the combination of the history, electrocardiogram(ECG), and biochemical markers of myocardial necrosis. At present, many biochemical markers are used to diagnose AMI. In this study, the predictive values of serum myoglobin and creatine kinase-MB(CK-MB) were compared in the emergency department. Methods: Fifty-four consecutive patients who presented within 12 hours from onset of chest pain of presumed cardiac origin were enrolled into the study. Patients with trauma or renal failure were excluded. The serial serum myoglobin and CK-MB levels were obtained prospectively at admission and 2, 4, 6, 12, 24 and 48 hours after admission. We compare the levels of serum myoglobin and CK-MB within 2, 4, 6, 8, 10, 12, 24, and 48 hours after symptom onset respectively. And we compare the sensitivity, specificity, positive predictive value, and negative predictive value of myoglobin and CK-MB. Results: using World Health Organization criteria, 28 AMI patients were identified. Mean time from symptom onset to presentation was 4.1±1.3 hours. The predictive values of serum myoglobin were better than those of CK-MB within 6 hours after symptom onset. But, 6 hours after symptom onset, the predictive values of CK-MB were better than those of serum myoglobin. The false positive cases of serum myoglobin were 3-one was lung cancer with pleural effusion and the others were unstable angina. The false positive cases of CK-MB were 6-one case was viral myocarditis and the ohters were unstable angina. Conclusion: To compare the serum myoglobin and CK-MB in the diagnosis of AMI, serum myoglobin had better predictive values than CK-MB within 6 hours after symptom onset and useful in the early diagnosis of AMI. But, 6 hours after symptom onset, CK-MB had better predictive values than serum myoglobin and useful in following up.

        전침과 부항을 이용한 말초성 안면마비 환자의 치험 2례

        Background: Bell’s palsy, an acute unilateral facial paralysis, is frequently treated with acupuncture in many countries. Bell’s palsy is easy to recur, and it is important to receive appropriate treatment quickly to give the dramatic effect of facial paralysis on a patient’s appearance, quality of life, and psychological well-being and to decrease the likelihood of incomplete recovery. Methods: We report two cases of facial nerve palsy treated by Electro-acupuncture on the affected side of the face using the points based on the RSN acupuncture method and by cupping therapy. Results: The clinical outcomes were assessed using the House-Brackmann Grading Scales (HBGS) before and after the treatment. In Case 1, a 44- year-old female with left facial palsy was treated for four weeks. HBGS VI before the treatment has improved to HBGS I after five treatments with Electro-acupuncture and cupping therapy. In Case 2, a 43-year-old female with right facial palsy was treated for three weeks. HBGS V before the treatment has improved to HBGS I after five treatments with Electro-acupuncture and cupping therapy. Conclusion: This study found that the RSN acupuncture method effectively improves Bell’s palsy during the progress phase. Electro-acupuncture and cupping therapy can be a safe and effective method to treat Bell’s palsy.

        Preparation and characterization of Melamine-Formaldehyde Resin Microcapsules Containing Fragrant Oil

        In this study, melamine-formaldehyde microcapsules were prepared via in situ polymerization using peppermint oil as a core material, melamine-formaldehyde as the wall material, Tween 20 as the emulsifier, and poly (vinyl alcohol) as a protective colloid. The melamine-formaldehyde microcapsules prepared in this study were then evaluated with regard to their structures, thermal properties, particle size distributions, morphologies, and release behaviors.

      • Photofragment Translational Spectroscopy of n-Butyl Iodide at 277 and 304 nm : Polarization Dependence and Energy Partitioning

        Photodissociation of n-C_4H_9I at excitation wavelenghts of 277 and 304 nm has been studied utilizing state-selective photofragment translational spectroscopy. The quantum yields of I^*(^2P_1/2) at these wavelenghts are determined to be 0.61 and 0.14, respectively. The anisotropy parameters observed at 277 nm are β(I) = 1.6 ± 0.1 for the ground-state I(^2P_3/2) and β ̄(I^*) = 0.9 ± 0.1 for the excited-state I^*(^2P_1/2) which substantially differs from limiting value. The results are interpreted in terms of dual path formation of iodine atoms from two different excited ^3Q_0 and ^1Q_1 states, i.e., a direct and an indirect dissociation via curve crossing between these states. The β ̄(I) (=1.9 ± 0.1) at 304 nm, close to its limiting value, shows that the primary processes for I and I^* formation channels proceed dominantly via a parallel transition to the ^Q_0 state in the long-wavelength region of the a band absorption profile. The translational energy distributions of recoil fragments show that a large fraction of the available energy goes into the internal excitation of the photofragment (<E_int>/E_avl ?? 0.7) for both dissociation channels. The quantitative analysis is presented for the state-dependent branching ratio and curve crossing probability combined with a quasidiatomic Landau-Zener description.
