파라콰트 중독 환자의 생존율에 영향을 미치는 초기 검사 지표

Purpose: Paraquat (1, 1-dimethyl-4, 4'-bipyridylium chloride) is widely a used non-selective herbicide. In spite of efforts to improve the outcome in patients poisoned with paraquat, the mortality rates still remains high. The purpose of this study is to identify initial stat laboratory parameters which can affect the survival rate of these patients. Methods: A retrospective analysis by chart review was done on 67 patients who had ingested paraquat and who had presented to the Emergency Medical Center of Chonnam University Hospital from June 1997 to July 2001. Results: The results were as follows: 1) Survivors were significantly younger than the nonsurvivors (38 years vs 44 years, p= 0.03). The volume of paraquat ingested by survivors was significantly smaller than that ingested by the deceased (1 mouthful vs 3 mouthfuls, p<0.001). 2) The WBC count and the levels of serum AST, BUN and serum creatinine in the deceased were significantly higher than those in the survivors. The levels of serum potassium and bicarbonate, arterial pH, and base excess in survivors were significantly higher than those in the deceased. 3) A multivariate analysis revealed that serum creatinine, serum potassium, and arterial base excess were associated with the fatality rate. Conclusion: Initial stat laboratory parameters including arterial blood gas analysis, renal function test, and serum electrolytes could be used to predict the outcome of patients poisoned with paraquat. However, the development of readily applicable and reliable indices predicting outcome is desired for the future.

이 글은 한국 근대기의 지식인 집단이 ‘룸펜’화 되어가는 과정을 통해 당시 이들에게 요구되었던 시대적 역할에 대한 기대가 어떻게 좌절되어 나가는지를 살펴본 것이다. 조선에 신학문이 보급되면서 배출되기 시작한 근대 인테리들은 그 희소성과 식민지라는 조선의 정치적 상황과 관련된 민족적 절망이 맞물리면서 사회의 기대를 한 몸에 받았었던 특권 집단이었다. 그러나 이들은 식민지적 조건에서 제1차 세계대공황이라는 세계사적 상황을 맞이하면서 직업 구조 안으로 흡수되지 못한 채 졸업과 동시에 룸펜의 길로 들어서게 된다. 당시 이러한 징후에 불안을 느낀 조선 사회는 범사회적으로 인테리들을 위한 취업강령을 내놓는 등 다양한 노력을 기울이지만, 이미 노동시장 자체가 왜곡되어 있는 상황에서 그것은 불가항력이었다. 결국 가난한 인테리들은 육체노동자가 되거나 호구지책을 위해 절도까지 자행하는 상태로 전락하게 된다. 더욱 극단적으로는 자살을 선택하기도 했다. 그러나 또 한편으로‘인테리 룸펜’이라는 용어에는 당시 지식인 집단이 갖고 있었던 자의식적 속물성이 결합되어 있기도 했다. 경제적 무능력과 동시에 인테리가 갖고 있던 속물성으로 인해 조선 사회는 그들을 비웃음거리로 전락시키는 집단적 공격을 보이게 되고, 더 나아가서는 그들에 대한 사회적 기대감까지 서서히 내려놓는 현상을 보여주고 있다. This article looked at how the Korean society' s expectation of the role by its intellectual class was shattered in the modem times, by exannmng its process of becoming lumpen. The intelligentsia, who started to be released along with the popularization of new learning (sinhangmun) in Joseon, were the privileged class who covered themselves with the society' s expectation, as their scarcity coincided with national despair related with Joseon’s political condition as Japan’s colony. However, they were faced with the global historical event of the first Great Depression, but were not absorbed into the employment structure and entered into the path of becorning lumpen right after graduation. The Joseon society which felt uneasy about such a sign made diverse efforts on a national scale such as coming up with a platform for employing them, but the problem was inevitable under the condition where the labor market itself was distorted. Poor intellectuals ended up with becoming physical labor workers or degrading into thieves who even committed theft as a means of livelihood. More extremely, some opted for suicide. However, the term "lumpen-intelligentsia" as well was a combination with self-conscious snobbery of the intellectual class at the tirnes. Their economic incompetence combined with such snobbery brought about the Joseon society' s collective attack, degenerating them into those subject to mockery, and it gradually gave up expectation of them.

서사물의 디지털콘텐츠화 전략 연구 - 문화원형 개발 사례를 중심으로 -

This paper investigates the developmental strategies required to turn literary narratives into digital contents and their potentiality for culture industries based on the established cases in the project of making the prototype of cultures digital contents. The investigation leads to the following statements: In order to make archetypal materials of narratives into the materials of the industry of culture contents, those materials should have user-friendly segmental units, and should not focus on end products. Segmentation and disjunction are the very strategy of narratives needed to satisfy these requirements. The episodes of their own narrative structures are made into a great narrative based on their temporal sequences, and then those episodes are selectively collected into a newly emerged narrative. In this case, strategies of paralleling independent episodes, connecting unit stories or archetypal episodes extracted from a great narrative, and producing indefinite episodes through hypertexts, are being used. Another developmental strategy is to make modern versions out ot traditional narratives. The strategy tries to remake a modern narrative out of the basic motifs of archetypal materials to secure the industrial popularity as well as the maintenance of tradition. The literary narratives developed on these strategies are not just confined to their own industrial values, but they can also create a series of values through window effect, such as animation, movie, game, character, and mobile.
