Exercise-induced anaphylaxis(EIA) is a medical emergency in that it derives from a physical allergy. We reported a case of food-independent EIA based on history and laboratory data. A 40-year-old male patient was presented with generalized urticaria, angioedema, and syncope after jogging. He had suffered from allergic rhinitis for 25 years. Increased level of serum tryptase was noted. Mast cell activation might be suggested in the pathophysiologic mechanism of EIA. (J Asthma Allergy Clin Immunol 24 : 152-5, 2004)
오스템퍼링한 구상흑연주철(ADI)의 드릴 가공시 경도 및 현미경조직변화에 관한 연구

This paper was carried out to know the influence of advanced austempered ductile cast iron (ADI) on the tool life and mechanical properties of drilling machinability. For manufactured method of ADI, the spheroidal graphite cast iron were austenized at 900℃ for 1 hour and then austempered for 2 hour at 370℃ in the salt bath. And interrelationship has been investigated between tool life and mechanical characteristics of specimen material on drilling condition when the ordinary and step-feed drilling are carried out to drill holes of specimens. Tensile strength and hardness of ADI decrease and elongation of ADI increases with the increase austempered temperature. It is known that about 2 times of tool life in the case of step-feed decreases compared with ordinary feed due to the high hardness of ADI and hardness ascribed to the fact that retained austenite beame to martensite state due to cutting heat in drilling. Under the constant feed rate 0.1㎜/rev relation between hardness and length of end tip after drilling can be formularized to Hv=788.46L^-0.096 for the cutting speed 6.1m/min.
Selective hydrophobic derivatization on the surface of helical silica nanotubes

<P><B>Abstract</B></P> <P>The chiral 1,2-diphenylethylenediamine derivative that is capable of spontaneous self-assembly was employed as an organogel template to produce a helical mesoporous silica nanotube containing gelators therein by following sol-gel polycondensation of TEOS. The synthesis enabled the successful introduction of the hydrocarbon of octyl silane (hydrophobic functional group) onto the outer surface of the silica nanotube while preserving the hydrophilic silanol (SiOH) group on internal surface of silica nanotube free from the gelators. This synthetic condition consists of a pre-stage of the introduction of a hydrophobic hydrocarbon functional group onto the outer surface of the silica nanotube selectively, and the post-stage washing of the gelators was presented together with a method analyzing the actions of organogels in the respective experimental processes.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A hydrophobic octyl groups were introduced on the surface of silica nanotube with organogels. </LI> <LI> The hydrocarbons were derivatized on the outer surface of silica nanotube via TEM image mapping and TOF-SIMS spectroscopy mode. </LI> <LI> A synthesis process of derivatizing hydrocarbons on the outer surface of the silica nanotubes has been proposed. </LI> <LI> EA, LC and TOF-SIMS analyses were performed to confirm that the organogels remained in the silica nanotubes during the washing process. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Dual antiplatelet therapy (DAPT) is recommended in patients receiving drug-eluting stents (DES). However, bleeding risk should be weighed against ischemic risk. Utility of GRACE risk score and ACUITY-HORIZONS bleeding risk score was assessed in patients with acute myocardial infarction (MI) according to use of P2Y12 blocker.</P> <P><B>Methods</B></P> <P>From the Korea Acute Myocardial Infarction Registry-National Institute of Health database, 7791 patients with acute MI receiving DES were divided into ticagrelor (<I>n</I> =1554) and clopidogrel (<I>n</I> =6237) groups. Propensity-matched 12-month mortality and bleeding event rates were compared according to GRACE and ACUITY-HORIZONS scores. Patients who received thrombolysis, prasugrel or anticoagulants, or who discontinued or switched DAPT were excluded.</P> <P><B>Results</B></P> <P>In all patients, high-risk patients more often received clopidogrel. After propensity score matching (<I>n</I> =1553 in each group), 12-month mortality was not different, but TIMI major bleeding rate was higher with ticagrelor (2.8% vs. 1.4%, <I>p</I> =0.007). On subgroup analysis, 12-month mortality was lower with ticagrelor in patients with high (>140) compared to low-to-moderate risk GRACE score (5.1% vs. 7.9%, <I>p</I> =0.04). When combined with ACUITY-HORIZONS bleeding score, 12-month mortality was lower with ticagrelor in patients with high GRACE score but without very high (≥20) ACUITY-HORIZONS score (2.4% vs. 5.3%, <I>p</I> =0.03).</P> <P><B>Conclusions</B></P> <P>In patients with acute MI receiving DES, GRACE and ACUITY-HORIZONS scores may help guide DAPT. In patients with high GRACE score, a more potent P2Y12 blocker may be considered, particularly in the subset not at very high risk of bleeding.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Both ischemic and bleeding risks are correlated with mortality post-percutaneous coronary intervention. </LI> <LI> GRACE and ACUITY-HORIZONS scores may help guide dual antiplatelet therapy (DAPT) in patients with acute myocardial infarction (MI). </LI> <LI> The optimal DAPT regimen in Asian patients with acute MI needs to be determined. </LI> </UL> </P>

Mesenteric venous thrombosis is an rare type of intestinal ischemia associated with significant mortality and morbidity because of its delay in diagnosis. Mesenteric vasculitis associated with allergic vasculitis can also produce an acute intestinal ischemia. We recently experienced a case of mesenteric venous thrombosis associated with allergic vasculitis, successfully treated with segmental bowel resection and anticoagulant therapy, we report this case with a brief review of the literatures. A 30-year-old man was admitted to our hospital because of severe abdominal pain and hematochezia. Abdominal CT scan showed superior mesenteric venous thrombosis extending into main portal vein. There were large amounts of ascites and the wall of the jejunum was markedly thickened. On the 5th hospital day, his abdomen was tense and diffusely tender: bowel sounds were absent. Exploratory laparotomy was performed under the impression of intestinal infarction. At laparotomy thrombosis of the mesenteric vein and gangrene of the jejunum were found. A small bowel resection with an end-to-end anastomosis was done. Histopathological examination confirmed venous bowel gangrene and showed leukocytoclastic vasculitis, the hallmark of allergic vasculitis. The patient recovered fully and was discharged on anticoagulant therapy. At follow-up after 6 months, the patient was well with no gastrointestinal symptoms.
Fracture patterns and causes in the craniofacial region: an 8-year review of 2076 patients

Background: For proper recovery from craniofacial fracture, it is necessary to establish guidelines based on trends. This study aimed to analyze the patterns and causes of craniofacial fractures. Methods: This retrospective study analyzed patients who underwent surgery for craniofacial fractures between 2010 and 2017 at a single center. Several parameters, including time of injury, region and cause of fracture, alcohol intoxication, time from injury to surgery, hospitalization period, and postoperative complications, were evaluated. Results: This study analyzed 2708 fracture lesions of 2076 patients, among whom males aged 10 to 39 years were the most numerous. The number of patients was significantly higher in the middle of a month. The most common fractures were a nasal bone fracture. The most common causes of fracture were ground accidents and personal assault, which tended to frequently cause more nasal bone fracture than other fractures. Traffic accidents and high falls tended to cause zygomatic arch and maxillary wall fractures more frequently. Postoperative complications-observed in 126 patients-had a significant relationship with the end of a month, mandible or panfacial fracture, and traffic accidents. Conclusions: The present findings on long-term craniofacial fracture trends should be considered by clinicians dealing with fractures and could be useful for policy decisions.

Unit 1 of the Kori Nuclear Power Plant (NPP) and Unit 1 of the Wolsong NPP are being prepared for decommissioning; their decommissioning is expected to generate large amounts of intermediate-level, low-level, and very low level Waste. Mixed waste containing both radioactive and hazardous substances is expected to be produced. Nevertheless, laws and regulations, such as the Korean Nuclear Safety Act and Waste Management Act, do not define clear regulatory guidelines for mixed waste. However, the United States has strictly enforced regulations on mixed waste, focusing on the human health and environmental effects of its hazardous components. The U.S. Nuclear Regulatory Commission and the U.S. Department of Energy regulate the radioactive components of mixed waste under the Atomic Energy Act. The U.S. Environmental Protection Agency regulates the hazardous waste component of mixed waste under the Resource Conservation and Recovery Act. In this study, the laws, regulations, and authorities pertaining to mixed waste in the United States are reviewed. Through comparison and analysis with waste management laws and regulations in Korea, a treatment direction for mixed waste is suggested. Such a treatment for mixed waste will increase the efficiency of managing mixed waste when decommissioning NPPs in the near future.

Henoch-$Sch\ddot{o}nlein$ purpura (HSP) is an immunologically mediated systemic vasculitis of small blood vessels that primarily involves the skin, gastrointestinal tract, joints and kidneys. HSP is a common vasculitic syndrome in children who, in most cases, achieve complete recovery. Pulmonary hemorrhage is a very rare manifestation of HSP. The authors present a case of a 46-year-old male presenting with pulmonary hemorrhage and renal involvement and the diagnosis of HSP. The patient responded to prednisolone therapy.
