        특발성 자발성 신장 파열 1예

        Primary aldosteronism is a syndrome characterized by hypertension, hypokalemia, suppressed plasma renin activity, and elevated serum aldosterone levels. Conn first described it 1955 in association with adrenal cortical adenoma(Conn's syndrome). Today, it can be divided into at least six distinctive sub-groups. The diagnosis of primary aldosteronism is usually suspected when the patient presents with poorly controlled hypertension with hypokalemia. The main issues in the evaluation of primary aldosteronism are to differentiate an adenoma from hyperplasia and to localize the adenoma. Basic hormonal studies and computed tomographic(CT) scanning can be used effectively for that differentiation. An adenoma is one of a few potentially curable forms of hypertension, and it is best treated by removing the adrenal tumor. We experienced a case of a typical adrenal adenoma. The patient was a 37-year-old male who had experienced in both his legs over a period of several days a weakness due to hypokalemia. He was diagnosed using basic hormonal studies and adrenal CT scanning. After, he received laparoscopic adrenalectomy and was discharged with improved condition.

        이층지반과 단일지반의 열전달 거동 특성 평가

        This paper presents how to analyze heat transfer characteristics of double-layered soils. Thermal response tests were conducted to measure the ground thermal conductivities of Joomunjm sand and douole layered soils filled in a steel box of which the size is 5m × 1m × 1m. Double-layered soils were composed of Joomunjm sand and Kaoline clay. Each Lhermal conductivity of Joomurym sand and Kaloine clay was measured by using Heat Flow Meter considering different void ratio. The ground thermal conductivity of doubleㅛlayered soils was 15% smaller than that of Joomunjin sand.

        FC-25계 회주철의 Boro-Nitriding 복합 표면처리에 따른 표면특성과 고온산화거동에 관한 연구

        Surface properties and oxidation behavior were studied for FC-25 gray cast iron (GCI), ion nitrided CCI (N CCI), powder boronized CCI (B GCI) and powder boronized N CCI (B-N GCI). A boride (Fe2B and FeB) layer with a tooth shape was formed by boronizing GCI at 650℃. The thickness and hardness of the layer were proportional to boronizing temperature and time. The hardness was decreased as the depth of the boronized layer increased. The activation energy for boride formation was about 74 kJ/mol. The thickness of the graphite layer formed by boronizing was increased by increasing temperature and time. A silicon layer insoluble to borides accumulated beneath the boride layer. The major phases were Fe4N, Fe3N and FeN in the nitride layer formed by ion nitriding. The thickness and hardness of the layer increased with increasing nitriding temperature and time under the Fe4N decomposition temperature of 680℃. The hardness was maximum at the surface and decreased with increasing nitrided depth on B-N GCI at 700℃, above the Fe4N decomposition temperature. Hardness of B-N GCI at the layer depth of 10 μm was 1000 Hv, which was lower than that of the B GCI at the same condition. Also the layer thickness was 50 gm; 10 μm deeper than that of B GCI. The reason was that the nitrogen gas given off by the decomposition made the layer porous and facilitated the diffusion of oxygen and other reactants. The samples were oxidated at 800℃ for 48hrs under atmospheric air. Oxidation rate was in decreasing order of N GCI, GCI, B GCI and B-N GCI. Oxidation rates were greatly influenced by the diffusion of nitrogen gas formed by Fe4N decomposition over the decomposition temperature of 680℃ and the oxidation barrier of the less corrosive boride layer.         FC-25계 회주철의 이온질화처리에 따른 표면특성과 고온산화거동에 관한 연구

        Surface properties and high temperature oxidation behavior were investigated for FC-25 Gray Cast Iron(GCI) and the ion nitrided GCI(N-GCI). The GCI was pre-cleaned to improve hardness to the optimum pre-sputtering parameters with an Ar/H₂ ratio of 1/2, working pressure of 3 torr, working temperature of 550℃ and working time of 1hour. The optimum nitriding conditions for the maximum hardness of 560575 Hv were an N₂/H₂ ratio of 3/1, working pressure of 3 torr, and working temperature of 575℃. The thickness of graphite in the GCI was increased by increasing the working temperature from 525℃ to 595℃ for the nitriding time of 6-18hrs. XRD patterns showed FeO and Fe₂O₃ peaks for both the oxidized N-GCI and GCI at temperatures of 600℃ and 800℃ under atmospheric environment for both 24 and 60hours. At 800℃, above the Fe4N decomposition temperature of 680℃, the oxidation rate of N-GCI was greater than that of the GCI. The most abundant nitride, Fe4N, was decomposed and the nitrogen gas given off by the decomposition made the protective film porous by degassing through the film. But at 600℃, below the decomposition temperature, the degree of oxidation of N-GCI was lower than that of the GCI because the nitride film worked as protective barrier for oxidation.         Clinical Outcomes of Intrascleral Fixation of Intraocular Lens Compared to Ciliary Sulcus Implantation and Transscleral Fixation

        Purpose: To compare the clinical outcomes of intrascleral fixation of the three-piece intraocular lenses (IOLs) 2.5 mm posteriorto the limbus with ciliary sulcus implantation and transscleral fixation 2.5 mm posterior to the limbus. Methods: Sixty-five eyes of 65 patients who underwent ciliary sulcus implantation or transscleral or intrascleral fixation ofthe AMO Sensar AR40e IOL were retrospectively reviewed. The postoperative refractive prediction error, back-calculatedeffective lens position (ELP), corrected distance visual acuity (CDVA), and postoperative residual cylinder were compared. Results: There were significant differences in the median (interquartile range) postoperative refractive prediction error (diopters[D]) among the three groups (p < 0.001): for ciliary sulcus implantation (33 eyes), −0.89 D (−1.21 to −0.56 D); for transscleralfixation (10 eyes), −0.40 D (−0.78 to −0.22 D); and for intrascleral fixation (22 eyes), 0.01 D (−0.28 to 0.34 D). Significantdifferences (p < 0.001) were observed in the median back-calculated ELP: for ciliary sulcus implantation, 4.35 mm (3.95 to4.55 mm); for transscleral fixation, 4.51 mm (4.34 to 4.76 mm); and for intrascleral fixation, 4.90 mm (4.56 to 5.35 mm). Therewere no differences in the median postoperative CDVA (0, 0.10, and 0 logarithm of the minimum angle of resolution, respectively;p = 0.083) and the residual cylinder (−0.75, −1.50, and −0.63 D, respectively; p = 0.074) among three groups. Conclusions: Intrascleral fixation showed no myopic shift and the most posterior lens position, while ciliary sulcus implantationinduced the greatest myopic shift and the most anterior lens position. However, there was no significant difference in thepostoperative CDVA or astigmatism among the eyes with different IOL insertion methods, demonstrating good IOL stabilityand vision outcomes.

        Facilitation of AMPA receptor-mediated steady-state current by extrasynaptic NMDA receptors in supraoptic magnocellular neurosecretory cells

        In addition to classical synaptic transmission, information is transmitted between cells via the activation of extrasynaptic receptors that generate persistent tonic current in the brain. While growing evidence supports the presence of tonic NMDA current (I<sub>NMDA</sub>) generated by extrasynaptic NMDA receptors (eNMDARs), the functional significance of tonic I<sub>NMDA</sub> in various brain regions remains poorly understood. Here, we demonstrate that activation of eNMDARs that generate INMDA facilitates the α-amino-3-hydroxy-5-methylisoxazole-4-proprionate receptor (AMPAR)-mediated steady-state current in supraoptic nucleus (SON) magnocellular neurosecretory cells (MNCs). In low-Mg<sup>2+</sup> artificial cerebrospinal fluid (aCSF), glutamate induced an inward shift in I<sub>holding</sub> (I<sub>GLU</sub>) at a holding potential (V<sub>holding</sub>) of –70 mV which was partly blocked by an AMPAR antagonist, NBQX. NBQX-sensitive IGLU was observed even in normal aCSF at V<sub>holding</sub> of –40 mV or –20 mV. IGLU was completely abolished by pretreatment with an NMDAR blocker, AP5, under all tested conditions. AMPA induced a reproducible inward shift in Iholding (I<sub>AMPA</sub>) in SON MNCs. Pretreatment with AP5 attenuated I<sub>AMPA</sub> amplitudes to ~60% of the control levels in low-Mg <sup>2+</sup> aCSF, but not in normal aCSF at V<sub>holding</sub> of –70 mV. I<sub>AMPA</sub> attenuation by AP5 was also prominent in normal aCSF at depolarized holding potentials. Memantine, an eNMDAR blocker, mimicked the AP5-induced I<sub>AMPA</sub> attenuation in SON MNCs. Finally, chronic dehydration did not affect I<sub>AMPA</sub> attenuation by AP5 in the neurons. These results suggest that tonic INMDA, mediated by eNMDAR, facilitates AMPAR function, changing the postsynaptic response to its agonists in normal and osmotically challenged SON MNCs.

        Case Report : Reactivation of Pulmonary Tuberculosis in a Patient with Polygonum multiflorum Thunb-Induced Hepatitis

        Several cases of Polygonum multiflorum Thunb-induced hepatitis have been reported worldwide. Anthraquinone is an active ingredient of P. multiflorum Thunb. that has been thought to play a role in its hepatotoxicity. Here we report the case of a 34-year-old Korean man who had P. multiflorum Thunb-induced hepatitis and reactivation of pulmonary tuberculosis caused by bone marrow suppression, which developed simultaneously. He was admitted to our hospital with recently developed fatigue and aggravated jaundice. He was a previously healthy man except for the sequelae of pulmonary tuberculosis seen on chest X-ray. He had a 30-day history of ingesting the root of P. multiflorum as a form of liquor and tea. The patient was diagnosed with P. multiflorum Thunb-induced hepatitis after excluding all other potential causes of acute hepatitis. Liver function gradually improved following the total cessation of the consumption of the material. However, he suffered from spiking fever with progressive pancytopenia during the hospital stay. A bone marrow biopsy showed markedly hypocellular marrow, suggesting transient bone marrow suppression, which was probably caused by extrinsic factors such as drugs, toxins, and viral infection. Although he began to complain of a dry cough, repeated sputum investigations revealed positive acid-fast bacillus staining. The fever subsided and pancytopenia improved after treatment for pulmonary tuberculosis. These observations suggest that P. multiflorum Thunb induces both bone marrow suppression and hepatotoxicity.         Dynamic Equivalent Battery as a Metric to Evaluate the Demand Response Performance of an EV Fleet

        Electric vehicles (EVs) are significant resources for demand response (DR). Thus, it is essential for EV aggregators to quantitatively evaluate their capability for DR. In this paper, a concept of dynamic equivalent battery (DEB) is proposed as a metric for evaluating the DR performance using EVs. The DEB is the available virtual battery for DR. The capacity of DEB is determined from stochastic calculation while satisfying the charging requirements of each EV, and it varies also with time. Further, a new indicator based on the DEB and time-varying electricity prices, named as value of DEB (VoDEB), is introduced to quantify the value of DEB coupled with the electricity prices. The effectiveness of the DEB and the VoDEB as metrics for the DR performance of EVs is verified with the simulations, where the difference of charging cost reduction between direct charging and optimized bidding methods is used to express the DR performance. The simulation results show that the proposed metrics accord well with the DR performance of an EV fleet. Thus, an EV aggregator may utilize the proposed concepts of DEB and VoDEB for designing an incentive scheme to EV users, who participate in a DR program.
